Efficacy of Anticancer Regimen Combined with Gefitinib in the Treatment of Cancer-related Fatigue in Advanced Non-small Cell Lung Cancer Patient

【Objective】
To investigate the efficacy of anticancer regimen combined with gefitinib in the treatment of cancer-related fatigue and its impact on quality of life.
【Methods】
A total of 123 patients with advanced non-small cell lung cancer (NSCLC) were divided into two groups according to the treatment methods. Patients in the control group(group B)received gefitinib, while patients in the observation group(group A) received gefitinib combined with anti-cancer treatment. The therapeutic effects, quality of life scores and the PFS score in the two groups before and after treatment were compared.
【Results】
The PFS scores of the two groups were lower than those before treatment (P<0.05), and the decrease in group A was higher than that in group B (P<0.05). After treatment, the fatigue, rib pain, five upset fever and pulse weakness scores of the two groups were lower than before treatment (P<0.05).Those scores in group A were significantly lower than group B (P<0.05). The total remission rate of group A was 69.68%, which was significantly higher than group B (40.98%) (P<0.05). After treatment, the scores of physical function, role function and emotional function in the two groups were higher than those before treatment (P<0.05). The increase in the above scores in group A was higher than group B (P<0.05). Regression analysis showed that age and pathological grade were negatively correlated with quality of life in patients with advanced NSCLC. However, psychological intervention and treatment were positively correlated with quality of life in those patients.
【Conclusion】
Anticancer combined with gefitinib is effective in treating cancer-related fatigue in patients with advanced NSCLC , and can significantly improve the quality of life of patients.

Drug resistance of Smear-positive Pulmonary Tuberculosis Patients Aged 15~24 in Foshan

【Objective】
To analyze the drug resistance of Smear-positive tuberculosis patients aged 15~24 years in this area.
【Methods】
The clinical data of 250 smear-positive pulmonary tuberculosis patients registered in Foshan tuberculosis control project from January 2017 to June 2018 were selected. 246 strains of Mycobacterium tuberculosis (MTB) were isolated by using the proportions recommended by WHO and the International Federation of Tuberculosis and Pulmonary Diseases to determine drug susceptibility tests of first and second-line anti-tuberculosis such as streptomycin (SM), isoniazid (INH), rifampicin (RFP), ethambutol (EMB) and ofloxacin (OFX). amikacin (AM), capreomycin (CM), isonicotinamide propionate (PTO) and aminosalicylic acid (PAS).
【Results】
Among 246 strains of Mtb,there were 51 strains (20.73%) with total drug resistance, 34 strains (13.82%) with single drug resistance, 12 strains (4.88%) with multi-drug resistance, 4 strains (1.63%) with multi-drug resistance and 1 strain (0.41%) with extensive drug resistance. The total drug resistance rate of male was 19.61%(30/154), which was lower than that of female 22.58% (21/93), but there was no significant difference between male and female (χ
2
<0.31,P>0.05). The total drug resistance rates were 19.44% (7/36) in Sanshui District, 18.18% (14/77) in Nanhai District, 24.53% (26/106) in Chancheng District and 14.81% (4/27) in Gaoming District, respectively. There was no significant difference among regions (χ

≤ 0.63,P>0.05).
【Conclusion】
The epidemic trend of drug resistance among Smear positive pulmonary tuberculosis patients aged 15~24 in Foshan City is not optimistic. It is necessary to strengthen the control of key links and reduce drug resistance.

Effect of AKIP1 on Apoptosis of Colorectal Cancer Cells and Its Molecular Mechanism

【Objective】
To investigate the effect and related molecular mechanism of AKIP1 on the apoptosis of human colorectal cancer cells HCT-116.
【Methods】
AKIP1 siRNA were designed and transfected into HCT-116 cells. Real-time PCR and Western Blot were used to detect the expression of AKIP1 mRNA and protein, respectively. Cell viability and apoptosis were measured by MTT and flow cytometry. We detected the mRNA expression of apoptosis associated genes by Real-time PCR, and the protein expression of NF-κB p65 in the cytoplasm and nucleus and the apoptosis-related proteins were detected by Western Blot.
【Results】
After transfection with AKIP1 siRNA, the expression levels of AKIP1 mRNA and protein were significantly down-regulated. MTT assay showed that AKIP1 siRNA significantly inhibited the viability of HCT-116 cells. Flow cytometry showed that AKIP1 siRNA significantly promoted apoptosis of HCT-116 cells. Meanwhile, AKIP1 siRNA up-regulated Bax, cleaved Caspase-3, and down-regulated bcl-2 expression. In addition, AKIP1 siRNA down-regulated the expression of NF-κB p65 in the nucleus and up-regulated the expression of NF-κB p65 in the cytoplasm.
【Conclusion】
AKIP1 may inhibit the apoptosis of HCT-116 cells by activating NF-κB p65 and promote the progression of colorectal cancer.

Evaluation Value of Urinary and Renal Function Indexes in Early Renal Injury of Diabetic Nephropathy

【Objective】
To explore the value of urinary and renal function indicators in early renal injury in diabetic nephropathy (DN).
【Methods】
Fifty patients with type 2 diabetes mellitus (T2DM) were selected from June 2017 to June 2018. According to the level of urinary microalbumin (MA), patients were divided into diabetic kidney injury group (MA>23 mg/L, observation group,n=18) and diabetic non-kidney injury group (MA<23 mg/L, control group,n=32). The levels of N-acetyl-D-glucosaminidase (NAG), MA, beta-2-microglobulin (beta-2-MG), cystatin-C (Cys-C), retinol binding protein (RBP) were compared between the two groups, and the ROC curves of early DN renal injury were analyzed.
【Results】
There was no significant difference in the levels of creatinine (Cr) and blood urea nitrogen (BUN) between the two groups (P>0.05); the levels of NAG, RBP, beta 2-MG and Cys-C in the observation group were higher than those in the control group (P<0.05). The AUCs of NAG, RBP, beta 2-MG and Cys-C in ROC curve were 0.894, 0.926, 0.937 and 0.984 respectively, and those of Cr and BUN were 0.513 and 0.433 respectively.
【Conclusion】
NAG, RBP, beta 2-MG and Cys-C are highly expressed in patients with early DN renal injury, and they are highly sensitive to the diagnosis of early DN renal injury, which can provide a basis for early diagnosis and treatment of diseases.

