961 Clinical Effect of Low Frequency Alternating Magnetic Therapy Combined with Levofloxacin can Improve the in treatment of Spinal Cord Injury Complicated with Acute Epididymitis

【Objective】To investigate the clinical effect of pulsed low-frequency magnetic field(PLFMF) therapy combined with levofloxacin in the treatment of spinal cord injury(SCI)complicated with acute epididymitis(AE).【Methods】A total of 42 patients with SCI and AE who were hospitalized in the Rehabilitation Medicine Department of Xijing Hospital, Air Force Military Medical University from December 2017 to December 2020 were selected. Patients were randomly divided into an observation group and a control group using a random number table method, with 21 patients in each group. Both groups were treated with levofloxacin for 14 days, while the observation group was also treated with PLFMF. The pain relief time, recovery time and clinical effect of the two groups were compared. 【Results】After 14 days of treatment, the pain relief time in the observation group was (3.9±1.3) d, which was lower than that in the control group (6.0±1.2) d, and the difference was statistically significant (t=-5.643,P=0.000).The recovery time of the observation group was (8.1±1.0)d, which was also lower than that of the control group (12.3±1.6)d with statistically significant difference (t=-7.651,P=0.000).The effective rate was 90.4% in the observation group and 61.9% in the control group. The effective rate in the observation group was higher than that in the control group, and the difference was statistically significant (χ2=4.725,P=0.03). 【Conclusion】PLFMF combined with levofloxacin therapy can reduce the pain of patients with SCI complicated with AE faster, shorten the course of the disease, and improve the curative effect.

964 Survey of Unmarried Teenage Pregnancy Outcomes

【Objective】To investigate whether unmarried adolescent pregnant women have higher rates of pregnancy complications and comorbidities, and explore the impact on maternal and infant outcomes. 【Methods】A retrospective cohort study was conducted on the general characteristics and pregnancy outcomes of adolescent pregnant women aged 11-19 years and adult pregnant women over 20 years old who were hospitalized in our hospital from January 2013 to December 2020. A comparison of pregnancy complications, adverse pregnancy outcomes, and delivery methods was made, and relative risks were calculated. 【Results】There were 743 pregnant women in the adolescent group, and the number and proportion showed a downward trend year by year.The proportion of pregnant women in the adolescent group was lower in the local population (64.1% vs 90.9%), lower proportion of multiparas (7.1% vs 34.3%), higher proportion of vaginal delivery (68.1% vs 46.6%). The average delivery gestational age was [(39.6±1.7) weeks vs (39.1±1.4) weeks], the birth weight of new-borns was [(3304.9±465.4) g vs ( 3356.3±443.1) g], and hospitalization expenses was [(6594.2±3012.7) yuan vs (8547.8±3694.0) yuan] as the adolescent pregnant group vs the adult pregnant group; And the differences were statistically significant (P<0.05).The adolescent group had higher rates of anemia and preeclampsia, while gestational diabetes and hypothyroidism were less common. When compared to adults aged 20-34 years, the incidence of hypertensive disorders during pregnancy was also higher in the adolescent group. The incidence of neonatal asphyxia in the adolescent group was higher than that in the adult group. There was no significant difference in other maternal and infant outcomes between the two groups. 【Conclusion】The incidence of anemia and gestational hypertension in adolescents is higher than that in adults, and the incidence of neonatal asphyxia is higher than that in adults. Despite the declining rate of hospital deliveries of adolescent pregnancy, it is vital to pay attention to adolescent sex education in order to prevent unintended pregnancy, and it is necessary to care for adolescent pregnant women in order to reduce adverse outcomes for the mother and infant.

968 The Application Value of IMB Concept Management in Breastfeeding for Primiparous Women

【Objective】To explore the application value of the Information Motivation Behavioral Skills Model (IMB) concept management in breastfeeding for primiparous women.【Methods】A total of 120 pregnant and postpartum women admitted to our hospital from January 2022 to December 2022 were selected. According to the method of random number table, they were divided into a control group (conventional postpartum care) and an observation group (IMB concept management based on conventional postpartum care), with 60 cases in each group.The breastfeeding knowledge questionnaire and the Breastfeeding Self Efficacy Short Form Scale (BSES) were used to evaluate the mastery of breastfeeding knowledge and self-efficacy of mothers after admission and before discharge; Before discharge, the visual simulation score (VSA) was used to assess postpartum pain in postpartum women, and the pure breastfeeding status of the women was calculated at 3 days, 1 month, 3 months, and 6 months postpartum, and the pure breastfeeding rate was calculated.【Results】The scores of wound pain, Breast pain and uterine contraction pain in the observation group were lower than those in the control group, with a statistically significant difference (P<0.05). After intervention, the mastery of breastfeeding knowledge in both groups was better than that before intervention, with a statistically significant difference (P<0.05); The observation group had better mastery of correct breastfeeding methods, breastfeeding time and frequency, breast health care methods, methods to prevent baby obesity, and baby care methods than those of the control group, with statistically significant differences (P<0.05). After intervention, the self-efficacy scores of breastfeeding in both groups were higher than before intervention, and the observation group was higher than the control group, with a statistically significant difference (P<0.05). The pure breastfeeding rate in the observation group was higher than that in the control group at 1 month, 3 months, and 6 months postpartum, with statistically significant differences (P<0.05). 【Conclusion】Health guidance based on IMB concept management can effectively alleviate postpartum pain, improve self-efficacy of breastfeeding, and increase the rate of pure breastfeeding, which is worth promoting in clinical practice.

972 Clinical Effect of the Spleen and Kidney Tonifying Recipe on Intervention of Perimenopausal Syndrome with Spleen and Kidney Deficiency Syndrome

【Objective】To explore the clinical effect of the Spleen and kidney Tonifying Recipe on the intervention of spleen and kidney deficiency syndrome in perimenopausal syndrome. 【Methods】A total of 80 patients with spleen and kidney deficiency syndrome admitted to Hunan Maternal and Child Health Hospital from February 2020 to February 2021 were randomly divided into an observation group and a control group, with 40 cases in each group. The observation group was treated with the self-made the Spleen and kidney Tonifying Recipe, and the control group was treated with estradiol/estradiol Dydrogesterone tablets orally, 28 days as a cycle, 3 consecutive cycles. The symptom score, clinical efficacy, endometrial thickness, uterine volume, ovarian volume, serum endocrine hormone [Luteinizing hormone (LH), Follicle-stimulating hormone (FSH), estradiol (E2)] levels, Calcitonin (CT), parathyroid hormone (PTH), β-Endorphins(β-EP), interleukin-2 (IL-2), and bone mineral density of the forearm, hip joint, and L1-4 lumbar spine were compared.【Results】The scores and total scores of hot flashes, sweating, abnormal sensation, insomnia, depression, anxiety, dizziness, fatigue, muscle Arthralgia, headache, palpitations, skin ants, vulva/vagina discomfort, urinary irritation symptoms in the observation group were lower than those in the control group (P<0.05). The total effective rate of patients in the observation group was 95.00% (38/40), significantly higher than 70.00% (28/40) in the control group, and the difference was statistically significant(χ2=8.658, P<0.05). The serum FSH level in the observation group was lower than that in the control group (P<0.05), while the serum E2 level was higher than that in the control group (P<0.05). The bone mineral density of the forearm, hip joint, and L1-4 lumbar vertebrae in the observation group was higher than that in the control group (P<0.05). The serum levels of CT,β-EP, and IL-2 in the observation group were higher than those in the control group (P<0.05), while the serum PTH levels were lower than those in the control group (P<0.05). 【Conclusion】The Spleen and Kidney Tonifying Recipe has a significant clinical effect on the treatment of perimenopausal syndrome with spleen and kidney deficiency syndrome, which can improve symptoms, hormone levels, immune function, and bone density of patients. It is worth promoting and applying in clinical practice.

