641 Efficacy of RPG Global Postural Rehabilitation Technique Combined with Pelvic Floor Electrical Stimulation, Manual Massage for Myofascial Chronic Pelvic Pain

【Objective】To explore the clinical efficacy of RPG global postural rehabilitation technique combined with pelvic floor electrical stimulation, manual massage in patients with myofascial chronic pelvic pain (MFPP).【Methods】A total of 80 female patients diagnosed as MFPP treated in the pelvic floor diagnosis and treatment center of Hunan Maternal and Child Health Hospital from March 2020 to December 2023 were selected and they were divided into the observation group (40 cases) and the control group (40 cases) through the random number table method. All two groups received 1 course of stimulation therapy + manual massage + pelvic floor muscle exercise therapy, while the observation group was also given the RPG global postural rehabilitation training. Treatment efficiency after treatment, degree of pelvic myofascial pain and abnormal posture recovery before and after treatment were compared between the two groups.【Results】The treatment effective rate was 87.50% in the observation group and 80.00% in the control group, with no significant difference between the two groups(P>0.05).The pain of the left side, right side, obturator, piriformis, and the mixed patten showed no statistically significant difference (P>0.05). After treatment, the proportion of patients with moderate and severe pain in both groups was significantly reduced compared with before treatment (P<0.05), and the proportion of patients without pain in the observation group was significantly higher than that in the control group (P<0.05).The treatment effective rate of abnormal posture improvement was 57.33% in the observation group and 37.5% in the control group. There was no statistically significant difference between the two groups (P>0.05).【Conclusion】RPG global postural rehabilitation technique combined with pelvic floor electrical stimulation, manual massage can significantly relieve the pain of MFPP, which is a safe and effective method for the treatment of MFPP. The long-term efficacy needs to be further observed.

645 Effect and Safety of Small Balloon Pre-Dilation before Catheter Thrombolysis in the Treatment of Acute Iliofemoral Vein Thrombosis Patients

【Objective】To explore the efficacy and safety of pre dilation of small balloons before catheter thrombolysis in the treatment of acute iliofemoral vein thrombosis patients.【Methods】A total of 116 patients with acute iliofemoral vein thrombosis admitted to our hospital from January 2018 to December 2021 were selected as the study subjects. According to the intervention method, they were divided into small balloon dilation combined with catheter thrombolysis treatment (observation group,n=53) and direct catheter thrombolysis treatment (control group, n=63). Comparison was made between two groups of patients before treatment and one week after treatment, including the circumference difference between the healthy and affected sides, venous patency score, venous patency rate, prothrombin time (PT), fibrinogen (FIB) levels, activated partial enzyme thrombin time (APTT), thrombin time (TT), and white blood cell count (WBC) D-dimer (D-D) was used to record the incidence of bleeding, thrombolysis time, dosage of urokinase, and length of hospital stay in two groups of patients.【Results】After treatment, the difference in thigh circumference and calf circumference between the two groups of patients was lower than before treatment, and the observation group was lower than the control group, with statistical significance (P<0.05). After treatment, the deep vein patency scores of both groups of patients were lower than before treatment, and the observation group was lower than the control group (P<0.05); The deep vein patency rate and bleeding incidence rate in the observation group were lower than those in the control group, and the difference was statistically significant (P<0.05). After treatment, two groups of patients TT The levels of APTT and PT were higher than before treatment, and the observation group was higher than the control group. The FIB levels in both groups were significantly lower than before treatment, and the observation group was lower than the control group (P<0.05). After treatment, the WBC and D-D levels in the observation group were lower than before treatment, and the observation group was lower than the control group (P<0.05). The thrombolysis time, urokinase dosage, and hospital stay in the observation group were all lower than those in the control group, and the differences were statistically significant (P<0.05).【Conclusion】The use of balloon pre dilation before catheter thrombolysis for the treatment of acute iliofemoral vein thrombosis can improve treatment efficacy, have high safety, and have advantages such as high venous patency rate, low dosage of thrombolytic drugs, short thrombolysis time, and low incidence of bleeding.

649 The Effect of Changes in Posterior Tibial Slope Angle on the Postoperative Efficacy of Fixed Platform Total Knee Arthroplasty

【Objective】To investigate the effect of changes in posterior tibial slope angle (PSA) on the postoperative efficacy of fixed platform total knee arthroplasty (PS-TKA).【Methods】A retrospective analysis was conducted on the clinical data of 138 patients who underwent TKA for the first time at the Xuzhou Medical University Affiliated Hospital from June 2016 to June 2019. According to the average change in PSA before and after surgery, they were divided into Group A (PSA change ≤ 6.4°,n=68) and Group B (PSA change>6.4°, n=70). After one year of postoperative follow-up, two groups of patients were compared in terms of Blackburn Peel Ratio (BPR), Modified Insell Salvati Ratio (mISR), Knee Range of Motion (ROM), and Knee Function Score.【Results】There was no statistically significant difference in the thickness of two sets of polyethylene gaskets (P>0.05); The joint forgetting score (FJS) of Group A was significantly higher than that of Group B, and the difference was statistically significant (P<0.05); The change in PSA before and after surgery in Group A was (4.8±2.1)°, significantly lower than that in the control group (10.3±2.4)°, and the difference was statistically significant (P<0.05). Postoperative PSA, mISR, BPR, VAS scores, Western Ontario and McMaster University Osteoarthritis Index (WOMAC) scores of the two sets were significantly lower than preoperative (P<0.05); Postoperative ROM, American Knee Society score (KSS) Clinical Score and the KSS functional score were significantly higher than preoperative (P<0.05). Postoperative PSA, ROM, KSS clinical score and the KSS functional score in Group A were significantly higher than those of Group B; Postoperative VAS score and WOMAC score of Group A was lower than those of Group B; However, there was no statistically significant difference between the postoperative mISR score and BPR score between the two groups (P>0.05).【Conclusion】The smaller the change in tibial plateau tilt angle after fixed platform total knee arthroplasty, the greater the postoperative knee joint mobility, and the better the clinical scoring effect, which will bring better postoperative efficacy.

653 The Predictive Efficacy of Different Nutritional Screening Tools on Postoperative Outcomes of Elderly Hip Fractures

【Objective】To explore the predictive efficacy of different nutritional screening tools on the postoperative outcomes of elderly hip fractures.【Methods】Clinical data of 314 patients who were hospitalized in the orthopedics department of the Zhangjiakou Second Hospital from January 2017 to December 2019 were collected. All patients were evaluated for their nutritional status using the Nutrition Risk Screening 2002 (NRS-2002), the malnutrition universal screening tool (MUST), the mini nutrition assessment-short form(MNA-SF), and the geriatric nutrition risk index (GNRI) before surgery. All patient clinical data were recorded, and the function independent measure (FIM) scale was used to evaluate daily living activities on the first day after surgery and at discharge. The FIM rehabilitation efficacy was calculated [(difference in FIM scores on the first day after surgery and discharge)/postoperative hospital stay], and the average walking speed of 15 meters at 14 days after surgery was evaluated. The correlation analysis between different nutritional status and prognostic indicators of various nutritional screening tools was conducted.【Results】A total of 343 patients were admitted, of which 29 were excluded and 314 met the inclusion criteria. The age was (81.54±7.34) years old, with 236 females (75.16%). According to MNA-SF nutritional screening, 80 cases (25.48%) were well nourished, 160 cases (50.96%) were at risk of malnutrition, and 74 cases (23.57%) were at risk of malnutrition; According to MUST nutritional screening, 145 cases (46.18%) were at low risk, 72 cases (22.93%) were at medium risk, and 97 cases (30.89%) were at high risk; According to NRS-2002 nutritional screening, 135 cases (42.99%) were well nourished, 101 cases (32.17%) had moderate risk, and 78 cases (24.84%) had malnutrition; According to GNRI nutritional screening, 68 cases (21.66%) were at no risk, 114 cases (36.31%) had mild risk, and 132 cases (42.04%) had significant risk. There was a statistically significant difference in the proportion of different nutritional status among various screening tools (χ2=45.82, P<0.001). There was a correlation between MNA-SF and discharge FIM score; Compared with good nutrition, both the risk of malnutrition and malnutrition had a negative impact on the discharge FIM score,FIM rehabilitation efficacy and average walking speed of 15 meters (P<0.05). NRS-2002 was only related to the discharge FIM score. When using GNRI evaluation, compared with no risk, minor and major risks only had a negative impact on the average walking speed of 15 meters (P<0.05), and were not correlated with the rehabilitation efficacy of FIMs and the average walking speed of 15 meters (P>0.05).There was no correlation between MUST and prognostic indicators (P>0.05).【Conclusion】In the acute phase of elderly patients with hip fractures, MNA-SF is the most relevant nutritional screening tool for early functional prognosis.

