平均血小板体积和红细胞分布宽度与系统性红斑狼疮活动指数评分的相关性

摘要【目的】探讨平均血小板体积(MPV)和红细胞分布宽度(RDW)与系统性红斑狼疮(SLE)活动指数评分(SLEDAI评分)的相关性。【方法】在本院接受治疗的120例SLE患者,根据SLEDAI评分分为活动组和非活动组。选取同期体检的健康志愿者50例为对照组。比较各组的相关资料,分析MPV和RDW与SLEDAI评分的相关性,采用ROC曲线评估MPV和RDW预测患者SLE活动度的价值。【结果】(1)活动组RDW、MPV、血沉(ESR)、C反应蛋白(CRP)及IgG明显高于非活动组和对照组(P<0.05);活动组血红蛋白浓度(HGB)、C3及C4水平明显低于非活动组和对照组(P<0.05)。非活动组RDW、MPV、ESR、CRP及IgG明显高于对照组(P<0.05);非活动组HGB、C3及C4水平明显低于对照组(P<0.05)。(2)Pearson相关性分析显示,SLEDAI评分与MPV、RDW均呈正相关(均P<0.05)。(3)MPV预测患者SLE活动度的曲线下面积(AUC)和95%CI分别为0.594和(0.468~0.697),截断点为12.98 fL时,灵敏度、特异度分别为56.9%和77.4%。RDW水平的预测患者SLE活动度的AUC和95%CI分别为0.687和(0.417~0.783),截断点为14.23%时,灵敏度、特异度分别为67.8%和86.7%。【结论】MPV和RDW 具有潜在的评估SLE疾病活动度的临床价值。

Abstract：【Objective】To investigate the correlation between mean platelet volume (MPV), red blood cell distribution width (RDW) and systemic lupus erythematosus activity index score (SLEDAI). 【Methods】According to SLEDAI score, 120 SLE patients were divided into active group and inactive group. 50 healthy volunteers in the same period were selected as the control group. The correlation between MPV and RDW and SLEDAI score was analyzed. ROC curve was used to evaluate the value of MPV and RDW in predicting patients' activity.【Results】(1) RDW, MPV, ESR, CRP and IgG in active group were significantly higher than those in inactive group and control group (P<0.05); HGB, C3 and C4 levels in active group were significantly lower than those in inactive group and control group (P<0.05). The levels of RDW, MPV, ESR, CRP and IgG in the inactive group were significantly higher than those in the control group (P<0.05); the levels of HGB, C3 and C4 in the inactive group were significantly lower than those in the control group (P<0.05). (2) Pearson correlation analysis showed that SLEDAI score was positively correlated with MPV and RDW (P<0.05). (3) The AUC and 95%CI of MPV were 0.594 and 0.468-0.697, respectively. When the cutoff point was 12.98 fL, the sensitivity and specificity were 56.9% and 77.4%, respectively.The prediction of RDW level shows that the AUC and 95%CI of activity were 0.687 and (0.417-0.783), respectively. When the cutoff point was 14.23%, the sensitivity and specificity were 67.8% and 86.7%, respectively.【Conclusion】 MPV and RDW have potential clinical value in assessing SLE disease activity.