976 Clinical Efficacy of Different Doses of Sakubactrovalsartan in Patients with Hypertension Complicated with Chronic Heart Failure

【Objective】 To investigate the clinical efficacy of different doses of Sakubatroxvalsartan in the treatment of hypertensive patients with CHF. 【Methods】 From January 2019 to January 2022, 100 patients with hypertension and CHF admitted to the Affiliated Hospital of Yan'an University were randomly divided into a large dose group (conventional treatment + Sakubatroxvalsartan tablets 100 mg) and a small dose group (conventional treatment + Sakubatroxvalsartan tablets 50 mg) according to the random number table. Two groups were compared in terms of efficacy, serum levels of high sensitivity C-reactive protein (hs-CRP), endothelin-1 (ET-1), N-terminal-proBNP, cardiac function indicators, and adverse reactions after treatment. 【Results】The total effective rate of the high-dose group was 90.00% (45/50), significantly higher than the 72.00% (36/50) of the low-dose group, with a statistically significant difference (P<0.05). After treatment, the serum levels of hs-CRP and ET-1 in both groups were lower than those before treatment, and the high-dose group was lower than the low-dose group, with a statistically significant difference (P<0.05); LVEDD and NT-proBNP in both groups were lower than those before treatment, while LVEF was higher than treatment; The indicators of the high-dose group were better than those of the low-dose group, and the difference was statistically significant (P<0.05). The incidence of adverse reactions was 20.00% (10/50) in the high-dose group and 10.00% (5/50) in the low-dose group, with no statistically significant difference (P>0.05). 【Conclusion】High dose Sakubatroxvalsartan is effective in the treatment of hypertension with CHF. It can reduce the level of inflammatory factors, improve the heart function of patients, and has good safety.

979 Value of Acupoint Injection of Neostigmine in the Rapid Recovery Management of Gastric Cancer Laparoscopic Radical Surgery

【Objective】To explore the value of acupoint injection of neostigmine in the rapid recovery management of patients undergoing gastric cancer laparoscopic radical surgery. 【Methods】Eighty patients undergoing gastric cancer laparoscopic radical surgery were randomly divided into two groups, with 40 patients in each group. The observation group received acupoint injection of neostigmine in addition to Fast Track Surgery (FTS) management during the perioperative period, while the control group received only FTS management during the perioperative period. Surgical-related indicators, serum levels of C-reactive protein (CRP) and albumin (ALB) at different time points, T lymphocyte level, incidence of adverse reactions, and 12-month survival rate were compared between the two groups. 【Results】The time of getting out of bed, the time of anal exhaust, the time of the first oral liquid diet, and the hospital stay in the observation group were significantly shorter than those in the control group (P<0.05). Compared to preoperative level, serum CRP level was significantly higher, while serum ALB level was significantly lower at 1 day and 3 days after surgery in both groups (P<0.05). The serum CRP level of the two groups at 3 days after surgery were significantly lower than those at 1 day after surgery, and ALB level at 3 days after surgery were significantly higher than those at 1 day after surgery in both groups (P<0.05). The observation group had significantly lower serum CRP level and significantly higher serum ALB level at different time points, if compared to the control group (P<0.05). CD3+, CD4+, and CD8+ levels at 3 days after surgery were significantly lower than preoperative level in both groups (P<0.05). The CD4+/CD8+ at 3 days after surgery showed no significant difference compared to preoperative level in the same group (P>0.05). The levels of CD3+, CD4+, CD8+, and CD4+/CD8+ at 3 days after surgery in the observation group were significantly higher than those in the control group (P<0.05). The incidence of adverse reactions was significantly lower in the observation group compared to the control group (P<0.05). There was no significant difference in the 12-month survival rate between the two groups (P>0.05). 【Conclusion】Adding acupoint injection of neostigmine to FTS management can facilitate faster recovery, reduce the body's stress response, improve nutritional status, regulate immune function, and reduce the incidence of adverse reactions in patients after laparoscopic gastric cancer radical surgery.

984 Clinical Efficacy of 1470 nm Laser Vaporization of the Prostate through Urethra in the Treatment of Benign Prostatic Hyperplasia

【Objective】 To investigate the efficacy of transurethral vaporization of the prostate with 1470 nm laser (1470 nm DiLVP) in the treatment of patients with benign prostatic hyperplasia (BPH). 【Methods】A total of 114 patients with BPH admitted to our hospital from February 2019 to February 2022 were selected and divided into group A (receiving 1470 nm DiLVP treatment) and group B (receiving TUPKP treatment) based on different surgical plans, with 57 patients in each group. Perioperative parameters, maximum urinary flow rate (Qmax) before and after operation, IPSS score, residual urine volume (RUV), serum prostate specific antigen (PSA) level, stress factors [cortisol (Cor), adrenaline (AD), Norepinephrine (NE)] level and the incidence of complications were compared between the two groups.【Results】The hospitalization time, bladder flushing time, catheter retention time, and surgical time in the Group A were shorter than those in the Group B, and the intraoperative blood loss was lower than that in the Group B (P<0.05). The IPSS score, RUV, and PSA levels of the two groups of patients at 1 and 3 months after surgery were lower than those before surgery, while Qmax was higher than that before surgery (P<0.05); The IPSS score, RUV, and PSA levels in the Group A were lower than those in the Group B at 1 and 3 months after surgery, while Qmax was higher in the Group B (P<0.05). The serum NE, Cor, and AD levels were higher in both groups at 12 hours and 24 hours after surgery than those before, while the group A was lower than the group B, with statistically significant differences (P<0.05). The incidence of postoperative complications in the Group A was 1.75% (1/57), lower than the Group B [15.79% (9/57)], with a statistically significant difference (P<0.05). 【Conclusion】The application of 1470 nm DiLVP in the treatment of BPH can optimize perioperative indicators, improve patient condition, improve urinary control function, reduce postoperative complications, and have a smaller impact on the body's traumatic stress response.

