摘要【目的】探讨自拟中药处方红花化瘀汤对膝关节镜术后关节肿胀、疼痛等并发症的疗效。【方法】随机将本院2016年11月至2019年11月诊治96例膝关节镜术后患者分为对照组和观察组,各48例。对照组采用膝关节镜术后常规康复理疗治疗,观察组在对照组的基础上增加中药红花化瘀汤局部熏洗术侧膝关节,两组疗程均为8周。观察两组患者康复治疗前、治疗4周末、治疗8周末治疗侧膝关节肿胀值、疼痛视觉模拟评分法(VAS)评分、美国特种外科医院膝关节功能评分(Hospital for special surgery,HSS)。【结果】治疗4周及8周后,两组患者治疗侧膝关节肿胀值、VAS评分较治疗前均显著下降(P<0.05),且观察组低于对照组(P<0.05),4周末、8周末两组患者治疗侧膝关节肿胀值差异无统计学意义(P>0.05),VAS评分比较差异具有统计学意义(P<0.05);治疗4周及8周后,两组患者HSS评分较治疗前均显著上升(P<0.05),且治疗组高于观察组(P<0.05),4周末、8周末两组患者HSS评分比较差异具有统计学意义(P<0.05)。【结论】自拟中药处方红花化瘀汤对膝关节镜术后患者康复效果显著,可缓解患者关节肿胀和疼痛,改善患者膝关节功能,且安全、无不良反应,值得临床推广应用。
Abstract：【Objective】To explore the curative effect of Safflower Huayu Decoction on knee joint swelling, pain and other complications after arthroscopic surgery. 【Methods】 A total of 96 cases of the arthroscopic surgery patients from November 2016 ~ November 2019were randomly divided into the control group and the treatment group, with 48 cases in each group. Patients in the control group received routine rehabilitation treatment after arthroscopic surgery , while patients in the treatment group received traditional Chinese medicine Safflower Huayu (local fumigation and washing around knee) in addition to routine rehabilitation therapy. The course was 8 weeks in both groups. The swelling degree of knee joint, visual analogue scale (VAS) score and knee joint function score (HSS) were observed before rehabilitation, 4 weeks and 8 weeks after treatment. 【Results】After 4 and 8 weeks of treatment, the knee joint swelling value and VAS score of the treatment in the two groups of patients were significantly lower than before treatment (P<0.05); and those indicators in the treatment group were lower than those in the control group (P<0.05). Afterwards there was no significant difference in the swelling value of the treated knee joint between the two groups (P>0.05), while the difference in VAS scores was still statistically significant (P<0.05). After 4 and 8 weeks of treatment, the HSS scores of the two groups were higher than before treatment (P<0.05), while the treatment group was higher than the control group (P<0.05). There was a statistically significant difference in HSS scores between the two groups at the end of the 4th week and the 8th week (P<0.05). 【Conclusion】The Safflower Huayu Decoction has a significant effect on the rehabilitation of patients after knee arthroscopy. It can relieve the swelling of the joints, improve the pain of the patients, improve the function of the knee joints, and is safe and free of adverse reactions. It is worthy of clinical application.
