Effects of Tranexamic Acid on Blood Loss, Blood Transfusion, and Complications in Patients with Acute Stanford A Type Aortic Dissection Treated By Sun's Procedure

【Objective】To explore the effects of tranexamic acid on blood loss, blood transfusion, and complications in patients with acute Stanford A type aortic dissection treated by Sun's procedure. 【Methods】Sixtytwo cases (including 8 off cases) of patients with acute Stanford A type aortic dissection who were admitted from September 2012 to September 2014 were selected as the study subjects. All patients underwent Sun's procedure. By the random number table method, the patients were divided into the control group (31 cases, 26 cases completed, 5 off cases) and the observation group (31 cases, 28 cases completed, 3 off cases). The observation group was treated with an intravenous injection of tranexamic acid (10 mg/kg) before the skin incision, and 30 min later the injection was completed. Then, an intravenous infusion of 10 mg/(kg•h) was performed until the end of surgery. The control group was given normal saline according to the same procedure as tranexamic acid was given to the observation group. Many intraoperative and postoperative indicator levels were observed between the two groups such as: total drainage volume 24h after surgery, homologous blood transfusion, hemostasis rate of a second open surgery, incidence of complications, and fatality rate 30 days after surgery. The interleukin6 (IL6), tumor necrosis factor alpha (TNFα), hemoglobin (Hb), and platelet count (PLT) were detected as well. 【Results】The mechanical ventilation time, length of ICU stay, and hospitalization time of the observation group was significantly shorter than those of the control group, and the total drainage volume in 24h postoperative and homologous blood transfusion was significantly less than that of the control group (P<0.05). There were no significant differences between the two groups in Hb and PLT after surgery and 24h after surgery (P>0.05). The hemostasis rate of the second open surgery in observation group (0) was significantly lower than that in the control group (15.38%) (P<0.05). The incidence rates of postoperative pulmonary infection, acute lung injury, and neurological dysfunction in the observation group (3.57%, 14.29%, 17.86%) were significantly lower than those in the control group (23.08%, 38.46%, 46.15%) (P<0.05). 【Conclusion】The application of tranexamic acid in acute Stanford A type aortic dissection can protect blood as well as reduce the incidence of postoperative complications.

Effects of 112 Intervention Mode on Patients with 2Diabetes Mellitus

【Objective】To investigate the effect of 112 intervention mode on patients with 2diabetes mellitus. 【Methods】 One hundred cases of patients with 2diabetes mellitus who were filed from July 2010 to March 2012 in the community health service station of the hospital were divided into intervention group and control group, 50 cases in each. Patients in the control group were given conventional treatment, while patients in the intervention group received 112 intervention mode. A 18 months' followup was conducted on both groups at the same time. Changes of glycosylated hemoglobin (HbA1c), fasting plasma glucose (FPG), postprandial plasma glucose (2hPG), three acyl glycerine (TG), low density lipoprotein (LDL) and high density lipoprotein (HDL) were detected and compared between the two groups of patients before and after the intervention. 【Results】 In the intervention group, FPG, 2hPG, HbA1c and TG were effectively controlled. The difference was statistically significant compared with those of the same group before treatment and those of the control group (P<0.05).【Conclusion】 112 intervention mode has good therapeutic effect in patients with T2DM, can reduce patients' blood sugar, promote patients with good behavior habit formation and reduce the complications. It is worthy of clinical use.

Exploration of HDAC2 in the Pathogenesis of Osteoarthritis

【Objective】To investigate the possible biological roles of HDAC2, IL1β, and NO in the pathogenesis of osteoarthritis, and to understand the possible correlation of HDAC2 with IL1β, and NO with HDAC2. 【Methods】 ELISA was used to determine the amount of HDAC2 and IL1 beta ,and the method of nitrate reduction was detected the amount of NO in the synovial fluid of 51 patients with osteoarthritis (observation group). The results were compared with corresponding levels of HDAC2, IL1, and NO in 26 normal patients (control group). In addition, the relationships of HDAC2 with IL1 beta and HDAC2 with NO were explored. 【Results】The levels of HDAC2, IL1, and NO in the synovial fluids of the patients were higher in the experimental (observation) group than in the control group, with the differences beingstatistically significant (P<0.01). Using Spearman correlation analysis, it was found that for HDAC2 with IL1 beta and NO, the correlation coefficients were r=0.872 and 0.824,respectively (P<0.01); all indicators were positively correlated. 【Conclusion】HDAC2 may be a chemokine of inflammatory cytokines IL1 and NO, which can promote the formation of local noninfective inflammation of osteoarthritis.

