Traumatic Diaphragmatic Hernia and Traumatic Diaphragmatic Injury Treated by Abdominal Operation:16 Cases of Report

【Objective】To discuss the surgical indications and the treatment results of traumatic diaphragmatic hernia(TDH)and traumatic diaphragmatic injury(TDI)treated by abdominal operation.【Methods】Retrospective analysis of 16 patients with treatment of TDH and TDI underwent abdominal surgery was conducted in terms of causes, clinical manifestation, preoperative examination, surgical approach selection and the result of surgical treatment. 【Results】Sixteen cases were included in the group: 14 males,2 females;13 left,1 right,2 double;all cases combined with united injury,11 cases combined with traumatic shock. 8 cases examined by Xray,5 diagnosed ;16 cases examined by computed tomography,135 diagnosed ; 2 cases examined by thoracoscopy,both with diaphragm injury;16 cases were treated by abdominal diaphragm repair,15 cases cured,3 cases with complications,2 cases with incision infection, 1 case with inflammatory intestinal obstruction and 1 case died of multiple organ failure.【Conclusion】 Most of TDI and TDH could be repaired by abdominal operation so as to treat bleeding of the abdominal organs timely ,and the double side of diaphragm could be examined to avoid the misdiagnosis and missed diagnosis. Meanwhile the abdominal operation could reduce the effect of respiratory and circulatory system caused by open chest operation , and will be benefit to postoperative discharge of phlegm and rehabilitation.

Importance of HATCH Score in Predicting the Effect of Atrial Fibrillation Radiofrequency Catheter Ablation

To evaluate the importance of the HATCH score on predicting effects of atrial fibrillation radiofrequency catheter ablation (AFRCA)

Methods
】
A retrospective analysis was carried out for 118 cases who underwent AFRCA with CARTO guidance from Dec. 2010 to Dec. 2013. They were divided into a recurrence group and a non
recurrence group based on one year follow
up of sinus rhythm preservation. The clinical data and HATCH scores of the two groups were analyzed.

The ratio of patients with hypertension, diabetes and stroke were higher in the

Expression and Clinical Significance of CD147 Protein in Human Prostate Cancer

Objective
】
To investigate the expression and clinical significance of CD147 protein in human prostate cancer and benign prostatic hyperplasia tissues.

Methods
】
The immunohistochemical S
P method was used to detect the expression of CD147 protein in 56 patients of prostate cancer tissues and 22 patients of benign prostatic hyperplasia tissues so as To analyze the correlation between CD147 protein and the clinical biological characteristics of prostate cancer.

Results
】
The positive rate of CD147 protein in the observation group was 9.1%, which was significantly higher than that in the control group , the difference
was statistically significant (

<0.01). There was no significant difference in CD147 protein expression in different age groups

>0.05). The positive expression of CD147 protein in different stages of Gleason staging and pathological grading was significantly different (
P
<0.05), The positive expression rate of CD147 protein in each grade group was statistically significant (

Conclusions
】
The high expression of CD147 in prostate cancer tissues is related to its pathological stage and clinical stage. CD147 has the potential to be an important indicator to judge the invasion of prostate cancer, and to block the expression of CD147 may become a new target for the treatment of prostate cancer.

Research on the Application of HACCP in the Supervision of Fever Clinics

【Objective】To master the major influence factors in the management of fever clinics; to identify the key controlling point (CCP) of fever clinics and work out plans on HACCP ; to improve the efficiency of health supervision on fever clinics. 【Methods】 The influence factors were investigated in accordance with the setting of fever clinic, the use of the situation, the management status and the management of infectious diseases. The potential hazard of each link was analyzed on the base of the important data (total surface colonies); The CCP and critical limits (CL) were determined according to the expert advice by developing the corresponding preventive measures and establishing the fever outpatient monitoring system for each CCP point.【Results】 In terms of the sanitary conditions of the objects, the desks of all medical institutions, instrument panels surface and handles of devices as well as other objects are all below qualification, among which the telephones and handles of taps are the worst. Eighteen hazard factors have been assured through sitevisiting and questionnaires. These facts include: patients fetch medicines outside fever clinics; the air and ventilation system is not independent and the rubbish in fever clinics are dealt with as medical waste; the mixture of treatment region and ordinary region. The existing problems such as medical processes, personal arrangement, personal protection awareness, and disinfection health problems are all set as key controlling point. Meanwhile, the plan on HACCP has been worked out, and the content, methods, frequency, monitoring, corrective actions, record and revise measures of the key controlling points(CCP) are indentified. 【Conclusion】The HACCP principle is applicable to the supervision of fever clinic in medical institutions. Upon the regulation of CCP and CL in trems of the daily health supervision can improve the efficiency of operation and the efficiency of health supervision, and reduce the risk of regional infectious diseases.