987 Diagnostic Value of Exosomal miRNA-21 in Early Renal Fibrosis of Lupus Nephritis Patients

【Objective】This study aims to investigate the diagnostic value of exosomal miRNA-21 in early fibrosis of lupus nephritis (LN). 【Methods】A total of 54 LN patients admitted to our nephrology department were enrolled. Based on the chronic fibrosis index (CI) of the kidneys, patients were divided into the low CI group (CI<2 scores, n=20), the moderate CI group (2 scores≤CI≤4 scores, n=27), and the high CI group (CI>4 scores, n=7). The level of miRNA-21 in urine samples from all patients were measured, and the differential expression of miRNA-21 in different degrees of renal fibrosis was analyzed. Pearson correlation analysis was performed to assess the relationship between miRNA-21 and clinical indicators. The diagnostic value of miRNA-21 in early renal fibrosis of LN was evaluated using receiver operating characteristic (ROC) curve analysis. 【Results】The high CI group had significantly lower estimated glomerular filtration rate (eGFR) level than that of the low CI and moderate CI groups (P<0.05). While the 24-hour urinary protein and miRNA-21 level in the high CI group were significantly higher than that in the low CI and moderate CI groups. The differences were statistically significant (P<0.05). The moderate CI group had lower eGFR level than the low CI group (P<0.05) and higher 24-hour urinary protein and miRNA-21 level than the low CI group (P<0.05).Pearson correlation analysis revealed a significant positive correlation of exosomal miRNA-21 with SLEDAI score, anti-dsDNA antibodies, blood urea nitrogen (BUN), serum creatinine (Scr), CI scores, and urinary protein (P<0.05). However, a significant negative correlation was observed between miRNA-21 and eGFR (P<0.05).Exosomal miRNA-21 was identified as an independent factor affecting early renal fibrosis in LN, with an odds ratio (OR) (95%CI) of 2.93 (1.18-5.36).The area under the curve (AUC) for exosomal miRNA-21 in identifying early fibrosis of LN was 0.85, with the optimal cutoff value at 5.76. 【Conclusion】Exosomal miRNA-21 shows potential value as a diagnostic marker for early renal fibrosis in LN patients.

991 Study on the Relationship between Classification and Clinical Characteristics of 64 Cases of Gastric Neuroendocrine Tumor

【Objective】To analyze the clinicopathological characteristics and prognosis of gastric Neuroendocrine tumor (G-NEN). 【Methods】A total of 64 cases of G-NEN in Yancheng Third People's Hospital from June 2016 to January 2022 were retrospectively analyzed. According to the fifth edition of the WHO digestive system tumor classification standard in 2019, their pathological classification and grading were carried out. Their clinical data, pathological characteristics, immunophenotype, treatment and prognosis were analyzed. 【Results】Sixty-four G-NEN patients had more males than females, with an average age of (65.7±9.01) years. Most patients presented with abdominal pain. Low grade G-NEN was mainly located in the gastric body, while high grade NEN and MiNEN were more common in the cardia. There were statistically significant differences in age, maximum tumor diameter, tumor location, number of tumors, morphology under gastroscopy, lymph node metastasis, and TNM staging among patients with different pathological types (P<0.05). The expression of Syn in immunohistochemistry was significantly higher than that of CgA, and the difference was statistically significant (P<0.05). Following up for 5-60 months, the prognosis of low-grade NEN was better than that of patients in the high-level NEN and MiNEN groups (P<0.05). There was no statistically significant difference in the prognosis between NET G3 and NEC, NEC and MiNEN (P>0.05). 【Conclusion】G-NEN is more common in elderly men, mainly manifested as abdominal pain. Different pathological types of G-NEN have different clinical and pathological characteristics and biological behaviors. High grade NEN and MiNEN have poor prognosis.

995 The Expression of PD-1 and PD-L1 in Elderly CHB Patients and their Correlation with T Lymphocyte Function

【Objective】 To investigate the expression of Programmed cell death protein-1 (PD-1) and its ligand (PD-L1) in the peripheral blood of elderly patients with chronic hepatitis B (CHB) and their correlation with T lymphocytes. 【Methods】A total of 86 elderly CHB patients (observation group) admitted to two hospitals from March 2020 to April 2021 were selected and divided into mild group (n=41), moderate group (n=32), and severe group (n=13) based on their severity. All patients were treated with Entecavir, and were divided into the acquired group (n=39) and the non acquired group (n=47) according to whether they received complete response after 48 weeks of treatment. The levels of PD-1, PD-L1, and T lymphocytes (CD3+, CD4+, CD8+, CD4+/CD8+) were compared between the observation group and the control group. We analyzed the correlation between the levels of PD-1 and PD-L1 in the study group and T lymphocytes, as well as the severity of the disease. ROC analysis was conducted to assess the predictive value of PD-1 and PD-L1 for complete response.【Results】Compared with the control group, the observation group showed a decrease in CD3+, CD4+, CD4+/CD8+, while PD-1, PD-L1, and CD8+increased (P<0.05). The levels of PD-1 and PD-L1 in T cells of the severe, moderate, and mild groups showed a decreasing trend (P<0.05). The levels of PD-1 and PD-L1 were negatively correlated with CD3+, CD4+, CD4+/CD8+, and positively correlated with CD8+and disease severity (P<0.05). The levels of PD-1 and PD-L1 in the acquired group were lower than those in the non acquired group, and the difference was statistically significant (P<0.05). The results of Receiver operating characteristic showed that PD-1 and PD-L1 had a certain predictive value for the clinical efficacy of antiviral therapy in elderly CHB patients (P<0.05). 【Conclusion】 The levels of PD-1 and PD-L1 in T cells of elderly CHB patients are elevated and closely related to T lymphocyte function, which can indirectly evaluate the progression of CHB and have high predictive value for the antiviral efficacy of patients.

998 Effect of GnRH-a Treatment on Natural Pregnancy after Laparoscopic Endometriosis

【Objective】To observe the effect of gonadotropin releasing hormone agonist (GnRH-a) on natural pregnancy after laparoscopic endometriosis. 【Methods】A total of 220 endometriosis patients diagnosed and treated in our hospital from October 2016 to October 2019 were selected and divided into the control group and the observation group according to the random number table method, with 110 cases in each group. Both groups were treated with laparoscopic surgery, while patients in the observation group received GnRH-a after laparoscopic endometriois. The patients were followed up for 36 months to compare the ovarian reserve function [the number of antral follicles, ovarian volume, and anti-Mullerian hormone (AMH) level], the number of natural pregnancy cases, postoperative natural pregnancy rate, and pregnancy outcome between the two groups of patients.【Results】The number of antral follicles in the observation group at 3 months and 6 months after the operation was significantly higher than that in the control group at the same period (P<0.05). There was no significant difference in ovarian volume between the two groups (P>0.05). There were 13 cases of hot flashes and 17 cases of bone mineral loss after operation in the observation group, the incidence of adverse reactions was 27.27%. The rate of natural pregnancy in the observation group at 24 months and 36 months was significantly higher than that in the control group (P<0.05). The pregnancy delivery rate, and the full-term delivery rate in the observation group were significantly higher than those in the control group 36 months after operation (P<0.05).However, the embryo developing arrest rateshowed no significant difference during 36-month follow-up between the two groups (P>0.05). The rate of premature delivery in the observation group was lower than that in the control group (P<0.05). The AMH level in the observation group was significantly higher than that in the control group at 24 months and 36 months after operation (P>0.05). 【Conclusion】Application of GnRH-a treatment in patients after laparoscopic endometriosis can promote the recovery of ovarian reserve function, increase the rate of natural pregnancy, and reduce the risk of fetal premature delivery.

