龙胆泻肝汤联合多柔吡星注射剂治疗带状疱疹后遗神经痛的疗效观察

摘要【目的】观察龙胆泻肝汤联合多柔吡星注射剂治疗带状疱疹后遗神经痛(PHN)的临床疗效。【方法】收集2017年12月至2020年12月陇县妇幼保健院收治的80例PHN患者为研究对象,随机分为观察组和对照组,每组40例。对照组采取CT引导下椎间孔内注射多柔吡星治疗,观察组采取龙胆泻肝汤联合多柔吡星治疗,疗程均为1个月。比较两组患者临床疗效、中医证候积分及疼痛视觉模拟量表(VAS)评分。于治疗前后检测两组患者血清中炎症因子[肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-2(IL-2)]及外周血中T细胞亚群[CD4⁺、CD8⁺、CD4⁺/CD8⁺]的表达水平。对比两组不良反应发生率。【结果】观察组治疗总有效率为90.00%,显著高于对照组的72.50%(P<0.05)。与治疗前比较,两组治疗后中医证候积分、VAS评分显著降低且观察组显著低于对照组(P<0.05)。治疗后,两组患者血清TNF-α、IL-1β水平降低(P<0.05),IL-2水平升高(P<0.05),且与对照组比较,观察组TNF-α、IL-1β显著降低,IL-2显著增高(P<0.05)。治疗后,两组CD4⁺及CD4⁺/CD8⁺升高(P<0.05),CD8⁺降低(P<0.05),且相比于对照组,观察组变化更明显(P<0.05)。观察组不良反应发生率为20.00%,与对照组的17.50%相比,差异无统计学意义(P>0.05)。【结论】龙胆泻肝汤联合多柔吡星注射剂治疗PHN疗效显著,可促进疼痛缓解,对减轻炎症反应和增强免疫功能有着积极作用,且有较高的安全性。

Abstract：【Objective】 To observe the efficacy of Longdan Xiegan decoction combined with doxorubicin injection in the treatment of postherpetic neuralgia (PHN). 【Methods】 A total of 80 PHN patients admitted to Longxian Maternal and Child Health Hospital from December 2017 to December 2020 were collected in the study. They were randomly divided into two groups, with 40 cases in each group. Patients in the control group w received intraforaminal injection of doxorubicin under CT guidance, while patients in the observation group received Longdan Xiegan Decoction + doxorubicin for 1 month. The clinical efficacy, TCM syndrome score and visual analog scale (VAS) score were compared between the two groups. Blood was collected before and after treatment to detect serum inflammatory factors [tumor necrosis factor-α (TNF-α), interleukin-1β (IL-1β), Interleukin-2 (IL-2)] and the expression of immune cells [CD4⁺, CD8⁺, CD4⁺/CD8⁺], and the incidence of adverse reactions between the two groups were compared. 【Results】 The total effective rate of the observation group was 90.00%, which was significantly higher than that of the control group (72.50%, P<0.05). Compared with before treatment, TCM syndrome score and VAS score of the two groups decreased significantly after treatment; And the observation group was significantly lower than the control group (P<0.05). After treatment, the contents of TNF-α and IL-1β in the two groups decreased (P<0.05), while the contents of IL-2 increased (P<0.05). Compared with the control group, TNF-α and IL-1β in the observation group decreased more, while IL-2 increased more; The differences were statistical significance (P<0.05). After treatment, CD4⁺ and CD4⁺/CD8⁺ increased (P<0.05) and CD8⁺ decreased (P<0.05) in the two groups, and the changes in the observation group were more obvious than those in the control group (P<0.05). The incidence of adverse reactions in observation group was 20.00%, which was not statistically significant compared with 17.50% in control group (P>0.05). 【Conclusion】 Longdan Xiegan Decoction combined with doxorubicin injection has obvious curative effect on PHN, which can promote pain relief, reduce inflammatory reaction and enhance immune function. It is safe for clinical application.
