Predictive Value of Uterine Artery Pulse Index Combined with Serum β-HCG Level in Late-Onset Fetal Intrauterine Growth Restriction in the Second Trimester
LI Xina, XU Ning, WANG Fuli
DOI: 10.3969/j.issn.1671-7171.2024.08.001
【Objective】To investigate the value of uterine artery pulsation index (PI) combined with serum β-subunit human chorionic gonadotropin (β-HCG) level in predicting intrauterine growth restriction in late-onset fetuses during the second trimester. 【Methods】A total of 179 pregnant women were selected from Xi'an People's Hospital(Xi'an Fourth Hospital) from January 2019 to December 2023. According to the occurrence of growth restriction, they were divided into the late-onset intrauterine growth restriction group and non-late-onset intrauterine growth restriction group. PI and β-HCG were measured during the second trimester. Logistic regression was used to analyze the risk factors of intrauterine growth restriction in late-onset fetuses. We also analyzed the value of uterine artery ultrasound PI combined with β-HCG in predicting late-onset fetal intrauterine growth restriction in the second trimester. 【Results】There were 31 cases of late-onset intrauterine growth restriction (17.32%) in pregnant women, which were classified as late-onset intrauterine growth restriction group and 148 cases of non-late-onset intrauterine growth restriction group. There was no statistical significance in age, gestational time, family history of hypertension during pregnancy, systolic blood pressure, TC and albumin between late and non-late intrauterine growth restriction group (P>0.05). The levels of PI, β-HCG and gestational age in the late intrauterine growth restriction group were higher than those in the non-late intrauterine growth restriction group (P<0.05). The occurrence of late-onset fetal intrauterine growth restriction was taken as the dependent variable, and PI and β-HCG were taken as independent variables for Logistic multivariate regression analysis (assignment: PI and β-HCG levels were continuous variables), the results showed that PI and β-HCG were risk factors for late-onset intrauterine growth restriction (P<0.05). ROC curve results showed that the AUC value of PI combined with β-HCG in predicting late-onset intrauterine growth restriction was 0.937, which was higher than that predicted alone (P<0.05). 【Conclusion】Both PI and β-HCG are closely related to intrauterine growth restriction of late-onset fetuses, and they can be used to predict intrauterine growth restriction of late-onset fetuses, while the prediction efficiency of PI and β-HCG combined value is higher than either PI or β-HCG alone.

ZHAI Junge, ZUO Yanyan, XIN Huiqin
DOI: 10.3969/j.issn.1671-7171.2024.08.002
【Objective】To explore the relationship of serum levels of C-reactive protein (CRP), homocysteine (Hcy), and matrix metalloproteinase-9 (MMP-9) with the occurrence of concurrent pneumonia in elderly patients with multiple cerebral infarctions .【Methods】The clinical data of 80 elderly patients with multiple cerebral infarctions were retrospectively analyzed. The patients were divided into two groups based on whether they had hospital-acquired pneumonia: the concurrent pneumonia group (n=18) and the non-concurrent pneumonia group (n=62). General data and serum levels of CRP, Hcy, and MMP-9 were compared between the two groups. The value of CRP, Hcy, and MMP-9 levels in predicting concurrent pneumonia was analyzed using receiver operating characteristic (ROC) curves. Multivariate Logistic regression analysis was applied to identify risk factors for concurrent pneumonia. 【Results】 The proportion of patients aged ≥70 years and those with a history of pulmonary disease was significantly higher in the concurrent pneumonia group than in the non-concurrent pneumonia group (P<0.05). The levels of CRP, Hcy, and MMP-9 were significantly higher in the concurrent pneumonia group compared to the non-concurrent pneumonia group (P<0.05). ROC and Logistic analysis showed that the area under the curve (AUC) values of CRP, Hcy, and MMP-9 in predicting concurrent pneumonia were 0.834, 0.850, and 0.752, respectively (P<0.05). Age ≥70 years, a history of pulmonary disease, CRP ≥21.725 mg/L, Hcy ≥21.420 μmol/L, and MMP-9 ≥385.255 ng/mL were identified as risk factors for concurrent pneumonia (P<0.05).【Conclusion】 Monitoring serum levels of CRP, Hcy, and MMP-9 has predictive value for concurrent pneumonia in elderly patients with multiple cerebral infarctions.