【Objective】This study aimed to explore the diagnostic value of intestinal fatty acid-binding protein (IFABP) combined with neutrophil-to-lymphocyte ratio (NLR) and procalcitonin (PCT) testing in pediatric strangulated intestinal obstruction. 【Methods】Ninety-six cases of intestinal obstruction in children admitted to two hospitals were enrolled as the observation group, and 84 healthy children undergoing medical examinations during the same period were selected as the control group. Serum levels of IFABP, NLR, and PCT were compared between the two groups. The occurrence of strangulated intestinal obstruction was recorded, and the clinical characteristics of children with strangulated intestinal obstruction were compared with those of children with simple intestinal obstruction. Logistic regression analysis was used to determine the factors influencing the occurrence of strangulated intestinal obstruction, and receiver operating characteristic (ROC) analysis was performed to evaluate the diagnostic value of serum IFABP combined with NLR and PCT levels in pediatric strangulated intestinal obstruction. 【Results】The serum levels of IFABP, NLR, and PCT in the observation group were significantly higher than those in the control group (P<0.05). The incidence of strangulated intestinal obstruction among the 96 cases of intestinal obstruction was 43.75% (42/96). Children with strangulated intestinal obstruction had a higher incidence of peritoneal irritation signs, history of abdominal surgery, and elevated serum levels of IFABP, NLR, and PCT compared to those with simple intestinal obstruction; And the differences were statistically significant (P<0.05). Logistic regression analysis showed that serum levels of IFABP, NLR, PCT, and history of abdominal surgery were independent risk factors for pediatric strangulated intestinal obstruction (P<0.05). ROC analysis indicated that the optimal cutoff points for serum IFABP, NLR, and PCT levels in diagnosing pediatric strangulated intestinal obstruction were 375.80 pg/mL, 3.82 ng/mL, and 0.34 ng/mL, respectively, with corresponding area under the curve (AUC) values of 0.818, 0.786, and 0.821. The combination of these markers yielded the highest specificity and AUC in diagnostic performance. 【Conclusion】Children with strangulated intestinal obstruction exhibit elevated serum levels of IFABP, NLR, and PCT, which can serve as sensitive indicators for diagnosing pediatric strangulated intestinal obstruction. Furthermore, their diagnostic value is enhanced when used in combination.

【Objective】To explore the bioinformatics screening of autophagy differentially expressed genes (ATDEGs) related to the early diagnosis of Endometriosis (EMT).【Methods】We used the Limma package to obtain differentially expressed genes (DEGs) from the Gene Expression Omnibus (GEO) database for EMT and healthy control (HC), and conducted GSEA enrichment analysis. The functional modules related to EMT were obtained through the weighted Gene co-expression network analysis, and the genes in the functional modules were enriched with GO and KEGG. ATDEGs had been downloaded from the Genecards database. Taking the intersection of three parts, we obtained ATDEGs related to EMT and constructed a protein interaction network diagram; The LASSO algorithm was used to screen diagnostic genes and evaluate their diagnostic ability through the receiver operating characteristic (ROC) curve.【Results】This study obtained 62 ATDEGs and identified the most significant module using Cytoscape's MCODE plugin, which includes 17 hub genes; Further LASSO analysis identified 7 diagnostic genes, which were validated in external datasets, indicating their good diagnostic ability.【Conclusion】 The results of this study contribute to the early diagnosis and intervention of EMT, and provide a theoretical basis for further clinical work and mechanism research.

1011 Diagnostic Efficacy of Ultrasound-guided and CT-guided Percutaneous Lung Biopsy for Peripheral Lung Lesions

【Objective】To explore the diagnostic efficacy of ultrasound-guided and CT-guided percutaneous lung biopsy for peripheral lung lesions. 【Methods】A retrospective analysis was conducted on the clinical data of 96 patients admitted to our hospital from March 2019 to March 2022 who were diagnosed with peripheral lung lesions through imaging examinations. They were divided into ultrasound-guided group and CT-guided group based on different guided biopsy methods, with 48 patients in each group. We compared the success rate, diagnosis rate, and pathological results of two groups of biopsies, as well as the total time, effective time, and average depth of puncture biopsy between the two groups. The incidence of complications during puncture biopsy were compared between the two groups. 【Results】There was no statistically significant difference in the success rate, diagnosis rate, and pathological results between the two groups (P>0.05). The total time, effective time, and average depth of puncture biopsy in the ultrasound-guided group were lower than those in the CT-guided group, with statistically significant differences (P<0.05). The total incidence of complications in the ultrasound-guided puncture biopsy group was 6.25% (3/48), which was lower than 20.83% (10/48) in the CT-guided group, with a statistically significant difference(χ2=4.360,P=0.037). 【Conclusion】 Both ultrasound-guided and CT-guided percutaneous lung biopsies have higher diagnostic efficacy for peripheral lung lesions, and the incidence of complications in ultrasound-guided percutaneous lung biopsy is relatively low.

1014 The Application Value of Contrast-Enhanced Ultrasound in the Diagnosis of Unknown Small Thyroid Nodules in Initial FNAC

【Objective】 To explore the application value of contrast-enhanced ultrasound in the diagnosis of unknown small thyroid nodules using initial fine needle aspiration cytology (FNAC). 【Methods】 A total of 86 patients (88 nodules) with unknown initial FNAC diagnosis of small thyroid nodules admitted to the Yan'an University Affiliated Hospital from February 2019 to February 2022 were selected for the study. All patients underwent contrast-enhanced ultrasound examination and secondary FNAC. The results of contrast-enhanced ultrasound examination and secondary FNAC diagnosis of benign and malignant thyroid nodules were recorded. Based on the pathological diagnosis results as the gold standard, the value of contrast-enhanced ultrasound examination and secondary FNAC diagnosis of unknown malignant thyroid nodules in the initial FNAC diagnosis was analyzed through consistency analysis. 【Results】Among the 88 nodules, pathological diagnosis showed a total of 46 malignant nodules and 42 benign nodules. The results of contrast-enhanced ultrasound examination showed 54 malignant nodules, 40 pathologically diagnosed malignant nodules, and 14 benign nodules; 18 benign nodules, 3 malignant nodules with pathological diagnosis, and 15 benign nodules; 16 nodules cannot be clearly diagnosed, 3 malignant nodules can be pathologically diagnosed, and 13 benign nodules can be diagnosed. The secondary FNAC diagnosis results showed 11 malignant nodules, 10 pathological diagnoses of malignant nodules, and 1 benign nodule; 18 benign nodules, 8 malignant nodules with pathological diagnosis, and 10 benign nodules; 59 nodules were not clearly diagnosed, 28 malignant nodules and 31 benign nodules were pathologically diagnosed. The consistency analysis results showed that the sensitivity, specificity, and accuracy of contrast-enhanced ultrasound in the diagnosis of unknown small thyroid nodule malignancy in the initial FNAC were 0.870, 0.667, and 0.773, respectively; The sensitivity, specificity, and accuracy of the second FNAC diagnosis for unknown small thyroid nodule malignancy in the first FNAC were 0.217, 0.976, and 0.580, respectively. 【Conclusion】 Contrast-enhanced ultrasound has a high diagnostic efficacy and clinical application value in the diagnosis of benign and malignant small thyroid nodules with unknown initial FNAC diagnosis.

