发病年龄、慢性高血糖和严重低血糖对T1DM儿童患者认知功能的影响

摘要【目的】探讨发病年龄、慢性高血糖和严重低血糖对1型糖尿病(T1DM)儿童患者认知功能的影响。【方法】收集2017年11月至2019年10月在中南大学湘雅二医院诊治的61例T1DM儿童患者的临床资料,分析糖尿病风险因素发病年龄、慢性高血糖和严重低血糖对患者认知功能的影响。【结果】61例T1DM患儿中,7岁前发病患儿14例(早发性T1DM组),7岁及7岁以后发病患儿47例(晚发性T1DM组)。早发性T1DM及晚发性T1DM组患儿年龄、体重指数(BMI)、发病年龄、严重的低血糖发作次数及瞬间言语记忆力、瞬间视觉记忆力和延迟视觉记忆力评分比较,差异有统计学意义(P<0.05)。阶层回归模型分析结果显示:在智力测试中,HbA1c水平对模型有显著的正向影响,占总变异的9.60%;严重低血糖发作次数在加入了HbA1c这一变量后对智力评分有显著影响;在注意力测试中,发病年龄和HbA1c水平可预测注意力集中程度,并分别占11.60%和8.70%的总变异;在记忆测试中,发病年龄主要影响患者的瞬时视觉记忆和延迟视觉记忆,占25%的总变异。【结论】发病年龄、严重低血糖、高血糖对T1DM患儿的认知功能产生不同的影响。
