脑动脉瘤术后并发肺部感染患者炎症因子、凝血功能指标水平变化及其与患者预后的相关性

摘要【目的】探讨脑动脉瘤术后并发肺部感染患者血清白细胞介素-6(IL-6)、基质金属蛋白酶-9(MMP-9)及凝血功能指标水平变化及其与患者预后的相关性。【方法】选取2017年1月至2020年5月本院收治的157例脑动脉瘤患者,根据患者术后是否并发肺部感染将其分为肺部感染组(n=56)与非肺部感染组(n=101)。比较两组及不同预后患者凝血功能指标及血清IL-6、MMP-9水平,采用受试者工作特征(ROC)曲线评价FIB、D-D、IL-6、MMP-9水平对脑动脉瘤术后并发肺部感染预后的预测价值。【结果】肺部感染组患者FIB、D-D、IL-6、MMP-9水平均高于非肺部感染组,差异均有统计学意义(P<0.05)。重度患者IL-6、MMP-9、FIB、D-D水平高于轻度、中度患者,差异有统计学意义(P<0.05)。预后良好患者IL-6、MMP-9、FIB、D-D水平均低于预后不良患者,差异有统计学意义(P<0.05)。ROC曲线结果显示,FIB、D-D、IL-6、MMP-9预测脑动脉瘤术后并发肺部感染预后的AUC分别为0.763、0.835、0.755、0.837,上述指标联合预测的AUC大于单一指标(P<0.05)。【结论】脑动脉瘤术后并发肺部感染患者FIB、D-D、IL-6、MMP-9水平呈高表达,与患者肺部感染病情程度及预后密切相关,对指导临床治疗具有重要意义。

Abstract：【Objective】To investigate the changes of serum interleukin-6 (IL-6), matrix metalloproteinase-9 (MMP-9) and coagulation function in patients with pulmonary infection after cerebral aneurysm surgery and their correlation with prognosis. 【Methods】A total of 157 patients with cerebral aneurysms admitted to our hospital from January 2017 to May 2020 were divided into pulmonary infection group (n=56) and non pulmonary infection group (n=101) according to whether the patients were complicated with pulmonary infection after surgery. The coagulation function indexes and serum IL-6 and MMP-9 levels were compared between the two groups and patients with different prognosis. The predictive value of FIB, D-D, IL-6 and MMP-9 levels on the prognosis of pulmonary infection after cerebral aneurysm surgery was evaluated by using the receiver operating characteristic (ROC) curve. 【Results】 The levels of FIB, D-D, IL-6, MMP-9 in patients with pulmonary infection were higher than those in patients without pulmonary infection (P<0.05). The levels of IL-6, MMP-9, FIB and D-D in severe patients were significantly higher than those in mild and moderate patients (P<0.05). The levels of IL-6, MMP-9, FIB and D-D in patients with good prognosis were lower than those in patients with poor prognosis (P<0.05). ROC curve results showed that the AUC of FIB, D-D, IL-6, MMP-9 predicting the prognosis of pulmonary infection after cerebral aneurysm surgery were 0.763, 0.835, 0.755, 0.837, respectively. The AUC predicted by the above indicators was greater than that by a single indicator (P<0.05). 【Conclusion】The high expression of FIB, D-D, IL-6, MMP-9 in patients with pulmonary infection after cerebral aneurysm surgery is closely related to the severity and prognosis of pulmonary infection, which is of great significance for guiding clinical treatment.
