GAO Yan-fang, ZHANG Ming, FAN Ju-xiang, et al
DOI: 10.3969/j.issn.1671-7171.2020.05.003
【Objective】 To investigate the effects of ubiquitin-like containing PHD and ring finger 1 (UHRF1) gene silencing by specific small interfering RNA (siRNA) on the proliferation and apoptosis in human cervical cancer HeLa cells in vitro.【Methods】 HeLa cells were randomly divided into three groups: the untreated control group without transfection, the negative control group with scramble siRNA transfection and the silencing group which were transfected with the UHRF1 specific gene-targeted siRNA. The mRNA and protein levels of UHRF1 were detected by RT-PCR and western blot, respectively. The effects of UHRF1 gene silencing on the proliferation and apoptosis were analyzed by conducting CCK-8 assay and flow cytometry with Annexin V-FITC/PI staining. 【Results】 The RT-PCR and western blot results showed the mRNA and protein levels of UHRF1 were significantly decreased in HeLa cells after the transfection of UHRF1 gene-targeted siRNA (P<0.01). The proliferation was inhibited and apoptosis was triggered via silencing UHRF1 gene, and the difference between the silencing group and the untreated control group/ negative control group was statistically significant (P<0.01). However,There was no significant difference between the untreated control group and the negative control group (P>0.05). 【Conclusion】 The expression of UHRF1 gene was efficiently interfered by transfecting the UHRF1-targeted siRNA, which resulted in decreased proliferation and enhanced apoptosis in HeLa cells.

LI Jiang, CHEN Tie-qiang, WU Min, et al
DOI: 10.3969/j.issn.1671-7171.2020.05.004
【Objective】 To investigate the effect of Budesonide aerosol inhalation in the treatment of bronchopulmonary dysplasia (BPD) in premature infants. 【Methods】 The clinical data of 92 cases of BPD in our hospital were analyzed retrospectively. According to the treatment methods, they were divided into 29 cases of budesonide group, 34 cases of dexamethasone group and 29 cases of control group (without glucocorticoid).【Results】 After treatment, BPD scores of the budesonide group and the dexamethasone group were lighter than those of the control group, but there was no significant difference between the budesonide group and the dexamethasone group (P>0.05). Compared with the dexamethasone group and the budesonide group, the time of oxygen delivery and the time of noninvasive mechanical ventilation in the control group were significantly longer (P<0.05), but there was no significant difference in the time of hospitalization and oxygen therapy after discharge in the three groups (P>0.05). There was no significant difference in the incidence of sepsis, patent ductus arteriosus (HsPDA), intraventricular hemorrhage (IVH)≥grade Ⅲ, PVL, necrotizing enterocolitis (NEC), gastrointestinal hemorrhage, pulmonary hypertension (PPHN) and hyperglycemia among the three groups (P>0.05).【Conclusion】 For premature infants who need continuous oxygen use or ventilator use, early (≥ 14 days) use of budesonide atomization inhalation has therapeutic effect on their BPD, which can reduce the time of oxygen use and mechanical ventilation, avoid the use of glucocorticoids in the whole body, and no short-term adverse complications are found, but the long-term prognosis still needs further study.

XIANG Yi, OU Bai-qing, XIAO Zhi-lin
DOI: 10.3969/j.issn.1671-7171.2020.05.006
【Objective】 To explore the correlation between 24-hour ambulatory blood pressure and severity of coronary artery disease.【Methods】 From January 2018 to December 2018, 265 patients with coronary angiography in the Department of Geriatric Cardiology of Xiangya Hospital were selected as the research objects. All patients were monitored 24 hours of ambulatory blood pressure before coronary angiography (CAG) operation, and relevant parameters were recorded. The severity of coronary stenosis was expressed by Gensini score. According to the results of CAG, the patients were divided into coronary heart disease group (169 cases) and non coronary heart disease group (96 cases), and the differences of ambulatory blood pressure parameters between the two groups were compared; finally, according to the severity of coronary artery disease (Gensini score), patients in coronary heart disease group were divided into mild disease group, moderate disease group and severe disease group, and the differences of dynamic blood pressure parameters in different Gensini integral groups were compared.【Results】 There was no significant difference in age, weight, blood sugar, blood lipid, liver and kidney function between the two groups (P>0.05). Pearson correlation analysis showed that only the coefficient of variation of blood pressure was significantly correlated with Gensini score (r=0.179,P=0.029). In different Gensini integral groups, only the coefficient of variation of blood pressure had significant difference, and with the increase of Gensini integral, the higher the coefficient of variation of blood pressure was.【Conclusion】 Blood pressure variability is closely related to the severity of coronary lesions. In patients with coronary heart disease, the coefficient of blood pressure variability can be used as a predictor of the severity of coronary lesions.