658 Correlation Analysis and Preventive Intervention Measures between Biological Parameters of the Eyeball and Myopia in Children

【Objective】To explore the correlation between biological parameters of the eyeball and myopia in children and preventive interventions.【Methods】A total of 500 children diagnosed and treated in the ophthalmology outpatient department of our hospital from January 2019 to December 2021 were selected. They were divided into myopia group and non myopia group based on whether they were nearsighted. The biological parameters of the eyeballs in the two groups were compared in terms of Axial length (AL), axial length to corneal curvature ratio (AL/CR), lens thickness (LT), white to white distance (WTW), corneal thickness (CT), vitreous cavity length (VCL), and intraocular pressure (IOP), gender, age, and behavioral patterns (table and chair height, time spent using electronic devices, outdoor exercise time, and eye use time); Logistic regression was used to analyze the influencing factors of myopia in children; Pearson analysis was used to investigate the correlation between myopia in children and ocular biological parameters.【Results】The AL, AL/CR, LT, CT,VCL, Age and behavior patterns of two groups of were compared, and the differences were statistically significant (all P<0.05).The results of logistic regression analysis showed that: AL, AL/CR, LT, CT, VCL, Age and behavioral patterns were both influencing factors for myopia in children. Pearson correlation analysis shows that AL, AL/CR, VCL were positively correlated with myopia in children (r=0.472, 0.369, 0.449, all P<0.05); LT, CT were negatively correlated with myopia in children (r=-0.210, -0.170, P<0.05).【Conclusion】Eye biological parameters, age, and behavioral habits are closely related to myopia in children.

661 Construction and Validation of a Nomogram Model for the Clinical Efficacy of Budesonide-Formoterol Combined with Pirfenidone Capsules in the Treatment of COPD with Pulmonary Interstitial Fibrosis

【Objective】To explore the efficacy of budesonide-formoterol combined with pirfenidone capsules in treating Chronic Obstructive Pulmonary Disease (COPD) with pulmonary interstitial fibrosis, and at constructing and validating a nomogram model for predicting clinical efficacy.【Methods】From September 2019 to December 2023, 107 patients with COPD combined with pulmonary interstitial fibrosis, who were treated with budesonide-formoterol combined with pirfenidone capsules in the Department of Respiratory and Critical Care Medicine at the First Affiliated Hospital of Henan University, were selected. Based on treatment outcomes, they were divided into an improvement group (markedly effective+effective) and an ineffective group (ineffective). Clinical data and laboratory results of both groups were analyzed through univariate analysis, and factors of statistical significance were subjected to multivariate regression analysis to construct a nomogram for identifying factors influencing treatment outcomes.【Results】Out of the 107 patients included, 83 were in the improvement group (77.57%) and 24 in the ineffective group (22.43%). There were no statistically significant differences between the two groups in terms of age, gender, BMI, history of alcohol abuse, diabetes, hypertension, heart rate, white blood cell count, PaO2, PaCO2, lactate, and CRP (P>0.05). Significant differences were found between the improvement and ineffective groups regarding smoking history, pulmonary function classification, COPD duration, APACHEⅡscore, and NT-proBNP (P<0.05). Multivariate regression analysis revealed that a history of smoking, pulmonary function classification of Ⅲ-Ⅳ,long duration of COPD, high APACHEⅡ score, and high NT-proBNP were independent risk factors for ineffective treatment in patients with COPD combined with pulmonary interstitial fibrosis (P<0.05). A nomogram was constructed, demonstrating good predictive performance.【Conclusion】The risk factors affecting the efficacy of budesonide-formoterol combined with pirfenidone capsules in treating patients with COPD combined with pulmonary interstitial fibrosis were identified as a history of smoking, pulmonary function classification of Ⅲ-Ⅳ, long duration of COPD, high APACHEⅡ score, and high NT-proBNP. These can serve as predictive tools for the efficacy in patients with COPD combined with pulmonary interstitial fibrosis, providing a reference for clinical treatment decision-making.

666 Application of Liver Stiffness, Serum CER and AST Levels in Evaluating Liver Fibrosis of Patients with ALT Elevated Hepatitis B

【Objective】To explore the value of ultrasound in detecting liver stiffness (LSM), serum ceruloplasmin (CER) and aspartate aminotransaminase (AST) in evaluating liver fibrosis of patients with mild elevation of alanine aminotransferase (ALT).【Methods】From June 2020 to June 2022, 100 patients with hepatitis B showing slightly elevated ATL admitted to our hospital were divided into non-fibrosis group (n=10) and fibrosis group (n=90) according to the stage of liver fibrosis. According to the severity of liver fibrosis, patients were divided into severe fibrosis group (n=42), moderate fibrosis group (n=36), and mild fibrosis group (n=12). The value of ALT, albumin (ALB), AST and CER, LSM was compared in different groups of patients. The ROC curve of subjects was drawn to analyze the evaluation value of CER, AST, LSM on liver fibrosis in hepatitis B patients with mild elevation of ALT.【Results】There was no statistically significant difference in ALT and ALB between the two groups of fibrosis and non-fibrosis patients (P>0.05); The AST and LSM of the fibrosis group were higher than those of the non-fibrosis group, and the CER of the fibrosis group was lower than that of the non-fibrosis group, with statistical significance (P<0.05). ROC curve analysis showed that the area under the curve of AST, CER and LSM for evaluating liver fibrosis in hepatitis B patients with slightly elevated ATL was 0.914, 0.616 and 0.946, respectively (P<0.05). The CER of the severe fibrosis group, the moderate fibrosis group, and the mild fibrosis group gradually increased, and the difference was statistically significant (P<0.05); The AST and LSM gradually decreased in the severe fibrosis group, moderate fibrosis group, and mild fibrosis group, and the difference was statistically significant (P<0.05).【Conclusion】There are differences in AST, CER and LSM in patients with hepatitis B with slightly elevated ATL at different levels of fibrosis. AST, CER and LSM have certain evaluation value for liver fibrosis and can be used for early diagnosis or evaluation of liver fibrosis in such patients.