1018 Effects of Different Methods of Lymph Node Dissection near Recurrent Laryngeal Nerve on Perioperative Parameters and Recurrent Laryngeal Nerve in Patients with Thyroid Papillary Carcinoma

【Objective】 To investigate the effect of different lymph node dissection methods near recurrent laryngeal nerve on perioperative parameters and Recurrent laryngeal nerve in patients with thyroid papillary carcinoma. 【Methods】A total of 98 patients with papillary thyroid cancer admitted to our hospital from May 2018 to May 2021 were selected and divided into an observation group (minimally invasive suspension surgery, n=52) and a control group (conventional surgery, n=46) based on different cleaning methods. The perioperative indexes (including operation time, intraoperative blood loss, chest tube retention time, hospital stay and postoperative thoracic drainage volume) of the two groups were compared, and the number of lymph node dissection, the success rate of lymph node dissection, the incidence of recurrent laryngeal nerve injury and complications were compared between the two groups. 【Results】There was no statistically significant difference between the two groups in terms of intraoperative bleeding volume, chest tube retention time, hospitalization time, and postoperative thoracic drainage volume. The surgical time in the observation group was longer than that in the control group, and the difference was statistically significant (P<0.05). The number of lymph node dissection and the success rate of lymph node dissection in the observation group were higher than those in the control group, and the difference was statistically significant (P<0.05). The incidence of recurrent laryngeal nerve injury in the observation group and the control group was 15.38% (8/52) and 13.04% (6/46) respectively, and there was no significant difference between the two groups(χ2=0.109,P=0.741).The total incidence of complications in the observation group and the control group was 5.77% (3/52) and 13.04% (6/46), respectively, with no statistically significant difference(χ2=1.549,P=0.213).【Conclusion】 Compared with conventional dissection, minimally invasive suspension dissection is a safe and reliable surgical method for the removal of lymph nodes near the recurrent laryngeal nerve in patients with thyroid papillary carcinoma.

1021 Correlation between CT Bhalla Score and Pulmonary Function in COPD Patients with Bronchiectasis

【Objective】To investigate the correlation between Bhalla score of computed tomography (CT) and pulmonary function in patients with chronic obstructive pulmonary disease (COPD) and Bronchiectasis. 【Methods】The clinical data of 100 patients with COPD admitted to our hospital from January 2021 to January 2022 were retrospectively analyzed. According to whether Bronchiectasis was combined or not, they were divided into a simple COPD group (COPD group, n=65) and a combined Bronchiectasis group (bronchiectasis group, n=35). CT Bhalla scores, lung function indexes (FEV1/FVC, PEF, 6MWT), interleukin-6 (IL-6), interleukin-33 (IL-3), C-reactive protein (CRP), and tumor necrosis factor-α(TNF-α) levels were compared between the two groups; The predictive value of CT Bhalla score, pulmonary function index, IL-6, IL-33, CRP, TNF-α in COPD with Bronchiectasis was analyzed by receiver operating characteristic (ROC) curve; Spearman rank correlation analysis was used to investigate the correlation between lung function indicators, IL-6, IL-33, CRP, TNF-α levels, and CT Bhalla score.【Results】The CT Bhalla score, IL-6, IL-33, CPR and TNF-α levels in the Bronchiectasis group were significantly higher than those in the COPD group, while 6MWT, FEV1/FVC, PEF were lower than those in the COPD group (P<0.05); Receiver operating characteristic analysis confirmed that CT Bhalla score, 6MWT, FEV1/FVC, PEF, IL-6, IL-33, CRP, TNF-α can be used to predict COPD with Bronchiectasis, and the area under the curve was 0.967, 0.906, 0.718, 0.783, 0.582, 0.825, 0.738, 0.745 respectively; The CT Bhalla score was negatively correlated with 6MWT, FEV1/FVC, and PEF (P<0.05), and positively correlated with IL-6, IL-33, CPR, and TNF-α (P<0.05). 【Conclusion】CT Bhalla score is significantly increased in COPD patients with Bronchiectasis, and is closely related to lung function, IL-6, IL-33, CPR and TNF-α levels.

1025 Relationship of COPD Complicated with Pulmonary Heart Disease with Levels of NT-proBNP, ET-1 and CGRP in Elderly Patients

【Objective】To investigate the correlation between severity of chronic obstructive pulmonary disease (COPD) complicated with pulmonary heart disease and levels of N-terminal B-type brain natriuretic peptide precursor (NT-proBNP), endothelin-1 (ET-1) and calcitonin gene-related peptide (CGRP) in elderly patients. 【Methods】A total of 108 elderly patients with COPD diagnosed and treated in the First People's Hospital of Xianyang City from January 2020 to January 2022 were selected in the study. According to whether the patients were complicated with pulmonary heart disease, they were divided into simple COPD group and COPD complicated with pulmonary heart disease group (combined group).Plasma levels of NT-proBNP, ET-1 and CGRP were detected in all patients. The value of plasma NT-proBNP, ET-1 and CGRP in the diagnosis of COPD complicated with pulmonary heart disease was analyzed by receiver operating characteristic (ROC) curve. Spearman correlation coefficient was used to analyze the correlation of plasma levels of NT-proBNP, ET-1 and CGRP with cardiac function classification of New York Heart Association (NYHA) in elderly COPD patients with pulmonary heart disease. 【Results】Compared with the simple COPD group, the levels of NT-proBNP and ET-1 were higher, while the level of CGRP weas lower in COPD group combined with pulmonary heart disease (P<0.05).ROC analysis showed that the best cutoff values of NT-proBNP, ET-1 and CGRP in the diagnosis of COPD complicated with pulmonary heart disease were 873.507 pg/mL, 117.625 ng/L and 33.228 pg/mL, respectively. In COPD with pulmonary heart disease group, the plasma levels of NT-proBNP and ET-1 in patients with different NYHA cardiac function grades were compared and showed in the order of grade Ⅱ<grade Ⅲ<grade Ⅳ, while CGRP level was opposite grade Ⅱ > grade Ⅲ > grade Ⅳ (P<0.05).Correlation analysis showed that plasma levels of NT-proBNP and ET-1 were positively correlated with NYHA cardiac function grade in COPD patients with pulmonary heart disease, while plasma level of CGRP was negatively correlated with NYHA cardiac function grade (P<0.05). 【Conclusion】Compared to simple COPD, levels of NT-proBNP and ET-1 in COPD patients with pulmonary heart disease were higher, while CGRP level was lower. The degree of cardiac function injury in COPD patients with pulmonary heart disease is positively correlated with the level of NT-proBNP and ET-1, and negatively correlated with the level of CGRP.

1029 The Relationship between MPV/PC, NF-κB, Fibulin-3 and the Prognosis of Patients with Aortic Dissection

【Objective】To investigate the levels of serum mean platelet volume (MPV)/platelet count (PC), nuclear factor-κB (NF-κB), and Fibrinolin-3 in patients with aortic dissection (AD) with different prognosis.【Methods】A total of 92 patients with Aortic dissection admitted to our hospital from March 2020 to February 2022 were selected, and they were divided into two groups according to their survival conditions:good prognosis group (survival, n=65) and poor prognosis group (death, n=27). We analyzed the levels of serum MPV/PC, NF-κB, and Fibulin-3 at admission and 7 days after surgery in the two groups, and analyzed the correlation between the serum indicators at admission and the risk score of Aortic dissection, cardiac function indicators [serum Troponin I (cTnI), left ventricular end diastolic diameter (LVEDD), left ventricular Ejection fraction (LVEF)]. The risk factors of poor prognosis in patients with Aortic dissection were analyzed by partial regression. The prognostic value of serum parameters 7 days after operation in patients with Aortic dissection was analyzed.【Results】7 days after surgery, the levels of MPV/PC and NF-κB in the poor prognosis group were higher than those in the good prognosis group, while the levels of Fibulin-3 were lower than those in the good prognosis group (P<0.05); MPV/PC, NF-κB, Fibulin-3 were closely related to cTnI, LVEDD, risk score, LVEF, and poor prognosis (P<0.05). The AUC of combined prediction of poor prognosis in patients with Aortic dissection was greater than that of single prediction (P<0.05).【Conclusion】 In patients with poor prognosis of aortic dissection, the levels of MPV/PC and NF-κB are increased, and the levels of Fibulin-3 are decreased. The joint prediction of poor prognosis of patients has high predictive value.