MA Chunhui, SUN Ningning, ZHANG Xiaoying, et al
DOI: 10.3969/j.issn.1671-7171.2024.08.003
【Objective】To investigate the effects of different femoral osteotomy methods on the complications and hip joint function of total hip replacement (THR) for developmental dysplasia of the hip (DDH). 【Methods】A total of 95 DDH patients who underwent THR treatment in our hospital from January 2021 to October 2022 were selected and divided into three groups according to different osteotomy methods: Group A [using short and shortened trochanter osteotomy (SSO), n=32], Group B [using extended trochanter osteotomy (ETO), n=32], and Group C (using large trochanter long oblique osteotomy, n=31). The surgical related indicators, hip joint function scores, and incidence of complications were compared among the three groups.【Results】The results showed that the operation time of Group C was shorter than that of Group A and Group B, and the total bleeding volume was less than that of Group A and Group B, with statistically significant differences (P<0.05). At 1 week, 1 month, and 3 months after surgery, the hip joint function scores of Group C were higher than those of Group A and Group B (P<0.05). The healing time of the osteotomy site and the difference in lower limb length one year after surgery in Group C were less than those in Group A and Group B (P<0.05); One week after surgery, the pain level in Group C was lower than that in Group A and Group B (P<0.05). There was no statistically significant difference in the incidence of complications among the three groups (P>0.05). 【Conclusion】 The use of different femoral osteotomy methods for THR treatment in DDH patients can effectively improve hip joint function and reduce the incidence of complications. However, the use of a large trochanter long oblique osteotomy method can significantly shorten the surgical time, reduce total bleeding, and promote patient recovery.

CHEN Yalin, WANG Xiaobin, WANG Jing, et al
DOI: 10.3969/j.issn.1671-7171.2024.08.005
【Objective】 To investigate the effect of non-insufflated and insufflated transaxillary endoscopic thyroidectomy on recurrent laryngeal nerve (RLN) injury and prognosis in patients with differentiated thyroid carcinoma (DTC).【Methods】 Eighty-eight DTC patients undergoing transaxillary endoscopic thyroidectomy in our hospital were divided into a non-insufflated group (40 cases) and an insufflated group (48 cases) according to the different surgical methods. The levels of parathyroid hormone (PTH), recurrence rate, operation time, hospital stay, blood loss, drainage volume, incidence of RLN injury, and quality of life (QOL) scores were compared between the two groups.【Results】 Postoperatively, the PTH level in the non-insufflated group was higher than that in the insufflated group (P<0.05). There was no statistically significant difference in the recurrence rates between the two groups (P>0.05). The operation time and hospital stay were shorter, and the blood loss and drainage volume were less in the non-insufflated group than in the insufflated group. The incidence of temporary RLN injury was lower in the non-insufflated group (P<0.05). At three months post-operation, the QOL scores in the non-insufflated group were higher than those in the insufflated group (P<0.05).【Conclusion】 Compared to insufflated surgery, non-insufflated transaxillary endoscopic thyroidectomy can shorten the operation and hospital stay, reduce blood loss and drainage volume, and lower the incidence of temporary RLN injury. Additionally, patients have better quality of life postoperatively, making it worthy of clinical promotion and application.