669 Effect of Simulated Sleep on the Expression of Circadian Clock Genes Bmal1, Per2, Immune Function, and Short-term Clinical Outcomes in Patients with Sepsis

【Objective】To explore the effect of simulated sleep on the expression of circadian clock genes Brain and Muscle ARNT-Like 1 (Bmal1) and Period 2 (Per2), immune function, and short-term clinical outcomes in patients with sepsis.【Methods】Eighty sepsis patients were randomly divided into the control group (with standard symptomatic treatment) and the observation group (with standard symptomatic treatment combined with simulated sleep intervention), with 40 patients in each group. The study compared the levels of inflammatory factors [interleukin-6 (IL-6), IL-8, tumor necrosis factor-alpha (TNF-α), and C-Reactive Protein (CRP)], Bmal1 and Per2 expression, immune function, Sequential Organ Failure Assessment (SOFA) score, Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ) score, mechanical ventilation duration, ICU stay duration, and 28-day mortality rate between the two groups before treatment and 7 days after treatment.【Results】After 7 days of treatment, both groups showed reduced levels of IL-6, IL-8, TNF-α, and CRP compared to before treatment (P<0.05), with the observation group presenting even lower levels than the control group (P<0.05). Both groups exhibited increased expression of Bmal1 and Per2 mRNA (P<0.05) after 7 days of treatment compared to before treatment, with higher expression of Bmal1 and Per2 in the observation group than the control group (P<0.05). The levels of CD4+, CD4+/CD8+ increased, while CD8+ decreased in both groups compared to before treatment (P<0.05), and the observation group showed higher CD4+, CD4+/CD8+ and lower CD8+ than the control group (P<0.05). Both groups had reduced SOFA and APACHE Ⅱ scores compared to before treatment (P<0.05), with lower scores in the observation group (P<0.05). The observation group had shorter mechanical ventilation and ICU stay durations (P<0.05), but the difference in 28-day mortality rate was not statistically significant between the two groups (P>0.05).【Conclusion】Simulated sleep can improve the dysregulation of circadian clock genes Bmal1 and Per2 in sepsis patients, enhance immune function, reduce systemic inflammatory response, and improve short-term clinical outcomes.

674 Clinical Efficacy of Skull Repair Surgery for Children with Traumatic Skull Defects at Different Times

【Objective】To explore the clinical efficacy of skull repair surgery at different times in the treatment of children with traumatic skull defects.【Methods】Sixty children with traumatic skull defects admitted to our hospital from June 2017 to September 2023 were selected. All children underwent decompressive craniectomy, and skull repair surgery was performed at a selected time after surgery. They were divided into the control group (n=35) and the observation group (n=25) based on the time of postoperative skull repair surgery. The control group patients underwent skull repair surgery within 3-6 months after decompressive craniectomy, while the observation group patients completed skull repair surgery within 2 months after decompressive craniectomy. We compared the quality of life, self-care ability, neurological function, and motor function recovery of two groups of patients before and after treatment, and compared the clinical efficacy and postoperative complications of the two groups of patients.【Results】After treatment, the quality of life scores of both groups of patients were higher than before treatment, and the observation group was higher than the control group, with a statistically significant difference (P<0.05). After treatment, the Barthel index and Fugl-Meyer score in both groups were higher than before treatment, and the observation group was higher than the control group; The NIHSS score was lower than before treatment, and the observation group was lower than the control group, with a statistically significant difference (P<0.05). The incidence of postoperative complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). The total effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05).【Conclusion】Early skull surgery for children with traumatic skull defects can improve their quality of life, improve their neurological and motor functions, and have good clinical effects.

677 The Relationship among Serum 25-Hydroxyvitamin D, Low-Density Lipoprotein Cholesterol and Hypertension Grading in Hypertensive Patients

【Objective】To explore the relationship among serum 25-hydroxyvitamin D [25-(OH)D] level, low-density lipoprotein cholesterol (LDL-C) level, and the classification of hypertension in hypertensive patients.【Methods】According to the hypertension grading standards, 371 hypertensive patients treated in our hospital were divided into grade 1 hypertension (120 cases), grade 2 hypertension (124 cases) and grade 3 hypertension (127 cases). While 391 healthy individuals undergoing physical examinations at our hospital during the same period were selected as the control group. General information of all participants was collected. Fasting blood glucose (FBG), triglycerides (TG), total cholesterol (TC), creatinine (Cr), high-density lipoprotein cholesterol (HDL-C), 25-(OH)D, and LDL-C levels were measured. Correlation analysis was conducted between 25-(OH)D and LDL-C levels. Multifactorial logistic regression analysis was used to analyze the risk factors influencing the occurrence of hypertension.【Results】Compared to the control group, the hypertensive group showed significantly higher levels of body mass index (BMI), smoking proportion, systolic blood pressure (SBP), diastolic blood pressure (DBP), FBG, and LDL-C (P<0.05); while the level of 25-(OH)D in the hypertensive group was significantly lower than that in the control group (P<0.05). The BMI, FBG, and LDL-C of hypertensive patients increased with the grade of hypertension (P<0.05), meanwhile the level of 25-(OH)D decreased with the grade of hypertension (P<0.05). The correlation analysis showed that patients' BMI, smoking, SBP, DBP, and FBG were positively correlated with serum LDL-C level (P<0.05) and negatively correlated with 25-(OH)D level (P<0.05). Multifactorial logistic regression analysis showed that FBG and LDL-C were independent risk factors for hypertension (P<0.05), while HDL-C and 25-(OH)D were protective factors against hypertension (P<0.05).【Conclusion】Low level of 25-(OH)D and high level of LDL-C in hypertensive patients are closely related to the blood pressure classification (grades) of hypertension.

681 Effect of Diabetic Retinopathy Degree and Glycosylated Hemoglobin Level on Corneal Endothelium in Diabetic Patients

【Objective】To investigate the effect of diabetic retinopathy (DR) and glycosylated hemoglobin A1c (HbA1c) on corneal endothelium in diabetic patients.【Methods】A total of 167 patients (334 eyes) with cataract diagnosed and treated in the Ophthalmology Department of Yuhuangding Hospital from February 2023 to June were selected in the study. Among them, 117 patients with diabetes (234 eyes) were included in the observation group, and 50 patients without diabetes (100 eyes) were included in the control group. According to the clinical stage of DR, the observation group was divided into no DR group (NDR group), non-proliferative diabetic retinopathy group (NPDR group) and proliferative diabetic retinopathy group (PDR group). According to the HbA1c results after admission, the patients were further divided into group A (HbA1c < 7.5%), group B (7.5%≤HbA1c < 8.5%), group C (8.5%≤HbA1c < 9.5%) and group D (HbA1c≥9.5%). Endothelial cell microscopy was used to detect the corneal endothelial density (ECD), cell area standard deviation (SD), and variation coefficient(CV), hexagonal cell percentage (HCR) and central corneal thickness (CCT) in all patients for comparison.【Results】Compared with the control group, the ECD of the observation group decreased, while SD and CV increased; And the difference was statistically significant (P<0.05) There was no statistically significant differences in HCR and CCT between the two groups (P>0.05). Compared with the NDR group and the NPDR group, ECD in the PDR group was significantly reduced, and the differences were statistically significant (P<0.05), however, there was no statistically significant difference between ECD in the NDR group and the NPDR group (P>0.05). There were statistically significant differences in CV of the NPDR group and the PDR group (P<0.05), if compared to the NDR group; But there were no statistically significant differences in SD of the NPDR group and the PDR group with the NDR group (P>0.05). There were no statistically significant differences in SD and CV between the NPDR group and the PDR group (P>0.05). There were no significant differences in HCR and CCT among the NPDR group, the PDR group and the NDR group (P>0.05). With the increase of HbA1c level, ECD in groups A, B, C and D decreased in order, and there were statistically significant differences in ECD and SD in groups B, C and D compared with group A (P<0.05), though there were no statistically significant differences in ECD and SD among groups B, C and D (P>0.05). There was statistical significance in CV between group A and group C (P<0.05), but there was no statistical significance in CV of group A comparing with group B and D (P>0.05). There were no significant differences in HCR and CCT among group A, group B, group C and group D (P>0.05). Age was negatively correlated with CCT (P<0.05). The duration of diabetes was negatively correlated with ECD (P<0.05), and positively correlated with SD and CV (P<0.05). The level of HbA1c and the severity of DR Were negatively correlated with ECD (P<0.05).【Conclusion】The increase of the degree of DR lesions and the increase of HbA1c level can lead to the decrease of ECD and the increase of CV. The number and morphology of corneal endothelial cells will change with the prolonged course of diabetes and unstable blood glucose control, which can lead to a decline in corneal endothelial cell function.