HBVDNA Load Testing of Newborns with Intrauterine HBV Infection Prediction Value Analysis

【Objective】To investigate the effect of hepatitis B virus intrauterine infection on neonatal infants. 【Methods】One hundred and six cases of pregnant women with HBsAg positive results in our hospital from Jan. 2010 to Dec. 2014 were used as an observation group. According to HBV Casset results, 106 cases of pregnant women were divided into a HBsAg, HBeAg, and HBcAb positive group (26 cases), a HBsAg, HBeAb, and HBcAb positive group (74 cases) and a HBsAg, HBcAb positive group (6 cases). Venous blood of pregnant women and cord blood of infants were collected to determine HBV 5 tests and HBV DNA quantification test. 【Results】Ten infants were found to be HBsAg positive among 106 cases of pregnant women with HBsAg positive results, accounting for 9.43% of the patients. There were no significant differences among the intrauterine infection rates of the 3 groups (F=1.204,P=0.163). However, there were significant differences among the groups' HBV DNA capacities (F=5.204,P<0.05). Furthermore, HBV DNA copy and intrauterine infection was positively corelated (r=0.672,P=0.015). 【Conclusion】Using HBV serum landmarks for intrauterine infection detecting is not suitable. Thus, detecting the serum HBV DNA copy of pregnant women will help decrease the risk of intrauterine infection.

Effect of Diabetes Drug Injection Guide on Training of the Injection Technique Among Clinical Nurses

【Objective】To investigate the understanding of injection technique and patient education in clinical nurses, and compare their understandings before and after Diabetes Drug Injection Guide training. 【Methods】Randomly chosen 90 clinical nurses were enrolled in our study. A questionnaire concerning injection technique was made before and after training. The questionnaire was designed according to the Chinese guideline. 【Results】Before the training, clinical nurses had a good understanding of both the position of injection and disinfection procedures (accuracy rate above 90%). However,the clinical nurses had poor knowledge about the psychological counseling, injection routine, examination et al (accuracy rate of 7.8~73.3%), and the situation of patient education was poor (accuracy rate of 3.3~54.4%). After the training, the knowledge of clinical nurses were improved (accuracy rate after training was 30.0~87.8%,the accuracy rate before training was 27.8~74.4%,P<0.05). 【Conclusion】The training improved the understanding and grasping of injection technique among clinical nurses, and promoted the situation of patient education.

Impact of 1, 25 dihydroxycholecalciferol on Kidney Injury of Type 2 Diabetic Rats

【Objective】To study the impact of 1, 25 dihydroxycholecalciferol on kidney injury of type 2 diabetic rats. 【Methods】30 male Wistar rats were randomly divided into normal diet group (N group, 10 rats) and high fat and high glucose diet group (model group, 20 rats), Type 2 diabetes model group were successfully set and randomly divided into two groups, with 10 rats in each group, diabetes mellitus group (M group) was treated by intraperitoneal injection of 2.5 g/(kg•d) of peanut oil, and D group by intraperitoneal injection 2.5 μg/(kg•d) 1, 25 dihydroxycholecalciferol dissolved in peanut oil. After 8 weeks, blood samples were collected to detect blood glucose, creatinine and urea nitrogen;At the same time, the rats were sacrificed and the kidney tissues were kept.Tumor necrosis factor alpha (TNFα), transforming growth factor beta 1 (TGFβ1) were detected by Blot Western and compared with that in renal tissue.【Results】Blood glucose, urea nitrogen and creatinine in M group and D group were significantly higher than those in N group (P<0.05), but the blood glucose, urea nitrogen and creatinine in D group were significantly lower than that in M group (P<0.05); The levels of TNFα、TGFβ1 in M group and D group were significantly higher than those in N group, while the D group was significantly lower than that in M group (P<0.05).【Conclusion】1, 25 dihydroxycholecalciferol can inhibit expression of TNFα、TGFβ1 of diabetic rats kidney and take protective effect on renal injury in rats with diabetes.

