Influential Factors of Carotid Plaque Instability in Patients with Large Atherosclerosis and Small Artery Occlusion in TOAST Classification of Acute Ischemic Stroke

Abstract:【Objective】 To investigate the influencing factors of carotid plaque instability in patients with acute ischemic stroke in TOAST classification. 【Methods】 Retrospective analysis was made on 127 patients with acute ischemic stroke admitted to our hospital from January 2019 to January 2022. According to TOAST classification, they were divided into two groups:large atherosclerosis group(n=48) and small artery occlusion group(n=69). The occurrence, stability and quantity of carotid plaque in the two groups were compared, and the related factors of plaque instability were analyzed by binomial logistic regression.【Results】Carotid plaque was found in 39 cases (81.25%) of the large atherosclerosis group and 59 cases (85.51%) of the small artery occlusion group. There was no significant difference between the two groups in the incidence of carotid plaque formation, the stability of carotid plaque, and the number of carotid plaque (P>0.05). The results of binomial logistic regression analysis showed that gender and plaque number were the influencing factors of carotid atherosclerotic plaque instability (P<0.05).【Conclusion】 Male and multiple plaques are the influencing factors of carotid plaque instability in patients with acute ischemic stroke. Clinical attention should be focused on such patients and intervention measures should be strengthened.
