Abstract：【Objective】To investigate the effect of immediate load and tilted implant in the restoration of tooth loss. 【Methods】From January 2015 to January 2017, 18 patients were prospectively selected. All patients were followed up for 3 years, staring from 6 months after surgery. Various indexes were recorded immediately after load (T0), 6-month (T1), 1-year (T2) and 3-year (T3) after treatment. There included marginal bone levels (MBLs), mid-buccal mucosal levels (MBMLs), implant success rate, implant stability quotient (ISQ), prosthodontic maintenance, prosthodontic complications, pink esthetic score (PES), and white esthetic score (WES). Then the aesthetic effect was discussed. 【Results】Of the 18 patients, 4 cases did not have complete follow-up records (1 implant failed before restoration, 1 case had bilateral first molars missing, and 2 cases were lost to follow-up). The remaining 14 cases with a total of 15 single implant crowns finished 4 years follow-up. The success rate of implant was 93.33% during the 3-year follow-up period (14/15). Compared to the data at T0, the value of MBLS and MBMLs increased at T1, T2 and T3 (P<0.05); and MBMLs value increased at T2 and T3 compared to T1 (P<0.05), however, the value of MBLS and MBMLs at T2 showed no significant difference compare to T3 (P>0.05). ISQ values of mesial and distal papilla at T1, T2 and T3 were lower than those at T0 (P<0.05),while the values at T2 and T3 had no obvious changes, which showed no significant different from those at T1 (P>0.05). Compared to T0, the RES and WES scores at T1, T2, and T3 were significantly higher, and the difference was statistically significant (P<0.05). After that, the RES and WES scores at T2 and T3 changed slightly, and there were no statistical differences compared to T1 (P>0.05).At T1, there were some prosthodontic maintenance issues involving the definitive implant crowns which included fracture of veneering ceramic and zirconia abutment as well as aesthetic concerns. At T2 and T3, there were no further prosthodontic maintenance issues reported and no screw loosening was evident. During the 3-year follow-up period, RES and WES had no significant changes. There were 1 case of implant loosening, 1 case of abutment pain and 1 case of gingivitis, all of which were significantly improved by active treatment. 【Conclusion】Application of immediate load and tilted implant in single-tooth implant restoration has high success rate and aesthetic effect.
