Clinical Study of Body Surface Fixation Device for Jejunostomy Tube Fixation in Patients with Minimally Invasive Resection of Esophageal Cancer

【Objective】To investigate the effect of different fixation methods of jejunostomy tube on postoperative recovery of patients with jejunostomy tube after minimally invasive resection of esophageal cancer.【Methods】A total of 108 patients who underwent minimally invasive esophagectomy (MIE) while indwelling jejunostomy tube were selected from the Chinese PLA General Hospital from September 2021 to February 2023, and were divided into observation group (n=56) and control group (n=52) according to random number table method. The observation team used a body surface fixation device developed by the General Hospital ,Chinese People's Liberation Army to fix the fistula tube. ) after surgery were compared between the two groups, as well as the occurrence of complications related to fixed devices and postoperative recovery [first exhaust, defecation time and ability of daily living scale (ADL)].【Results】The rate of unplanned extubation in T

after operation in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05). The incidence of fistula displacement, fistula leakage and peritubular infection during catheterization in two groups was lower than that in control group, and the total incidence of complications in the observation group was significantly lower than that in the control group, with statistical significance (P<0.05). The first postoperative exhaust time and defecation time of the observation group were shorter than those of the control group, and the ADL scores of the observation group were higher than those of the control group at 7 and 14 days after the operation, with statistical significance (P<0.05).【Conclusion】The body surface fixation device of jejunostomy tube can enhance the fixation function of jejunostomy tube, reduce the rate of unplanned extubation, improve the patients' ability of daily living, and reduce the incidence of complications caused by poor fixation of jejunostomy tube.

Comparison of 577 nm Laser Micropulse Mode and 532 nm Laser Photocoagulation in the Treatment of Patients with Central Serous Chorioretinopathy

【Objective】To compare the efficacy of 577 nm laser micropulse mode and 532 nm laser photocoagulation in the treatment of central serous chorioretinopathy (CSC).【Methods】Sixty patients (60 eyes) with CSC diagnosed by fluorescein fundus angiography, indocyanine green angiography and optical coherence tomography (OCT) were selected from September 2021 to August 2023 in our hospital. They were voluntarily divided into the observation group (receiving 577 nm laser micropulse mode treatment) and the control group (receiving 532 nm laser photocoagulation treatment), with 30 cases (30 eyes) in each group. The best corrected visual acuity (BCVA), central macular thickness (CMT) and subfovea choroidal thickness (SCT) and subretinal fluid (SRF) were compared between the two groups before treatment and at 1 month, 3 months and 6 months after treatment.【Results】Compared with before treatment, the BCVA, CMT and SRF heights of both groups were improved (P<0.05) after 1 month of treatment, while SCT was not significantly improved (P>0.05). After 3 months of treatment, SCT decreased in both groups (P<0.05). There was no significant difference in BCVA, CMT and SRF height between the two groups at 1 month, 3 months and 6 months after treatment (P>0.05). There was no significant difference in SCT at 1 month and 6 months after treatment between the two groups (P>0.05). The SCT of the observation group was lower than that of the control group at 3 months after treatment, and the difference was statistically significant (P<0.05). There was no significant difference in SRF absorption rate at 1 month, 3 months and 6 months after treatment between the two groups (P>0.05).【Conclusion】Both 577 nm laser micropulse mode and 532 nm laser photocoagulation can improve visual acuity and reduce the height of CMT and SRF in CSC patients after 1 month. Both laser treatments reduced SCT after 3 months, but the reduction was more pronounced in the 577 nm laser micropulse mode.

Qualitative Study on Cancer Pain Experience and Expectation of Cancer Pain Management of Patients with Lung Cancer

【Objective】To explore the cancer pain experience and expectations of cancer pain management in patients with lung cancer.【Methods】A total of 17 patients with lung cancer pain who were hospitalized in our hospital from March to October 2022 were selected and semi-structured in-depth interviews were conducted. It was to understand and record the experience of cancer pain and expectations for cancer pain management. The Giorgio six step method was used to analyze and sort out the data for refining the theme.【Results】The experience of cancer pain in patients with lung cancer can be divided into four themes: Patients had varying degrees of cancer pain; Cancer pain had a great impact on patients; Patients had insufficient cognition and response to cancer pain, including pain tolerance, guilt, misunderstanding of pain relief treatment and lack of patients' ability to cope with cancer pain; Patients expected more attention and support to better manage pain.【Conclusion】The current situation of cancer pain experience in patients with lung cancer is not optimistic. We should pay attention to the experience of cancer pain in patients with lung cancer, pay attention to the treatment of cancer pain, strengthen patients' health education, improve the ability of cancer pain management, improve the social support system, and enhance the ability of home care. Finally, improve the quality of life of patients.