Evaluation of Risk Factors for Stent Patency in Patients with Lower Limb Deep Vein Thrombosis Post Interventional Syndrome Using a Risk Column Chart Model
QI Yongle, LIU Juanfang, YUAN Huifeng
DOI: 10.3969/j.issn.1671-7171.2024.08.007
【Objective】 To explore the risk factors for assessing stent patency in patients with deep vein thrombosis syndrome (PTS) after interventional procedures using a risk column chart model. 【Methods】 A total of 108 PTS patients who underwent iliac femoral vein balloon dilation combined with stent implantation in both hospitals from October 2019 to June 2022. Based on the stent patency during a 2-year follow-up, they were divided into a patency group (n=90) and a non patency group (n=18). Using logistic regression analysis to identify factors affecting postoperative stent patency, a column chart prediction model was established based on the results, and validated using calibration curves. 【Results】 There was a statistically significant difference in BMI(P<0.05), antiplatelet therapy, stent number, thrombus blockage location, and elastic sock support treatment between the two groups(P<0.05). Logistic multiple regression analysis showed that high BMI, lack of standardized antiplatelet therapy, number of stents ≥2, and non use of elastic stockings were risk factors affecting stent patency in PTS patients after intervention (P<0.05). The column chart model results showed that PTS patients with high BMI, no standardized antiplatelet therapy, number of stents ≥2, and no use of elastic stockings had a higher risk of stent patency after intervention. The decision curve results showed that when the risk threshold was between 0.2 and 0.8, the Nomogram curve was higher than the All and None curves, indicating that there was the maximum net benefit when the risk threshold was between 0.2 and 0.8. 【Conclusion】PTS patients with high BMI, lack of standardized antiplatelet therapy, ≥2 stents, and no use of elastic stockings have a higher risk of stent patency after intervention.

TAN Bin, CHEN Ao, YANG Fan, et al
DOI: 10.3969/j.issn.1671-7171.2024.08.008
【Objective】To compare the clinical outcomes of endoscopic transnasal submucosal approach and traditional transnasal transsphenoidal approach for pituitary adenoma resection. 【Methods】A retrospective analysis was conducted on the clinical data of 85 patients with pituitary adenomas admitted to our hospital from March 2019 to April 2024. According to the surgical approach plan, they were divided into a nasal septum submucosal group (transnasal septum submucosal approach, n=45) and a nasal butterfly group (transnasal butterfly approach, n=40). The incidence of postoperative complications, olfactory function, nasal ventilation function, and hormone levels before and after surgery were compared between the two groups. 【Results】There was no statistically significant difference in the total effective rate between the two groups of patients (P>0.05). The intraoperative tumor resection rate and cerebrospinal fluid leakage rate in the submucosal group of the nasal septum were not significantly different from those in the nasal butterfly group (P>0.05); The operation time and bleeding volume of the submucosal group of the nasal septum were lower than those of the nasal butterfly group, and the difference was statistically significant (P<0.05). Compared with preoperative values, the olfactory recognition thresholds of the two groups were higher at 1 month, 3 months, and 6 months after surgery, and the nasal septum submucosal group was lower than the nasal butterfly group, with statistical significance (P<0.05). At 3 and 6 months after surgery, the nasal inspiratory and expiratory resistance in both groups were higher than before surgery, and the submucosal group of the nasal septum was lower than the nasal butterfly group, with a statistically significant difference (P<0.05). Six months after surgery, the levels of serum prolactin, growth hormone, and adrenocorticotropic hormone in both groups were lower than before surgery (P<0.05), but there was no significant difference in these indicators between the two groups at six months after surgery (P>0.05). The total incidence of postoperative complications in the submucosal group of the nasal septum was lower than that in the nasal butterfly group, and the difference was statistically significant (P<0.05). 【Conclusion】Neuroendoscopic transnasal submucosal approach for pituitary adenoma resection has minimal damage to the patient's olfactory function, low incidence of postoperative complications, and high clinical significance.

