Impact of Post-General Anesthesia Sore Throat Nursing Indicators on Patient Comfort Based on XGBoost Algorithm and SHAP Interpretive Analysis

【Objective】
To explore the use of machine learning eXtreme Gradient Boosting(XGBoost) algorithm combined with the SHapley Additive exPlanations(SHAP) interpretability method to analyze the impact of post-general anesthesia(GA) sore throat nursing indicators on postoperative patient comfort.
【Methods】
A retrospective analysis was conducted on the clinical data of 191 patients who underwent general anesthesia. The patients were divided into a comfortable group and an uncomfortable group according to their comfort level. An XGBoost classification prediction model was constructed, and model parameters were optimized via the grid search method. SHAP values were used to quantify the contribution of each nursing indicator to patient comfort.
【Results】
The accuracy of the model was 49%, and the Area Under the Curve(AUC) of the Receiver Operating Characteristic(ROC) curve was 0.41. SHAP analysis showed that cuff pressure monitoring(mean SHAP value: 0.22), prophylactic medication administration(mean SHAP value: 0.18), and application of laryngeal pain assessment tools(mean SHAP value: 0.15) contributed the most to predicting postoperative discomfort.
【Conclusion】
The XGBoost-based algorithm combined with SHAP can identify key nursing factors affecting postoperative patient comfort. In clinical nursing practice, priority should be given to cuff pressure management and prophylactic medication use to improve the postoperative comfort of patients with sore throat after general anesthesia.

Efficacy and Safety Analysis of Ceftazidime-Avibactam in the Treatment of Severe Pneumonia Caused by Carbapenem-Resistant Gram-Negative Bacilli Infection

【Objective】
To investigate the efficacy and safety of ceftazidime-avibactam(CAZ-AVI) in the treatment of patients with severe pneumonia caused by carbapenem-resistant organism(CRO) infection.
【Methods】
A total of 71 patients with severe pneumonia caused by CRO infection admitted to our hospital from September 2019 to October 2020 were selected. Among them, 38 cases received CAZ-AVI-based combined therapy and were included in the CAZ-AVI group, and 33 cases received anti-CRO combined therapy except CAZ-AVI and were included in the control group. The white blood cell count(WBC), C-reactive protein(CRP), procalcitonin(PCT), interleukin-6(IL-6), Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) score, Sequential Organ Failure Assessment(SOFA) score before and after anti-infective treatment, duration of antibiotic use, mechanical ventilation time, length of stay in respiratory intensive care unit(RICU), RICU survival rate, 28-day survival rate and incidence of adverse reactions were compared between the two groups.
【Results】
After treatment, the levels of WBC, CRP, PCT and IL-6 in the CAZ-AVI group were significantly lower than those in the control group(P<0.05), and the APACHE Ⅱ score and SOFA score were significantly lower than those in the control group(P<0.05). The incidence of hyperpigmentation in the CAZ-AVI group was significantly lower than that in the control group(P<0.05). The duration of anti-infective treatment and mechanical ventilation time in the CAZ-AVI group were shorter than those in the control group(P<0.05), and the RICU survival rate and 28-day survival rate were higher than those in the control group(P<0.05).
【Conclusion】
For patients with severe pneumonia caused by CRO infection, CAZ-AVI-based combined therapy is more effective and safer, with higher survival rate, which has a good clinical application prospect.

Expression and Clinical Significance of Supervillin and IL-1β Proteins in Epithelial Ovarian Cancer Tissues

【Objective】
To investigate the expression and clinical significance of Supervillin(SVIL) and interleukin-1β(IL-1β) proteins in epithelial ovarian cancer(EOC) tissues.
【Methods】
A total of ovarian epithelial tumor tissue specimens resected surgically and confirmed by pathological examination in our hospital from January 2010 to June 2022 were selected, including 40 cases of EOC tissue specimens(group A), 20 cases of ovarian epithelial borderline tumor tissue specimens(group B) and 20 cases of ovarian epithelial benign tumor tissue specimens(group C). Immunohistochemistry was used to detect the expression levels of SVIL and IL-1β proteins in each group, and their clinical significance was analyzed.
【Results】
Immunohistochemical results showed that the positive expression rates of SVIL and IL-1β proteins in group A and group B were significantly higher than those in group C, with statistically significant differences(P<0.05), while there were no statistically significant differences in the positive expression rates of SVIL and IL-1β proteins between group A and group B(P>0.05). The positive expression rates of SVIL and IL-1β proteins in EOC tissues were correlated with the degree of tissue differentiation and clinical stage(P<0.05), but not significantly correlated with age, histological type, presence of ascites and presence of lymph node metastasis(P>0.05). The expression of SVIL protein was positively correlated with that of IL-1β protein in EOC tissue specimens(r=0.373, P=0.018).
【Conclusion】
SVIL and IL-1β proteins may be involved in the occurrence and development of EOC, and there is a significant correlation between them.

Application Effect of Full-Cycle Rehabilitation Nursing Path in Patients Undergoing Surgery for Schatzker Type Ⅳ-Ⅵ Tibial Plateau Fractures

【Objective】
To explore the application effect of full-cycle rehabilitation nursing path in patients undergoing surgery for Schatzker type Ⅳ-Ⅵ tibial plateau fractures.
【Methods】
A total of 88 patients who underwent surgery for Schatzker type Ⅳ-Ⅵ tibial plateau fractures admitted to our hospital from January to December 2024 were selected and divided into an observation group(intervened with full-cycle rehabilitation nursing path) and a control group(intervened with routine nursing) using the random number table method, with 44 cases in each group. The rehabilitation status, knee joint function score, joint range of motion, pain degree, swelling score and quality of life were compared between the two groups.
【Results】
Three months after the intervention, the length of hospital stay, fracture healing time and full weight-bearing time of the observation group were shorter than those of the control group(P<0.05); the Rasmussen knee function score and Hospital for Special Surgery(HSS) knee function score of the observation group were higher than those of the control group(P<0.05); the flexion angle of the observation group was larger than that of the control group(P<0.05), and the extension limitation angle was smaller than that of the control group(P<0.05). The Visual Analogue Scale(VAS) score and swelling score of the observation group were lower than those of the control group(P<0.05). At 1 month and 3 months after the intervention, the Knee Injury and Osteoarthritis Outcome Score(KOOS) of the observation group was higher than that of the control group(P<0.05).
【Conclusion】
The full-cycle rehabilitation nursing path can effectively promote the postoperative knee joint function recovery of patients with Schatzker type Ⅳ-Ⅵ tibial plateau fractures, relieve pain and improve quality of life, which is worthy of clinical promotion and application.