Effect of Long-term Electroacupuncture Stimulation on the Recovery of Sensory and Motor Function after Peripheral Nerve Anastomosis in Rats

【Objective】
To evaluate the effect of long-term electroacupuncture stimulation (EA) on sensory and motor function recovery after PNI anastomosis by the establishment of a rat model of peripheral nerve injury (PNI).
【Methods】
Ninety male rats were selected and divided into control group, PNI group and PNI+EA group according to the method of computer random number table. 30 rats in each group were divided into PNI group and PNI+EA group. The PNI model was established by transection of sciatic nerve in PNI group and PNI+EA group. The PNI + EA group was treated with discontinuous wave EA, while the PNI group and the control group were not treated. The experiment lasted for 8 weeks. The effects of EA on PNI nerve regeneration were assessed by leg function index (LFI), stimulus reflex threshold (ST), temperature stimulation threshold (TST) and nerve conduction function (NCF).
【Results】
The LFI values of the PNI + EA group and the PNI group decreased rapidly in the first week of the experiment, and began to rise in the second to the eighth week, while the LFI values of the PNI + EA group were higher than those of the PNI group in the fifth to seventh week, but there was no significant difference between the two groups (P>0.05). The ST value of the PNI + EA group and the PNI group decreased gradually from 2 to 8 weeks after the experiment. At the 8th week, the ST value of the PNI + EA group was significantly lower than that of the PNI group (P<0.05). The TST value of PNI + EA group decreased gradually at the 2nd to 6th week, and the TST value of the PNI + EA group was significantly lower than that of the PNI group at the 4th to 8th week (P<0.05). The NCF values of the PNI group and the PNI + EA group were lower than those of the control group at 8 weeks of experiment, while those of the PNI + EA group were higher than those of the PNI group (P<0.05).
【Conclusion】
Short-term EA may have no significant effect on the regeneration of injured peripheral nerves in rats. Long-term sustained stimulation can promote nerve regeneration and functional recovery.

Clinical Efficacy of Radiofrequency Ablation in the Treatment of Benign Thyroid Tumors

【Objective】
To investigate the clinical efficacy and application of radiofrequency ablation in the treatment of benign thyroid tumors.
【Methods】
A total of 63 patients with benign thyroid tumors were treated with radiofrequency ablation. The patient's thyroid function, tumor shrinkage, quality of life, cosmetic satisfaction, and complications were examined.
【Results】
①The blood levels of free triiodothyronine (FT3), free tetraiodothyronine (FT4) and thyroid stimulating hormone (TSH) were within the normal range before and 1 month after treatment. ②The tumor volumes at 1 month, 3 months and 6 months after treatment were significantly reduced compared to before treatment (P<0.05). ③The total effective rate of cosmetic satisfaction after treatment was 96.83% (61/63). The quality of life improved significantly after treatment.④There were 4 cases of complications; the incidence rate was 6.35%. All patients had no serious complications such as skin burns, thyroid damage, and bleeding.
【Conclusion】
Radiofrequency ablation technique has a good therapeutic effect on the treatment of benign thyroid tumors. RFA does not damage the thyroid gland, which can improve the quality of life and reduce the complications of patients. Thus, it has a good clinical application prospect.

Expression and Significance of miR-29b and miR-126 in Iris Tissues of Diabetic Neovascular Glaucoma

【Objective】
To observe the expression of micro RNA-29b (miR-29b) and micro RNA-126 (miR-126) in the iris tissues of diabetes mellitus (DM) neovascular glaucoma (NVG) in order to explore their significance in the pathogenesis and development of DM with NVG.
【Methods】
One hundred cases of DM complicated with NVG were selected as DM-NVG group, 54 cases of typically simple glaucoma were selected as glaucoma group (NVG group) and 15 cases of healthy iris tissue donors were as control group (CON group). The expression levels of miR-29b and miR-126 were detected by fluorescence quantitative polymerase chain reaction (PCR). The relationship between the two genes' expression with the clinicopathological characteristics of patients with DM combined with NVG was analyzed. Pearson correlation analysis was used to analyze the relationship between miR-29b and miR-126 expression in the iris tissue of patients. Logistic multiple regression analysis was used to analyze the risk factors of NVG in DM patients.
【Results】
The expression of miR-29b in the iris tissues of the DM-NVG group and NVG group was significantly lower than that of the control group (P<0.05). However, the expression of miR-126 in the DM-NVG and NVG groups was significantly higher than that of the CON group (P<0.05). The expression of miR-29b in the iris of DM-NVG group was significantly lower than that of the NVG group (P<0.05), and its expression of miR-126 was significantly higher than that of the NVG group (P<0.05). The expression of miR-29b in iris tissues was related to intraocular pressure, visual acuity index and VEGF (P<0.05). The expression of miR-126 was related to intraocular pressure and VEGF (P<0.05). There was a significant negative correlation between miR-29b and miR-126 expression in iris tissues of patients with DM combined with NVG (P<0.001). Multivariate regression analysis showed that VEGF, DM type, miR-29b and miR-126 were the risk factors for DM complicated with NVG (P<0.05).
【Conclusion】
The expression of miR-29b is low in the iris tissues of diabetic neovascular glaucoma, while miR-126 is highly expressed, which may be involved in regulating the occurrence and development of diabetic neovascular glaucoma. Thus,it provides a reference for clinical treatment of diabetic neovascular glaucoma.