YI Xinming, WAN Shunxin, ZHANG Jie, et al
DOI: 10.3969/j.issn.1671-7171.2024.08.009
【Objective】To establish and validate a prediction model for poor prognosis after total hip replacement in older adult patients. 【Methods】A total of 85 elderly patients who underwent hip arthroplasty from February 2019 to July 2022 were selected. After 1 year follow-up, patients were divided into the poor prognosis group and the good prognosis group according to the prognosis of older adult patients after hip arthroplasty. By comparing the clinical data of the two groups, the influencing factors of the poor prognosis after total hip replacement in elderly patients were analyzed by binary Logistic regression method, the prediction model of the poor prognosis after total hip replacement in older adult patients was constructed by Logistic method, and the fitting degree was evaluated by Hosmer-Lemeshow test. Receiver operating characteristic curve (ROC) was plotted to evaluate the prediction model differentiation. 【Results】After 1 year of follow-up, 2 cases of the 85 patients were lost to follow-up, the follow-up rate was 97.65%. Among them, 22 patients had poor prognosis, the incidence of poor prognosis was 26.51%(22/83). The surgical severity score (P-POSSUM score) ≥15 points, rehabilitation intervention time of 7 days after surgery, bone density <-3 SD, controlled nutritional status (CONUT) score ≥5 points, monocyte-to-lymphocyte ratio (MLR) ≥1.13, and bone alkaline phosphatase (BLAP) and thyroid hormone (PTH) levels in the poor prognosis group were significantly higher than those in the good prognosis group (P<0.05).Binary Logistic regression analysis showed that P-POSSUM score, rehabilitation intervention time, bone mineral density, CONUT score and MLR were the influencing factors for poor prognosis after total hip replacement in older adult patients (P<0.05). Taking the above influencing factors as predictive variables, a nomogram prediction model was established. The total score of each factor ranged from 76 to 426, and the corresponding risk rate ranged from 0.05 to 0.80. The C-index of the nomogram model was 0.825, and the correction curve for predicting the risk of poor prognosis after total hip replacement in older adult patients was close to the ideal curve (P>0.05). ROC curve results showed that the sensitivity and specificity of the nomogram model in predicting total poor prognosis after hip replacement in older adult patients were 90.91% and 88.52%, respectively. The area under the curve (AUC) was 0.903. 【Conclusion】P-POSSUM score, rehabilitation intervention time, bone mineral density, CONUT score and MLR are associated with the risk of poor prognosis after total hip arthroplasty in older adult patients. The construction of a nomogram model is helpful for early screening of the risk of poor prognosis after total hip arthroplasty in older adult patients.

DOI: 10.3969/j.issn.1671-7171.2024.08.010
【Objective】To investigate the clinical efficacy of phased radiofrequency microneedling combined with minocycline in the treatment of moderate to severe acne. 【Methods】Sixty patients with moderate to severe acne who visited our clinic from July 2022 to December 2023 were randomly divided into the observation group (treated with phased radiofrequency microneedle combined with minocycline) and the control group (treated with intense pulsed light combined with minocycline) using a random number table method, with 30 patients in each group. The observation group was treated with phase controlled radiofrequency microneedle combined with minocycline, while the control group was treated with intense pulsed light combined with minocycline. Both groups received single treatment. We compared the scores of the Global Acne Grading System (GAGS) and the conditions of erythema, pores, and amethyst between two groups before treatment, 1 month after treatment, and 3 months after treatment. We also compared the treatment efficacy and incidence of adverse reactions between the two groups.【Results】One month after treatment, the effective rate of the observation group was 93.33%, which was not significantly different from the control group's 86.67% (P>0.05). After treatment, the GAGS scores of both groups decreased compared to before treatment, and the GAGS score of the observation group was lower than that of the control group (P<0.05). After treatment, both groups showed significant improvement in erythema, purple pigment, and pores compared to before treatment (P<0.05); And the improvement of erythema and pores in the observation group was better than that in the control group (P<0.05), but there was no statistically significant difference in the percentage of purple matter between the two groups (P>0.05). All patients had no significant adverse reactions. 【Conclusion】The combination of phase controlled radiofrequency microneedles or intense pulsed light with minocycline is safe and effective in treating moderate to severe acne vulgaris. However, the combination of phase controlled radiofrequency microneedles and minocycline has a more significant improvement in enlarged pores and fine skin texture.

