脊柱骨折固定手术前后入路的临床疗效比较

摘要【目的】比较脊柱骨折前后入路内固定手术的临床治疗效果。【方法】选取本院2018年2月至2019年3月收治的150例脊柱骨折患者为研究对象,采取抽签法随机分为观察组与对照组各75例。观察组采用前路内固定手术,对照组采用后路内固定手术。统计两组手术切口长度、手术时间、术中出血量、术后引流量、术后下床时间、术后排气时间、住院时间;检测两组治疗前后伤椎变化情况[伤椎前缘高度百分比、伤椎后缘高度百分比、Cobb角];检测两组治疗前后应激反应指标[皮质醇(Cor)、C反应蛋白(CRP)、促肾上腺皮质激素(ACTH)、肿瘤坏死因子-α(TNF-α)],统计两组总并发症发生率。【结果】观察组手术切口长度、手术时间、术中出血量、住院时间显著高于对照组,术后引流量显著低于对照组(P<0.05);观察组术后伤椎前缘高度百分比、伤椎后缘高度百分比显著高于对照组,Cobb角显著低于对照组(P<0.05);观察组术后Cor、CRP、ACTH、TNF-α显著高于对照组(P<0.05);观察组总并发症发生率显著低于对照组(P<0.05)。【结论】较后路手术而言,前路内固定手术治疗脊柱骨折具有虽创伤大,但能有效改善脊髓损伤及脊髓功能的恢复,并发症少,应结合患者实际情况选择入路方案。