Influencing Factors and Significance of Lower Extremity Motor and Sensory Impairment in Patients with Spinal Tuberculosis

【Objective】
To explore the influencing factors and significance of lower extremity motor and sensory deficits (LMSD) in patients with spinal tuberculosis (STB).
【Methods】
A total of 82 STB patients from January 2015 to January 2018 were selected as the study subjects. According to the presence or absence of LMSD, 42 patients were divided into LMSD group and 40 patients without LMSD group. Age, sex, duration of symptoms, deterioration of disease, clinical symptoms, clinical signs, imaging features, kyphosis angle, Oswestry dysfunction index (ODI) and visual analogue score (VAS) were compared between the two groups.
【Results】
Multivariate logistic regression analysis showed that age, deterioration of disease, vertebral position and spinal compression were independent risk factors for LMSD. Sixty-five patients underwent surgical treatment. The kyphosis angle improved from (26.8±9.3) degree to (12.0±7.6) degree, the average correction degree was (11.9±4.3) degree, and the average correction loss at the final follow-up was (1.9±1.5) degree. There were significant differences in preoperative and final ODI and VAS scores between the two groups (P < 0.05).
【Conclusion】
Patients with spinal tuberculosis involving cervical or lumbar spine, if they have a history of disease progression and history of spinal compression, may cause symptoms such as movement disorders or sensory disturbances. Appropriate treatment options such as surgery should be used to achieve complete debridement. Sufficient stability of the spine to better restore function prevents STB from becoming worse.

Effects of Different Doses of Dexmedetomidine on Sedation and Heart Rate Variability in ICU Patients without Mechanical Ventilation

【Objective】
To study the effects of different doses of dexmedetomidine on sedation and analgesia and heart rate variability of ICU patients during non-mechanical ventilation.
【Methods】
Sixty-eight patients with non-mechanical ventilation admitted to ICU in our hospital from January 2018 to January 2019 were divided into high-dose group and low-dose group by random number table, 34 cases in each group. Both groups were given ICU routine treatment. The high-dose group was given the initial dose of dexmedetomidine 1.0 μg/(kg?h) on the basis of routine treatment, while the low-dose group was given the initial dose of dexmedetomidine 0.6 μg/(kg?h) on the basis of routine treatment. At the end of the treatment, the clinical related indicators such as the target score of RASS, the dosage of fentanyl, norepinephrine, propofol and the length of stay in ICU were compared between the two groups including the RASS score and CPOT score within 72 hours between the two groups; the adverse reactions such as delirium, bradycardia and hypotension between the two groups; the indicators of variability of heart rates such as low frequency power (LF) and high frequency power (HF) between the two groups.
【Results】
There were no significant differences in RASS score (-2~+1) time, CPOT score within 72 hours, propofol dosage, ICU hospitalization days, time of shallow sedation and delirium incidence between the two groups (P>0.05). The dosage of fentanyl, duration of deep sedation and HF value in the high dose group were significantly lower than those in the low dose group, while the dosage of norepinephrine, incidence of hypotension, bradycardia and LF value in the high dose group were significantly higher than those in the low dose group (P<0.05).
【Conclusion】
The initial dose of 0.6 ug/Kg/h dexmedetomidine is more conducive to sedation of non-mechanical ventilation patients in ICU, with lower incidence of adverse reactions and lower heart variability rate.

Expression and Clinical Significance of SIK1 and SIK2 in Thyroid Microcarcinoma and Adjacent Tissues

【Objective】
To investigate the expression and clinical significance of SIK1 (salt-induced protein kinase 1) and SIK2 (salt-induced protein kinase 2) in thyroid microcarcinoma and adjacent tissues.
【Methods】
A total of 80 cases of pathology-confirmed thyroid microcarcinoma and matched paracancerous tissues from January 2014 to December 2016 were collected from the specimens bank of thyroid cancer in Shenzhen Baoan District People's Hospital. The expression levels of SIK1 and SIK2 in thyroid microcarcinoma and adjacent tissues were detected by immunohistochemistry and Q-PCR. The correlations of gene expression with patients' age, gender, tumor diameter, TNM stage and vascular invasion were analyzed.
【Results】
The results of immunohistochemistry showed that the expression levels of SIK1 and SIK2 in cancer tissues were higher than those in adjacent tissues (P<0.05). The results of PCR showed that the expression level of SIK1 in cancer tissue was significantly higher than that in adjacent tissues (P<0.05). However, there was no significant difference in the expression level of SIK2 between cancer tissues and adjacent tissues (P>0.05). Logistic regression analysis showed that the expression level of SIK1 in thyroid microcarcinoma tissue was positively correlated with tumor diameter, vascular invasion, lymphatic metastasis and distant metastasis (P<0.05). Logistic regression analysis also showed that the expression level of SIK2 was positively correlated with tumor size, lymphatic metastasis and whether distant metastasis (P<0.05).
【Conclusion】
The expression levels of SIK1 and SIK2 in thyroid microcarcinoma tissues are significantly higher than those in tumor adjacent tissues. The levels of SIK1 and SIK2 expression are positively correlated with tumor diameter, lymphatic metastasis and distant metastasis, suggesting that SIK1 and SIK2 are related to the progression and prognosis. SIK1 and SIK2 can be used as new indicators of thyroid microcarcinoma diagnosis and treatment.

Changes of Serum Levels of IGFBP2 and miR-29c in Patients with Glioma and their Clinical Significance

【Objective】
To investigate the changes of serum insulin-like growth factor binding protein 2 (IGFBP2) and microRNA-29c (miR-29c) levels in patients with glioma and their clinical significance.
【Methods】
Eighty patients with glioma diagnosed in our hospital from January 2016 to December 2017 (case group), 80 healthy subjects with age, gender and case group were selected as the control group, and the serum of the two groups were tested. IGFBP2 and miR-29c levels were analyzed by stratification according to tumor diameter, pathological type, WHO classification, and tumor location.
【Results】
The level of serum IGFBP2 in the observation group was significantly higher than that in the control group, and the level of serum microRNA-29c was significantly lower than that in the control group, with statistical significance (P<0.05). The serum levels of IGFBP2 and microRNA-29c in patients with WHO grade Ⅲ were significantly higher than those in patients with WHO grade Ⅰ and Ⅱ, while the serum levels of microRNA-29c were significantly lower than those in patients with WHO grade Ⅰ and Ⅱ (P<0.05); there was no significant difference in serum levels of IGFBP2 and microRNA-29c between patients with different pathological types, lesion diameters and tumor sites (P>0.05).
【Conclusion】
The level of serum IGFBP2 increases and the expression of microRNA-29c decreases in glioma patients, which is correlated with the grade of glioma.