685 Correlation of the Severity of Parkinson's Disease with Serum Levels of cys C, UA, Hcy, and NSE

【Objective】To explore the correlation of the severity of Parkinson's disease with the serum levels of cystatin C (cys C), uric acid (UA), homocysteine (Hcy), and neuron-specific enolase (NSE).【Methods】A retrospective analysis was conducted on the clinical data of 120 Parkinson's disease patients. Patient demographics, lipid profiles, coagulation markers, and levels of cys C, UA, Hcy, and NSE were statistically analyzed. Multivariate logistic regression was used to identify independent risk factors affecting the severity of the disease, while the Spearman test was utilized to analyze the correlation between these biomarkers and disease severity.【Results】Of the 120 patients, 43 (35.83%) were in the early stage of the disease, classified as the early group; the remaining 77 (64.17%) were in the middle to late stages, classified as the middle-late group. Comparisons of disease duration, age, gender, hemoglobin, platelets, coagulation markers, blood pressure, and lipid levels between the groups showed no statistically significant differences (P>0.05). The levels of cys C, Hcy, and NSE were significantly higher in the middle-late stage group compared to the early stage group (P<0.05), while UA level in the middle-late stage group was lower than that in the early stage group (P<0.05). Multivariate logistic regression analysis indicated that cys C, Hcy, and NSE were independent risk factors for the severity of Parkinson's disease (P< 0.05), whereas UA served as a protective factor (P<0.05). Spearman test results showed a positive correlation of disease severity with levels of cys C, Hcy, and NSE, while a negative correlation of the severity with UA (P<0.05).【Conclusion】cys C, Hcy, and NSE are risk factors that affect the severity of Parkinson's disease, while UA acts as a protective factor. The levels of these biomarkers are closely related to the severity of the condition, which should be paid good attention clinically.

【Objective】To explore the molecular mechanisms of the N-cadherin and the Wnt/β-catenin pathway facilitate drug resistance in leukemia stem cells (LSC) and mesenchymal stem cells (MSC).【Methods】The study included 65 patients with acute myeloid leukemia (AML), of whom 41 achieved complete remission (CR group) and 24 did not (non-remission, NR group). The expression of N-cadherin in CD34+CD38- stem cells was compared between these two groups. The CD34+CD38- Kg1α cells were further divided into six groups based on different treatments with MSC or idarubicin (IDA) that cell proliferation, apoptosis, and levels of N-cadherin and β-catenin were assessed. The cell adhesion and anti-apoptotic capabilities were also evaluated.【Results】The N-cadherin expression in the NR group compared was significantly higher than that in the CR group (P<0.01).The apoptosis rate in the LSC+IDA group (Group B) was significantly higher than that in the LSC and MSC co-culture + IDA group (Group D) (P<0.05). At IDA concentrations of 100 and 200 nmol/L, the proliferation inhibition rate in Group B was significantly higher than that in Group D (P<0.05). The colony formation rate in Group B was significantly lower than that in Group D (P<0.05). The Western blot results showed that the expression levels of both N-cadherin and β-catenin were higher in direct co-culture condition of MSC than in solo cultures or non-contact co-cultures. In co-culture condition with MSC, LSC showed increased expression levels of N-cadherin and β-catenin, along with an increase in nuclear β-catenin level.【Conclusion】MSCs in the bone marrow microenvironment can support the proliferation of LSC through N-cadherin-mediated adhesion and promote the activation of the Wnt/β-catenin pathway in LSC, thereby enabling them to evade the cytotoxic effects of chemotherapy.

693 Clinical Effect of Yinchenhao Decoction on Postoperative Diabetes Foot

【Objective】To investigate the clinical efficacy of Yinchenhao Decoction in the treatment of diabetes foot after operation.【Methods】The clinical data of 95 patients with diabetes foot who underwent surgical treatment in our hospital from January to December 2023 were retrospectively analyzed. According to the different treatment methods, they were divided into the control group (n=47, conventional treatment) and the observation group (n=48, conventional treatment + Yinchenhao Decoction modified treatment). We compared the clinical efficacy, postoperative recovery (bed rest time, bed rest time, anal exhaust time, hospital stay), wound improvement (wound area and depth, granulation tissue growth degree, wound healing time), and incidence of adverse reactions between two groups,and the symptom scores and serum inflammatory factors[ C-reactive protein(CRP), tumor necrosis factor-α(TNF-α), interleukin-4(IL-4), interleukin-5(IL-5)],blood glucose [fasting blood glucose(FPG), glycated hemoglobin (HbA1c)] and the levels of hemorheological indicators[erythrocyte sedimentation rate (ESR), fibrinogen (Fib), and plasma viscosity (PV) ]before and after treatment were statistically analyzed for both groups.【Results】The total effective rate of the observation group was 93.75%, significantly higher than the control group's 78.72% (P<0.05). Compared with the control group, the observation group had a shorter postoperative recovery time (P<0.05). After treatment, the wound area and depth of both groups decreased, and the observation group showed a more significant decrease (P<0.05); The growth degree of granulation tissue in the observation group was greater than that in the control group, and the wound healing time was shorter than that in the control group (P<0.05). After treatment, the levels of the symptom scores FPG, HbA1c, CRP, TNF-α, ESR, Fib, and PV for both groups were all reduced, and the observation group was lower than the control group (P<0.05); The levels of IL-4 and IL-5 both increased, and the observation group was higher than the control group (P<0.05). Both groups of patients showed no significant adverse reactions.【Conclusion】Yinchenhao Decoction can promote postoperative recovery and wound repair of diabetes foot, control blood sugar level, relieve symptoms, alleviate inflammatory reaction, improve hemorheology, improve efficacy, and will not increase the incidence of adverse reactions.

697 Clinical Efficacy of Jicheng Yushang Tablets in the Treatment of Osteoporotic Thoracolumbar Compression Fractures

【Objective】To explore the clinical efficacy of Jicheng Yushang tablets in the treatment of osteoporotic thoracolumbar vertebral compression fractures (OVCF).【Methods】A total of 64 OVCF patients who underwent conservative treatment in our hospital from June 2022 to June 2023 were selected and randomly divided into an observation group and a control group, with 32 cases in each group, using a random number table method. The control group was treated with calcium carbonate D600 tablets combined with alpha calciferol in addition to basic treatment, while the observation group was treated with our hospital's Jicheng Yushang with callus tablets in addition to the control group, both of which were treated for 3 months.We compared the visual analogue scale (VAS) scores Oswestry Disability Index (ODI), Traditional Chinese Medicine Symptoms and Signs/TCM Syndrome Score between two groups before treatment, 1 week after treatment, 4 weeks after treatment, and 3 months after treatment. The two groups were compared in terms of bone density, Cobb angle of injured vertebrae, and recovery rate of injured vertebrae height and Cobb angle improvement rate before and after 3 months of treatment.【Results】After 3 months of treatment, the VAS score of the observation group ODI,the scores of traditional Chinese medicine symptoms and signs/syndromes were better than those of the control group (P<0.05). However, in the early stages (after 1 week and 4 weeks of treatment), there was no statistically significant difference in the above indicators between the two groups (P>0.05). After 3 months of treatment, the bone density value in the observation group was significantly improved compared to the control group (P<0.05), and the recovery rate of injured vertebral height in both groups after 3 months of treatment was not significant, there was no statistically significant difference in the improvement rate of Cobb angle (P>0.05).【Conclusion】Jicheng Yushang tablets can alleviate pain, improve functional impairments, symptoms, and signs in patients with OVCF, and are worthy of clinical promotion and application.

