Abstract：[Obj ective]To explore the application of microanatomy of petrosal vein in microsurgical decom-pression for trigeminal neuralgia.[Methods]Clinical data of 547 patients with trigeminal neuralgia undergoing microsurgical decompression in our hospital were analyzed retrospectively.[Results]Of 127 patients undergo-ing surgical management of petrosal vein,5 patients died and 1 patient suffered cerebellum and brainstem in-farction.Of 420 patients who did not undergo surgical management of petrosal vein,no one died.[Conclusion]Petrosal vein compressing trigeminal nerve is one of the causes of trigeminal neuralgia.During microsurgical decompression for the treatment of trigeminal neuralgia,the protection and rational management of petroal vein are the important factors of decreasing the mortality.The management of petroal vein should be careful.