Analysis of Quality of Life and Sleep in Patients with End-stage Uremia after Maintenance Hemodialysis

【Objective】
To explore the related factors of the effect of maintenance hemodialysis (MHD) on the quality of life and sleep of end-stage uremic patients.
【Methods】
Two hundred patients with end-stage uremia in our hospital from June 2015 to December 2018 were selected as the study subjects. All patients received MHD treatment. Pittsburgh Sleep Quality Index (PSQI) was used to evaluate the sleep quality of patients, SF-36 was used to evaluate the quality of life of patients, general data of patients and related disease factors were collected, and logistic multiple regression analysis was used to analyze the related factors of sleep quality of patients.
【Results】
Among 200 patients in this study, 138 had poor sleep, the incidence of which was 69.00%. By univariate analysis, there were significant differences in sex composition, age, marriage, work, albumin level, whether to drink tea, whether to take sleeping pills, whether to take anxiety and depression (all P<0.05). Among them, taking sleeping pills, drinking tea, anxiety and depression were significant differences. The SF-36 score of patients was (53.58±10.97). There were significant differences in the quality of life of patients with different ages, educational levels and vascular access (P<0.05). Age and vascular access were independent factors influencing the quality of life of patients (P<0.05).
【Conclusion】
MHD has adverse effects on the quality of life and sleep of end-stage uremia patients. Effective interventions should be taken clinically to improve the quality of sleep and life of patients.

PET-CT Features of Pulmonary Blastoma (a Case Report and Literature Review)

【Objective】
To analyze the PET-CT imaging features of pulmonary blastoma.
【Methods】
A 27-year-old male was admitted to hospital with fever with pharyngeal pain and improved after anti-infection treatment. space occupying lesions in the lower lobe of the right lung were found during the treatment.Routine chest enhanced CT, bronchoscopy and PET-CT, puncture biopsies were used, and pulmonary blastocytoma to was diagnosed finally. Combined with the diagnosis and treatment process of this case, the clinical and imaging features of pulmonary blastocytoma were analyzed retrospectively.
【Results】
Enhanced chest CT showed that the density of space-occupying lesions in the lower lobe of the right lung was not uniform, lobulated and vascular cluster signs were seen, and uneven enhancement of enhanced scan was observed.

F-FDG PET-CT showed increased glucose metabolism in the lesion. Lumpectomy was performed. Postoperative pathological examination confirmed the diagnosis of pulmonary blastoma.

【Conclusion】

F-FDG PET-CT examination has good value in localization and qualitative diagnosis of pulmonary blastoma. PET-CT examination can be used early in suspected cases of pulmonary blastoma.

【Objective】
To investigate the effect of Visfatin-1535C/T gene polymorphism on acute myocardial infarction (AMI).
【Methods】
The clinical data of 142 patients with AMI admitted to our hospital from February 2015 to May 2018 (observation group) were selected. Meanwhile, 280 healthy volunteers were selected as control group. Polymerase chain reaction-restriction fragment length polymorphism (PCR-RLFP) was used to detect the genetic polymorphism of Visfatin-1535 in each group, and ELISA was used to detect the level of serum Visfatin.
【Results】
The polymorphism distribution of 1535 locus of Visfatin gene in observation group and control group accorded with Hardy-Weinberg equilibrium. The proportion of CC genotype and C allele in 1535 locus of Visfatin gene in the observation group was significantly higher than that in the control group, with statistical significance (P<0.05). There was no significant difference between ST-segment elevation myocardial infarction (STEMI) and non-ST-segment elevation myocardial infarction (NSTEMI) in the proportion of Visfatin 1535 locus genotype and allele (P>0.05). The serum Visfatin level of CC genotype patients was significantly higher than that of CT and TT genotypes, and the CT genotype was significantly higher than that of TT genotype patients (P<0.05). There was no significant difference in gender and age of patients with different genotypes (P>0.05).
【Conclusion】
The polymorphism of Visfatin-1535C/T gene is related to the pathogenesis of AMI and the level of serum Visfatin. Allele C plays an important role in the pathogenesis of AMI.

Effects of Matrix Metalloproteinase-10 and Insulin-like Growth Factor-1 on Metastasis of Histiocytosarcoma

【Objective】
To establish a mouse histiocytosarcoma model and screen a high invasive tumor strain. The expression of IGF-1 and MMP-10 protein in the tumor strain and clinical specimens were detected and the clinical significance was explored.
【Methods】
Primary L-II tumor cells were inoculated subcutaneously into the axilla of the mice, and the lung metastases were taken to make a suspension to inoculate another mouse, and then the lung metastases were taken for further passage. A mouse xenograft model was established using primary cells and ninth generation cells, respectively. The survival time and tumor size of primary tumor and metastatic tumor were compared. Immunohistochemistry and Western blot were used to detect the expression of IGF-1 and MMP-10.
【Results】
Mouse histiocytic sarcoma L-II to passage 9 were screened by serial transfer in vivo. The results showed that the selected tumor strains grew faster than the pre-screened tumor strains. The survival time of mice before and after screening was 29.23±2.90 days and 23.66±2.97 days, respectively. The difference was significant (P<0.05). The expression of IGF-1 and MMP-10 protein in the 9th generation tumor strain of mice was up-regulated compared with the expression of the tumor strain before screening.
【Conclusion】
The expressions of IGF-1 and MMP-10 were both enhanced in the mouse model of the 9th generation tumor cells, which suggests that IGF-1 and MMP-10 may play a role in the process of histiocytosarcoma metastasis.