Mechanistic Study on the Role of SNHG5 in Sepsis-Induced Coronary Artery Injury

【Objective】
To explore the mechanism of SNHG5, a long non-coding RNA(lncRNA) associated with inflammation, in sepsis-induced coronary artery injury.
【Methods】
Ten male C57BL/6 mice were randomly divided into a sham group(Sham) and a sepsis group(Sepsis), with five mice in each group. A sepsis mouse model was established using the cecal ligation and puncture method. The expression levels of inflammation-related lncRNAs in coronary artery tissue were detected. The target genes of SNHG5 were predicted and analyzed using the starBase v2.0 database. In in-vitro experiments, mouse aortic endothelial cells(mAECs) were cultured. And 30% serum from Sham and Sepsis groups of mice was used to stimulate the mAECs. Dual-luciferase assays were used to analyze the regulatory relationship among SNHG5, miR-377-3p, and methionine aminopeptidase 2(METAP2).
【Results】
SNHG5 and METAP2 were highly expressed in the coronary arteries of Sepsis mice, while miR-377-3p was lowly expressed(P<0.05). The expression levels of miR-377-3p were negatively correlated with those of SNHG5 and METAP2(P<0.05), while SNHG5 expression was significantly positively correlated with METAP2 expression(P<0.05). Compared to Sham mice, serum from Sepsis mice significantly upregulated SNHG5 expression in mAECs(P<0.05). Serum from Sepsis mice also significantly upregulated METAP2 expression and downregulated miR-377-3p expression(P<0.05). Knockdown of SNHG5 promoted miR-377-3p expression and inhibited METAP2 expression(P<0.05).
【Conclusion】
SNHG5 promotes apoptosis of mAECs induced by sepsis and may act as a competitive endogenous RNA regulator of miR-377-3p and METAP2, participating in sepsis-induced coronary artery injury.

Application Effect of Operating Room Intervention Guided by ERAS Concept in Patients Undergoing Laparoscopic Cholecystectomy

【Objective】
To explore the application effect of operating room intervention guided by the Enhanced Recovery After Surgery(ERAS) concept in patients undergoing laparoscopic cholecystectomy.
【Methods】
A total of 100 patients with gallstones who underwent laparoscopic cholecystectomy in our hospital from January 2022 to January 2024 were selected. The patients were divided into the observation group and the control group by random number table method, with 50 cases in each group. The observation group was given operating room intervention guided by the ERAS concept, while the control group was given routine nursing intervention. The body temperature of the two groups was compared at 30 minutes after anesthesia(T

The time of endotracheal tube extubation, stay time in PACU, first ambulation time, first anal exhaust time, resumption of eating time, and hospitalization time of the observation group were shorter than those of the control group, with statistically significant differences(P<0.05). After intervention, the SDSS, HAMA, and HAMD scores of both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences(P<0.05). After intervention, the scores of social relations, physical function, environmental dimension, psychological function and total score of both groups were higher than those before intervention, and the observation group was higher than the control group, with statistically significant differences(P<0.05).
【Conclusion】
The operating room intervention guided by the ERAS concept has a good application effect in patients undergoing laparoscopic cholecystectomy. It can effectively reduce the fluctuation range of perioperative body temperature, alleviate postoperative pain, shorten the recovery time, improve social function and psychological stress state, and enhance the quality of life, which is worthy of promotion.

Impact of Proactive Risk Prevention Nursing Procedure on Psychological Stress and Risk Events in Patients Undergoing Digestive Endoscopy

【Objective】
To explore the impact of proactive risk prevention nursing procedure on psychological stress and risk events in patients undergoing digestive endoscopy.
【Methods】
A total of 98 patients who underwent digestive endoscopy in our hospital from September 2022 to August 2024 were selected and divided into an observation group and a control group using the random number table method, with 49 cases in each group. The control group was given routine nursing intervention, while the observation group was given proactive risk prevention nursing procedure intervention. The heart rate(HR), blood pressure fluctuations and psychological status of the two groups before and after the examination were compared, and the incidence of risk events in the two groups was counted.
【Results】
Before the examination, there were no statistically significant differences in HR and mean arterial pressure(MAP) between the two groups(P>0.05). At 30 minutes during the examination and after the examination, the HR and MAP of both groups were higher than those before the examination, while those of the observation group were lower than those of the control group(P<0.05). After the examination, the HR and MAP of both groups were higher than those at 30 minutes during the examination, while those of the observation group were lower than those of the control group(P<0.05). After the examination, the scores of hostility, phobia, anxiety and depression of both groups were lower than those before the examination, and the scores of the observation group were significantly lower than those of the control group, with statistically significant differences(P<0.05). The incidence of risk events in the observation group was 4.08%(2/49), which was significantly lower than 18.37%(9/49) in the control group, with a statistically significant difference(P<0.05).
【Conclusion】
Proactive risk prevention nursing procedure helps stabilize the HR and blood pressure of patients undergoing digestive endoscopy, alleviate their psychological stress responses, and reduce the incidence of risk events.