The Efficacy of Lu's Acupuncture on Gastrointestinal Dysfunction and Postoperative Pain in Patients with Gastrointestinal Tumors Following Laparoscopic Surgery

【Objective】To investigate the efficacy of Lu's acupuncture on gastrointestinal dysfunction and pain in patients with gastrointestinal tumors after laparoscopic surgery.【Methods】Sixty patients requiring laparoscopic surgery for gastrointestinal tumors were randomly divided into an observation group and a control group, with 30 patients in each group. The control group received conventional postoperative treatment, while the observation group received intervention with Lu's acupuncture to strengthen the spleen and promote bowel function. The postoperative inflammatory factors [interleukin-6(IL-6), procalcitonin(PCT), and high-sensitivity C-reactive protein (hs-CRP) levels], pain scores at 48 hours post-surgery, incidence of gastrointestinal adverse reactions, and recovery status were compared between the two groups.【Results】On the fifth postoperative day, the levels of IL-6, PCT, and hs-CRP, the pain score (VAS) at 48 hours post-surgery, and the incidence of gastrointestinal adverse reactions in the observation group were significantly lower than those in the control group (P<0.05). The recovery time for bowel sounds, the time of first getting out of bed and the time of first eating after surgery were all shorter in the observation group compared to the control group (P<0.05).【Conclusion】The application of Lu's acupuncture significantly improves gastrointestinal function and pain level in patients with gastrointestinal tumors after laparoscopic surgery and reduces the incidence of gastrointestinal adverse reactions.

The Role of FVH/DWI Mismatch in Predicting the Recanalization Rate after Mechanical Thrombectomy in Acute Anterior Circulation Large Vessel Occlusion

【Objective】To investigate the relationship of fluid-attenuated inversion recovery vascular hyperintensity (FVH)/diffusion-weighted imaging (DWI) mismatch with the recanalization rate after mechanical thrombectomy in patients with acute anterior circulation large vessel occlusion (LVO).【Methods】A total of 127 patients with acute anterior circulation LVO underwent MRI and mechanical thrombectomy, and their post-procedural recanalization rates were evaluated.【Results】Of the 127 patients, 98 achieved complete recanalization (Group A), while 29 had no or partial recanalization (Group B), with an overall recanalization rate of 77.17%. No statistically significant differences were observed between Group A and Group B regarding gender, age, comorbidities, occlusion site, time from onset to MRI, or NIHSS scores on admission (P>0.05). Group A had a higher proportion of good collateral circulation and lower 3-month modified Rankin Scale (mRS) scores compared to Group B (P<0.05). Group A also had higher FVH scores, a greater proportion of FVH/DWI mismatch, and smaller DWI infarct volumes compared to Group B (P<0.05). Spearman correlation analysis showed a positive correlation between FVH/DWI mismatch and recanalization rate after mechanical thrombectomy in acute anterior circulation LVO (r

=0.415, P=0.005). Logistic regression analysis identified collateral circulation, 3-month mRS score, and FVH/DWI mismatch as factors associated with recanalization (P<0.05).【Conclusion】Patients with FVH/DWI mismatch have a higher recanalization rate after mechanical thrombectomy for acute anterior circulation large vessel occlusion, which warrants clinical attention.

Analysis on the Use of Category Ⅱ Psychotropic Drugs in Two Tertiary Hospitals of Hunan from 2015 to 2018

【Objective】To analyze the use of Category Ⅱ psychotropic drugs and evaluate the rationality of their use.【Methods】The Category Ⅱ psychoactive use in the two hospitals of from 2015 to 2018 was retrospectively analyzed, and the types, sales volume, consumption sum, DDDs, DDC and drug ranking ratio (B/A) of drugs were statistically analyzed.【Results】There were 11 kinds of the Category Ⅱ psychotropic drugs in our hospital, and their drug use trend was stable, and benzodiazepines (BZDs) were dominant. From 2015 to 2018, the sales volume and consumption amount of the second category of psychotropic drugs showed a downward trend. DDDs and DDC of midazolam injection ranked first, DDDs of Zopiclone tablet showed an increasing trend year by year, and B/A of Alprazolam tablet was greater than 1.【Conclusion】The use of the Category Ⅱ psychotropic drugs in our hospital is basically reasonable, but supervision and management are still needed to guard against the possibility of drug abuse.

Investigation and Analysis of Knowledge and Behavior of Respiratory Treatment-related Infection Prevention and Control among Clinical Medical Staff

【Objective】To investigate the knowledge and behavior of respiratory therapy related infection prevention and control among clinical staff and analyze the influencing factors.【Methods】From May 15 to 30, 2020, 249 medical personnel from professional wechat groups such as the National Academic Committee and academic groups were selected as the investigation objects, and the knowledge and behavior of respiratory treaty-related infection prevention and control questionnaire was used to investigate the clinical medical personnel.【Results】The total score of 249 people was (53.84±6.02) scores. The knowledge score of the medical staff who participated in respiratory therapy was higher than that of the non-participants, and the knowledge score of the medical staff who engaged in respiratory therapy was higher than that of the non-full-time medical staff, with statistical significance (P<0.05). Among the prevention and control behaviors of respiratory treatment-related infections, the standardized use rate of personal protective measures was 100%, and the three lowest use rates of respiratory treatment professional equipment and materials were: Non-invasive ventilation uses semi-automatic humidification tank with closed automatic water refill function or self-made, non-invasive ventilation uses closed mask + bacterial filter + exhalation valve connection mode, and dedicated atomizing device adoption rate.【Conclusion】The knowledge of infection prevention and control related to respiratory therapy is generally well mastered by clinical front-line medical staff, but the implementation rate of infection prevention and control behavior needs to be improved.

