Effects of Arthroscopic Meningoplasty Combined with Sodium Hyaluronate Injection on Curative Efficacy, Pain Degree and Knee Range of Motion in Patients with Meniscus Injury

摘要【目的】探究关节镜半月板成形术联合玻璃酸钠注射对半月板损伤患者疗效、疼痛程度及膝关节活动度的影响。【方法】选取2017年5月至2020年6月西安医学院第二附属医院收治的94例半月板损伤患者,采用随机数字表法分为观察组(给予关节镜半月板成形术联合玻璃酸钠注射治疗,成形术后立即予以2.5 mL透明质酸钠关节腔内注射,每周1次,连续4周)与对照组(给予关节镜半月板成形术治疗),每组47例。对比两组患者临床疗效、疼痛程度、膝关节活动度与不良反应发生情况。【结果】观察组患者优良率高于对照组(93.62% vs 78.72%)。术后4周,两组患者膝关节液炎症因子水平与术前比较均明显下降,且观察组白介素-6(IL-6)、白介素-1β(IL-1β)、肿瘤坏死因子-α(TNF-α)水平及VAS评分均低于对照组,膝关节活动度与膝关节功能评分均高于对照组,差异均有统计学意义(P<0.05)。两组患者并发症发生率(4.26% vs 2.13%)比较,差异无统计学意义(P>0.05)。【结论】使用关节镜半月板成形术联合玻璃酸钠注射治疗半月板损伤,疗效较好,可有效减轻炎症和疼痛,促进膝关节功能恢复。
Abstract:【Objective】To explore the effect of arthroscopic meningoplasty combined with sodium hyaluronate injection on the curative efficacy, degree of pain and range of motion of knee joint in patients with meniscus injury. 【Methods】 A total of 94 patients with meniscus injury treated in the Second Affiliated Hospital of Xi'an Medical College from May 2017 to June 2020 were randomly divided into observation group (treated with arthroscopic meningoplasty combined with sodium hyaluronate injection, 2.5 ml sodium hyaluronate intra-articular injection immediately after surgery, once a week for 4 weeks) and control group (treated with arthroscopic meningoplasty), with 47 cases in each group. The clinical efficacy, degree of pain, range of motion of knee joint and incidence of adverse reactions were compared between the two groups. 【Results】The excellent and good rate of the observation group was higher than that of the control group (93.62% vs 78.72%). Four weeks after operation, the levels of inflammatory factors in knee joint fluid of the two groups decreased significantly compared with those before operation, and the levels of interleukin-6 (IL-6) and interleukin-1β (IL-1 β),tumor necrosis factor-α(TNF-α) and VAS score in the observation group were lower than those in the control group, and the scores of knee activity and knee function were higher than those in the control group (P<0.05). There was no significant difference in the incidence of complications between the two groups (4.26% vs 2.13%) (P>0.05). 【Conclusion】 The combination of arthroscopic meningoplasty and sodium hyaluronate injection is effective in the treatment of meniscus injury, which can effectively reduce inflammation and pain and promote the recovery of knee function.