Application Effect of Systematic Functional Exercise Combined with Enhanced Social Support System Intervention in Patients with Avascular Necrosis of the Femoral Head Complicated with AIDS after Surgery

【Objective】
To explore the application effect of systematic functional exercise combined with enhanced social support system intervention in patients with avascular necrosis of the femoral head complicated with AIDS after surgery.
【Methods】
A total of 80 patients with avascular necrosis of the femoral head complicated with AIDS after surgery admitted to our hospital from January 2021 to March 2025 were selected and divided into the observation group and the control group by lottery method, with 40 cases in each group. The control group received routine intervention, while the observation group received systematic functional exercise combined with enhanced social support system intervention. The joint injury status(osteoarthritis index), psychological stress status [Stress Response Questionnaire(SRQ) scores], incidence of postoperative complications, and hip joint function [Harris Hip Score(HHS)] were compared between the two groups.
【Results】
After 14 days of intervention, the osteoarthritis index of the observation group was lower than that of the control group(P<0.05); all SRQ scores of the observation group were lower than those of the control group(P<0.05). There was no statistically significant difference in the total incidence of postoperative complications between the two groups(P>0.05). After 3 months of intervention, all HHS scores of the observation group were higher than those of the control group(P<0.05).
【Conclusion】
Systematic functional exercise combined with enhanced social support system intervention can effectively improve the joint injury status and hip joint function of patients with avascular necrosis of the femoral head complicated with AIDS after surgery, reduce psychological stress responses, decrease the incidence of postoperative complications, and promote postoperative rehabilitation of patients.

【Methods】
Eighty patients with suspected complex anal fistula admitted to Huai'an Hospital of Traditional Chinese Medicine from May 2020 to May 2025 were included. All patients underwent magnetic resonance DWI and T

WI fat suppression sequence examinations. Using surgical pathology results as the gold standard, we observed the visualization of the internal opening and fistula tract of anal fistula using different examination sequences. The sensitivity, specificity, accuracy, positive predictive value and negative predictive value of DWI alone, T

WI fat suppression sequence alone, and their combination in diagnosing complex anal fistula were calculated and analyzed.
【Results】
Eighty patients suspected of having complex anal fistulas were confirmed by surgical pathology to have 85 internal fistula openings and 102 fistula tracts. The combined diagnostic method(both T

WI fat-suppressed sequence and DWI sequence) showed a significantly higher detection rate of internal fistula openings and fistula tracts compared to DWI and T

Effect of Auricular Scraping with Copper Bian-Stone Combined with Auricular Point Pressing on Blood Pressure, Sleep Quality and Quality of Life in Elderly Patients with Hypertension

【Objective】
To explore the effect of auricular scraping with copper bian-stone combined with auricular point pressing on blood pressure, sleep quality and quality of life in elderly patients with hypertension.
【Methods】
A total of 130 elderly patients with hypertension admitted to our hospital from March 2022 to April 2025 were selected and divided into the control group and the observation group by random number table method, with 65 cases in each group. The control group was treated with routine antihypertensive drugs, while the observation group was given the intervention of auricular scraping with copper bian-stone combined with auricular point pressing on the basis of the control group. The clinical efficacy, blood pressure levels, sleep quality [Pittsburgh Sleep Quality Index(PSQI) scores] and quality of life [36-Item Short Form Health Survey(SF-36) scores] of the two groups were compared before and after treatment.
【Results】
After treatment, the systolic blood pressure(SBP) and diastolic blood pressure(DBP) of both groups were lower than those before treatment, and those of the observation group were lower than those of the control group, with statistically significant differences(P<0.05). After treatment, the total effective rate of the observation group was 92.31%(60/65), which was higher than 64.62%(42/65) of the control group, with a statistically significant difference(χ
2
=14.748, P<0.05). After treatment, the PSQI scores of both groups were lower than those before treatment, and the score of the observation group was lower than that of the control group, with a statistically significant difference(P<0.05). After treatment, the scores of all dimensions of SF-36 in both groups were higher than those before treatment, and the scores of the observation group were higher than those of the control group, with statistically significant differences(P<0.05).
【Conclusion】
Auricular scraping with copper bian-stone combined with auricular point pressing has a good auxiliary effect on lowering blood pressure in elderly patients with hypertension, can improve their sleep quality and enhance their quality of life.

Impact of Clinical Prognosis-Oriented Home Transitional Care Program on Cardiac Function and Self-Management Ability in Patients after PCI

【Objective】
To explore the impact of a clinical prognosis-oriented home transitional care program on cardiac function and self-management ability in patients after percutaneous coronary intervention(PCI).
【Methods】
A total of 881 patients who underwent emergency PCI in our hospital from January 2022 to April 2024 were selected. The hospital launched the clinical prognosis-oriented nursing intervention in March 2023. According to the admission time, the patients were divided into a control group(January 2022 to February 2023, n=417) and an observation group(March 2023 to April 2024, n=464). The control group received routine postoperative nursing after PCI, while the observation group received the clinical prognosis-oriented home transitional care program. The postoperative cardiac function [6-minute walking distance(6MWD), left ventricular ejection fraction(LVEF), left ventricular end-diastolic diameter(LVEDD)], self-management ability, and the incidence of major adverse cardiovascular events(MACE) were compared between the two groups.
【Results】
At 3 months after discharge, the 6MWD and LVEF of both groups were higher than those on the 1st day before discharge, and the LVEDD was lower than that on the 1st day before discharge. Moreover, the 6MWD and LVEF of the observation group were higher than those of the control group, and the LVEDD was lower than that of the control group, with statistically significant differences(P<0.05). At 3 months after discharge, the scores of daily life management, disease knowledge management, treatment compliance, emotional management, first-aid skills and the total score of both groups were higher than those on the 1st day before discharge, and the scores of the observation group were higher than those of the control group, with statistically significant differences(P<0.05). The incidence rates of postoperative revascularization, ventricular arrhythmia, post-infarction angina pectoris and in-stent restenosis in the observation group were lower than those in the control group, with statistically significant differences(P<0.05).
【Conclusion】
The clinical prognosis-oriented home transitional care program can significantly improve postoperative cardiac function, enhance self-management ability, and reduce the risk of MACE after PCI.