Impact of Fluence Smoothing Setting in Monaco Treatment Planning System on IMRT Plans for Cervical Cancer

The use of the High fluence smoothing parameter with a maximum of 30 control points per field in the design of IMRT plans for postoperative concurrent dose escalation in cervical cancer improves plan efficiency.

Effects of Oxiracetam Combined with Betahistine on Cerebral Blood Flow and Serum Levels of Ang Ⅱ and PDGF-BB in Patients with Post-Stroke Dizziness

【Objective】To investigate the effects of combined treatment of Oxiracetam with Betahistine on cerebral blood flow and levels of angiotensin Ⅱ(Ang Ⅱ) and platelet-derived growth factor-BB (PDGF-BB) in patients with post-stroke dizziness.【Methods】Seventy-eight patients with post-stroke dizziness were randomly divided into the control group and the observation group, with 39 patients in each group. The control group was treated with betahistine mesylate tablets, while the observation group received additional treatment with xxiracetam in addition to betahistine. The cerebral blood flow, serum levels of Ang Ⅱ and PDGF-BB, dizziness severity (as assessed by the UCLA-DQ), and motion sensitivity (as assessed by the MSQ) were compared between the two groups before and after treatment.【Results】After treatment, both groups showed a significant increase in cerebral blood flow compared to before treatment (P<0.05), with the observation group exhibiting a greater increase than the control group (P<0.05). The levels of Ang Ⅱ and PDGF-BB significantly decreased after treatment in both groups (P<0.05), with lower levels observed in the observation group compared to the control group (P<0.05). The UCLA-DQ and MSQ scores significantly decreased after treatment in both groups (P<0.05), with the observation group showing lower scores than the control group (P<0.05).【Conclusion】The combination of oxiracetam and betahistine significantly improves cerebral blood flow in the affected regions of patients with post-stroke dizziness, reduces serum levels of Ang Ⅱ and PDGF-BB, alleviates dizziness symptoms, and enhances the quality of life. This treatment approach is worthy of clinical promotion and application.

Efficacy and Safety of Stanford Accelerated Intelligent Neuromodulation Therapy in Combination with Medication for First-episode Depressive Disorder

【Objective】To investigate the efficacy and safety of Stanford accelerated intelligent neuromodulation therapy (SNT) combined with medication in the treatment of first-episode depressive disorder.【Methods】A total of 56 patients with depressive disorder hospitalized in the Hunan Brain Hospital from December 2022 to December 2023 were selected and divided into SNT combined with drug therapy group (SNT group) or repetitive transcranial magnetic stimulation (rTMS) combined with drug therapy group (rTMS group) according to random number table method, with 28 cases in each group. Effect of Sevelamer Combined with Hemoperfusion Therapy on Itching Severity and Calcium-phosphorus Metabolism Indicators in Patients with Uremic Skin Itching

【Objective】To explore the effect of sevelamer combined with hemoperfusion therapy on itching severity and calcium-phosphorus metabolism in patients with uremic skin itching.【Methods】A total of 76 patients with uremic skin itching admitted to the hospital from July 2022 to February 2024 were randomly divided into two groups using a random number table. The control group (n=38 cases) received blood perfusion therapy, while the observation group (n=38 cases) received sevelamer in addition to hemoperfusion of the control group. The itch severity score, inflammatory factors, calcium and phosphorus metabolism indicator, and quality of life were compared between two groups.【Results】After treatment, the sleep score, symptom and sign score, social psychological score, high-sensitivity C-reactive protein (hs-CRP), procalcitonin (PCT), interleukin-6 (IL-6), blood phosphorus, blood calcium, and total parathyroid hormone (iPTH) levels in the observation group were all lower than those in the control group (P<0.05). The World Health Organization Quality of Life Inventory (WHOQOL-BREF) scores were higher than those in the control group, with significantly statistical differences (P<0.05).【Conclusion】The application of sevelamer combined with blood perfusion therapy has significant therapeutic effects, which can alleviate itching symptoms, reduce inflammatory reactions, improve calcium and phosphorus metabolism indicators, and enhance quality of life.