700 The Predictive Value of Preoperative Serum Lp-PLA2 and NLRP3 Levels for MACE in High-Risk Coronary Heart Disease Patients Treated with IABP Assisted PCI

【Objective】To explore the predictive value of preoperative serum lipoprotein associated phospholipase A2 (Lp-PLA2) and nucleotide binding oligomeric domain like receptor protein 3 (NLRP3) levels for cardiovascular adverse events (MACE) in high-risk coronary heart disease patients undergoing intra-aortic balloon counterpulsation (IABP) assisted percutaneous coronary intervention (PCI).【Methods】A total of 120 high-risk coronary heart disease patients admitted to our hospital were selected and all underwent IABP assisted PCI treatment. The incidence of MACE within 6 months after surgery was statistically analyzed and divided into the non MACE group and the MACE group. The preoperative serum Lp-PLA2 levels were compared between the two groups NLRP3 level. The correlation between preoperative serum Lp-PLA2 and NLRP3 levels and the degree of coronary stenosis were analyzed. The receiver operating characteristic (ROC) curve was used to analyze the value of preoperative serum Lp-PLA2 and NLRP3 level in predicting postoperative MACE.【Results】The incidence of MACE in 120 high-risk coronary heart disease patients after IABP assisted PCI was 34.17% (41/120); Preoperative serum Lp-PLA2 in the MACE group The levels of NLRP3 were higher than those in the non MACE group (P<0.05). Spearman correlation analysis showed that preoperative serum Lp-PLA2 The level of NLRP3 was positively correlated with the degree of coronary stenosis (rs=0.750, 0.815, all P<0.05). The level of preoperative serum Lp-PLA2 and NLRP3 in severe patients was higher those in moderate and mild patients, with moderate patients having higher levels than mild patients (P<0.05). ROC curve analysis showed that the area under the curve predicted by preoperative serum Lp-PLA2 and NLRP3 level alone was 0.770 and 0.844, respectively. The joint prediction of AUC by the two was 0.909 (P<0.05).【Conclusion】The level of Preoperative serum Lp-PLA2 and NLRP3 was positively correlated with the degree of coronary artery stenosis in high-risk coronary heart disease patients. The combined detection of the two can provide a certain reference for clinical prediction of MACE in high-risk coronary heart disease patients after IABP assisted PCI surgery.

704 Effect of Pre-Chemotherapy Blood Lipid Levels on Liver Dysfunction after Chemotherapy in Patients with Esophageal Cancer

【Objective】To explore the impact of blood lipid levels before the first chemotherapy on liver dysfunction after chemotherapy in patients with esophageal cancer.【Methods】Eighty-four esophageal cancer patients with normal liver function undergoing their first chemotherapy were divided into two groups based on pre-chemotherapy blood lipid levels: the abnormal lipid group (n=49) and the normal lipid group (n=35). The occurrence of liver dysfunction after chemotherapy was compared between the two groups. Logistic regression analysis was used to assess the impact of pre-chemotherapy blood lipid levels on post-chemotherapy liver dysfunction.【Results】Before chemotherapy, there were no significant differences in serum alanine aminotransferase (ALT), aspartate aminotransferase (AST), and alkaline phosphatase (ALP) levels between the abnormal and normal lipid groups (P>0.05). Patients in the abnormal lipid group had higher levels of total cholesterol (TC), triglycerides (TG), and low-density lipoprotein cholesterol (LDL-C) (P<0.05), and lower level of high-density lipoprotein cholesterol (HDL-C) compared to the normal lipid group (P<0.05). The occurrence rate of liver dysfunction after chemotherapy was 44.90%(22/49) in the abnormal lipid group, which was higher than that in the normal lipid group at 20.00%(7/35) (P<0.05).After chemotherapy, patients with liver dysfunction had higher serum TC, TG, and LDL-C levels, and lower HDL-C level compared to those with normal liver function (P<0.05). Serum TC, TG, LDL-C, and HDL-C were independent risk factors for liver dysfunction after chemotherapy in patients with esophageal cancer (P<0.05).【Conclusion】Patients with esophageal cancer who have abnormal blood lipid parameters before the first chemotherapy are at a higher risk of developing liver dysfunction afterwards. Clinicians should strengthen preventive measures.

707 Clinical Application Value of Multimodal Ultrasound in Cervical Lymph Node Metastasis of Papillary Thyroid Carcinoma

【Objective】To explore the clinical application value of multimodal ultrasound in cervical lymph node metastasis of papillary thyroid carcinoma.【Methods】A retrospective analysis was conducted on the clinical data of 90 patients with PTC and cervical lymph node enlargement admitted to our hospital from August 2021 to July 2023. All selected patients underwent DUS CUES, UE inspection. Using pathological results as reference standards, DUS was analyzed Sensitivity, specificity, and accuracy of CUES and UE single and combined examinations in diagnosing cervical lymph node metastasis in PTC. The ultrasound characteristics of PTC patients with cervical lymph node metastasis and those without metastasis were analyzed.【Results】The pathological results of 90 patients with PTC and cervical lymph node enlargement showed that 55 patients (61.11%) had cervical lymph node metastasis, and 35 patients (38.89%) did not have cervical lymph node metastasis. The accuracy of DUS, CUES, and UE combined examination in diagnosing PTC with cervical lymph node metastasis was 92.22%(83/90), which was significantly higher than DUS (73.33%), CUES (80.00%) and UE(81.11%) single check, the difference was statistically significant (P<0.05). There was no significant difference in the sensitivity and specificity of joint diagnosis compared with CUES, but they were all higher than DUS, UE single item examination with statistically significant differences (P<0.05). The blood supply pattern, lesion tissue location, perfusion uniformity, elasticity score, edge regularity, and microcalcification of patients in the metastatic group were compared with those in the non metastatic group, and the differences were statistically significant (P<0.05).【Conclusion】The application of multimodal ultrasound in the clinical diagnosis of PTC cervical lymph node metastasis has high diagnostic efficacy. Clinical attention should be paid to lesions that exhibit peripheral blood flow patterns, lesions located in the upper and middle parts of the neck, irregular edges, microcalcifications, and other ultrasound features to improve diagnostic accuracy.