The Application Value of Standardized Operating Room Nursing Path in Laparoscopic Bariatric Double-Tract Surgery

【Objective】
To explore the application effect of standardized operating room nursing path in laparoscopic bariatric double-tract surgery.
【Methods】
A total of 90 obese patients who underwent laparoscopic bariatric double-tract surgery in our hospital from January 2020 to January 2025 were selected and divided into a control group and an observation group by random number table method, with 45 cases in each group. The control group was given the traditional nursing path, while the observation group was applied the standardized operating room nursing path. The operation time, exhaust time, incidence of intraoperative complications, operation success rate, nursing satisfaction and nursing quality were compared between the two groups.
【Results】
The operation time and exhaust time of the observation group were shorter than those of the control group, the incidence of intraoperative complications was lower than that of the control group, and the operation success rate was higher than that of the control group, with statistically significant differences(P<0.05). The scores of psychological nursing, preoperative preparation and postoperative preparation in the observation group were higher than those in the control group, with statistically significant differences(P<0.05). The scores of basic nursing, education and communication, ward management and nursing safety in the observation group were higher than those in the control group, with statistically significant differences(P<0.05).
【Conclusion】
The standardized operating room nursing path can effectively improve the nursing quality of laparoscopic bariatric double-tract surgery, reduce the incidence of intraoperative complications, and enhance patients' nursing satisfaction.

【Objective】
To investigate the progression of acute pain after single-aperture thoracoscopic surgery(VATS) in early non-small cell lung cancer(NSCLC), and to analyze the factors influencing the progression of acute pain.
【Methods】
A total of 171 early-stage NSCLC patients admitted to our hospital from July 2021 to November 2023 were selected, all of whom underwent uniportal VATS. Postoperative pain was assessed using the Numerical Rating Scale(NRS) within 1 to 7 days after surgery. The Group-Based Trajectory Model(GBTM) was employed to describe the development trajectory of acute postoperative pain in early-stage NSCLC patients undergoing uniportal VATS, categorized into Group A(n=61, mild to moderate pain significantly alleviated), Group B(n=81, moderate to severe pain gradually alleviated), and Group C(n=29, persistent high pain). A multifactorial logistic regression model was used to analyze the influencing factors of postoperative acute pain trajectory types.
【Results】
Over time, NRS scores gradually decreased in Groups A, B, and C(P<0.05), with Group C showing significantly higher NRS scores than Groups A and B(P<0.05). Statistically significant differences were observed among Groups A, B, and C in terms of operative time, body mass index, age, incision rib gap width, patient-controlled intravenous analgesia(PCIA) postoperatively, drainage tube retention time, the proportion of patients with ≥2 drainage tubes, preoperative Hospital Depression Scale(HDS) scores, preoperative Hospital Anxiety Scale(HAS) scores, and preoperative Pain Catastrophizing Scale(PCS) scores(P<0.05). Group C exhibited higher operative time, body mass index, age, postoperative PCIA proportion, drainage tube retention time, proportion of patients with ≥2 drainage tubes, preoperative HDS scores, preoperative HAS scores, and preoperative PCS scores compared to Groups A and B(P<0.05). Multivariate logistic regression analysis identified incision rib gap width, drainage tube retention time, preoperative HDS scores, operative time, and postoperative analgesic regimen as influencing factors of acute postoperative pain trajectory types in early-stage NSCLC patients undergoing uniportal VATS(P<0.05).
【Conclusion】
Patients with early NSCLC present heterogeneous acute pain development trajectory after single-aperture thoracoscopic surgery. Clinical attention should be paid to patients with narrow incisional costal space width, longer retention time of drainage tube, higher preoperative PCS score, longer operation time, and postoperative intravenous self-controlled pump, whose appropriate intervention measures should be adopted.

Diagnostic and Prognostic Predictive Value of CT Perfusion Imaging Combined with MRA in Ischemic Stroke

【Objective】
To explore the clinical diagnostic efficacy and prognostic predictive value of computed tomography(CT) perfusion imaging combined with magnetic resonance angiography(MRA) in patients with cerebral ischemic stroke(CIS).
【Methods】
The clinical data of 92 patients suspected of CIS admitted to our hospital from March 2023 to March 2025 were retrospectively analyzed. According to the results of digital subtraction angiography(DSA), the patients were divided into the CIS group(n=58) and the non-CIS group(n=34). The patients in the CIS group were further divided into the good prognosis group(n=21) and the poor prognosis group(n=37) based on the modified Rankin Scale(mRS) scores at discharge. The CT perfusion imaging parameters [cerebral blood flow(CBF), cerebral blood volume(CBV), mean transit time(MTT), time to peak(TTP)] and MRA findings [maximum intensity projection(MIP), multi-planar reconstruction(MPR)] of the above patients were compared. The receiver operating characteristic(ROC) curve was used to analyze the diagnostic and prognostic predictive value of CT perfusion imaging combined with MRA in patients with CIS.
【Results】
The diagnostic accuracy, sensitivity, and specificity of MRA for CIS were 93.48%, 93.10%, and 94.12%, respectively; the misdiagnosis rate was 5.88%, the missed diagnosis rate was 6.90%, the positive predictive value was 0.964, the negative predictive value was 0.889, and the Kappa value was 0.862. The levels of CBF and CBV in the CIS group were lower than those in the non-CIS group, while MTT and TTP were longer than those in the non-CIS group, with statistically significant differences(P<0.05). ROC curve analysis showed that the combined diagnosis had the highest value for CIS, with an area under the curve(AUC) [95% confidence interval(

)] of 0.925(0.874-0.977), a sensitivity of 84.5%, and a specificity of 94.1%. Meanwhile, the combined assessment also presented the highest value for predicting the prognosis of CIS, with an AUC(95%
CI
) of 0.987(0.967-1.000), a sensitivity of 97.3%, and a specificity of 90.5%.
【Conclusion】
CT perfusion imaging combined with MRA can significantly improve the diagnostic accuracy and prognostic predictive efficacy of ischemic stroke, which is worthy of clinical application and promotion.

