Abstract:【Objective】To investigate the effect of different doses of morphine and propofol combined with brachial plexus block in upper limb surgery in children. 【Methods】A total of 122 children who received upper limb surgery in our hospital were randomly divided into two groups with 61 cases in each group. The control group was given 0.05-0.1 mg morphine and 4-12 mg/h propofol,while the observation group was given 0.5 μg/kg morphine and 4-12 mg/h propofol. Brachial plexus block was performed in both groups. The recovery time,recovery time of spontaneous respiration,visual analogue scale (VAS) score,levels of oxygen saturation,heart rate,angiotensin Ⅱ (Ang Ⅱ) and aldosterone (ALD) before surgery (T1),0 horr (T2) and 24 hour (T3) after operation,and anesthetic related adverse reactions were compared between the two groups. 【Results】There was no significant difference in the recovery time of spontaneous breathing and VAS score between the two groups (P>0.05). The surgery recovery time of children in the observation group was shorter than that in the control group (P<0.05). There was no significant difference in oxygen saturation at each time point between the two groups (P>0.05). The heart rates at T2 and T3 in the two groups were lower than those at T1 in the same group; And the heart rate in the observation group was lower than that in the control group (P<0.05). There was no significant difference in the levels of Ang Ⅱ and aldosterone at T1 between the two groups (P>0.05). The levels of Ang Ⅱ and aldosterone at T2 and T3 were higher than those at T1 in the group (P<0.05),and the levels of Ang Ⅱ and aldosterone at T2 and T3 in the observation group were lower than those in the control group on average (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). 【Conclusion】Low doses morphine and propofol combined with brachial plexus block for upper limb surgery in children can shorten the recovery time,relieve pain,stabilize blood gas indicators,and reduce stress response. Thus,it is safe.