Effect of Different Anesthesia Methods on Vaginal Delivery in Puerperal

【Objective】
To compare the effects of different anesthesia methods on maternal vaginal delivery.
【Methods】
A total of 200 women with vaginal delivery in our hospital were randomly divided into two groups according to different anesthesia methods, with 100 cases in each group. In the observation group, 1% lidocaine was used to block the bilateral perineal nerve block while fetal exposed part was lowered to about 0.5 ~ 1cm under the sciatic spine. Then the local infiltration anesthesia was performed before perineotomy. In the control group, when the fetal head was exposed, 1% lidocaine local infiltration anesthesia and perineal nerve block anesthesia were performed before episiotomy. Perineal pain and tear in the two groups were observed and compared.
【Results】
The incidence of perineal pain in the observation group was significantly lower than that in the control group (P<0.05). The proportion of perineal tears in the observation group was significantly lower than that in the control group (P<0.05). The second stage of labor and postpartum hemorrhage in the observation group were significantly lower than those in the control group (P<0.05). The time to get out of bed after surgery and length of hospitalization in the observation group were significantly less than those in the control group
(P<0.05).
【Conclusion】
1% lidocaine of bilateral perineal nerve block and local infiltration anesthesia before perineotomy has a good effect, which can reduce the pain and tear of the perineum and shorten the second stage of labor.

Risk Factors of Ventilator-associated Pneumonia in Stroke Patients in ICU

【Objective】
To explore the risk factors of ventilator-associated pneumonia in ICU stroke patients in order to provide knowledge for improving the prognosis of stroke patients.
【Methods】
Clinical data of 119 patients with mechanical ventilation stroke from June 2016 to June 2018 in the intensive care department of the southern district of Anhui provincial hospital were retrospectively analyzed. Patients were divided into 60 patients in the VAP group and 59 patients in the non-vap group (control group) according to the occurrence of ventilator-associated pneumonia (VAP). Gender, age, underlying disease, albumin, lymphocyte absolute value, GCS scores, APACHEⅡ scores, swallowing function, surgical treatment, broad-spectrum antibiotics, hormones, vasoactive drugs, sedatives and acid inhibitors were compared between the two groups. Multivariate regression analysis was performed on the indicators with statistically significant differences between the two groups to analyze the risk factors of VAP during mechanical ventilation in ICU stroke patients.
【Results】
Age, APACHEⅡ scores, GCS scores, dysphagia, accompaniment with chronic diseases, use of broad-spectrum antibiotics, sedatives and antacids in patients with stroke in the VAP group showed statistically significant differences from the control group (P<0.05). There was no statistically significant difference between the VAP group and the control group in terms of gender, albumin, lymphocyte absolute value, surgical treatment or not, broad-spectrum antibiotics, hormones and vasoactive drugs (P>0.05). Multivariate regression analysis showed that the occurrence of stroke VAP was associated with advanced age, chronic underlying disease, dysphagia, low GCS scores and high APACHEⅡ scores, sedation and acid suppressant application.
【Conclusions】
Elderly age, accompaniment with chronic diseases, swallowing dysfunction, low GCS scores and high APACHEⅡ scores, use of sedatives and antacids are risk factors for VAP in stroke patients and should be prevented.

Effect of Midazolam Combined with Remifentanil on Anesthesia and Hemodynamics in Spinal Fracture Surgery

To investigate the effect of midazolam combined with remifentanil on anesthesia and hemodynamics in patients with spinal fractures.

Eighty-four patients with vertebral fracture treated in our hospital from June 2016 to February 2018 were divided into control group and observation group according to random number table method, 42 cases in each group. The control group was anesthetized with midazolam injection, while the observation group was anesthetized with remifentanil on the basis of the control group. The anesthetic effect, hemodynamics and incidence of anesthetic complications were compared between the two groups.
【Results】

Influence of Video Combined with Feedback Health Education on Primiparas' Cognitive Level and Outcome of Breastfeeding

【Objective】
To investigate the influence of video combined with feedback health education on primiparas' cognitive level and outcome of breastfeeding.
【Methods】
A total of 80 primiparas who were hospitalized for baby delivery in our hospital from June to December of 2016 were selected as the control group.They received routine health education for breastfeeding. A total of 80 primiparas who hospitalized in our hospital from January to June of 2017 were selected as the observation group, and they received video combined with feedback health education in addition to above routine breastfeeding education. During the follow-up to 42 days postpartum, the complications of nipple cleft palate, breast tenderness and mastitis were compared between the two groups. The follow-up to 6 months postpartum, the exclusive breastfeeding rate in the two groups of primiparas was compared.
【Results】
There were 76 and 72 cases who actually completed the study in the observation group and in the control group, respectively. The breastfeeding basic knowledge score and skill knowledge score in observation group were significantly higher than those in control group (t=3.876, 3.240,P<0.01). When follow-up visits were made 42 days after delivery, the incidence rate of breast complications in the observation group was 13.16%, which was significantly lower than 29.17% in control group (χ
2
=5.722,P<0.05). During the follow-up visit to 6 months after delivery, the exclusive breastfeeding rates in the observation group at 1 month, 3 months and 6 months after delivery were 93.42%, 90.79% and 81.58%, respectively, which were significantly higher than those in the control group 77.78%, 58.33% and 45.83%, respectively (χ

【Conclusion】

Efficacy and Safety of Modified HoLEP and TURP in the Treatment of Benign Prostatic Hyperplasia

【Objective】
To analyze the efficacy and safety of modified HoLEP and TURP in the treatment of benign prostatic hyperplasia.
【Methods】
A retrospective analysis of 236 patients with benign prostatic hyperplasia who were admitted to our hospital from January to December 2018 and divided into two groups according to the treatment plan. Among them, 143 patients underwent holmium laser prostate removal (HoLEP), while the remaining 93 patients underwent conventional transurethral resection of the prostate (TURP). The maximum urinary flow rate (Qmax), residual bladder volume (RUV), overactive bladder symptom score (OABSS), international prostate symptom score (IPSS), and quality of life score (QOL) were analyzed before and 3 months after surgery. Perioperative related indicators and postoperative complications were recorded.
【Results】
The operation time and the weight of resected tissue in the HoLEP group were significantly higher than those in the TURP group, however, the intraoperative blood loss, urinary catheter indwelling time and hospitalization time were significantly lower than those in the TURP group (P<0.05). After 3 months, 27.96% of patients in the TURP group had complications, and 10.49% of patients in the HoLEP group had complications; the difference was statistically significant (P<0.05). Compared to those before surgery, the Qmax, RUV, OABSS, IPSS and QOL of the two groups were improved after surgery(P<0.05). However, the differences between the groups were not statistically significant (P>0.05).
【Conclusion】
HoLEP and TURP are equivalent to the clinical efficacy of benign prostatic hyperplasia. HoLEP has less damage, more safety and faster recovery after surgery.