The Anesthetic Effects of CSEA and LPSB in Elderly Patients Undergoing Hip Fracture Surgery and their Impact on Cognitive Function

【Objective】To observe the anesthetic effects of combined spinal-epidural anesthesia (CSEA) and lumbar plexus-sciatic nerve block (LPSB) in elderly patients undergoing hip fracture surgery and their impact on cognitive function.【Methods】Eighty elderly patients undergoing internal fixation surgery for hip fractures were randomly divided into an observation group and a control group, with 40 patients in each group. The control group received CSEA anesthesia, while the observation group received LPSB anesthesia. The surgical conditions, hemodynamic parameters at various time points [heart rate (HR), mean arterial pressure (MAP), oxygen saturation (SpO
2
)], stress response indicators [glucose (GLU), interleukin-6 (IL-6), cortisol (COR)], pain levels before and after surgery, cognitive function changes, and adverse events were compared between the two groups.【Results】There were no statistically significant differences in surgery time and blood loss between the two groups (P>0.05). However, the observation group had lower infusion volumes, longer anesthesia onset times, and longer block duration compared to the control group (P<0.05). At 30 minutes intraoperatively, both groups showed lower HR and MAP than preoperatively and at 1 hour postoperatively (P<0.05), with the observation group having higher HR and MAP than the control group (P<0.05). The GLU, IL-6, and COR levels at 30 min intraoperatively and 1 hour postoperatively were higher than those before operation in both groups, which showed a continuous increase (P<0.05). However, the observation group had lower GLU, IL-6, and COR levels than the control group at these time points (P<0.05). Postoperatively, both groups had lower visual analog scale (VAS) pain scores and higher Montreal Cognitive Assessment (MoCA) scores than preoperatively (P<0.05), with the observation group showing lower VAS and higher MoCA scores than the control group (P<0.05). There was no statistically significant difference in the overall incidence of adverse events between the two groups (P>0.05).【Conclusion】LPSB offers better anesthetic effects than CSEA for elderly patients undergoing hip fracture surgery. It helps stabilize blood circulation, reduces stress responses, has a smaller impact on cognitive function, and does not result in significant adverse events, which makes it suitable for clinical use.

Effect of Minimally Invasive Endometrial Therapy under Hysteroscopy on Uterine Endometrial Receptivity in Women with Infertility

【Objective】To investigate the effect of hysteroscopic endometrial minimally invasive surgery on postoperative endometrial receptivity in female infertility patients.【Methods】A total of 50 female infertility patients diagnosed and treated in our hospital from January 2021 to December 2022 were selected and divided into the control group (receiving conventional drug treatment) and the observation group (receiving hysteroscopic endometrial minimally invasive surgery on the basis of control group) according to random number table, with 25 cases in each group. The endometrial blood flow parameters and endometrial receptivity were compared between the two groups before and after treatment.【Results】Before treatment, there were no significant differences in uterine artery blood flow pulsatility index (PI) and resistance index (RI) as well as endometrial thickness between the two groups (P>0.05). One week after treatment, the PI and RI of uterine artery blood flow and endometrial thickness of the two groups were improved in both groups (P<0.05). In addition, the PI and RI of uterine artery blood flow and endometrial thickness in the observation group improved more significantly, and the differences were statistically significant (P<0.05).【Conclusion】Hysteroscopic endometrial minimally invasive surgery for female infertility patients can improve endometrial blood flow parameters and endometrial acceptability to improve the implantation rate and pregnancy rate.

Clinical Effect of Statins on Patients with Coronary Heart Disease Complicated with Carotid Atherosclerotic Plaque

【Objective】To investigate the effect of rosuvastatin on low density lipoprotein cholesterol (LDL-C) compliance rate, symptom remission and carotid plaque in patients with coronary heart disease and carotid plaque.【Methods】A total of 90 patients with coronary heart disease combined with carotid plaque admitted to our hospital from April 2019 to October 2020 were selected and divided into the observation group and the control group according to random number method, with 45 cases in each group.The observation group received rosuvastatin lipid-lowering treatment,the control group received simvastatin treatment. LDL-C level and compliance rate, occurrence of adverse reactions and changes of carotid plaque [including carotid intima-media thickness (IMT), carotid plaque area, thickness and number] were compared between the two groups after treatment.【Results】After treatment, serum LDL-C levels in 2 groups were lower than before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). The LDL-C compliance rate in the observation group was significantly higher than that in the control group, with statistical significance (P<0.05). After treatment, IMT and plaque thickness in the observation group were lower than those in the control group, and the difference was statistically significant (P<0.05). There was no significant difference in other carotid plaque indexes between the two groups (P>0.05). After treatment, the frequency of angina pectoris attacks in 2 groups was lower than before treatment, the duration of angina pectoris attacks was shorter than before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05).【Conclusion】Rosuvastatin has better lipid lowering effect, and is beneficial to improve the symptoms of patients with coronary heart disease and delay the progression of carotid plaque.