Correlative Analysis of Urinary Albumin Excretion with Retinopathy in Patients with Type 2 Diabetes Mellitus

【Objective】 To analyze the relationship between urinary albumin excretion and diabetic retinopathy in type 2 diabetics mellitus. 【Methods】 Two hundred and sixty patients with type 2 diabetics mellitus were divided into normal albuminuria group(NA,n=41, 24hurinary albumin≤30mg), microalbuminuria group (MAU,n=118,30mg<24hurinary albumin<300mg) and macroalbuminuria group(CAU,n=101, 24hurinary albumin≥300mg). All the patients were measured in terms of 24hurinary albumin and examined by digital retinal photographs, thus the relationship between urinary albumin excretion and diabetic retinopathy in type 2 diabetics can be analyzed. 【Results】 The non proliferative diabetic retinopathy (NPDR) detection rate in the NA group was 9.8%, and the detection rate of proliferative diabetic retinopathy (PDR) was 4.8%; The detection rate of NPDR in the MAU group was 43.2%, and the detection rate of PDR was 16.1%; The detection rate of NPDR in the CAU group was 42.6%, and the detection rate of PDR was 34.7%; There were significant differences in the detection rates of NPDR and PDR in each group (P<0.05); Correlation analysis showed that urinary albumin excretion rate was positively correlated with the rate of DR in T2DM patients (P<0.05).【Conclusion】 The urinary albumin excretion rate of T2DM patients is significantly correlated with the occurrence of DR. The above factors may have a common pathophysiological basis.

Effects of CD44v6 and Integrin Beta1 on the Prognosis of Surgical Patients with Pancreatic Cancer

【Objective】To investigate the relationships between cell adhesion molecule CD44 variant exon6 (CD44v6) and integrin beta 1 with the prognosis of surgical patients with pancreatic cancer. 【Methods】Eightyfive patients with pancreatic cancer who received treatment in our hospital were chosen as the study subjects. The positive expression rates of CD44v6 and integrin beta1 in the pancreatic cancer tissues of the patients with different clinicalpathological features were deterimined. The correlation between the positive expression rates of CD44v6, integrin beta 1 and the levels of tumor markers (CEA, CA199 and CA125) was analyzed. 【Results】The positive expression rates of CD44v6, integrin beta 1, antigen (CEA), carbohydrate antigen 199 (CA199), and carbohydrate antigen 125 (CA125) in the death group were significantly higher than those in the survival group, the difference was statistically significant (P<0.05). Pancreatic cancer patients with old age, lower differentiation, distant metastasis, and invasion of vascular showed higher positive expression rates in CD44v6 and integrin beta1 , the differences were statistical significance (P<0.05). There were no significant differences showing the expression rates of CD44v6 and integrin beta 1 with patient gender, tumor size, and local lymph node invasion (P<0.05). . The positive expression rate of CD44v6 and integrin beta1 in pancreatic cancer patients were significantly positively correlated with CA199, CA125 and CEA, and the difference was statistically significant (P<0.05). 【Conclusion】The high expression of CD44v6 and integrin beta1 contributes to the poor prognosis in patients with pancreatic cancer, and is associated with tumor markers (CEA,CA199 and CA125). It can be used as an important indicator of the prognosis as well.

Evaluate of Sensation Restoration of Neurorrhaphy of Anterolateral Femoral Perforated Skin Flap in Repairing the Defect after Head and Neck Cancer Resection

【Objective】 To evaluate the sensation restoration of neurorrhaphy of anterolateral femoral perforated skin flaps in repairing the defect after head and neck cancer resection.【Methods】 Thirtyfive patients underwent simultaneous sensation restoration of neurorrhaphy of anterolateral femoral perforated skin flap from May,2013 to May, 2015 were selected . Among which 27 cases were of endtoend neurorrhaphy and 8 cases of endtoside neurorrhaphy; 9 cases of epineurium neurorrhaphy, and the rest of 26 cases of epineurium neurorrhaphy plus nerve bundle neurorrhaphy. The effect on the restoration of sensation was reviewed at 4,8,16 and 24 weeks after operation respectively. 【Results】All of the 35 flaps survived. Satisfying morphology for the maxillofacial region and PhaseOne healing on the donor site were obtained. During the 1 to 6 months' followup, the sensation of pain, touch and twopoint discrimination of the flaps were restored somewhat 4 weeks after surgery. The sensory function of flaps had reached a level above S2, which acquired the protective sensation. The excellent and good rate of sensation restoration was 65.7%. The color, texture and elasticity of skin flaps were satisfied. 【Conclusion】The lateral femoral cutaneous nerve of the donor site is characterized with its regular formation which is superficial ,flexible for flap designing, easy and convenient for operation etc.; on the recipient site, there is plenty of sensory nerves in maxillofacial region, and it is of great value to reconstruct the defect after resection of tumor in this region with anterolateral femoral perforated skin flaps to restore the sensation function therewith.