711 Correlation Analysis of Serum miR-345, miR-138 and miR-22 with Chemotherapy Sensitivity in Advanced Gastric Cancer Patients

【Objective】To explore serum microRNA-345 (miR-345) The correlation between miR-138 and miR-22 and chemotherapy sensitivity in advanced gastric cancer patients. 【Methods】A total of 100 patients with advanced gastric cancer who received the same chemotherapy regimen were divided into an effective group of 69 cases and an ineffective group of 31 cases based on their efficacy. The clinical and pathological characteristics and serum related indicators of the two groups were compared, and the receiver operating characteristic (ROC) curve was plotted. The serum miR-345 was analyzed. The value of miR-138 and miR-22 in predicting the chemotherapy efficacy of advanced gastric cancer was analyzed using multivariate logistic regression to identify the factors that affect the chemotherapy efficacy of patients.【Results】The relative expression level of serum miR-345 in stage Ⅲ patients was low than those of stage Ⅳ patients (P<0.05). The relative expression levels of miR-138 and miR-22 were higher than those of stage Ⅳ patients (P<0.05); miR-345 in the effective group was lower than that in the ineffective group, miR-138 and miR-22 were higher than those in the ineffective group (P<0.05). Confirmed by ROC analysis, serum miR-345, miR-138 and miR-22 can be used to predict the chemotherapy efficacy of advanced gastric cancer, with sensitivity and specificity of 0.855, 0.935, 0.971, 0.839, and 0.884, 0.903, respectively (all P<0.05). Multivariate logistic regression analysis showed that the pathological stage Ⅳ, miR-345 ≥ 14.5, miR-138 ≤ 2, and miR-22 ≤ 3.12 were risk factors for poor chemotherapy efficacy in advanced gastric cancer patients (P<0.05).【Conclusion】Serum miR-345≥14.5, miR-138 ≤ 2 and miR-22 ≤ 3.12 are both risk factors for poor chemotherapy efficacy, and can be used as biological markers to evaluate the chemotherapy efficacy of patients.

【Objective】To explore the risk factors of postpartum infection in patients with scarred uterus after cesarean section.【Methods】A retrospective analysis was conducted on the clinical data of 80 patients with scarred uterus admitted to our hospital from January 2018 to October 2023. All patients underwent cesarean section again and were divided into an infected group (n=20) and an uninfected group (n=60) based on whether they developed postpartum infection. Two sets of general information were compared, including the presence of reproductive tract infections during pregnancy and the presence of upper respiratory tract infections; Multiple logistic regression analysis was used to identify the risk factors for postpartum infection in patients with scarred uterus after cesarean section.【Results】There was no statistically significant difference in age, body mass index (BMI), gestational age, surgical time, anemia, presence of placenta previa, gestational hypertension, hyperlipidemia, proportion of upper respiratory tract infections, and gestational age between the two groups (P>0.05); The proportions of premature rupture of membranes, postpartum hemorrhage, diabetes in pregnancy, and reproductive tract infection in pregnancy in the infected group were significantly higher than those in the uninfected group (P<0.05). Multivariate logistic regression analysis showed that premature rupture of membranes, postpartum hemorrhage, pregnancy diabetes, pregnancy genital tract infection were the risk factors for puerperal infection after cesarean section in women with scar uterus (all P<0.05).【Conclusion】The puerperal infection of patients with scar uterus after cesarean section is affected by many factors, among which premature rupture of membranes, postpartum hemorrhage, pregnancy diabetes, pregnancy genital tract infection can predict the puerperal infection of such patients after cesarean section, which should be paid close attention to clinically.

718 The Effect of Zishen Runfeifang in Combined with Chemotherapy on Clinical Efficacy with Advanced Lung Cancer Exhibiting Both Qi and Yin Deficiencies

【Objective】To explore the efficacy of Zishen Runfeifang in combined with chemotherapy in patients with advanced lung cancer exhibiting both Qi and Yin deficiencies.【Methods】Eighty patients with advanced lung cancer exhibiting both Qi and Yin deficiencies, who received treatment in our two hospitals, were randomly divided into a chemotherapy group and a combination group, with 40 patients in each group. The chemotherapy group received conventional lung cancer chemotherapy, while the combination group received Zishen Runfeifang orally in addition to the chemotherapy. The efficacy, time to disease progression (TTP), 1-year survival rate, 1-year progression-free survival (PFS) rate, improvements in Traditional Chinese Medicine symptom scores, and Karnofsky Performance Scale (KPS) scores before and after treatment, as well as the occurrence of bone marrow suppression, were compared between the two groups.【Results】The total effective rate (RR) in the combination group was 37.5% (15/40) and the disease control rate (DCR) was 82.5% (33/40), whereas the chemotherapy group had an RR of 27.5%(11/40) and a DCR of 70.0%(28/40). The differences between the groups were not statistically significant(P>0.05). The TTP in the chemotherapy group was 6.5(4.2-8.4) months, with a 1-year survival rate of 30.0%(12/40), and a 1-year PFS rate of 85.0%(34/40); the combination group had a TTP of 7.2(5.5-9.3) months, a 1-year survival rate of 35.0%(14/40), and a 1-year PFS rate of 87.5%(35/40). The differences between the two groups were not statistically significant(P>0.05). Compared to before treatment, both groups showed a significant reduction in Traditional Chinese Medicine symptom scores after treatment, with more significant reductions in the combination group(P<0.05). After treatment, the stability rate of KPS scores in the combination group was 90.00%(36/40), significantly higher than the 70.00%(28/40) in the chemotherapy group(P< 0.05). The total occurrence rate of bone marrow suppression in the combination group was 17.5%(7/40), lower than 42.5%(17/40) in the chemotherapy group.【Conclusion】Zishen Runfeifang combined with chemotherapy can effectively improve patients' quality of life and reduce the occurrence rate of bone marrow suppression.

Serum inflammatory factors [interleukin-6(IL-6), tumor necrosis factor-α(TNF-α), C-reactive protein(CRP)], blood gas analysis indicators [PaO2, SaO2 and PaCO2], acute physiology and chronic health status score System Ⅱ(APACHEⅡ) score were compared as well.【Results】The total effective rate of the observation group was 90.91%, which was significantly higher than 76.36% of the control group, and the difference was statistically significant(P<0.05). The time of fever reduction and disappearance of lung rales and cough in the observation group were significantly shorter than those in the control group(P<0.05). After 2 weeks of treatment, the levels of IL-6, TNF-α, CRP and PaCO2 in observation group were lower than those in control group(P<0.05), while PaO2 and SaO2 were significantly higher than those in control group(P<0.05). After 2 weeks of treatment, the physiological scores and chronic health status scores of the observation group were significantly lower than those of the control group, with statistical significance(P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).【Conclusion】Inhaled acetylcysteine combined with ulinastatin is effective in the treatment of patients with severe pneumonia, which can effectively improve respiratory function and reduce inflammation without increasing the incidence of adverse reactions.It is worthy of clinical application.

725 The Therapeutic Effect of Getong Tongluo Capsule Combined with Butylphthalide in the Treatment of Acute Posterior Circulation Infarction and its Effect on Serum TGF-β1/Smad3 Signaling Pathway and Platelet Related Factors

【Objective】To explore the therapeutic effect of Getong Tongluo capsule combined with butylphthalide in the treatment of in posterior circulation infarction (POCI),and its effects on the serum transforming growth factor-β1 (TGF-β1)/Smad3 signaling pathway and platelet related factors.【Methods】A retrospective analysis was conducted on the clinical data of 114 POCI patients admitted to our hospital from January 2020 to January 2023. According to different treatment methods, they were divided into the observation group and the control group, with 57 cases in each group. The control group was treated with butylphthalide, while the observation group was treated with Getong Tongluo capsules and butylphthalide. Two groups were compared in terms of efficacy, National Institute of Health stroke scale (NIHSS),Barthel index, serum TGF -β1/Smad3 signaling pathway related protein expression (Smad3,TGF-β1, Smad1),platelet related factors [vascular endothelial growth factor (VEGF), platelet activating factor (PAF), angiotensin Ⅱ(Ang Ⅱ), matrix metalloproteinase-9 (MMP-9)], inflammatory response indicators [soluble tumor necrosis factor related apoptosis inducing ligand (sTRAIL), hypersensitive C-reactive protein (hs-CRP), monocyte chemotactic protein-1 (MCP-1)].【Results】The effective rate of the observation group was 94.74%(54/57), which was higher than the control group's 80.70%(46/57), and the difference was statistically significant (χ2=5.211, P=0.022). After treatment, the NIHSS scores in both groups were lower than before treatment, and the observation group was lower than the control group. The Barthel index score was higher than that before treatment, and the observation group was higher than the control group (P<0.05). After treatment, Smad3,TGF-β1 and Smad1 of the two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05). After treatment, the serum VEGF levels in both groups were higher than those before treatment, and the observation group was higher than the control group. The level of the serum PAF,Ang Ⅱ and MMP-9 was lower than those before treatment, and the observation group was lower than the control group (P<0.05). After treatment, the levels of serum hs-CRP, sTRAIL and MCP-1 of the two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05).【Conclusion】The combination of Getong Tongluo capsules and butylphthalide can inhibit the expression of TGF-β1/Smad3 signaling pathway related proteins, inhibit platelet activation, alleviate inflammatory response, and improve patient neurological function.