Application of Comprehensive Enteral Nutrition Intervention Based on Timing Selection in Intensive Care Unit

【Objective】To explore the application of comprehensive enteral nutrition (EN) intervention based on timing selection for patients in the Intensive Care Unit (ICU).【Methods】A total of 106 ICU patients were randomly divided into a control group and an observation group, with 53 patients in each group. The control group received routine EN care, while the observation group implemented a comprehensive EN intervention based on timing selection. The serum prealbumin (PA), albumin (ALB), serum potassium, sleep quality, feeding intolerance rate, and refeeding syndrome incidence were compared before and after the intervention between the two groups.【Results】After the intervention, the levels of ALB, PA, and serum potassium in the observation group were significantly higher than those in the control group. The feeding intolerance rate and refeeding syndrome incidence in the observation group were both lower than those in the control group. Additionally, the sleep quality index scores of the observation group were lower than those of the control group, with statistically significant differences (P<0.05).【Conclusion】Comprehensive EN intervention based on timing selection can significantly improve biochemical indicators and sleep quality in ICU patients while reducing the rates of feeding intolerance and refeeding syndrome.

Expression and Clinical Significance of Galectin-3 and Apelin-12 in Serum of Patients with Hypertensive Heart Disease

【Objective】To investigate the serum levels of galectin-3 and Apelin-12 in patients with hypertensive heart disease (HHD) and its clinical significance.【Methods】From October 2019 to October 2021, 160 patients with simple hypertension were selected as the hypertension group, and 137 patients with HHD were selected as HHD group. The left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD), interventricular septal thickness (IVST), left ventricular posterior wall thickness (LVPWT), left atrial diameter (LAD), left ventricular ejection fraction (LVEF), mitral valvular early diastolic peak velocity (E peak) and mitral valvular end diastolic peak velocity (A peak) were measured, and the E/A was calculated. The enzyme linked immunosorbent assay (ELISA) was used to detect serum levels of galectin-3 and Apelin-12, Pearson test was used to analyze the correlation between serum galectin-3 and Apelin-12 levels and cardiac color Doppler ultrasound parameters in patients with HHD; and receiver operating characteristic (ROC) curve was drawn to evaluate the diagnostic value of serum galectin-3 and Apelin-12 levels in HHD;Multivariate logistic regression model was used to analyze the risk factors of HHD in patients with hypertension.【Results】The systolic blood pressure(SBP), diastolic blood pressure(DBP), LVESD, LVEDD, IVST, LVPWT, LAD and serum galectin-3 level in HHD group were significantly higher than those in hypertension group (P<0.05), and the course of hypertension in HHD group was significantly longer than that in hypertension group (P<0.05). While LVEF, E/A and serum Apelin-12 level in HHD group were significantly lower than those in hypertension group (P<0.05). Pearson analysis showed that serum galectin-3 level was positively correlated with LVESD, LVEDD and LAD (P<0.05), and negatively correlated with LVEF and E/A (P<0.05); Apelin-12 level was negatively correlated with LVESD, LVEDD and LAD (P<0.05), and positively correlated with LVEF and E/A (P<0.05). The cutoff value of galectin-3 was 7.55 ng/mL, and the sensitivity and specificity were 67.20% and 82.50%, respectively, the cutoff value of Apelin-12 was 305.34 ng/L, the sensitivity and specificity were 73.70% and 71.90%, respectively, the sensitivity and specificity of galectin-3 and Apelin-12 combined diagnosis were 76.60% and 80.60% respectively. The course of hypertension and serum levels of galectin-3 and Apelin-12 were independent risk factors for HHD in patients with hypertension (P<0.05).【Conclusion】Galectin-3 is highly expressed in HHD patients, and the expression of Apelin-12 is lowly expressed. They are closely related to the structure and function of left ventricle.

The recovery of symptoms, lung function, medical fear, cooperation, and complications were compared between the two groups.【Results】The fever reduction time, cough disappearance time, and lung rales disappearance time of the observation group were shorter than those of the control group (P<0.05), the cooperation degree was higher than that of the control group (P<0.05), and the total incidence of complications was lower than that of the control group (P<0.05). Application of WeChat Platform Intervention Combined with Peer Support Nursing in Patients with Sudden Hearing Loss

【Objective】To explore the effectiveness of WeChat platform intervention combined with peer support nursing in patients with sudden hearing loss.【Methods】A total of 98 patients with sudden hearing loss admitted to our hospital between May 2022 and May 2024 were selected. They were evenly divided into an experimental group and a control group, with 49 cases in each group using the odd-even grouping method. The control group received WeChat platform intervention in addition to routine care, while the experimental group received peer support nursing on top of the intervention provided to the control group. Negative emotions, stress levels, and the degree of social support were compared between the two groups.【Results】The patient health questionnaire-9(PHQ-9) and generalized anxiety disorder(GAD-7) scores of the experimental group were significantly lower than those of the control group (P<0.05). Scores for detachment, re-experiencing, avoidance, irritability, and impairment were also significantly lower in the experimental group (P<0.05). In addition, the scores for subjective support, objective support, and support utilization were significantly higher in the experimental group (P<0.05).【Conclusion】The combination of WeChat platform intervention with peer support nursing effectively reduces negative emotions, alleviates stress, and improves the level of social support in patients with sudden hearing loss.