Impact of Arousal Interventions with Different Frequencies on Postoperative Awakening Outcomes in Patients Undergoing Hepatic Tumor Resection under General Anesthesia

【Objective】
To explore the impact of arousal interventions with different frequencies on the postoperative awakening outcomes of patients who undergo hepatic tumor resection under general anesthesia(GA).
【Methods】
One hundred and fifty patients undergoing hepatic tumor resection under GA were randomly assigned to a control group and an observation group, with 75 cases in each group. The control group received arousal intervention at a frequency of once every 10 minutes, whereas the observation group received the intervention once every 5 minutes. The changes in surgical and related indicators, as well as the patients' satisfaction with the intervention were compared between the two groups.
【Results】
The tracheal extubation time, awakening time after extubation and total awakening time of the observation group were all shorter than those of the control group(P<0.05). No statistically significant differences were found between the two groups in the incidence rates of post-extubation hypoxemia and mean arterial pressure(MAP) fluctuation(P>0.05). The occurrence rates of coughing and restlessness also showed no notable differences between the two groups(P>0.05). The satisfaction rate with intervention in the observation group reached 94.67%, which was higher than the 80.00% of the control group(P<0.05).
【Conclusion】
A high-frequency arousal intervention(once every 5 minutes) can effectively optimize the postoperative awakening process and improve nursing satisfaction in patients undergoing hepatic tumor resection under GA.

Application Effect of Transitional Care Based on Triangle Hierarchical Management Model for Chronic Diseases in Patients with Diabetic Foot

【Objective】
To explore the application effect of transitional care based on the Triangle hierarchical management model for chronic diseases in patients with diabetic foot.
【Methods】
A total of 98 patients with diabetic foot admitted to our hospital from January 2022 to February 2024 were selected and divided into an observation group(transitional care based on the Triangle hierarchical management model for chronic diseases) and a control group(routine care) using the random number table method, with 49 cases in each group. The scores of the Diabetes Foot Self-Care Questionnaire-University of Malaga(DFSQ-UMA), Summary of Diabetes Self-Care Activities(SDSCA), Diabetes Specific Quality of Life Scale(DSQL), and the incidence of diabetic foot ulcers were compared between the two groups before intervention and 3 months after intervention.
【Results】
Three months after intervention, the scores of each dimension of DFSQ-UMA and SDSCA in the observation group were significantly higher than those in the control group(P<0.05), while the scores of each dimension of DSQL and the incidence of foot ulcers were significantly lower than those in the control group(P<0.05).
【Conclusion】
Transitional care based on the Triangle hierarchical management model for chronic diseases can significantly improve the self-care ability, self-management behavior and quality of life of patients with diabetic foot, and reduce the incidence of diabetic foot ulcers, thus effectively improving the therapeutic effect and quality of life of patients with diabetic foot.

Expression Changes and Clinical Significance of CPEB4 in Clear Cell Renal Cell Carcinoma Tissues

【Objective】
To investigate the expression of cytoplasmic polyadenylation element binding protein 4(CPEB4) in clear cell renal cell carcinoma(CCRCC) tissues and its clinical significance.
【Methods】
Cancer tissues and adjacent normal tissues were collected from 80 patients with CCRCC. The mRNA and protein expression levels of CPEB4 were detected. The relationships between CPEB4 expression and clinicopathological characteristics as well as prognosis were analyzed.
【Results】
The relative mRNA expression level of CPEB4 in tumor tissues(1.883 ± 0.053) was significantly higher than that in adjacent normal tissues(0.809 ± 0.032)(P<0.05). The positive rate of CPEB4 protein expression in tumor tissues was 63.8%(51/80), which was higher than that in normal tissues(31.3%, 25/80)(P<0.05). CPEB4 expression was associated with TNM stage, Fuhrman grade, and distant metastasis. Patients with negative CPEB4 expression had better survival than those with positive expression(P<0.05).
【Conclusion】
CPEB4 is highly expressed in CCRCC tissues and is closely associated with clinicopathological features and prognosis.

Expression and Clinical Significance of FSCN1 and hTERT in Ovarian Cancer

【Objective】
To investigate the expression of fascin-1(FSCN1) and human telomerase reverse transcriptase(hTERT) in ovarian cancer and their clinical significance.
【Methods】
A total of 44 patients with ovarian cancer were included as the observation group, and 44 patients with benign epithelial ovarian tumors during the same period were selected as the control group. The expression levels of FSCN1 and hTERT proteins in ovarian lesion tissues were compared between the two groups. The correlations between FSCN1 and hTERT expression and clinicopathological characteristics were analyzed in the observation group.
【Results】
The high expression rates of FSCN1 and hTERT in the observation group were 70.45% and 72.73%, respectively, which were significantly higher than those in the control group(36.36% and 31.82%, respectively)(P<0.05). In the observation group, FSCN1 and hTERT expression were not associated with age, tumor size, pathological type, or lymph node metastasis(P>0.05), but were related to FIGO stage(P<0.05). Higher FIGO stages were associated with higher expression levels of FSCN1 and hTERT.
【Conclusion】
FSCN1 and hTERT are highly expressed in ovarian cancer, and their expression levels are associated with FIGO stage.

Correlation Analysis of Serum CEA, CA19-9, and β2-MG Levels with Prognosis in Bladder Cancer Patients

【Objective】
To explore the correlation between prognosis and levels of serum carcinoembryonic antigen(CEA), carbohydrate antigen 19-9(CA19-9), and β2-microglobulin(β2-MG) in bladder cancer patients.
【Methods】
The serum levels of CEA, CA19-9, and β2-MG were measured in 106 bladder cancer patients(the observation group) and 85 healthy individuals(the control group). The relationship between these markers and prognosis as well as the risk factors for death in bladder cancer patients were analyzed.
【Results】
The levels of CEA, CA19-9, and β2-MG in the observation group were higher than those in the control group. Furthermore, the preoperative levels of these markers in the observation group were higher than their postoperative levels(all P<0.05). During a 36-month follow-up, 63 of the 106 bladder cancer patients survived(the survival group), while 43 died(the death group). The death group had a higher proportion of patients in TNM stages Ⅲ-Ⅳ and poorly differentiated tumors, as well as higher expression levels of CEA, CA19-9, and β2-MG compared to the survival group(P<0.05). Logistic multivariate regression analysis revealed that TNM stages Ⅲ-Ⅳ, poorly differentiated tumors, and high expression of CEA, CA19-9, and β2-MG were significant risk factors for death in bladder cancer patients(P<0.05). The 3-year survival rate in the high expression groups of CEA, CA19-9, and β2-MG was significantly lower than that in the low expression groups(P<0.05).
【Conclusion】
Serum levels of CEA, CA19-9, and β2-MG are closely related to the prognosis of bladder cancer patients. These markers can serve as independent prognostic indicators for bladder cancer.