Correlation Analysis between Blood Pressure and Neck Circumference in Healthy Adults

【Objective】
This study explored the relationship between blood pressure and neck circumference in adults.
【Methods】
A total of 181 adults (aged from 22 to 50 years old) who conducted medical examinations from January to October,2017 in the First Hopital of Qinhuangdao were enrolled. Neck circumference(NC),systolic blood pressure(SBP),diastolic blood pressure(DBP),body height,weight,and other parameters (FPG,TG,TC,LDL-C and HDL-C) were measured for statistical analysis.The subjects were divided into two groups: the hypertension group (systolic pressure≥140 mmHg and/or diastolic pressure≥90 mmHg) and the normal blood pressure group (systolic pressure≤140 mmHg and diastolic pressure≤90 mmHg).
【Results】
The average age of the subjects was 43.31 ± 6.25 years old,including 92 males(43.95±5.06 years old) and 89 females( 42.43±7.56 years old). There were no significant differences in age, height, FPG, TG, TC, LDL-C, body fat content and total fat mass between the two groups (P>0.05).Weight, waist circumference, neck circumference, upper arm circumference, BMI, systolic pressure, diastolic pressure in hypertension group were all higher than normal blood pressure group, while HDL-C was lower than normal blood pressure group. The differences were statistically significant (P<0.05). There was no correlation of SBP with FPG, TG, TC and total fat mass. There was positive correlation between systolic blood pressure (SBP) and age, height, weight, waist circumference, neck circumference, upper arm circumference, BMI, LDL-C, and body fat content, and SBP was negativelt correlated with HDL-C. There was no correlation of diastolic blood pressure (DBP) with FPG, TG, TC, LDL-C and total fat mass. However, DBP was positively correlated with body mass, waist circumference, neck circumference, upper arm circumference, BMI, LDL-C and body fat content. DBP was negatively correlated with HDL-C (P<0.05). The results of multivariate linear regression analysis indicate that systolic pressure was mainly affected by neck circumference, age and upper arm circumference (P<0.05), while diastolic pressure was mainly affected by neck circumference, upper arm circumference and overall fat quality (P<0.05).
【Conclusion】
Neck circumference is closely related to blood pressure in adults.

Effects and Novel Mechanism of Emodin on Cytochrome C, bcl-2, AKT and ERK in Intestinal Epithelial Inflammatory Model Caco-2 Cells

【Objective】
To explore the effects of emodin on the expression of cytochrome C, bcl-2, PI3-K/Akt and ERK signaling pathways in inflammatory model of intestinal epithelial cell Caco-2.
【Methods】
A lipopolysaccharide (LPS)-induced Caco-2 inflammatory cell model was established and treated with different concentrations of emodin. The apoptosis, phosphorylation of Akt and ERK, cytochrome C and bcl-2 protein expression in Caco-2 cells before and after different concentrations of emodin treatment were compared.
【Results】
After induction of Caco-2 cells by LPS for 24 h, the apoptosis of Caco-2 cells was increased compared to vehicle control cells. Meanwhile the expression of cytochrome C was increased, and the expression of bcl-2 protein was decreased. After treatment with different concentrations of emodin for 24 h in LPS-induced Caco-2 cells, cell apoptosis was inhibited, cytochrome C expression decreased with increasing emodin concentration, and bcl-2 protein expression increased. Pretreatment with phosphatidylinositol kinase (PI3K) inhibitor LY294002 and extracellular signal-regulated kinase (ERK1/2) PD98059 reversed the inhibition of apoptosis by emodin, while increasing cytochrome C expression and decreasing the expression of bcl-2 as well. Also, emodin treatment inhibited the phosphorylation of Akt and ERK.
【Conclusion】
In the inflammatory model of intestinal epithelial cell line Caco-2, emodin inhibits cell apoptosis by regulating PI3-K/Akt and ERK signaling pathways through inhibiting phosphorylation of Akt and ERK. Emodin also reduced the expression of apoptotic cytochrome C and enhanced the expression of bcl-2.

Expression and Clinical Significance of Cys-c, β2-microglobulin and RBP in Patients with Type 2 Diabetic Nephropathy

【Objective】
To study the expression and clinical significance of Cys-c (cystatin C), β2-MG (β2-microglobulin) and RBP (retinol binding protein) in patients with type 2 diabetic nephropathy.
【Methods】
Eighty-one patients with type 2 diabetic nephropathy were enrolled in this study. According to urinary albumin excretion rate (UAER), patients were divided into group A (30 cases), group B (24 cases) and group C (27 cases). In the same period, 26 healthy volunteers were enrolled as the control group. The levels of Cys-c, β2-microglobulin and RBP in the four groups of patients were recorded. ROC curve was used to analyze the sensitivity and specificity of serum Cys-c, β2-MG and RBP content in the diagnosis of type 2 diabetic nephropathy .
【Results】
The levels of Cys-c, β2-MG and RBP in the control group were not significantly different from those in group A (P>0.05). The levels of Cys-c, β2-MG and RBP in the control group and group A were significantly lower than those in group B and group C (P<0.05). The levels of Cys-c, β2-MG and RBP in group B were significantly lower than those of group C (P<0.05).
【Conclusion】
Cys-c, β2-microglobulin and RBP play an important role in the clinical diagnosis, treatment and disease monitoring of patients with type 2 diabetic nephropathy, all of which have high sensitivity and specificity.