Clinical Efficacy of Sulodexide Combined with Epalrestat in the Treatment of Type 2 Diabetes Mellitus Complicated with Diabetic Peripheral Neuropathy

【Objective】To investigate the clinical efficacy of Sulodexide combined with epalrestat in the treatment of type 2 diabetesmellitus (T2DM) patients with diabetic peripheral neuropathy (DPN).【Methods】A total of 95 T2DM patients with DPN treated in our hospital from February 2020 to March 2023 were selected and divided into control group (receiving epalrestat treatment, n=46) and observation group (receiving Sulodexide treatment on the basis of control group treatment, n=49) according to different treatment methods. The clinical efficacy, clinical score, nerve conduction velocity, glycated hemoglobin(HbA1c), fasting blood glucose (FPG), serological indexes and incidence of adverse reactions were compared between the two groups.【Results】The total effective rate of the observation group was higher than that of the control group (P<0.05). After treatment, the pain, paresthesia, burning sensation, numbness scores and serum malondialdehyde (MDA) and endothelin-1 (ET-1) levels in both groups were lower than before treatment (P<0.05), and the pain, paresthesia, burning sensation, numbness scores and serum MDA and ET-1 levels in the observation group were lower than those in the control group (P<0.05). After treatment, the sensory conduction velocity (SNCV), motor conduction velocity (MNCV) and serum nitric oxide (NO) levels of the median nerve and common peroneal nerve in both groups were higher than before treatment (P<0.05), and the levels of SNCV, MNCV and NO in the median nerve and common peroneal nerve in the observation group were higher than those in the control group (P<0.05). After treatment, the HbA1c and FPG of both groups were lower than before treatment (P<0.05), and the HbA1c and FPG of observation group were lower than those of control group (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).【Conclusion】Sulodexide combined with epalrestat is effective in the treatment of T2DM patients with DPN, and can improve blood glucose control, clinical symptoms, oxidative stress index, vascular endothelial function index, nerve conduction velocity, and is safe and reliable.

Differential Comparison of Different MRI Sequences in Grading Diagnosis of Early Knee Cartilage Injury

【Objective】To compare the differences in early grading diagnosis of knee cartilage using different sequences of magnetic resonance imaging (MRI).【Methods】A total of 85 patients with early knee cartilage injuries admitted to our hospital from January 2019 to January 2022 were selected and underwent MRI three-dimensional and two-dimensional sequence examinations. The results of knee arthroscopy were used as the diagnostic criteria to analyze and compare the efficacy of different sequences in the diagnosis of early knee cartilage injuries.【Results】There were 130 knee cartilage injured lesions in 85 patients. Arthroscopy grading showed that 51 lesions were grade 0, 19 were grade 1, 26 were grade 2, 18 were grade 3, and 16 were grade 4. The MRI two-dimensional sequence scan showed 57 lesions at grade 0, 16 at grade 1, 25 at grade 2, 17 at grade 3, and 15 at grade 4. The MRI three-dimensional sequence scan showed that there were 52 cases at grade 0, 22 cases at grade 1, 24 cases at grade 2, 17 cases at garde 3, and 15 garde at level 4. The consistency results between MRI two-dimensional sequence and arthroscopy showed that the Kappa values for grades 0-4 were 0.715, 0.175, 0.341, 0.504, and 0.670, respectively. The consistency analysis between MRI three-dimensional sequence and arthroscopy showed that the Kappa values for grades 0-4 were 0.887, 0.566, 0.653, 0.702, and 0.817, respectively. The sensitivity of MRI three-dimensional sequence in diagnosing grade 0, 2, 3, and 4 knee cartilage injuries was higher than those of two-dimensional sequence, but the difference was not statistically significant (P>0.05); The sensitivity of three-dimensional sequence in diagnosing grade 1 knee cartilage injury is higher than that of two-dimensional sequence, and the difference was statistically significant (P<0.05).【Conclusion】Both 3D and 2D MRI sequences are effective diagnostic methods for early knee cartilage injury, but the sensitivity of MRI 3D sequence grading in diagnosing grade 1 knee cartilage injury is higher than that of MRI 2D sequence.