Analysis of the Clinical Characteristics and Risk Factors of Heart Failure in Patients with Maintenance Dialysis

【Objective】To investigate the clinical characteristics and risk factors of heart failure in patients with maintenance dialysis. 【Methods】Five hundred patients receiving maintenance dialysis in our hospital were selected as the study subjects. According to onset of the disease, they were divided into three groups: the diastolic heart failure (DHF) group, systolic heart failure (SHF) group, and normal cardiac function group. The clinical characteristics of the maintenance dialysis complicated with DHF and SHF were analyzed. Univariate and multivariate analyses were performed. 【Results】Of the 500 patients with maintenance dialysis, there were 184 (36.8%) complicated with DHF and 80 (16.0%) with SHF. The odds of hypertension, diabetes, and advanced age (≥70 years old) in the DHF and SHF groups were significantly higher than those in the normal cardiac function group. The incidence of hypertension in DHF group (30.43%,56/184) was significantly higher than that in SHF group (13.75%,11/80) (P<0.05). There were significant differences in age, systolic blood pressure, BNP(Btype natriuretic peptide), and serum uric acid between the DHF group, SHF group, and normal cardiac function group (P<0.05). The systolic blood pressure of the DHF group was higher than that of SHF group, while BNP and blood uric acid were significantly lower than those in SHF group (P<0.05). The multivariate Logistic regression analysis showed that the effects of hypertension and high systolic blood pressure on DHF were more significant, while the effects of high BNP and high blood uric acid on the SHF was more significant. 【Conclusion】There are differences in clinical manifestation and risk factors between maintenance dialysis complicated with DHF and SHF. In the clinic, the treatment plan should be made according to the type of heart failure in patients.

Clinical Value of AntiCyclic Citrullinated Peptide Antibodies Detection in Patients with Primary Sjogren's Syndrome

【Objective】To investigate the clinical application of anticyclic citrullinated peptide (CCP)antibodies detection in the development and prognosis of primary Sjogren's syndrome (PSS) . 【Methods】Clinical data and laboratory related indicators of 85 patients with PSS had been analyzed statistically. 【Results】 Anti CCP antibody presented 16.47% positive (14/85 cases) and 83.53% negative (71/85 cases); the difference of the specificity of anti CCP antibody in positive and negative patients, the positive rate of autoantibodies (SSA, SSB and total antibody ANA) and levels of ESR, CRP , IgA, IgG, IgM and RFIgM was not statistically significant (P>0.05); and the difference between the occurrence of joint pain and the incidence of erosive arthritis was statistically significant (P<0.05). 【Conclusion】 In PSS patients, the anti CCP antibody positive may be related to the occurrence of joint pain and erosive arthritis ; Higher anti CCP antibody levels are more likely to lead to the development of RA in patients with PSS.

Application Value of lung Computer Tomography Angiography in Diagnosing Pulmonary Embolism

【Objective】To investigate the application value of lung computer tomography angiography (CTA) in the diagnosis of pulmonary embolism (PE). 【Methods】Clinical data and CTA data of 67 cases of suspected PE patients in our hospital received during December 2013 to December 2015 were retrospectively analyzed. Comprehensive analysis of CTA imaging features, clinical manifestations, misdiagnosis, and embolus characteristics of pulmonary embolism were conducted based on the pathological examination. 【Results】There were 67 cases of suspected PE, in which 63 cases of PE were diagnosed by CTA and 62 PE patients were diagnosed pathologically. So the misdiagnosis rate of PE by CTA was only 1.58%, and there were no missed diagnosis. Among the 62 cases of patients with PE, 9 cases were of multisite embolism in pulmonary trunk, leaves, segment pulmonary, 12 cases involving the main pulmonary artery, 34 cases involving the right pulmonary artery, and 22 cases involving the left pulmonary artery. There were 78 emboli total among the 62 cases, include thrombosis (59), fat plugs (11), air plugs (5) and others (3). Thrombosis came from different places, such as pelvic thrombosis (24) and lower extremity deep venous thrombosis (19). 【Conclusion】CTA in the diagnosis of pulmonary embolism not only can display the position, shape, duration time, and indirect signs of embolus, but also can diagnose a disease that shows similar symptoms to PE. CTA is an intuitive and reliable method for diagnosing PE that has a low rate of misdiagnoses and missed diagnoses. It also can help find the source of emboli, which may provide the basis for clinical thrombolytic therapy.