WANG Xiaojian
DOI: 10.3969/j.issn.1671-7171.2024.08.012
【Objective】To investigate the relationship between the expression of epidermal growth factor receptor (EGFR) and Ki-67 in cancer tissues and the postoperative prognosis of primary liver cancer (HCC).【Methods】A total of 103 HCC patients admitted to the hospital from June 2015 to June 2018 were selected. All patients underwent HCC radical surgery and postoperative prophylactic transcatheter arterial chemoembolization, and were followed up for 3 years after surgery. We detected and compared the expression of EGFR and Ki-67 in cancer tissues and adjacent tissues, analyzed the relationship between EGFR and Ki-67 expression in cancer tissues and pathological factors, analyzed the factors affecting the prognosis of HCC radical surgery, and analyzed the relationship between EGFR and Ki-67 expression in cancer tissues and postoperative prognosis of HCC.【Results】The positive expression rates of EGFR and Ki-67 in cancer tissues were higher than those in adjacent tissues (P<0.05). The positive expression rates of EGFR and Ki-67 in cancer tissues of patients with microvascular tumor thrombus, incomplete tumor capsule, clinical stage Ⅲ, and low differentiation were higher than those of patients without microvascular tumor thrombus, intact tumor capsule, clinical stage Ⅰ-Ⅱ, and moderately to well differentiated patients (P<0.05). Cox regression analysis showed that microvascular tumor thrombus, tumor capsule, clinical stage, differentiation degree, EGFR expression, Ki-67 expression were related factors affecting disease-free survival rate (P<0.05); Tumor capsule, clinical stage, differentiation degree, EGFR expression, and Ki-67 expression are related factors affecting overall survival rate (P<0.05). The disease-free survival rate and overall survival rate of EGFR and Ki-67 negative expression patients were better than those of positive expression patients, and the differences were statistically significant (P<0.05).【Conclusion】EGFR and Ki-67 are abnormally overexpressed in cancer tissues of HCC patients. EGFR and Ki-67 expression are associated with the pathology and prognosis of HCC patients after surgery, and those with positive expression have a higher risk of poor prognosis.

YAN Yunpeng, WANG Miao
DOI: 10.3969/j.issn.1671-7171.2024.08.013
【Objective】To investigate the effects of ezetimibe on cardiac function and the phosphatidylinositol 3-kinase/protein kinase B/glycogen synthase kinase 3β(PI3K/AKT/GSK3β) pathway after acute myocardial infarction in hyperlipidemic rats. 【Methods】Fifty successfully modeled hyperlipidemic rats were randomly divided into a high-fat sham surgery group (n=16), a high-fat AMI group (n=17), and an ezetimibe group (n=17). Eight rats were randomly selected and fed with regular feed (normal control group). The high-fat AMI group and ezetimibe group rats underwent coronary artery ligation surgery to establish AMI models (the high-fat sham surgery group only had suture without ligation). 48 hours after surgery, there were 10, 9, and 9 surviving rats in the high-fat sham surgery group, high-fat AMI group, and ezetimibe group, respectively. We compared the levels of left ventricular end systolic volume (LVESV), left ventricular end diastolic diameter (LVEDV), and left ventricular ejection fraction (LVEF) in each group of rats after gastric lavage, observed the pathological changes in rat myocardial tissue, compared the apoptosis index (AI) of myocardial cells in each group, and compared the changes in PI3K, p-PI3K, AKT, p-AKT, GSK3β, and p-GSK3β protein levels in myocardial tissue. 【Results】Compared with the normal control group and high-fat sham surgery group, the levels of LVEDV and LVESV increased (P<0.05) and LVEF decreased in the high-fat AMI group and ezetimibe group rats(P<0.05); The LVEDV and LVESV levels in the ezetimibe group were lower than those in the high-fat AMI group (P<0.05), and the LVEF level was higher than that in the high-fat AMI group (P<0.05). Compared with the high-fat AMI group rats, the pathological changes in the myocardial tissue of the ezetimibe group rats were improved. Compared with the normal control group and the high-fat sham surgery group, the AI levels of myocardial cells in the high-fat AMI group and the ezetimibe group were increased (P<0.05); The AI level of myocardial cells in the ezetimibe group was lower than that in the high-fat AMI group (P<0.05). DOI: 10.3969/j.issn.1671-7171.2024.08.014