Clinical Effect of Percutaneous Spinal Endoscopic Translaminar Approach in the Treatment of Lumbar Instability with Lumbar Disc Herniation

【Objective】To investigate the clinical effect of percutaneous spinal endoscopic translaminar approach in the treatment of lumbar instability with lumbar disc herniation.【Methods】A retrospective analysis was performed on 164 patients with lumbar instability with lumbar disc herniation (group A) who underwent percutaneous spinal endoscopic translaminectomy in our hospital from May 2019 to May 2022, and 97 patients (group B) who underwent routine open surgery in our hospital during the same period. The preoperative and postoperative lumbar pain [visual analogue scale (VAS)], the degree of lumbar dysfunction [Oswestry Disability Index (ODI)], the therapeutic effect and the incidence of complications were compared between the two groups.【Results】There was no significant difference in preoperative VAS and ODI scores between the two groups (P>0.05). VAS scores and ODI scores of back and leg pain 1 d, 1 month and 3 months after surgery and ODI scores of 12 months after surgery between the two groups had statistical significance (P<0.05), while VAS scores of back and leg pain 12 months after surgery had no statistical significance between the two groups (P>0.05). VAS scores and ODI scores in the group A were lower than those in the group B at 1 day, 1 month and 3 months after surgery, and ODI scores at 12 months after surgery. 12 months after operation, the curative effect of the group A was better than that of the group B (P<0.05). There was no significant difference in the total incidence of adverse reactions between the two groups (P>0.05).【Conclusion】Compared with conventional open surgery, percutaneous spinal endoscopic translaminar approach for the treatment of patients with lumbar instability and lumbar disc herniation has significant clinical efficacy, can improve the pain degree of patients and lumbar rehabilitation effect, and has good safety.

Effect of Ultrasound-Guided Sciatic Nerve Block Anesthesia on Lower Limb Blood Circulation in Patients Undergoing Foot Surgery

【Objective】To investigate the impact of ultrasound-guided sciatic nerve block anesthesia on lower limb blood circulation in patients undergoing foot surgery.【Methods】A total of 78 patients undergoing foot surgery from our two hospitals were randomly divided into the observation group and the control group, with 39 patients in each group. The anesthetic effects and related indicators were compared between the two groups. Hemodynamic parameters such as heart rate (HR) and mean arterial pressure (MAP) were measured before anesthesia (T

). Coagulation function indices, including thrombin time (TT), prothrombin time (PT), and activated partial thromboplastin time (APTT), were compared before and after surgery. The incidence of adverse reactions was also compared between the two groups.【Results】The duration of motor and sensory block in the observation group was significantly longer than that in the control group, while the time to urination and lower limb motor recovery of the observation group was shorter than of in the control group, with statistically significant differences (P<0.05). in the observation group were higher than those in the control group at the same time points (P<0.05). Postoperative levels of TT, PT, and APTT were higher than preoperative levels in both groups (P<0.05), and postoperative TT, PT, and APTT levels in the observation group were significantly higher than those in the control group (P<0.05). The excellent rate of anesthesia in the observation group (97.44%) was significantly higher than in the control group (79.49%) (P<0.05). The total incidence of adverse reactions in the observation group was 7.69%, which was significantly lower than that in the control group (25.64%) (P<0.05).【Conclusion】Ultrasound-guided sciatic nerve block anesthesia for foot surgery has excellent anesthetic effects and safe clinical application, which effectively improves coagulation function and has fewer adverse reactions.

Effect of Enteral Nutrition Support on Cardiac Function and Prognosis in Patients with Early Acute Decompensated Heart Failure

【Objective】To investigate the effect of enteral nutrition on cardiac function and prognosis of patients with early acute decompensated heart failure.【Methods】The clinical data of 100 patients with early acute decompensated heart failure admitted to our hospital from January 2019 to January 2021 were retrospectively analyzed, and they were divided into the observation group and the control group according to different treatment methods, with 50 cases in each group. The control group received conventional anti-heart failure treatment, and the observation group was given enteral nutrition support on the basis of the control group. The changes of cardiac function indexes, nutritional status indexes and follow-up results within 1 year after discharge were compared between the two groups.【Results】After treatment, left ventricular end-diastolic diameter, left ventricular end-systolic diameter and serum brain natriuretic peptide levels in 2 groups were lower than before treatment, and the observation group was lower than the control group, the difference was statistically significant (P<0.05). Left ventricular ejection fraction in both groups was higher than before treatment, and the observation group was higher than the control group, the difference was statistically significant (P<0.05). After treatment, serum albumin, transferrin, prealbumin and hemoglobin in 2 groups were higher than before treatment, and the observer group was higher than the control group, the difference was statistically significant (P<0.05). The recurrence rate of heart failure, mortality rate and disease deterioration rate of patients in the observation group were significantly lower than those in the control group within 1 year after discharge, with statistical significance (P<0.05).【Conclusion】Enteral nutrition support can improve the cardiac function, nutritional status and prognosis of patients with early acute decompensated heart failure, and is worthy of clinical application.