1032 Clinical Efficacy of PFNA and InterTan in the Treatment of Unstable Intertrochanteric Fractures of the Femur

【Objective】 To explore the clinical efficacy of proximal femoral anti rotation intramedullary nail (PFNA) and InterTan internal fixation in the treatment of intertrochanteric fractures of the femur. 【Methods】 A retrospective analysis was conducted on 100 patients with intertrochanteric fractures of the femur admitted to our hospital from February 2018 to February 2022. They were divided into PFNA group and InterTan group based on different surgical methods, with 50 patients in each group. The perioperative related indicators, fracture healing time, postoperative complications, blood routine, D-dimer (D-D), pain score, ability of daily living, and clinical efficacy were compared between the two groups.【Results】The surgical time, hospitalization time, and intraoperative blood loss in the PFNA group were lower than those in the InterTan group, with statistically significant differences (P<0.05). The incidence of postoperative complications in the PFNA group was 10.00% (5/50), which was higher than 4.00% (2/50) in the InterTan group, and the difference was statistically significant(χ2=6.061, P<0.05). Seven days after surgery, the levels of HCT and Hb in both groups of patients were lower than those before surgery (P<0.05), while D-D levels were higher than those before surgery (P<0.05). The levels of HCT and Hb in the InterTan group were lower than those in the PFNA group (P<0.05), and D-D levels were higher than those in the PFNA group (P<0.05). Three months after surgery, the VAS score and Barthel score of both groups of patients were higher than those before surgery (P<0.05), while the InterTan group was higher than the PFNA group (P<0.05). There was no statistically significant difference in the total effective rate between the InterTan group and the PFNA group (P>0.05). 【Conclusion】The efficacy of InterTan in the treatment of intertrochanteric fractures of the femur is comparable to that of PFNA. It is necessary to comprehensively evaluate the patient's basic condition and choose an appropriate internal fixation method before surgery, but there are relatively few postoperative complications of InterTan.

【Objective】 To investigate the clinical efficacy of Xinmailong injection combined with Ribavirin in the treatment of viral Myocarditis with cardiac insufficiency. 【Methods】From September 2019 to November 2021, 106 patients with viral myocarditis and cardiac insufficiency caused by coxsackie virus infection in our hospital were selected and divided into an observation group and a control group according to the different treatment methods, with 53 cases in each group. The control group was treated with Ribavirin, and the observation group was treated with Xinmailong injection on the basis of the control group. The clinical efficacy, ultrasonic cardiac function indexes [left ventricular Ejection fraction (LVEF), left ventricular short axis shortening rate (FS), left ventricular end diastolic diameter (LVEDD), cardiac index (CI)], serum brain natriuretic peptide (BNP), Superoxide dismutase (SOD), Malondialdehyde (MDA), and glutathione (GSH) levels were compared between the two groups. 【Results】The total clinical effective rate of the observation group was 96.23% (51/53), which was higher than 81.13% (43/53) of the control group, with a statistically significant difference (P<0.05). After treatment, LVEF, FS, and CI in both groups were higher than before treatment, while the observation group was higher than the control group (P<0.05); The LVEDD of the two groups was lower than before treatment, while the observation group was lower than the control group (P<0.05); The levels of serum BNP, CK-MB, LDH, and CK in the two groups were lower than before treatment, while those in the observation group were lower than those in the control group (P<0.05); The serum MDA levels in both groups were lower than before treatment, while those in the observation group were lower than those in the control group (P<0.05); The serum SOD and GSH levels in both groups were higher than before treatment, while the observation group was higher than the control group (P<0.05). There was no statistically significant difference in the total incidence of adverse reactions between the two group (P>0.05). 【Conclusion】 Xinmailong injection combined with Ribavirin has a better clinical effect in the treatment of viral Myocarditis with cardiac insufficiency, which can improve the cardiac function of patients and improve the oxidative stress state of the body.

1039 Effect of Carituzumab Monoclonal Antibody Combined with Chemotherapy on Efficacy and Survival in Elderly Patients with Lung Cancer

【Objective】This study aimed to investigate the impact of carituzumab monoclonal antibody combined with chemotherapy on the three-year survival time in elderly patients with lung cancer. 【Methods】Eighty elderly patients with lung cancer admitted to our hospital were randomly divided into two groups, with 40 patients in each group. The control group received conventional chemotherapy, while the observation group received carituzumab monoclonal antibody treatment on the basis of chemotherapy. The clinical efficacy, serum T lymphocyte subsets levels before and after treatment, Karnofsky performance status (KPS) score, adverse event incidence, and three-year survival were compared between the two groups. 【Results】The objective response rate in the observation group was 55.00% (22/40), which was significantly higher than that in the control group 32.50% (13/40) (P<0.05). There was no significant difference in disease control rate between the two groups (P>0.05). After treatment, the levels of CD4+ and CD4+/CD8+ in both groups increased, while CD8+ decreased compared to before treatment (P<0.05). after treatment, the levels of CD4+ and CD4+/CD8+ in the observation group were higher than those in the control group, while CD8+ of the observation group was lower that of the control group (P<0.05). The KPS scores of both groups after treatment was higher than those before treatment (P<0.05); And the observation group had a higher KPS score than the control group (P<0.05). There was no significant difference in the overall incidence of adverse reactions between the two groups (P>0.05). During the 3-year follow-up period, 8 patients were lost to follow-up in the observation group, while there were 23 deaths and 9 survivors. Meanwhile, in the control group, 6 patients were lost to follow-up; And there were 28 deaths and 6 survivors. Kaplan-Meier survival analysis showed no significant difference in survival rates between the two groups (P>0.05). 【Conclusion】Carituzumab monoclonal antibody combined with chemotherapy has better efficacy in the treatment of elderly patients with lung cancer, which improves patient immune function. And it is safe and reliable.

【Objective】To investigate the therapeutic effect of acupuncture combined with rTMS on post-stroke comorbid anxiety and depression (PSCAD), and its impact on patients' serum 5-hydroxytryptamine (5-HT) and homocysteine (Hcy) levels. 【Methods】A total of 76 PSCAD patients were randomly divided into the observation group and the control group, with 38 cases in each. Based on the routine symptomatic treatment, the control group was given acupuncture treatment, while the observation group was given acupuncture combined with rTMS treatment. Both groups were treated for 8 weeks on an outpatient basis. The scores of Hamilton Depression Scale(HAMD) and Hamilton Anxiety Scale(HAMA), inflammatory factors [interleukin (IL)-2, IL-6, tumor necrosis factor-α (TNF-α)], neuro-damage indicators [neuron-specific enolase (NSE), brain-derived neurotrophic factor (BDNF), and neuropeptide Y (NPY)], and serum 5-HT and Hcy levels were compared before treatment and 8 weeks after treatment in both groups. 【Results】After treatment, the HAMA and HAMD scores in both groups were reduced compared to pre-treatment (P<0.05), and the scores in the observation group were lower than that in the control group (P<0.05). After treatment, the levels of IL-2, IL-6, TNF-α, NSE, and Hcy were decreased compared to pre-treatment (P<0.05), and their levels in the observation group were lower than those in the control group (P<0.05). After 8 weeks of treatment, the levels of BDNF, NPY, and 5-HT were increased compared to pre-treatment (P<0.05), and the levels in the observation group were higher than those in the control group (P<0.05). 【Conclusion】The combined treatment of acupuncture and rTMS can effectively improve the negative emotions of PSCAD patients, inhibit the inflammatory response, alleviate neuro-damage, and regulate serum 5-HT and Hcy levels.