Influencing Factors of Thyroid Nodules in Elderly Patients with Type 2 Diabetes Mellitus

【Objective】
To explore the influencing factors of thyroid nodules in patients with type 2 diabetes mellitus (T2DM).
【Methods】
A total of 385 patients with T2DM from June 2015 to June 2017 were selected as the research subjects , and were divided into thyroid nodules group (n=204) and non nodular group
(n=181) according to whether they had thyroid nodules. General data of two groups of patients were collected. Serum triglycerides (TG), total cholesterol (TC), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), glycosylated hemoglobin (HbA1C), free triiodothyronine 3 (FT3), free thyroxine 4 (FT4) and thyrotropin (TSH), thyroglobulin antibody (TgAb) and thyroid peroxidase antibody (TPOAb) level were detected. Logistic regression analysis was used to analyze the related factors of T2DM patients complicated with thyroid nodules and postoperative recurrence. Recieved operating characteristic curve (ROC) was used to analyze the effectiveness of various factors in diagnosing thyroid nodules and nodules with high ultrasound scores. The correlation between the factors was analyzed by Pearson.
【Results】
The levels of age, course of disease, TC and TG in the nodule group were higher than those in the non-nodule group, while the levels of TSH, HDL-C, FT3 and TPOAb in the nodule group were lower than those in the non-nodule group (P<0.05). Multivariate regression analysis showed that gender, age, TC, TSH and TPOAb were risk factors for thyroid nodules in patients with T2DM. Multivariate analysis showed that age > 60 years old, female and nodules occurred bilaterally and multiple were independent risk factors for recurrence.
【Conclusion】
Elderly patients with TC >4.40 mmol/L, TPOAb>10.17×10
3
IU/L, age >58.75 years, TSH >1.73 μIU/mL should be alert to the occurrence of thyroid nodules, and elderly women with multiple nodules on both sides should pay close attention to prevent recurrence after radiofrequency ablation.

Preventive effect of vitamin D calcium chewable tablets on relapse of idiopathic benign paroxysmal positional vertigo

【Objective】
To explore whether vitamin D calcium chewable tablets can prevent the recurrence of idiopathic benign paroxysmal positional vertigo (BPPV).
【Methods】
From December 2015 to December 2017, 138 patients with idiopathic BPPV were randomly divided into the observation group and the control group, 69 cases in each group. Both groups were treated with manual reduction to achieve clinical cure. The observation group was treated with vitamin D calcium chewable tablets (Diqiao) 750 mg, once a day, orally. The control group was given placebo orally. BPPV patients in both groups were followed up for 1 year, once every 4 months, and the recurrence times were recorded.
【Results】
After one year of follow-up, 67 patients in the observation group had a recurrence rate of 11.9% (8/67) and 68 patients in the control group had a recurrence rate of 27.9% (18/68). The recurrence rate in the observation group was significantly lower than that in the control group (P<0.05).
【Conclusion】
Vitamin D calcium chewable tablets can significantly reduce the recurrence rate of idiopathic BPPV.

Effect of Pelvic Floor Rehabilitation Technology on Pelvic Organ Prolapse, Urinary Incontinence, and Sexual Dysfunction in Patients with Pelvic Floor Dysfunction

【Objective】
To investigate the improvement of pelvic floor rehabilitation technology on the incidence of urinary incontinence, pelvic organ prolapse and sexual dysfunction in patients with pelvic floor dysfunction.
【Methods】
The clinical data of 150 patients with pelvic floor dysfunction admitted to our hospital from December 2015 to December 2017 were analyzed. Patients were divided into two groups: the observation group and the control group, according to their wishes. Patients in the control group received conventional treatment, while patients in the observation group received pelvic floor rehabilitation technology treatment on the basis of conventional treatment, and the clinical efficacy of the two groups was analyzed after treatment.
【Results】
Pressure test (vaginal resting pressure, pelvic floor systolic blood pressure), index of group I muscle fiber (duration of continuous systolic blood pressure) and index of group II muscle fiber (number of rapid systolic blood pressure) of the observation group were significantly higher than that of the control group (P<0.05).A total of 4 cases of pelvic organ prolapse, 5 cases of urinary incontinence and 5 cases of sexual dysfunction occurred in the observation group, with an incidence of 5.33%, 6.67% and 6.67%, respectively, while there were 15 cases of pelvic organ prolapse, 15 cases of urinary incontinence and 13 cases of sexual dysfunction occurred in the control group, with an incidence of 20.00%, 20.00% and 17.33%, respectively. The differences were statistically significant (P<0.05). The total satisfaction of the observation group was 96.00%; which was significantly higher than the control group of 84.00% (P<0.05).
【Conclusion】
Pelvic floor rehabilitation technology in the treatment of pelvic floor dysfunction, can effectively promote patients with pelvic floor muscle strength recovery, reduce the incidence of pelvic organ prolapse, urinary incontinence and sexual dysfunction, and improve the patient's rehabilitation satisfaction.

The expressions of PR-A, PR-B and PR-M in uterine leiomyoma were 0.79±0.09, 0.93±0.10, 0.09±0.005, higher than those in normal tissues (P<0.001); the expressions of PR-A, PR-B and PR-M were higher in patients with uterine leiomyoma whose number of leiomyomas was more than 2 and the diameter of leiomyoma was more than 5 cm. The expressions of PR-A, PR-B and PR-M were higher in patients with uterine leiomyoma at different ages, reproductive history, hypertension and diabetes mellitus. The expression levels of PR-A, PR-B and PR-M protein in leiomyoma tissues of patients with uterine leiomyoma were not affected (P>0.05). Multivariate regression analysis showed that the diameter of leiomyoma and the number of leiomyomas were the influencing factors of the expression of PR-A, PR-B and PR-M protein in patients with uterine leiomyoma.
【Conclusion】
The expression of PR-A, PR-B and PR-M in uterine leiomyoma is high, and is affected by the number of leiomyoma and the diameter of leiomyoma.