Efficacy of Elemene Combined with Lobaplatin in the Treatment of Malignant Pleural Effusion of Lung Cancer Patients

【Objective】
To investigate the clinical efficacy of elemene combined with lobaplatin in patients with malignant pleural effusion caused by lung cancer.
【Methods】
A total of 133 patients with lung cancer–related malignant pleural effusion were randomly divided into an observation group(n=66) and a control group(n=67). The control group received lobaplatin in addition to conventional treatment, while the observation group received lobaplatin combined with elemene. Clinical efficacy, as well as levels of cytokeratin 19 fragment antigen 21-1(CYFRA21-1), neuron-specific enolase(NSE), T-lymphocyte subsets, and inflammatory factors before and after treatment were compared between the two groups.
【Results】
The disease control rate in the observation group was 80.30%, which was significantly higher than 47.76% in the control group(P<0.05). After treatment, CYFRA21-1, NSE, CD8

Impact of Cognitive Behavioral Intervention on Psychological State and Sleep Quality of Puerperae Undergoing Medically Indicated Induction of Labor

【Objective】
To explore the impact of cognitive behavioral intervention on the psychological state and sleep quality of puerperae undergoing medically indicated induction of labor.
【Methods】
A total of 100 puerperae undergoing medically indicated induction of labor admitted to our hospital from January 2021 to October 2023 were selected and divided into a control group(receiving routine intervention) and an observation group(receiving cognitive behavioral intervention) using the random number table method, with 50 cases in each group. The anxiety level [Generalized Anxiety Disorder-7(GAD-7) score], depression level [Edinburgh Postnatal Depression Scale(EPDS) score], sleep quality [Pittsburgh Sleep Quality Index(PSQI) score], post-traumatic growth level [Chinese version of Post-traumatic Growth Inventory(C-PTGI) score] and maternal satisfaction were compared between the two groups before and after the intervention.
【Results】
After the intervention, the GAD-7 score and EPDS score of the observation group were lower than those of the control group(P<0.05), and the PSQI score of the observation group was higher than that of the control group(P<0.05). The scores of all dimensions and the total score of C-PTGI in the observation group were higher than those in the control group(P<0.05). The maternal satisfaction of the observation group was higher than that of the control group(P<0.05).
【Conclusion】
Cognitive behavioral intervention can improve the psychological state and sleep quality of puerperae undergoing medically indicated induction of labor, and enhance their post-traumatic growth level, which is worthy of clinical promotion and application.

Application Effect of Endotracheal Intubation Sequential Nasal High-Flow Oxygen Therapy in ICU Patients after Weaning from Mechanical Ventilation and Extubation

【Objective】
To explore the application effect of endotracheal intubation sequential nasal high-flow oxygen therapy in patients in the intensive care unit(ICU) after weaning from mechanical ventilation and extubation.
【Methods】
A total of 100 ICU patients receiving mechanical ventilation who were treated in our hospital from January 2023 to January 2025 were selected as the research subjects. They were divided into a control group and an observation group by the random number table method, with 50 cases in each group. Both groups received extubation after weaning from mechanical ventilation in accordance with unified standards. After extubation, the observation group was given endotracheal intubation sequential nasal high-flow oxygen therapy, while the control group was given oxygen inhalation via face mask at a flow rate of 6 L/min. The blood oxygen indicators [oxygen saturation(SaO
2
), partial pressure of oxygen in artery(PaO
2
), oxygenation index(OI)], reintubation rate within 48 hours after extubation, and ICU length of stay were compared between the two groups.
【Results】
Forty-eight hours after the intervention, the SaO
2
, PaO

and OI values of both groups were higher than those before the intervention, and the indexes of the observation group were higher than those of the control group(P<0.05). There were no statistically significant differences in the reintubation rate within 48 hours after extubation and ICU length of stay between the two groups(P>0.05).
【Conclusion】
Endotracheal intubation sequential nasal high-flow oxygen therapy has a significant application effect in ICU patients after weaning from mechanical ventilation and extubation, which can significantly improve the blood oxygen indicators of patients with high safety.

Predictive Value of Ultrasonographic Indicators for Postoperative Complications after AVF Fomation in Patients with Chronic Renal Failure

【Objective】
To investigate the predictive value of ultrasonographic indicators for postoperative complications following cephalic vein-radial artery arteriovenous fistula(AVF) formation in patients with chronic renal failure(CRF).
【Methods】
A total of 126 CRF patients who underwent AVF creation were enrolled. All patients received postoperative ultrasound examination on day 1 to assess parameters including cephalic vein diameter(CVD), radial artery diameter(RAD), and brachial artery flow volume(FV). Patients were divided into a complication group and a non-complication group based on postoperative outcomes. General clinical data and ultrasound parameters were compared between the two groups. Receiver operating characteristic(ROC) curves were used to analyze the diagnostic performance of each ultrasound parameter in predicting postoperative complications.
【Results】
Patients in the occurrence group had lower CVD, RAD, and FV, and a higher prevalence of diabetes mellitus than those in the non-occurrence group(P<0.05). Multivariate analysis revealed that decreased CVD, reduced RAD, and diminished FV were independent risk factors(P<0.05). ROC analysis indicated that the combined detection of CVD, RAD, and FV had significantly better diagnostic performance for postoperative complications than any single parameter alone(P<0.05).
【Conclusion】
Ultrasonographic indicators CVD, RAD, and FV can serve as effective predictors of postoperative complications after AVF creation in CRF patients, and combined assessment of the three parameters markedly enhances predictive accuracy.