1047 The Value of Invasive Intracranial Pressure Monitoring in Decompressive Craniectomy for Severe Traumatic Brain Injury

【Objective】To explore the value of invasive Intracranial pressure monitoring in decompressive craniectomy for severe traumatic brain injury (sTBI). 【Methods】A total of 78 sTBI patients who underwent decompressive craniectomy in our hospital from July 2018 to July 2021 were selected and randomly divided into an observation group and a control group, with 39 patients in each group. The control group received decompressive craniectomy, and the observation group used invasive Intracranial pressure monitoring while decompressive craniectomy. The clinical efficacy and postoperative complications of the two groups were compared, including the length of stay in the NICU, the amount of Mannitol, the use time of Mannitol, Intracranial pressure at different time points before and after surgery, and biochemical indicators before and after surgery [interleukin-6 (IL-6), high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-α(TNF-α), neuron specific Enolase (NSE)].【Results】The good prognosis rate of the observation group was significantly higher than that of the control group, and the difference was statistically significant (P<0.05). The NICU hospitalization time, Mannitol dosage and Mannitol use time in the observation group were significantly shorter than those in the control group (P<0.05). The intracranial pressure of patients in the two groups was lower than that before operation on the 1st, 3rd and 7th days after operation, with a statistically significant difference (P<0.05); The intracranial pressure of the observation group was lower than that of the control group 3 days and 7 days after operation, with a statistically significant difference (P<0.05). After surgery, two groups of serum IL-6, hs-CRP, TNF-α,the NSE level was lower than those before surgery, and the observation group was lower than the control group, with a statistically significant difference (P<0.05). The total incidence of postoperative complications in the observation group was lower than that in the control group, and the difference was statistically significant (P<0.05). 【Conclusions】 The use of invasive Intracranial pressure monitoring in decompressive craniectomy for sTBI patients can effectively shorten the hospital stay of NICU, reduce the dosage of Mannitol, restore Intracranial pressure, and significantly improve the neurological function and prognosis of patients.

1050 Effects of Lipid-Regulating Decoction Combined with Prednisone on Glycolipid Metabolism Levels in Patients with Nephrotic Syndrome

【Objective】This study aimed to investigate the effects of lipid-regulating decoction combined with prednisone on the glycolipid metabolism levels in patients with nephrotic syndrome (NS). 【Methods】A total of 98 patients with NS treated in our hospital were divided into an observation group and a control group by random number table method, with 49 patients in each group. The control group was treated with prednisone, while the observation group was treated with lipid-regulating decoction combined with prednisone. The clinical efficacy, traditional Chinese medicine symptom scores before and after treatment, levels of glycolipid metabolism, kidney function indicators, and the incidence of adverse reactions in the two groups were compared. 【Results】The total effective rate in the observation group was 91.84% (45/49), significantly higher than 77.55% (38/49) in the control group (P<0.05). After treatment, the scores of facial and limb edema, fatigue, and anorexia of the observation group were all lower than those before treatment of the observation group and after treatment of the control group (P<0.05). After treatment, the levels of fasting blood glucose (FPG), triglycerides (TG), and total cholesterol (TC) in the observation group were significantly lower than those before treatment in the observation group and after treatment in the control group (P<0.05). After treatment, serum creatinine (Scr), blood urea nitrogen (BUN), and 24-hour urinary protein quantification in the observation group were lower than those before treatment and after treatment in the control group (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】The treatment of NS patients with lipid-regulating decoction combined with prednisone can significantly reduce the traditional Chinese medicine symptom scores, improve clinical efficacy, and significantly improve glycolipid metabolism levels and kidney function indicators with minimal adverse reactions.

1054 Effect of Shenmai Injection on Inflammatory Factors and Renal Function of Septic Acute Kidney Injury

【Objective】To investigate the protective effects of Shenmai injection on inflammatory factors and renal function in patients with septic acute kidney injury (AKI). 【Methods】A total of 120 patients with septic AKI were randomly divided into the control group (n=60) and the observation group (n=60). The control group was given routine treatment. While the observation group was treated with shenmai injection on the basis of control group. Both groupswere treated continuously for 7 days. Acute physiology and chronic health evaluation system Ⅱ (APACHEⅡ), serum creatinine (Scr), cystatin C (cys C), heparin binding protein (HBP), tumor necrosis factor α (TNF-α) and interleukin 6 (IL-6) levels were compared between two groups before treatment and on 3 d and 7 d after treatment. The duration of mechanical vasoactive drug(noradrenaline)and ventilation was compared between the two groups as well. 【Results】 Before treatment, there were no significant differences in the levels of Scr, cys C, HBP, TNF-α, IL-6 and APACHEⅡ scores between two groups (P<0.05). On day 3 and day 7 after treatment, all the above indexes decreased compared to those before treatment; And on day 7, the indexes decreased more significantly in the observation group than those in the control group (P<0.05). The days of ventilator and vasoactive drug used in the observation group were significantly lower than those in the control group (P<0.05). 【Conclusion】 Shenmai injection may play a protective role in patients with septic AKI by regulating inflammatory mediators.

1057 Effect of Laparoscopic Assisted Transanal Total Mesorectal Excision on the Prognosis of Patients with Advanced Middle and Low Rectal Cancer

【Objective】To investigate the effect of laparoscopic assisted total mesorectal excision on the prognosis of patients with advanced middle and low rectal cancer.【Methods】A total of 120 patients with advanced middle and low rectal cancer in our hospital were divided into control group (n=60, total mesorectal excision) and observation group (n=60, laparoscopic assisted total mesorectal excision) according to different treatment methods. Two groups of patients were compared in terms of surgical related indicators, postoperative KPS score, incidence of complications, and prognosis. 【Results】Compared with the control group, the observation group had shorter surgical time and first postoperative exhaust time, while the observation group had less intraoperative bleeding than the control group (P<0.05). After treatment, the KPS scores of both groups of patients improved compared to before treatment, and the KPS scores of the observation group were higher than those of the control group (P<0.05). The total incidence of complications during the treatment period in the observation group was 5.00% (3/60), lower than the 16.67% (10/60) in the control group (P<0.05). The distant metastasis rate and local recurrence rate of the observation group were 3.33% (2/60) and 1.67% (1/60), both lower than the control group's 13.33% (8/60) and 11.67% (7/60), and the differences between the two groups were statistically significant (both P<0.05). 【Conclusion】Laparoscopic assisted transanal total mesorectal excision has a good effect, which is conducive to promoting the rehabilitation of patients with advanced middle and low rectal cancer, further improving their quality of life, and reducing the incidence of complications, distant metastasis and local recurrence.