Correlation and Clinical Significance of Serum Cystatin C, Urinary Kidney Injury Molecule 1 with Contrast-Induced Nephropathy

【Objective】
To investigate the correlation and clinical significance of serum cystatin C(Cys-C), urinary kidney injury molecule 1 (Kim-1) with contrast-induced nephropathy(CIN).
【Methods】
A total of 43 patients with CIN admitted to our hospital were enrolled in the observation group, while 43 cases of healthy volunteers in the same period were selected as the control group. The differences in the expression levels of Cys-C and Kim-1 were compared between the two groups, and the correlation between clinical features and biochemical indicators was observed.
【Results】
In terms of related factor levels, the levels of Cys-C and Kim-1 in the observation group were significantly higher than those in the control group (P<0.05). The levels of Cys-C and Kim-1 in patients with CIN gradually increased with the severity of the disease. The levels of Cys-C and Kim-1 in severe patients were significantly higher than those in moderate and mild CIN patients (P<0.05). The levels of Cys-C and Kim-1 were higher in patients than in mild patients, and the differences were significant (P<0.05). Correlation analysis showed that gender, age, contrast agent dosage, Kim-1and Cys-C levels were were significantly correlated with the incidence of CIN (P<0.05).
【Conclusion】
Indicators such as Cys-C and Kim-1 are closely related to patients with CIN , which can reflect the severity of the disease and provide reliable evidence for the diagnosis and treatment of early disease.

【Objective】

【Results】
The overall infection rate of H.pylori in the two groups was 56.3%. The detection rate of H.pylori infection in the NAFLD group was 65.4%, which was significantly higher than that in the control group 47.3% (P<0.01). Logistic regression analysis showed that H.pylori infection, Body Mass Index, and Low Density Lipoprotein Cholesterol (LDL-C) were closely related to NAFLD.Among them, H.pylori infection was positively correlated with NAFLD (OR=3.66).
【Conclusion】
The H.pylori infection may be an independent risk factor for NAFLD.

Effect of Humanistic Care in Combination with Information Support on Psychological Crisis and Coping Style of Patients Undergone Total Hysterectomy

【Objective】
To investigate the effect of humanistic care in combination with information support on the psychological crisis and coping style of patients undergone total hysterectomy.
【Methods】
A total of 76 patients with total hysterectomy admitted to our hospital from August 2015 to July 2016 were enrolled in the study. They were divided into the observation group and the control group according to the random number table method, with 38 cases in each group. Patients in the control group received routine nursing intervention of total hysterectomy, while patients in the observation group received both humanistic care and information support in addition to conventional nursing care. The psychological crisis assessment scale and Medical Coping Modes Questionnaire (MCMQ) were used to evaluate the psychological crisis and coping style of the patients.
【Results】
The scores of emotional, cognitive, behavioral and psychological crisis in the observation group were significantly lower than those in control group (t=10.695, 10.739, 7.111, 9.777, P<0.05, P<0.01). The active confrontation score in the observation group was significantly higher than that in the control group, and the scores of avoidance and surrender were significantly lower than in the control group (t=4.719, 12.102, 8.668, P<0.05, P<0.01).
【Conclusion】
Humanistic care in combination with information support can help resolve the psychological crisis such as emotional, cognitive and behavioral crisis of patients who have undergone total hysterectomy, avoid negative behaviors such as avoidance and surrender, and improve the ability of active confrontation.

Analysis of Effect of Abdominal Pressure on Emergency Cardiopulmonary Resuscitation

【Objective】
To explore the effect of abdominal pressure on cardiopulmonary resuscitation (CPR) in emergency patients.
【Methods】
A total of 68 patients with cardiac arrest (CA) were randomly divided into two groups (n=34) .Patients in the control group were given traditional chest compressions to perform CPR, while patients in the observation group were given abdominal pressure CPR. The indexes of therapeutic effects and outcomes after 30 minutes of CPR in the two groups of patients were observed and compared.
【Results】
The heart rate, mean arterial pressure and arterial oxygen partial pressure in the observation group were significantly better than those in the control group after 30 minutes of CPR, while the arterial carbon dioxide partial pressure were significantly decreased in the control group. The differences were statistically significant between the two groups (P<0.05). In the observation group, the CPR success rate was 67.65% (23/34) , the 24-hour survival rate was 61.76% (21/34) and the survival rate after discharge was 52.94% (18/34), which were significantly higher than those in the control group (41.18%, 14/34), 32.35% (11/34) and 26.67% (9/34),respectively. Those differences were statistically significant between the two groups (P<0.05). The incidence of complications and rib fractures in the observation group was lower than that in the control group, the difference was statistically significant (P<0.05).
【Conclusion】
Compared to traditional chest compression CPR, the effect of abdominal pressure CPR has more significant effects. It is worth further clinical application.

Diagnostic Value of Serum and Cerebrospinal Fluid Albumin and IgG in Neurosyphilis

【Objective】
To explore the clinical diagnostic value of serum and cerebrospinal fluid albumin and IgG in neurosyphilis.
【Methods】
Thirty patients with Treponema pallidum antibody gelatin granule agglutination test (TPPA) positive were selected as the research objects. According to the results of TRUST and TRUST Laboratory of cerebrospinal fluid, they were divided into 10 cases of the syphilis cure group, 12 cases of the syphilis unhealed group and 8 cases of the neurosyphilis group. Albumin and IgG in serum and cerebrospinal fluid of the three groups were detected, and albumin index, IgG index and IgG synthesis rate of the three groups were compared.
【Results】
The albumin index, cerebrospinal fluid IgG index and IgG synthesis rate of the cured syphilis group were (5.32±1.15), (0.56±0.07), (0.78±1.28) mg/d, while those of the uncured syphilis group were (6.25±2.24), (0.60±0.13), (4.54±1.70) mg/d, and those of the neurosyphilis group were (20.92±10.65), (2.81±1.86), (86.43±5.91) mg/d, respectively. The values of each index in the neurosyphilis group were significantly higher than those in the other two groups (
P
<0.05), but there was no significant difference between the cured group and the unhealed group (P>0.05).
【Conclusion】
Albumin index, IgG index and IgG synthesis rate are significantly increased in the neurosyphilis group, which provides a basis for early diagnosis and treatment of neurosyphilis.
