L-精氨酸联合整蛋白复方制剂对食管癌根治术后患者免疫功能及肿瘤标志物水平的影响

摘要【目的】探讨L-精氨酸联合整蛋白复方制剂对食管癌根治术后患者免疫功能及肿瘤标志物水平的影响。【方法】在本院行食管癌根治术的98例患者,随机分为观察组与对照组,每组各49例。观察组患者在术后接受L-精氨酸联合整蛋白复方制剂治疗,对照组患者在术后给予葡萄糖盐水肠内营养。比较两组患者治疗前后的T淋巴细胞亚群(CD4⁺、CD8⁺、CD4⁺/CD8⁺)、癌胚抗原(CEA)、糖类抗原19-9(CA19-9)、糖类抗原125(CA125)、细胞角蛋白19血清片段21-1(CYFRA21-1)、白蛋白(ALB)、前白蛋白(PA)、转铁蛋白(TRF)、尿素氮(BUN)、血肌酐(Scr)、丙氨酰氨基转移酶(ALT)、门冬氨酸转移酶(AST)水平。【结果】两组治疗后CD4⁺、CD4⁺/CD8⁺显著高于治疗前,CD8⁺显著低于治疗前(P<0.05),观察组治疗后CD4⁺、CD4⁺/CD8⁺显著高于对照组,CD8⁺显著低于对照组(P<0.05);两组治疗后CEA、CA19-9、CYFRA21-1、CA125显著低于治疗前,且观察组显著低于对照组(P<0.05);两组治疗后ALB、PA、TRF显著高于治疗前,且观察组显著高于对照组(P<0.05);两组治疗前、治疗后及治疗前后BUN、Scr、ALT、AST比较,差异均无统计学意义(P>0.05)。【结论】L-精氨酸联合整蛋白复方制剂可改善食管癌根治术后患者的营养状态和免疫功能,降低肿瘤标志物水平。

Abstract：【Objective】To investigate the effect of L-arginine combined with whole protein compound preparation on immune function and tumor marker level in patients after radical resection of esophageal cancer. 【Methods】 A total of 98 patients undergoing radical resection of esophageal cancer in our hospital were randomly divided into observation group and control group, with 49 patients in each group. Patients in the observation group received L-arginine combined with whole protein compound preparation after operation, and patients in the control group received glucose saline enteral nutrition after operation. The levels of T lymphocyte subsets (CD4⁺, CD8⁺, CD4⁺/CD8⁺), carcinoembryonic antigen (CEA), carbohydrate antigen 19-9 (CA19-9), carbohydrate antigen 125 (CA125), cytokeratin 19 serum segment 21-1 (CYFRA21-1), albumin (ALB), prealbumin (PA), transferrin (TRF), urea nitrogen (BUN), serum creatinine (Scr), alanyl aminotransferase (ALT) Aspartate transferase (AST) in the two groups before treatment and after treatment were compared. 【Results】 After treatment, CD4⁺, CD4⁺/CD8⁺ in the two groups were significantly higher than those before treatment, and CD8⁺ was significantly lower than those before treatment (P<0.05). After treatment, CD4⁺, CD4⁺/CD8⁺ in the observation group were significantly higher than those in the control group, and CD8⁺ was significantly lower than those in the control group (P<0.05); After treatment, CEA, CA19-9, CA125, CYFRA21-1 in both groups were significantly lower than those before treatment, and those in the observation group were significantly lower than those in the control group (P<0.05); After treatment, ALB, PA and TRF in the two groups were significantly higher than those before treatment, and those in the observation group were significantly higher than those in the control group (P<0.05); There was no significant difference in BUN, Scr, ALT and AST between the two groups before and after treatment (P>0.05). 【Conclusion】 L-arginine combined with whole protein compound preparation can improve the nutritional status and immune function of patients after radical resection of esophageal cancer, and reduce the level of tumor markers.