1060 Effect of Moxibustion Combined with Leukotriene Receptor Antagonist in Treating Adenoid Hypertrophy Children

【Objective】To investigate the therapeutic effect of moxibustion combined with leukotriene receptor antagonist in the treatment of adenoid hypertrophy children. 【Methods】Eighty-two children with adenoid hypertrophy admitted to our hospital were randomly divided into two groups, with 41 children in each group. The observation group was treated with moxibustion combined with leukotriene receptor antagonist, while the control group was treated with leukotriene receptor antagonist alone. The disappearance time of nasal congestion, the recovery time of sleeping, hearing, and breathing in the two groups of children, the Obstructive Sleep Apnea-18 (OSA-18) score before treatment and 3 months after treatment, clinical efficacy, and the occurrence of complications were compared. 【Results】The disappearance time of nasal congestion, the sleep recovery time, hearing recovery time, and breathing recovery time in the observation group were all shorter than those in the control group (P<0.05). The OSA-18 scores of children in both groups decreased 3 months after treatment (P<0.05), while the OSA-18 score in the observation group was lower than that in the control group (P<0.05). The total effective rate of the observation group was 92.68% (38/41), which was higher than that of the control group 73.17% (30/41); And the difference between the two groups was statistically significant (P<0.05). There was no statistical significance in the total incidence of complications between the two groups (P>0.05). 【Conclusion】Moxibustion combined with leukotriene receptor antagonist treatment of childhood adenoid hypertrophy is conducive to promoting the rapid recovery of patients, improving the symptoms of obstructive sleep apnea in patients, and enhancing the treatment effect of patients, while it does not increase the incidence rate of complications.

1063 Relationship of Severity and Prognosis of Acute Pulmonary Embolism with Levels of Copeptin, D-dimer, and Blood Sodium

【Objective】To investigate the relationship of the severity and prognosis of acute pulmonary embolism (APE) with levels of Copeptin, D-dimer, and blood sodium. 【Methods】A total of 84 patients diagnosed with acute pulmonary embolism in our hospital were selected as the observation group, and 84 healthy individuals undergoing physical examination during the same period were selected as the control group. The levels of Copeptin, D-dimer (D-D), and blood sodium were compared between the two groups. According to the Pulmonary Embolism Severity Index (PESI), the levels of Copeptin, D-D, and blood sodium in patients with different severity grades were compared in the observation group. The prognosis of the observation group was analyzed, and the levels of Copeptin, D-D, and blood sodium were compared among patients with different prognosis. The relationship of Copeptin, D-D and blood sodium levels with the severity and prognosis of acute pulmonary embolism was analyzed. 【Results】The levels of Copeptin and D-D in the observation group were significantly higher than those in the control group (P<0.05), while the blood sodium level was lower than that in the control group (P<0.05). According to the PESI grading, there were 22 cases of grade Ⅰ, 30 cases of grade Ⅱ, 20 cases of grade Ⅲ, 12 cases of grade Ⅳ, and no cases of grade Ⅴ in the 84 patients in the observation group. With the increase in PESI grade, the levels of Copeptin and D-D gradually increased, while the blood sodium level gradually decreased. The differences between groups were statistically significant (P<0.05). Among the 84 patients in the observation group, 9 patients (10.71%) died in the death group and 75 patients (89.29%) survived in the survival group. The levels of Copeptin and D-D in the death group were higher than those in the survival group (P<0.05), while the blood sodium level was lower than that in the survival group (P<0.05). Spearman's correlation analysis showed that Copeptin and D-D levels in patients with acute pulmonary embolism were positively correlated with the severity and prognosis (P<0.05), while blood sodium levels were negatively correlated with the severity and prognosis (P<0.05). 【Conclusion】Levels of Copeptin and D-D are significantly elevated, while blood sodium level is significantly reduced in patients with acute pulmonary embolism. Those levels are correlated with the severity and prognosis of the disease.

1066 Changes of Serum Pannexin1 Levels before and after Recanalization of Acute Posterior Circulation Ischemic Cerebral Infarction and its Relationship with Clinical Features

【Objective】To investigate the level changes of serum Pannexin1 and its relationship with clinical characteristics in patients with acute post-circulatory ischemic stroke before and after vascular recanalization. 【Methods】A total of 82 patients with acute post-circulatory ischemic stroke who underwent vascular recanalization were selected as the study subjects. Serum Pannexin1 levels before and after vascular recanalization were compared, and the correlation of serum Pannexin1 levels with stroke volume and NIHSS score was analyzed. The predictive value of serum Pannexin1 level for patient prognosis was also evaluated. 【Results】Serum Pannexin1 level was lower after vascular recanalization compared to before recanalization (P<0.05). Mild neurological deficit group had significantly lower serum Pannexin1 level than the severe neurological deficit group before and after recanalization (P<0.05). The mild stroke group had lower serum Pannexin1 level than the severe stroke group (P<0.05). The good prognosis group had lower serum Pannexin1 level than the poor prognosis group (P<0.05). The serum Pannexin1 levels in all groups after vascular recanalization were lower than those before recanalization (all P<0.05). Serum Pannexin1 level was positively correlated with stroke volume and NIHSS score (rs=0.649, 0.614, P<0.05). ROC analysis showed that the area under the curve (AUC) of serum Pannexin1 level for predicting patient prognosis was 0.778. 【Conclusion】Serum Pannexin1 level is elevated in patients with acute post-circulatory ischemic stroke compared to healthy volunteers, and the level of Pannexin1 is positively correlated with stroke volume and NIHSS score, which can provide certain reference value for the clinical prognosis of patients with acute posterior circulation ischemic cerebral infarction.

1070 Expression and Clinical Significance of Adipsin and A-FABP in Serum of Pregnant Diabetes Patients with Insulin Resistance

【Objective】To investigate the expression and clinical significance of lipase (adipsin) and adipocyte fatty acid binding protein (A-FABP) in the serum of patients with insulin resistance in gestational diabetes (GDM).【Method】A total of 100 GDM patients admitted to Ankang Maternal and Child Health Hospital from June 2019 to June 2022 were selected as the observation group, while 100 healthy pregnant women with normal prenatal examination in the same period were selected as the control group. The levels of triglyceride (TG), total cholesterol (TC), High-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), fasting blood glucose (FPG), fasting insulin (FINS), glycosylated hemoglobin (HbA1c), homeostasis model insulin resistance index (HOMA-IR), adipsin, and A-FABP were compared between the two groups of pregnant women. The Spearman correlation coefficient model was used to analyze the relationship among the indexes of glycolipid metabolism, adipsin, A-FABP and HOMA-IR. The correlation between A-FABP and HOMA-IR was analyzed using unconditional logistic stepwise regression to determine the influencing factors of insulin resistance in GDM patients.【Result】The TG,FPG, HbAlc, FINS, HOMA-IR,adiposin and A-FABP of the observation group were significantly higher than those of the control group (P<0.05). However, there were no statistically significant differences in TC, HDL-C, and LDL-C between the two groups (P>0.05). The correlation analysis showed that a positive correlation with HOMA-IR with levels of FPG, HbAlc, FINS, TG, adiposin, and A-FABP showed(P<0.05). The results of multivariate logistic regression analysis showed that FPG, HbAlc, FINS, TG, adiposin, and A-FABP were the influencing factors for insulin resistance in GDM patients (P<0.05).【Conclusion 】Adipsin and A-FABP are significantly correlated with GDM insulin resistance and they are important indicators for predicting GDM insulin resistance.