Multivariate Logistic Regression Analysis of Anesthesia Recovery Agitation

【Objective】To explore the risk factors of emergence agitation (EA) of patients in anesthesia recovery period.【Methods】 896 patients underwent anesthesia in our hospital were selected. The risk factors of EA were analyzed by single factor analyzing mode and multifactor Logistic regression analysis mode.【Results】 Of 896 cases in patients underwent general anesthesia, EA occurred in 99 cases , and the EA incidence rate was 11.05%; Single factor analysis showed that sex, age, ASA classification, intraoperative fluid replacement and postoperative analgesia, time of catheter placement, hypoxia and PACU residence time were significantly correlated with EA (P<0.05); The results of Logistics regression analysis showed that age and postoperative analgesia were protective factors, and the ASA classification , the time of catheter placement and hypoxia were independent risk factors. 【Conclusion】 EA is related to many factors, among which age, postoperative analgesia, ASA grading, catheter time and hypoxia are the independent influencing factors and needs to be stressed clinically.

Protection of Quercetin through peroxidase 6 Against Liver Ischemia Reperfusion Injury

【Objective】To explore the protection mechanism of peroxidase 6 (PRDX6) against liver ischemia reperfusion injury. 【Methods】A liver ischemia reperfusion injury model was established. And normal control group, sham group, ischemia reperfusion injury group, and quercetin group were set. Liver tissue was collected from all groups and total proteins were analyzed, separated by twodimensional gel electrophoresis and identified by matrixassisted laser desorption ionization time of flight mass spectrometry, PRDX6 was verificated by immunohistochemical staining. 【Results】Twodimensional gel electrophoresis showed that there were about 825±29 protein spots in the ischemia reperfusion injury group, and 831±27 protein spots in quercetin group. There was a 44 protein spot difference between the two groups, 25 were being upregulated, and 19 being downregulated. Experimental results suggest quercetin can induce the expression of PRDX6, and the highest expression was at 12 h after reperfusion. 【Conclusion】PRDX6 may be related with protection of quercetin against liver ischemia reperfusion injury, which is beneficial for the understanding of the mechanism of protection of quercetin against liver ischemia reperfusion injury.

Multivariate Analysis on Hypomenorrhea of Women in Reproductive Years

【Objective】To analyze the influencing factors of hypomenorrhea patients in our hospital. 【Methods】Unified questionnaires were used to screen out patients with hypomenorrhea, and patients with normal mestruation who had the same physical conditions were chosen and paired with hypomenorrhea at a ratio of 1:2. A casecontrol study was conducted, and single factor and multivariable logistic regression analyses were done. 【Results】Singlefactor analysis showed that hypomenorrhea has correlation with intrauterine adhesion, stress at work, abortion (over two times), irregular work and rest patterns, and unsatisfactory marriage (P<0.01, 0.05, 0.05, 0.05, 0.01). Multivariable nonconditional logistic regression analysis showed that there was a positive correlation between hypomenorrhea with intrauterine adhesion, stress at work, abortion (over two times), and irregular work and rest patterns (P=0.006,0.207,0.09,0.126).【Conclusion】Intrauterine adhesion, work stress, abortion (over two times), work and rest irregularity, and unhappy marriage are all factors of hypomenorrhea. These factors have a certain correlation with hypomenorrhea, and will be a theoretical foundation of further large sample and multicenter epidemiological investigation.

Clinical Analysis of Pneumonectomy for 138 Patients with TuberculosisDestroyed Lungs

【Objective】To perform clinical analysis of Pneumonectomy for 138 patients with tuberculosisdestroyed lungs. 【Methods】Retrospective analysis of 138 patients and their data from June of 1985 to October of 2010. Of them, 76 had tuberculosisdestroyed left lung and 62 had tuberculosisdestroyed right lung. Operative success, negative sputum postoperation, complication of surgery, and death rates were all measured and compared in the patients. 【Results】Of the total 138 patients, all patients had successful removal of tuberculosisdestroyed lung by surgery, however, 89.86% (124/138) patients were cured, 92.75% (128/138) showed negative sputum after operation, 16.67% (23/138) had postoperation complication, and mortality rates were 3.62% (5/138). 【Conclusion】Pneumonectomy is quite successful and objectively, it can be seen that the procedure can save many lives.