729 The Application Value of Intervention Mode Dominated by Tongue Muscle Function Training in Early Correction of Malocclusion Patients

【Objective】To explore the application value of intervention mode dominated by tongue muscle function training in early correction of malocclusion patients.【Methods】A total of 122 patients with malocclusion admitted to Qingdao Central Hospital from November 2018 to February 2021 were selected and divided into the observation group (n=70) and the control group (n=52) according to the wishes of their families. The control group received routine intervention mode, while the observation group received tongue muscle function training as the dominant intervention mode on the basis of routine intervention. We compared the occlusal function and oral related quality of life between two groups of patients before and after intervention.【Results】After intervention, AOF, AOA and OII of two groups were all lower than those before intervention, NOC was higher than that before intervention, and AOF, AOA and OII in the observation group were lower than those of the control group. The NOC was higher than that of the control group, and the difference was statistically significant (P<0.05). After intervention, the OHIP-14 scores in various fields and total scores in both groups were lower than those before intervention, and the observation group was lower than the control group, with a statistically significant difference (P<0.05).【Conclusion】The intervention model led by tongue muscle function training in early correction of malocclusion in patients can effectively improve their bite ability and is of great significance for improving their oral quality of life.

732 The Effect of Cisapride Combined with Hemoperfusion on Gastrointestinal Symptoms and Gastrointestinal Hormone Levels in CRF Maintenance Hemodialysis Patients

【Objective】To explore the effects of cisapride assisted blood perfusion therapy on gastrointestinal symptoms and gastrointestinal hormone levels in maintenance hemodialysis patients with chronic renal failure (CRF).【Methods】A retrospective analysis was conducted on the clinical data of 72 patients with CRF admitted to our hospital from February 2021 to January 2023. According to the treatment method, they were divided into an observation group and a control group, with 36 cases in each group. Both groups received maintenance hemodialysis treatment, the control group received hemoperfusion treatment, and the observation group received cisapride treatment on the basis of the control group. After 2 weeks of treatment, renal function (Scr), urea nitrogen (BUN), glomerular filtration rate (GFR), gastrointestinal symptoms, gastrointestinal hormones [somatostatin (SS), motilin (MOT), vasoactive intestinal peptide (VIP)], serum fibroblast growth factor 23 (FGF-23), calcitonin gene related peptide (CGRP), tumor necrosis factor alpha (TNF-α) levels, and incidence of adverse reactions were compared between the two groups.【Results】After treatment, Scr and BUN of two groups decreased compared to before treatment, EGER increased compared to that before treatment (P<0.05); There was no statistically significant difference between Scr,BUN and eGER in the two sets (P>0.05). After treatment, the scores of acid reflux, nausea and vomiting, bloating, belching, and constipation in both groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05).After treatment, The level of serum MOT and SS in the observation group was lower than that before treatment and that in the control group, VIP was higher than that before treatment and that in the control group (P<0.05). After treatment, The levels of serum FGF-23 CGRP and TNF-α in two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05).【Conclusion】Cisapride assisted hemoperfusion therapy for CRF maintenance hemodialysis can regulate gastrointestinal hormone levels, improve gastrointestinal symptoms, alleviate inflammatory damage, and has high safety.

【Objective】To explore the therapeutic effect of milrinone combined with recombinant human brain natriuretic peptide (rhBNP) in the treatment of chronic heart failure and its impact on the heart and lung function of patients.【Methods】A retrospective analysis was conducted on the clinical data of 120 patients with chronic heart failure admitted to our hospital from October 2021 to October 2022. According to the quality method, they were divided into the observation group (treated with milrinone combined with rhBNP) and the control group (treated with milrinone), with 60 cases in each group. We compared the treatment effects and adverse reactions between two groups, and also compared the heart function indicators [cardiac output (CO), stroke output (SV), left ventricular ejection fraction (LVEF)], lung function indicators [percentage of forced expiratory volume at 1 second to expected value (FEV1%), forced lung activity percentage to expected value (FVC%), FEV1/FVC], as well as serum BNP and uric acid (UA) levels before and after treatment.【Results】The total effective rate of the observation group was significantly higher than that of the control group (P<0.05). After treatment, the CO, SV, and LVEF of the two groups were higher than before treatment, and the observation group was higher than the control group, with statistical significance (P<0.05). After treatment, the FEV1%, FVC%, and FEV1/FVC of the two groups were higher than before treatment, and the observation group was higher than the control group, with statistical significance (P<0.05). After treatment, the serum BNP and UA levels in both groups were lower than before treatment, and the observation group was lower than the control group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two groups (P>0.05).【Conclusion】The combination of milrinone and rhBNP is safe and effective in treating chronic heart failure, and can improve the heart and lung function of patients, which is worth clinical reference.

739 Effects of Sevoflurane Combined Anesthesia on Hemodynamics and Pain Factors in Patients Undergoing Gynecological Laparoscopic Surgery

【Objective】To investigate the effects of sevoflurane combined anesthesia on hemodynamics and pain factors in patients undergoing gynecological laparoscopic surgery.【Methods】There were 86 patients with gynecological diseases treated by laparoscopic surgery in our hospital and random divided into the observation group and the control group (n=43).The control group received propofol combined anesthesia, and the observation group received sevoflurane combined anesthesia. The anesthesia effects, heart rate (HR), mean arterial pressure (MAP) and other hemodynamic indicators before induction (T0), intubation (T1), intubation 5 min (T2), skin incision (T3), post-pneumoperitoneum (T4), and extubation (T5) were compared between the two groups. Serum levels of pain factors such as 5-HT, PGE2, β-endorphin (β-EP) and substance P (SP) at 6 h, 12 h and 24 h postoperatively, and the occurrence of postoperative adverse reactions of the two groups was compared as well.【Results】There was no difference in anesthesia time between the two groups (P>0.05).The recovery time of spontaneous breathing, eye opening time, language response time, recovery time of orientation and extubation time in observation group were shorter than those in control group (P<0.05). At T3, HR of observation group was higher than that of control group at the same point (P<0.05). At T2, T3 and T4, the MAP of observation group was higher than that of control group (P<0.05). There were no differences in HR and MAP between the two groups at other time points (P>0.05). There were no differences in the incidence of 5-HT, PGE2, β-EP, SP and adverse reactions between two groups before and after operation (P>0.05).【Conclusion】sevoflurane combined anesthesia can maintain hemodynamic stability, promote postoperative resuscitation and effectively regulate pain factors in patients undergoing gynecological laparoscopic surgery, and the incidence of adverse reactions is low.