【Objective】 To observe the efficacy and safety of different surgical approaches in infantile cataract surgery. 【Methods】 A total of 86 cases (137 eyes) of pediatric cataract patients treated in our hospital from July 2015 to September 2019 were included in the study. The patients were divided into an anterior approach group (47 cases) and a posterior approach group (39 cases) according to the surgical approaches. The children were followed up for 3 years after discharge. The axial length and corneal curvature of the two groups were measured and compared preoperatively, 6 months postoperatively, 1 year postoperatively, and 3 years postoperatively. The best corrected visual acuity of one eye and both eyes at the last follow-up visit, as well as the incidence of complications, were also compared between the two groups. 【Results】There were no significant differences in the axial length and corneal curvature between the two groups preoperatively, 6 months postoperatively, 1 year postoperatively, and 3 years postoperatively (P>0.05). The axial lengths at 6 months postoperatively, 1 year postoperatively, and 3 years postoperatively were significantly longer than those preoperatively, and the corneal curvatures were significantly smaller than those preoperatively in both groups (P<0.05). There were no significant differences in the best corrected visual acuity of one eye and both eyes at the last follow-up visit between the two groups (P>0.05). There were no significant differences in the incidences of iris adhesions, intraocular lens displacement, and secondary cataracts between the two groups (P>0.05). The anterior approach group had significantly lower incidences of strabismus and nystagmus than the posterior approach group (P<0.05). 【Conclusion】 Both posterior and anterior approaches in infantile cataract surgery can effectively treat cataracts. The anterior approach can reduce postoperative complications and has better clinical application value.

【Objective】To investigate the clinical efficacy of thalidomide combined with prednisone in the treatment of ulcerative colitis (UC) and its effects on intestinal barrier function and inflammatory indicators in patients.【Methods】A retrospective analysis was conducted on the clinical data of 92 UC patients admitted to our hospital l from April 2021 to March 2023. According to the treatment plan, they were divided into the control group (treated with prednisone in addition to conventional treatment) and the observation group (treated with thalidomide in addition to the control group), with 46 patients in each group. After 8 weeks of treatment, the efficacy, disease-related scores (Mayo endoscopic score, UC endoscopic severity index (UCEIS) score), intestinal barrier function related indicators [diamine oxidase (DAO), blood lactate (LAC), bacterial lipopolysaccharide (LPS), D-lactate (D-LA)], inflammation related indicators [Toll like receptor 4 (TLR-4), nuclear factor kappa B (NF-κB), soluble late glycation end product receptor (sRAGE)], and adverse reactions were compared between the two groups.【Results】The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the Mayo score and UCEIS score of both groups were lower than before treatment, and the observation group was lower than the control group, with statistical significance (P<0.05); The levels of serum DAO, LAC, LPS, and D-LA in both groups were lower than before treatment, and the observation group was lower than the control group, with statistical significance (P<0.05); The levels of NF-κB, TLR-4, and sRAGE in both groups were lower than before treatment, and the observation group was lower than the control group, with statistically significant differences (P<0.05). There was no statistically significant difference in the total effective rate of adverse reactions between the two groups (P>0.05).【Conclusion】The combination of thalidomide and prednisone has a good therapeutic effect on UC, which can reduce intestinal mucosal inflammation, improve intestinal barrier function, and is relatively safe.