Application of Dexmedetomidine Pretreatment Combined with Low-Dose Propofol in Aortic Dissection Interventional Surgery

【Objective】
To investigate the clinical effects of dexmedetomidine(Dex) pretreatment combined with low-dose propofol in patients undergoing aortic dissection(AD) interventional surgery.
【Methods】
Eighty-four patients scheduled for AD interventional surgery were randomly assigned to an observation group or a control group, with 42 cases in each group. The control group received conventional full-dose propofol induction and maintenance, while the observation group received Dex pretreatment before anesthesia induction combined with low-dose propofol induction. Hemodynamic parameters[mean arterial pressure(MAP) and heart rate(HR)]were compared between the two groups before induction, at 1 minute after intubation, 30 minutes after the start of surgery, and at the end of surgery. Additional comparisons included operation time, propofol dosage, awakening time, mechanical ventilation duration, sedation and analgesia outcomes before surgery and 2 hours postoperatively [Ramsay sedation score and visual analogue scale(VAS)], as well as safety indicators.
【Results】
At 1 minute after intubation, 30 minutes after surgery initiation, and at the end of surgery, the observation group had significantly higher MAP and lower HR compared with the control group(P<0.05). There was no significant difference in operation time between the two groups(P>0.05). The observation group required less propofol and had shorter awakening and mechanical ventilation times than the control group(P<0.05). Two hours postoperatively, the observation group had higher Ramsay scores and lower VAS scores(P<0.05) compared to the control group. The incidence of adverse events showed no statistically significant difference between the two groups(P>0.05).
【Conclusion】
Dex pretreatment combined with low-dose propofol can effectively maintain hemodynamic stability in patients undergoing AD interventional surgery, reduce propofol requirements, promote rapid postoperative awakening, shorten mechanical ventilation time, improve postoperative sedation and analgesia quality, and demonstrates good safety.

Effects of Atorvastatin Combined with Sacubitril/Valsartan Combination on Blood Pressure and Heart Function in Stable Coronary Heart Disease Patients Post-Percutaneous Coronary Intervention

【Objective】
To explore the effects of atorvastatin calcium combined with sacubitril valsartan sodium on postoperative blood pressure and cardiac function in patients with stable coronary heart disease undergoing percutaneous coronary intervention(PCI).
【Methods】
A retrospective analysis was conducted on the clinical data of 82 stable coronary heart disease patients admitted to our hospital from October 2021 to October 2024. According to different treatment plans, the patients were divided into an observation group and a control group, with 41 cases in each group. Both groups of patients underwent PCI surgery. The control group received atorvastatin calcium after surgery, while the observation group received a combination of sacubitril and valsartan sodium in addition to the control group. The blood pressure [systolic blood pressure(SBP), diastolic blood pressure(DBP)], blood lipid levels[high-density lipoprotein cholesterol(HDL-C), total cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), triglycerides(TG)], cardiac function [left ventricular end diastolic diameter(LVEDD), left ventricular end systolic diameter(LVESD), left ventricular ejection fraction(LVEF)], and plaque indicators [carotid intima-media thickness(IMT), plaque thickness, plaque area] were compared between two groups of patients before and after treatment.
【Results】
After treatment, DBP and SBP in both groups of patients were lower than before treatment; And both DBP and SBP in the observation group were lower than those in the control group, with a statistically significant difference(P<0.05). The levels of serum TC, TG, and LDL-C in both groups were lower than before treatment, while HDL-C was higher than before treatment. The observation group had lower levels of serum TC, TG, and LDL-C than the control group, while HDL-C was higher than the control group, with statistical significance(P<0.05). The LVEDD and LVESD of the two groups were lower than before treatment, and the LVEF was higher than before treatment. The LVEDD and LVESD of the observation group were lower than those of the control group, and the LVEF was higher than that of the control group, with statistical significance(P<0.05). The IMT, plaque thickness, and plaque area of the two groups were lower than before treatment, and the values in the observation group were lower than those in the control group, with statistically significant differences(P<0.05).
【Conclusion】
Taking atorvastatin calcium and sacubitril valsartan sodium after PCI can effectively improve patients' cardiac function and blood pressure, reduce blood lipid levels, inhibit plaque accumulation, and have good application effects.

【Objective】
To explore the impact of enhanced recovery after surgery(ERAS) nursing combined with feeding detail guidance on swallowing function in patients after oral cancer surgery.
【Methods】
A total of 84 patients who underwent oral cancer surgery in our hospital from January 2021 to January 2023 were selected and divided into a control group and an observation group using the random number table method, with 42 cases in each group. The control group was given routine clinical nursing, while the observation group was given enhanced recovery after surgery nursing combined with feeding detail guidance. The clinical nursing effects of the two groups were compared.
【Results】
After nursing intervention, the pharyngeal constriction rates of the observation group for thin liquid diet, thick liquid diet and pureed diet were significantly lower than those of the control group. Meanwhile, the levels of nutritional status indicators and quality of life scores of the observation group were significantly higher than those of the control group, with statistically significant differences(P<0.05).
【Conclusion】
Enhanced recovery after surgery nursing combined with feeding detail guidance can significantly improve the swallowing function of patients after oral cancer surgery, thereby ensuring that patients take in sufficient nutrients and ultimately achieving the goal of improving their quality of life.