DOI: 10.3969/j.issn.1671-7171.2020.05.018
【Objective】 To explore the relationship between peripheral blood CD64 level and Wagner grading level of diabetic foot. 【Methods】 From November 2017 to December 2018, 162 patients with diabetic foot were divided into two groups according to the Wagner classification (severity): Wagner 2-3 (93 cases, group A), Wagner 4-5 (69 cases, group B). The levels of peripheral blood CD64, serum procalcitonin (PCT), C-reactive protein (CRP) and leukocyte (WBC) were compared between the two groups, The correlation between peripheral blood CD64 and other indexes was analyzed, and the relationship between CD64 and Wagner grade of diabetic foot was analyzed by logistic regression. The ROC curve was drawn, the area under the curve was calculated, and the sensitivity and specificity of CD64 in predicting the severity of diabetic foot were analyzed.【Results】 The levels of CD64, CRP, PCT and WBC in the group A were significantly higher than those in the group B (P<0.05). The correlation analysis showed that CD64 was positively correlated with CRP, PCT and WBC (r=0.86, 0.84, 0.62, respectively; P<0.05). Multivariate logistic regression analysis: the course of diabetic foot, CD64 and smoking were independent risk factors for the severity of diabetic foot disease (P<0.05). The area under ROC curve (0.892) showed that CD64 was related to the severity of diabetic foot. According to the maximum Youden index, when the critical value of CD64 was 3.38, its sensitivity was 81.2%, and its specificity was 91.4%.【Conclusion】 CD64 is related to Wagner's grade of diabetic foot, and high expression level of CD64 indicates that the condition of diabetic foot is more serious.

DOI: 10.3969/j.issn.1671-7171.2020.05.031
【Objective】 To observe the anesthesia effect of dexmedetomidine combined with remifentanil in gynecological daytime operation.【Methods】 One hundred cases of gynecological day surgery were selected, and were divided into dexmedetomidine combined with remifentanil group (observation group) and propofol group (control group) according to the random number table method. Both groups were treated with plasma concentration target controlled infusion of intravenous anesthesia (TCI), the observation group with dual-channel target controlled infusion of dexmedetomidine (plasma target concentration 0.3 ng/mL) and remifentanil (plasma target concentration 1.5 ng/mL), and the control group with target controlled infusion of propofol (plasma target concentration 3.0μg/mL). When the patient showed body movement during operation, the observation group was injected with remifentanil and the control group with propofol until body movement disappeared. The mean arterial pressure (map), blood oxygen saturation (SpO2), heart rate (HR) of patients before anesthesia induction (T1), before operation (T2), during operation (T3) and during recovery (T4) were recorded; the physical activity of patients during operation was recorded; the respiratory inhibition (SpO2 < 90% or apnea > 15s) was recorded ;and the recovery time (from the end of operation to wake up and open eyes, handshake according to instructions) and the occurrence of postoperative uterine contraction pain (VAS score) were recorded, and the time from drug withdrawal to anesthesia induction in the two groups were recorded.【Results】 There was no significant difference in map, SpO2 and HR between the two groups at T1 and T3 (P>0.05); map, SpO2 and HR in the observation group at T2 were significantly higher than those in the control group (P<0.05); The MAP and HR in the control group at T4 were significantly higher than those in the observation group (P<0.05), but there was no significant difference in SpO2 between the two groups (P>0.05). The number of body movement of patients in the observation group was significantly less than that in the control group (P<0.05), and the recovery time was significantly shorter than that in the control group (P<0.05), but there was no significant difference in the incidence of respiratory inhibition between the two groups (P>0.05). The time of anesthesia induction in the observation group was longer than that in the control group (P<0.05). 【Conclusion】 In the gynecological daytime operation, dexmedetomidine combined with remifentanil has a definite anesthesia effect, can obviously reduce the postoperative uterine contraction pain, shorten the recovery time and quality of patients, and improve the comfort of patients and operation. However, it is more expensive and longer than propofol. For the patients with propofol contraindications in gynecological daytime surgery, the combination of dextromethoridine and remifentanil anesthesia is an appropriate choice.

