斯坦福加速智能神经调节疗法联合药物治疗首发抑郁障碍患者的疗效及安全性*

摘要 【目的】探讨斯坦福加速智能神经调节疗法(SNT)联合药物治疗首发抑郁障碍患者的疗效及安全性。【方法】选择2022年12月至2023年12月在湖南省脑科医院住院治疗的56例抑郁障碍患者,根据随机数字表法将其分为SNT联合药物治疗组(SNT组)和高频重复经颅磁刺激(rTMS)联合药物治疗组(rTMS组),每组28例。治疗1周、治疗4周,采用汉密尔顿焦虑量表(HAMA)和17项汉密尔顿抑郁量表(HAMD17)评估患者的抑郁、焦虑症状,采用自编不良事件记录表记录不良事件。【结果】治疗1周、治疗4周,两组患者HAMD17评分、HAMA评分均低于治疗前,SNT组治疗1周HAMD17评分、HAMA评分低于rTMS组,差异均有统计学意义(P>0.05)。治疗1周,rTMS组的HAMD17评分、HAMA评分有效率均为36%,SNT组均为100%,SNT组临床有效率均高于rTMS组,差异有统计学意义(P<0.05);治疗4周,rTMS组的HAMD17评分、HAMA评分有效率分别为96%、100%,SNT组均为100%,两组有效率比较,差异无统计学意义(P>0.05)。【结论】SNT和rTMS治疗首发抑郁障碍患者均安全有效,但SNT较rTMS能更快且可显著改善患者的抑郁、焦虑症状。

Abstract:【Objective】To investigate the efficacy and safety of Stanford accelerated intelligent neuromodulation therapy (SNT) combined with medication in the treatment of first-episode depressive disorder.【Methods】A total of 56 patients with depressive disorder hospitalized in the Hunan Brain Hospital from December 2022 to December 2023 were selected and divided into SNT combined with drug therapy group (SNT group) or repetitive transcranial magnetic stimulation (rTMS) combined with drug therapy group (rTMS group) according to random number table method, with 28 cases in each group. After 1 week and 4 weeks of treatment, 17-item Hamilton Depression Scale (HAMD17) and Hamilton Anxiety Scale (HAMA) were used to evaluate the depression and anxiety symptoms of the patients, and adverse events were recorded by self-designed adverse event record sheet.【Results】After 1 week of treatment and 4 weeks of treatment, HAMD17 scores and HAMA scores in two groups were lower than before treatment, and HAMD17 scores and HAMA scores in the SNT group were lower than those of the rTMS group after 1 week of treatment, with statistical significance (P>0.05). After 1 week of treatment, the effective rate of HAMD17 score and HAMA score in the rTMS group was 36%, and that in the SNT group was 100%. The clinical effective rate in the SNT group was higher than that in the rTMS group, with statistical significance (P<0.05). After 4 weeks of treatment, the effective rates of HAMD17 score and HAMA score in the rTMS group were 96% and 100%, respectively, while those in the SNT group were 100%. There was no statistical significance in the effective rates between the two groups (P>0.05).【Conclusion】SNT and rTMS are safe and effective in the treatment of first-episode depressive disorder. SNT model can significantly improve depression and anxiety symptoms faster than rTMS.