Efficacy and Safety Comparison of Sublingual and Subcutaneous Specific Immunotherapy in Children with Allergic Rhinitis

【Objective】
To compare the efficacy and safety of sublingual specific immunotherapy(SLIT) and subcutaneous specific immunotherapy(SCIT) in children with allergic rhinitis.
【Methods】
A total of 102 children with dust mite-induced allergic rhinitis admitted to our hospital from January 2022 to May 2024 were selected as research subjects, and divided into an observation group(treated with SLIT) and a control group(treated with SCIT) using the random number table method, with 51 cases in each group. The total nasal symptom score(TNSS) and visual analogue scale(VAS) score of the two groups before treatment and 1 year after treatment were compared, as well as the total medication score(TMS), total treatment cost, incidence of adverse reactions and treatment dropout rate of the two groups after 1 year of treatment.
【Results】
One year after treatment, the TNSS and VAS scores of both groups were lower than those before treatment, with statistically significant differences(P<0.05), but there were no statistically significant differences in TNSS and VAS scores between the two groups(P>0.05). One year after treatment, the TMS of the observation group was higher than that of the control group, while the total treatment cost, total incidence of adverse reactions and total treatment dropout rate of the observation group were lower than those of the control group, with statistically significant differences(P<0.05).
【Conclusion】
SLIT and SCIT have comparable efficacy in improving the symptoms of allergic rhinitis in children, but SLIT has higher safety, better compliance and lower cost, making it more suitable as the preferred clinical desensitization regimen. It provides practical evidence for standardized desensitization treatment of allergic rhinitis in children.

Influencing Factors of Quality of Life and its Correlation with Social Support in Patients with Chronic Heart Failure

【Objective】
To explore the related factors affecting the quality of life of patients with chronic heart failure and their correlation with social support.
【Methods】
A total of 104 patients with chronic heart failure admitted to our hospital from June 2020 to June 2024 were selected. The Minnesota Living with Heart Failure Questionnaire(MLHFQ) was used to evaluate the quality of life of the patients, and the Social Support Rating Scale(SSRS) was used to evaluate the social support of the patients. Pearson correlation analysis was used to analyze the correlation between social support score and quality of life. Univariate analysis and multiple linear regression analysis were used to analyze the influencing factors of quality of life in patients with chronic heart failure.
【Results】
There were statistically significant differences in MLHFQ scores of patients with chronic heart failure with different genders, ages, cardiac function classifications, number of comorbidities, activities of daily living, educational backgrounds, monthly household incomes, exercise training status and marital status(P<0.05). The scores of subjective support, objective support and social support utilization of patients with chronic heart failure were(8.11±1.12) points,(11.47±2.08) points and(6.05±1.27) points, respectively. Pearson correlation analysis showed that the scores of subjective support, objective support and social support utilization of patients with chronic heart failure were negatively correlated with MLHFQ(r=-0.591, -0.759, -0.678, all P<0.05). Multiple linear regression analysis showed that age ≥ 60 years old, cardiac function classification grade Ⅲ~Ⅳ, number of comorbidities ≥ 4 types, abnormal activities of daily living, educational background ≤ junior high school, monthly household income < 3000 yuan per month, subjective support, objective support and social support utilization were independent influencing factors of quality of life in patients with chronic heart failure(P<0.05).
【Conclusion】
Age, cardiac function classification, number of comorbidities, abnormal activities of daily living, educational background, monthly household income per capita, subjective support, objective support and social support utilization are the related factors affecting the quality of life of patients with chronic heart failure. Clinical medical staff can formulate relevant social support programs according to the specific conditions of patients with chronic heart failure, which is of great significance for improving the quality of life of patients with chronic heart failure.

Impact of Hierarchical Management Based on CICARE Communication Model on Psychological State and Quality of Life in Diabetic Patients

【Objective】
To explore the impact of hierarchical management based on the CICARE communication model on the psychological state and quality of life in diabetic patients.
【Methods】
A total of 117 diabetic patients diagnosed and treated in our hospital from January to December 2024 were selected and divided into a control group(receiving routine nursing care, n=58) and an observation group(receiving hierarchical management based on the CICARE communication model, n=59) using the random number table method. The psychological state [Symptom Checklist 90(SCL-90) score], disease cognitive level, self-management behavior [Summary of Diabetes Self-Care Activities(SDSCA) score] and quality of life [Diabetes Specific Quality of Life Scale(DSQL) score] of the two groups were compared before nursing intervention and 3 months after nursing intervention.
【Results】
Three months after nursing intervention, the scores of all dimensions of SCL-90 and DSQL in both groups were lower than those before intervention, and the scores in the observation group were lower than those in the control group. The disease cognitive level and SDSCA scores in both groups were higher than those before intervention, and the indexes in the observation group were higher than those in the control group. All the differences were statistically significant(P<0.05).
【Conclusion】
Hierarchical management based on the CICARE communication model can effectively improve the psychological state and quality of life of diabetic patients, enhance their disease cognitive level and self-management ability, which is worthy of clinical promotion and application.

Clinical Efficacy of Xanthium Rhinitis Dripping Pills Combined with Cefuroxime in the Treatment of Patients with Acute Sinusitis

【Objective】
To explore the clinical efficacy of Xanthium Rhinitis Dripping Pills combined with cefuroxime in the treatment of patients with acute sinusitis.
【Methods】
A total of 101 patients with acute sinusitis admitted to our hospital from March 2023 to March 2025 were selected and divided into a control group(n=51, treated with cefuroxime plus placebo with identical appearance) and an observation group(n=50, treated with cefuroxime plus Xanthium Rhinitis Dripping Pills) using a computer-generated random sequence(completed by an independent statistician). During the study period, 3 cases in the control group(2 lost to follow-up, 1 withdrew voluntarily) and 2 cases in the observation group(lost to follow-up) dropped out, and finally 96 cases completed all follow-up visits(48 cases in each group). The clinical efficacy, inflammatory indicators, nasal endoscopy scores(Lund-Kennedy scores), clinical symptoms and incidence of adverse reactions were compared between the two groups.
【Results】
The total effective rate of the observation group was higher than that of the control group(P<0.05). After treatment, the serum levels of hypersensitive C-reactive protein(hs-CRP) and procalcitonin(PCT) in the observation group were lower than those in the control group(P<0.05), and the Lund-Kennedy scores and the scores of all dimensions of the Sinonasal Outcome Test-20(SNOT-20) in the observation group were lower than those in the control group(P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups(P>0.05).
【Conclusion】
Xanthium Rhinitis Dripping Pills combined with cefuroxime can improve the clinical efficacy of patients with acute sinusitis, alleviate the inflammatory response, and has good safety, which is worthy of clinical promotion and application.