WANG Hongzhi
DOI: 10.3969/j.issn.1671-7171.2024.08.027
【Objective】 To explore the value of platelet aggregation rate and blood viscosity in predicting major adverse cardiovascular events (MACE) after percutaneous coronary intervention (PCI) in patients with acute myocardial infarction (AMI). 【Methods】Ninety eight patients with acute myocardial infarction (AMI) who underwent PCI treatment in our hospital from March 2020 to February 2022 were selected. They were divided into an occurrence group (n=16) and a non occurrence group (n=82) based on the occurrence of MACE within 30 days after surgery. Comparing the clinical data, platelet aggregation rate, and blood viscosity of two groups of patients, logistic multiple regression analysis was used to identify the influencing factors of MACE occurrence after PCI. Receiver operating characteristic (ROC) curves were used to analyze the predictive value of platelet aggregation rate and blood viscosity on the risk of MACE occurrence in AMI patients after PCI. 【Results】The incidence of MACE in AMI patients after PCI is 16.33%. The left ventricular ejection fraction was lower in the occurrence group than in the non occurrence group, and the left ventricular end systolic volume, left ventricular end diastolic volume, platelet aggregation rate, and blood viscosity were higher in the occurrence group than in the non occurrence group, with statistically significant differences (P<0.05). Logistic multiple regression analysis showed that platelet aggregation rate and blood viscosity were both related factors affecting the occurrence of MACE in AMI patients after interventional surgery (P<0.05). The sensitivity, specificity, and AUC of platelet aggregation rate, blood viscosity, and their combined prediction of MACE after PCI were 68.75%, 75.00%, and 68.75%, 78.05%, 73.17%, and 97.56%, respectively, and 0.721, 0.748, and 0.887, respectively (P<0.05).【Conclusion】The combination of platelet aggregation rate and blood viscosity has a high predictive value for the occurrence of intracavitary MACE after AMI intervention.

DOI: 10.3969/j.issn.1671-7171.2024.08.028
【Objectives】To investigate the clinical efficacy of entecavir combined with silibinin in the treatment of patients with viral hepatitis B (CHB) complicated with non-alcoholic fatty liver disease (NAFLD) and its impact on liver function. 【Methods】A total of 104 patients with CHB and NAFLD admitted to our hospital from June 2021 to January 2023 were selected and divided into the control group (treated with entecavir in addition to conventional treatment) and the observation group (treated with silibinin in addition to conventional treatment) according to the treatment method, with 52 patients in each group. Two groups were compared in terms of clinical efficacy, liver function [alanine aminotransferase (ALT), aspartate aminotransferase (AST), tumor marker alpha fetoprotein (AFP)], liver fibrosis indicators [laminin (LN), type Ⅲ procollagen (PC-Ⅲ), type Ⅳ collagen (Ⅳ-C), hyaluronic acid (HA)], inflammatory factors [interleukin-6 (IL-6), tumor necrosis factor alpha (TNF-α), nuclear factor kappa B (NF-κB)]. 【Results】The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, ALT, AST, and AFP levels in both groups were lower than before treatment, and the observation group was lower than the control group (P<0.05); The serum levels of LN, PC-Ⅲ,Ⅳ-C, and HA in both groups were lower than before treatment, and the observation group was lower than the control group (P<0.05); The serum levels of IL-6, TNF-α, and NF-κB in both groups were lower than before treatment, and the observation group was lower than the control group (P<0.05).【Conclusion】 The combination of entecavir and silibinin in the treatment of CHB patients with NAFLD can improve liver function, reduce inflammatory response, and alleviate the progression of liver fibrosis.