Effect of Family Collaborative Nursing under Co-Parenting Concept on the Feeding Behavior and Psychological Resilience of Primiparous Women

【Objective】To investigate the influence of family cooperative nursing based on co-parenting on feeding behavior and psychological resilience of primiparous women.【Methods】A total of 206 primiparous women admitted to our hospital from December 2020 to December 2023 were selected and divided into the control group (receiving conventional nursing,n=99) and the observation group (receiving family cooperative nursing based on co-parenting,n=101) by random number table method. Each group had 103 cases. The scores of breastfeeding knowledges, self-efficacy (BSES-SF score), breastfeeding compliance rate, quality of co-parenting (Brief-CRS score), and psychological resilience level (CD-RISC score) were compared between the two groups.【Results】The breastfeeding knowledge score, BSES-SF score, Brief-CRS score, and CD-RISC score of both groups were higher than those before intervention (P<0.05), and the scores of the above indicators in the observation group were higher than those in the control group (P<0.05). Three months after delivery, the breastfeeding compliance rate of the observation group was 94.06%, which was significantly higher than that of the control group (82.83%, P<0.05).【Conclusion】Implementing family cooperative nursing based on shared co-parenting can effectively improve the breastfeeding knowledge level and self-efficacy of primiparous women, significantly improve the quality of co-parenting, increase the rate of breastfeeding compliance, and enhance the psychological resilience level of primiparous women. It is worthwhile for clinical reference.

Correlation Analysis of Serum PTH, 25(OH)D Levels with Urinary Red Blood Cells and Urinary Calcium in Children with Idiopathic Hypercalcemia

【Objective】To investigate the correlation of serum parathyroid hormone (PTH), 25-hydroxyvitamin D[25(OH)D], calcium and phosphorus levels with urinary red blood cells and urinary calcium at 24 h in children with idiopathic hypercalcemia.【Methods】A total of 26 children with idiopathic hypercalcemia admitted to our hospital from January 2019 to December 2023 (the observation group) were selected, and 33 healthy volunteers who underwent physical examination in our hospital were selected as the control group. The serum levels of PTH, 25(OH)D, phosphorus and calcium were compared between the two groups, and the correlation between urinary red blood cells and 24 h urinary calcium levels and serum PTH, 25(OH)D, phosphorus and calcium levels in the observation group was analyzed.【Results】The levels of serum PTH and 25(OH)D in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05). There was no significant difference in serum phosphorus and calcium levels between the two groups (P>0.05). Pearson correlation analysis showed that PTH level in the observation group was positively correlated with urinary red blood cells and 24 h urinary calcium (r=0.877, 0.717, P<0.01). Serum 25(OH)D levels were positively correlated with urinary red blood cells and 24 h urinary calcium (r=0.837, 0.746, P<0.01). There was no correlation between serum phosphorus and urinary red blood cells and urinary calcium at 24 h (P>0.05). There was no correlation between serum calcium and urinary red blood cells and urinary calcium at 24 h (P>0.05).【Conclusion】The serum levels of PTH and 25(OH)D were significantly increased in children with idiopathic hypercalcemia, which was positively correlated with urinary calcium and hematuria.

Effect of Different Semen Optimization Methods on Clinical Pregnancy Outcome of Intrauterine Insemination by AIH

【Objective】To investigate the effect of different semen optimization methods on the clinical outcome of intrauterine insemination by AIH.【Methods】A total of 124 patients with IUI admitted to our hospital from January 2020 to December 2023 were selected and divided into the upstream group (n=40) and the density gradient centrifuge group (n=84) according to different semen optimization treatment methods. The pregnancy status and pregnancy outcome of the two groups were compared.【Results】Of 124 IUI patients, 20 were pregnant (16.13%). There were 7 clinical pregnancies (17.50%) in the upstream group and 13 clinical pregnancies (15.48%) in the density gradient centrifuge group. There was no biochemical pregnancy between the two groups. There was no statistical significance in clinical pregnancy between the two groups (P>0.05). In the upstream group of 7 clinical pregnancies, there were 2 live births (28.57%) and 5 abortions (71.43%), including 4 abortions caused by fetal stop and 1 abortion caused by ectopic pregnancy. Among the 13 clinical pregnancies in the density gradient centrifuge group, 11 cases (84.62%) of live births and 2 cases (15.38%) of miscarriages were caused by fetal stop. The live birth rate of the density gradient centrifugation group was significantly higher than that of the upstream group, and the abortion rate was significantly lower than that of the upstream group, with statistical significance (P<0.05).【Conclusion】There is no difference between upstream method and density gradient centrifugation method on the pregnancy rate of intrauterine insemination by AIH. The live birth rate of density gradient centrifugation method is higher than upstream method, and the abortion rate is lower than upstream method. When intrauterine insemination is performed, different semen optimization methods should be selected according to the specific conditions of patients.

Effects of Remimazolam Combined with Dexmedetomidine on Cerebral Oxygen Metabolism and Cognitive Function in Elderly Patients Undergoing Lumbar Spine Surgery
