Abstract:【Objective】To investigate the expression and clinical significance of α1-antitrypsin (α1-AT)/DNase Ⅰ(DNase Ⅰ),β-Defensins 1 (HBD-1),and ferritin (SF) in children with severe community acquired pneumonia (SCAP). 【Methods】 A total of 98 children with SCAP admitted to Kaifeng Children's Hospital from January 2020 to May 2021 were selected as the observation group. They were divided into low risk group (n=15),medium risk group (n=52),and high risk group (n=31) based on the pneumonia severity index (PSI). According to the test results,they were divided into bacterial pneumonia group (n=50),mycoplasma pneumonia group (n=22),and viral pneumonia group (n=26). At the same time,62 healthy children who underwent physical examination in our hospital were selected as the control group. The serum levels of α1-AT/DNase Ⅰ,HBD-1,and SF were compared between different groups. The Spearman method was used to analyze the correlation between α1-AT/DNase Ⅰ,HBD-1,and SF and disease severity. The diagnostic value of the above for SCAP was evaluated using the receiver operating characteristic (ROC) curve.【Results】The serum levels of α1-AT/DNase Ⅰ,HBD-1,and SF in the observation group were higher than those in the control group,and the difference was statistically significant (P<0.05). Univariate analysis of variance showed that there was a statistically significant difference in serum α1-AT/DNase Ⅰ,HBD-1,and SF levels among SCAP patients with different disease severity levels (P<0.05). The serum levels of α1-AT/DNase Ⅰ,HBD-1,and SF in the medium and high-risk groups were higher than those in the low risk group,and the high-risk group was higher than the medium risk group,with statistical significance (P<0.05). The levels of α1-AT/DNase Ⅰ,HBD-1,and SF in the mycoplasma pneumonia group and viral pneumonia group were higher than those in the bacterial pneumonia group (P<0.05). There was no statistically significant difference between the mycoplasma pneumonia group and viral pneumonia group (P>0.05). Spearman correlation analysis showed that α1-AT/DNase Ⅰ,HBD-1,and SF were positively correlated with disease severity (rs=0.632,0.524,0.531,all P<0.01). ROC curve analysis showed that the AUC area,sensitivity,and specificity of α1-AT/DNase Ⅰ,HBD-1,and SF combined detection were significantly higher than those of single detection (P<0.05). 【Conclusion】 The serum levels of α1-AT/DNase Ⅰ,HBD-1,and SF in children with SCAP are abnormally elevated and related to the severity of the disease. Combined detection can improve the diagnostic value of SCAP.