【Objective】 To observe the effect of ultrasound-guided transverse abdominal muscle block on preventive analgesia and inflammatory factors in patients undergoing gynecological laparoscopic surgery. 【Methods】 From February 2016 to January 2018, 120 patients undergoing gynecological laparoscopic surgery were randomly divided into observation group (n=60) and control group (n=60).The observation group underwent ultrasound-guided abdominal transverse muscle plane block anesthesia before anesthesia, while the control group underwent ultrasound-guided abdominal transverse muscle plane block anesthesia after operation. The pain status (pain VAS score),analgesic recovery rate, comfort scale (BCS) score and inflammatory factors (tumor necrosis factor-α, IL-6, IL-2, IL-8 and postoperative infection) levels of the two groups at different time points were compared.【Results】 The rest and exercise VAS scores of the patients in the observation group were significantly lower than 1 h after operation (P<0.05); the VAS scores of the patients in the control group at different time points after operation were significantly lower than those in the control group (P<0.01). The postoperative remediation rate (1.67%) in the observation group was significantly lower than that in the control group (11.67%)(P<0.05); the BCS score in the observation group was significantly lower than that in the control group at different time points after operation (P<0.05); and the TNF-α、IL-6、IL-2、IL-8 level in the observation group was significantly lower than that in the control group at 24 h after operation (P<0.05). 【Conclusion】 The use of ultrasound-guided horizontal abdominal muscle block before anesthesia in gynecological laparoscopic surgery can effectively play a preventive analgesic role, reduce the level of postoperative inflammatory factors, and promote the postoperative rehabilitation of gynecological laparoscopic surgery patients.

DOI: 10.3969/j.issn.1671-7171.2020.05.033
【Objective】 To investigate the levels and significance of serum activin A (ACT-A), cystatin C (Cys-C) and B-type brain natriuretic peptide (BNP) in patients with acute heart failure. 【Methods】 From January 2017 to March 2018, 98 AHF patients (observation group) and 100 healthy persons in the same period were selected as the control group. The levels of ACT-A, BNP and Cys-C were compared between the two groups, and the levels of ACT-A, BNP and Cys-C in AHF patients with different NYHA grades were compared.【Results】 The serum levels of act-a, Cys-C and BNP in the observation group were significantly higher than those in the control group (P<0.05). The serum levels of ACT-A, Cys-C and BNP in patients with NYHA grade Ⅲ~Ⅳ in the observation group were significantly higher than those in patients with gradeⅠ~Ⅱ (P<0.05). Pearson correlation analysis showed that ACT-A was positively correlated with BNP (r=0.672, P<0.05), but not with Cys-C (P>0.05); Cys-C was not correlated with BNP (P>0.05).【Conclusion】 The serum levels of ACT-A, Cys-C and BNP are significantly increased in AHF patients, which is related to the severity of the disease.

【Objective】 To analyze the influence of wechat platform continuous nursing on the quality of life and psychological status of cancer pain patients.【Methods】 The clinical data of 240 patients with advanced cancer pain admitted to our hospital from July 2016 to July 2019 were analyzed retrospectively. According to the different ways of follow-up nursing after discharge, they were divided into observation group (wechat platform continuous nursing) and control group (telephone continuous follow-up nursing), 120 cases in each group. EORTC QLQ-C30, SAS, SDS and NRS were used to assess the quality of life of cancer patients. The quality of life, anxiety, depression and pain tolerance of the two groups were compared before and after nursing. 【Results】 The scores of body, role, emotion, society and cognitive function in the observation group were higher than those in the control group, and the scores of fatigue, pain, nausea and vomiting were lower than those in the control group (P<0.05). The scores of dyspnea, economic difficulty, insomnia, loss of appetite, constipation and diarrhea in the observation group were lower than those in the control group (P<0.05). The incidence rate of mild anxiety, moderate anxiety, severe anxiety and depression in observation group was lower than that in control group, the difference was statistically significant. The NRS score of the observation group was lower than that of the control group, and the satisfaction rate of return visit and health education was higher than that of the control group (P<0.05).【Conclusion】 Wechat platform continuous nursing can effectively alleviate the pain of cancer pain patients, improve the quality of life and satisfaction of cancer pain patients