743 Biomechanical Characteristics of CPC Fillers and the Effect on Lumbar Function during PKP in Patients with Traumatic Thoracolumbar Fracture

【Objective】To investigate the biomechanical characteristics of calcium phosphate bone cement (CPC) filler and its effect on lumbar function during percutaneous kyphoplasty (PKP) in patients with traumatic thoracolumbar fracture.【Methods】A total of 100 patients with traumatic thoracolumbar fracture treated in our hospital from April 2019 to April 2021 were selected and divided into the observation group and the control group according to different surgical methods, with 50 cases in each group. Patients in the observation group received treatment of PKP combined with CPC filler, while patients the control group received PKP combined with polymethyl methacrylate bone cement (PMMA) filler. The biomechanical characteristics of the two groups were compared, and the operative time, intraoperative blood loss, bed-staying length, hospitalization time, postoperative efficacy and complications were compared between the two groups. The vertebral height, visual analogue scale (VAS) score, Cobb Angle, Oswestry Disability Index (ODI) score of the injured vertebra were evaluated before and 6 months after surgery.【Results】The flexural strength, fracture work and elastic modulus of CPC filler were superior to those of PMMA filler (P<0.05). There was no significant difference in total curative effect between the two groups (P>0.05). There was no significant difference in operation time, intraoperative blood loss, bed-staying time and hospitalization between the two groups (P>0.05). There was no significant difference in the height of middle body, front edge and posterior edge between the two groups before and after surgery (P>0.05). The anterior margin height, middle height and posterior margin height of injured vertebrae in both groups were significantly higher than those before surgery (P<0.05). There was no significant difference in VAS score, Cobb Angle and ODI score between the two groups before and after surgery (P>0.05). VAS score, Cobb Angle and ODI score in the two groups after surgery were lower than those before surgery (P<0.05). There was no significant difference in the total incidence of complications between the two groups (P>0.05).【Conclusion】CPC and PMMA were used in PKP for the treatment of traumatic thoracolumbar fractures. Both methods can improve the treatment efficiency and lumbar function, but CPC has higher and better biomechanical properties and is more in line with the biological requirements of orthopedic fillings.

【Objective】To explore the effects of Danshen Chuanxiongqin Injection combined with Trimetazidine on electrocardiogram and cardiac function in patients with unstable angina pectoris (UAP).【Methods】A retrospective analysis was conducted on the clinical data of 106 UAP patients admitted to our hospital from January 2021 to May 2023. According to different treatment methods, they were divided into a control group and an observation group, with 53 cases in each group. The control group was treated with trimetazidine, while the observation group was treated with Danshen Chuanxiongqin Injection+trimetazidine. We compared between two groups in terms of the clinical efficacy, seizure frequency, nitroglycerin dosage, duration, cardiac function [cardiac output (CO), left ventricular ejection fraction (LVEF), stroke output (SV)], and electrocardiogram changes [ST segment depression amplitude], ST segment depression frequency, serum biochemical indicators, angiopoietin like protein 2 (Angptls2), oxidized low-density lipoprotein (ox-LDL),C1q tumor necrosis factor related protein 1 (CTRP1) and occurrence of adverse reactions.【Results】The effective rate of the observation group was higher than that of the control group, and the difference was statistically significant (P<0.05). After treatment, the seizure frequency, nitroglycerin dosage, and duration of the two groups were lower than before treatment, and the observation group was lower than the control group (P<0.05); CO, LVEF and SV of two sets were both higher than those before treatment, and the observation group was higher than the control group, with a statistically significant difference (P<0.05); The ST segment depression amplitude and the number of ST segment depressions of two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05); The levels of Serum Angptls2, ox-LDL and CTRP1 of the two groups were lower than those before treatment, and the observation group was lower than the control group (P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05).【Conclusion】The combination of Danshen Chuanxiongqin Injection and Trimetazidine has a good clinical efficacy in the treatment of UAP, which helps to alleviate patient symptoms, improve heart function, and has high safety.

750 Predictive Value of Immune Cells and Inflammatory Markers on the Efficacy of First-Line Chemotherapy in Advanced Lung Cancer

【Objective】This study aims to explore the predictive value of T lymphocyte subsets, tumor-infiltrating lymphocytes (Tils), and inflammatory markers on the effectiveness of first-line chemotherapy in advanced lung cancer.【Methods】Serum levels of interleukin 1α (IL-1α), IL-6, IL-17, and interferon-gamma (INF-γ), as well as the proportions of T lymphocyte subsets including CD4+T cells, CD8+T cells, regulatory T cells, CD57+ cells, Granzyme B+ cells, and CD45RO+ cells were measured in 98 newly diagnosed patients with stage Ⅲ/Ⅳ non-small cell lung adenocarcinoma. All patients received a chemotherapy regimen of paclitaxel injection and cisplatin for four cycles. Treatment effectiveness was assessed post-treatment, and patients were categorized into the responsive and non-responsive groups. Factors influencing the non-responsiveness and potential biomarkers for predicting treatment outcomes were analyzed.【Results】Out of 98 patients, 69 were responsive to chemotherapy, while 29 were non-responsive. Non-responsive patients had a higher proportion of lymph node metastasis, and higher levels of regulatory T cells and IL-1α expression (P<0.05), while their proportions of CD57+ and CD45RO+ cells were lower than those in the responsive group (P<0.05). Multivariate stepwise logistic regression analysis indicated that high levels of regulatory T cells and IL-1α were risk factors for ineffective chemotherapy response (P<0.05), whereas CD57+ and CD45RO+ cells were protective factors (P<0.05). Receiver operating characteristic (ROC) curve analysis showed sensitivities of 82.76%, 86.21%, 89.66%, and 93.10% for regulatory T cells, CD57+ cells, CD45RO+ cells, and IL-1α levels, respectively. The combined sensitivity, specificity, and area under the curve (AUC) of regulatory T cells, CD57+ cells, CD45RO+ cells, and IL-1α levels were 82.76%, 97.10%, and 0.957, respectively.【Conclusion】T lymphocyte subsets, Tils, and inflammatory markers are closely related to the treatment outcomes of advanced lung cancer and can serve as sensitive indicators for predicting therapeutic efficacy.

754 Analysis of the Correlation of Bacterial Biofilm Phenotype and Genotype with Multidrug Resistance in Escherichia Coli

【Objective】To explore the correlation of the bacterial biofilm phenotype and genotype with multidrug resistance in Escherichia coli.【Methods】A total of 230 clinical isolates of Escherichia coli were collected. Genotyping was performed using multiplex PCR, biofilm formation ability was determined using a modified crystal violet assay, and antibiotic susceptibility was tested using the Kirby-Bauer method. Statistical analysis was conducted on the antibiotic resistance patterns of Escherichia coli, the relationship between bacterial biofilm phenotype and multidrug resistance, and the relationship between Escherichia coli genotype and multidrug resistance.【Results】Escherichia coli showed high resistance rates to ampicillin, cefuroxime, and ciprofloxacin, but lower resistance to piperacillin and no resistance to imipenem. Biofilm phenotypes of Escherichia coli were classified as strong, moderate, weak and none, accounting for 3.91%, 31.30%, 57.83% and 6.96%, respectively. The common biofilm phenotypes in Escherichia coli were moderate and weak phenotypes. The stronger biofilm formation ability was associated with higher rate of multidrug resistance. The CTX-M-9 genotype was the most prevalent, accounting for 35.65%. Spearman correlation analysis showed that the biofilm formation ability of Escherichia coli was positively correlated with multidrug resistance rate (P=0.012).【Conclusion】The biofilm formation ability of Escherichia coli is associated with multidrug resistance. Different genotypes exhibit resistance to multiple antibiotics, highlighting the need for strict regulation of antibiotic use in clinical settings.
