Diagnostic Value of Transabdominal and Transperineal Ultrasonography in Cervical Insufficiency

Abstract  【Objective】To explore the value of transabdominal and transperineal ultrasonography in the diagnosis of cervical insufficiency.【Methods】A total of 84 pregnant women with suspected cervical insufficiency treated in our hospital from January 2017 to June 2019 were randomly divided into transabdominal ultrasound group (transabdominal ultrasound, n=42) and transperineal ultrasound group (transperineal ultrasound group, n=42).Two examination methods were used to compare the projection of amniotic sac into the cervix and the degree of opening of cervical canal, and the diagnostic value of transabdominal ultrasonography and transperineal ultrasonography in cervical insufficiency was analyzed. 【Results】The detection rate of amniotic sac protruding into the cervix in transabdominal ultrasound group was 9.52% (4/42), and there was no significant difference compared with 14.29% (6/42) in the transperineal ultrasound group(χ2=0.454,P=0.500). There was no significant difference between transabdominal ultrasonography and transperineal ultrasonography in the diagnosis of the degree of cervical opening (P>0.05). Compared with the transabdominal ultrasound group, the sensitivity, specificity and diagnostic rate of the transperineal ultrasound group increased, and the missed diagnosis rate and misdiagnosis rate decreased. 【Conclusion】Compared with transabdominal ultrasonography, transperineal ultrasonography has a higher diagnostic rate of cervical insufficiency, and the missed diagnosis rate and misdiagnosis rate are reduced. It is worthy of clinical application.
