摘要【目的】探讨不同侧髂骨截骨方式对股骨远端C3骨折术后膝关节功能康复的影响。【方法】选取2017年7月至2019年7月于本院就诊收治的52例股骨远端C3型骨折患者作为研究对象,根据数字奇偶法分为对照组和观察组,每组各26例。术中植骨时对照组采取同侧髂骨截骨方式,观察组采取对侧髂骨截骨方式,对比两组患者手术时间、 膝关节活动时间,术后6个月膝关节功能评分优良率以及术后并发症发生率。【结果】观察组手术时间明显短于对照组,差异具有统计学意义(P<0.05),观察组患者患侧膝关节主动伸直至10°以及主动屈曲至120°时间均短于对照组,差异均有统计学意义(P<0.05);观察组患者膝关节功能评分优良率明显优于对照组,差异具有统计学意义(80.77% vs 53.85%,χ2=4.282,P=0.039);两组患者取骨区并发症发生率相比无明显差异(P>0.05)。【结论】对侧髂骨取骨与同侧取骨相比,对侧取骨有利于股骨远端C3型骨折术后膝关节功能康复。
Abstract:【Objective】To investigate the effect of different methods of iliac osteotomy on the rehabilitation of knee function after C3 fracture of distal femur. 【Methods】A total of 52 patients with C3 fracture of distal femur treated in our hospital from July 2017 to July 2019 were selected as the research objects. They were divided into control group and observation group according to the digital parity method, with 26 cases in each group. During intraoperative bone grafting, ipsilateral iliac osteotomy was used in the control group and contralateral iliac osteotomy was used in the observation group. The operation time, knee activity time, excellent and good rate of knee function score 6 months after operation and the incidence of postoperative complications were compared between the two groups.【Results】The operation time in the observation group was significantly shorter than that in the control group(P<0.05). The time of active straightening to 10 ° and active flexion to 120 ° in the observation group was shorter than that in the control group(P<0.05); The excellent and good rate of knee function score in the observation group was significantly better than that in the control group(80.77% vs 53.85%,χ2=4.282,P=0.039); There was no significant difference in the incidence of complications between the two groups(P>0.05).【Conclusion】Compared with ipsilateral bone extraction, contralateral bone extraction is conducive to the rehabilitation of knee function after operation of C3 fracture of distal femur.