Analysis of the Correlation between Vascular Endothelial Dysfunction and Coronary Heart Disease in Elderly Patients with Obstructive Sleep Apnea Syndrome

【Objective】To analyze the correlation between vascular endothelial dysfunction and coronary heart disease (CHD) in elderly patients with obstructive sleep apnea syndrome (OSAS). 【Methods】 A total of 32 cases of patients with OSAS in our hospital from February 2014 to August 2015 were selected as the OSAS group. Another 35 cases of patients with CHD and OSAS in the same time period at our hospital were selected as the OSAS and CHD group. Apneahypopnea index (AHI) and oxygen desaturation index (ODI) of patients in both groups were detected. Meanwhile, plasma endothelin (ET), nitric oxide (NO) and NO/ET ratio were compared between patients of the two groups, and the correlation between OSAS patients with vascular endothelial dysfunction and coronary heart disease were analyzed. 【Results】Compared to OSAS and CHD group, ODI, AHI, the lowest SpO2, and mean SpO2 were significantly decreased in the OSAS group; the difference had statistical significance (P<0.05). ET and NO/ET also decreased significantly in the OSAS group compared to the OSAS and CHD group. In contrast, NO levels significantly increase in the OSAS group compared to the OSAS and CHD group; the difference was statistically significant (P<0.05). 【Conclusion】Obstructive sleep apnea syndrome complicated with coronary heart disease in elderly patients has obvious effect on endothelial dysfunction and vascular endothelial. Therefore, active prevention and treatment of vascular endothelial dysfunction can improve the prognosis of the patients with OSAS and CHD.

Effects of Maca on the Reproductive Capacities and Sleep of Drosophila Melanogasters and Mice

【Objective】To study the effects of maca on the sexual ability, reproductive capacity, and sleep of Drosophila melanogasters and male mice. 【Methods】 Firstly, the mating numbers and hatching numbers of Drosophila melanogaster were detected by culturing with medium containing maca powder (0.1%, 0.2%). Then, the reproductive capacity of mice with the treatment of maca solution(0.1 g/kg, 0.2 g/kg) by gavage for 15 days were evaluated through tests of semen quality, histologic changes of the testis and epididymis, and filial mice. In addition, the effects of maca (0.1 g/kg, 0.2 g/kg, 0.4 g/kg) on sleep of mice were evaluated through observations of the direct sleeping number, sleeping latency and sleeping time induced by pentobarbital sodium, and sleep onset of mice treated by subhypnotic dosage of pentobarbital sodium. 【Results】Maca could significantly improve the mating rate and proliferation rate of flies. It enhanced the testis development and semen quality of male mice and increased fertility rate, the average litter size of parent mice, the average litter weight, and survival rate of filial mice. Sleeping test illustrated that maca didn't cause direct sleep of mice; however, it shortened the sleeping latency time, prolonged the sleeping time of mice induced by pentobarbital sodium, and raised the sleeping number of mice treated by subhypnotic dosage of pentobarbital sodium either. 【Conclusion】Maca shows effects on improving sexual ability and reproductive capacity in different animal models. Moreover, maca also improves sleep.

Analysis of Related Factors of Hemorrhage Events in Patients with Acute Coronary Syndrome

【Objective】To investigate the related factors of hemorrhage events in patients with acute coronary syndrome (ACS).【Methods】 Seven hundred and fiftythree cases diagnosed as ACS in our hospital from May 2005 to May 2015 were selected, and 12 of them complicated with hemorrhage were regarded as hemorrhage group according to the criterion of hemorrhagic events,while 48 of them without hemorrhage were selected by stratified random sampling method as no hemorrhage group. The clinical features, complications, treatment and drug application of the two groups were compared.【Results】The complicated hemorrhage rate of this research was 1.6%(12/753). The patients' age in the hemorrhage group were older ,and their hospitalstay time were longer than those in the no hemorrhage group (P<0.05). Additionally, patients complicated with cardiac insufficiency, renal insufficiency and past history of hemorrhage got higher rate of hemorrhage (P<0.05). ACS patients treated with clopidogrel and platelet membrane proteinⅡb/Ⅲa(GPⅡb/Ⅲa)receptor antagonist or underwent PCI and (or) CABG in superior hospitals got a higher rate of hemorrhage (P<0.05). 【Conclusion】 ACS patients with senior age, combined cardiac insufficiency, renal insufficiency, previous history of bleeding, once underwent PCI and / or CABG, or treated with clopidogrel or GP Ⅱb/ Ⅲa receptor antagonists are more vulnerable to hemorrhage.
