七氟醚麻醉对肺癌根治手术患者炎性及氧化应激指标的影响

摘要 【目的】 探讨七氟醚麻醉对肺癌根治手术患者炎性及氧化应激指标的影响。【方法】 拟实施肺癌根治手术的患者90例,随机分为两组,各45例;观察组采用七氟醚实施并维持麻醉、对照组采用丙泊酚实施并维持麻醉。记录并比较两组的手术时间、苏醒时间、拔管时间、自主呼吸恢复时间、住院时间,手术前后患者血清E-选择素、可溶性白细胞介素2受体(sIL-2)、高迁移率族蛋白B1(HMGB1)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、丙二醛(MDA)、超氧化物歧化酶(SOD)、基质金属蛋白酶-2(MMP-2)、基质金属蛋白酶-9(MMP-9)水平。【结果】 观察组的苏醒时间、拔管时间、自主呼吸恢复时间均短于对照组(P<0.05),两组的手术时间、住院时间相比较差异无显著性(P>0.05);术前,两组患者的血清E-选择素、sIL-2、HMGB1、TNF-α、IL-6和MDA、SOD、MMP-2、MMP-9水平相比较差异均无显著性(P>0.05);术后24 h,观察组血清E-选择素、sIL-2、HMGB1、TNF-α、IL-6、MMP-2、MMP-9、MDA水平均低于对照组(P<0.05),SOD水平高于对照组(P<0.05)。【结论】 肺癌根治术中应用七氟醚麻醉有利于减轻手术引起的炎性反应及氧化应激反应。

Abstract【Objective】 To investigate the effect of sevoflurane anesthesia on inflammatory and oxidative stress indexes in patients with lung cancer undergoing radical operation.【Methods】 A total of 90 patients with lung cancer were randomly divided into two groups, 45 cases in each group; the observation group was given sevoflurane and maintained anesthesia, and the control group was given propofol and maintained anesthesia. The operation time, recovery time, extubation time, spontaneous breathing recovery time, hospital stay, serum E-selectin, soluble interleukin-2 receptor (SIL-2), high mobility group box 1 (HMGB1), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), malondialdehyde (MDA) and superoxide dismutase (SOD) were recorded and compared between the two groups Matrix metalloproteinase-2 (MMP-2), matrix metalloproteinase-9(MMP-9)levels were recorded and compared between the two groups.【Results】 The recovery time, extubation time and spontaneous breathing recovery time of the observation group were shorter than those of the control group (P<0.05), and there was no significant difference in the operation time and hospitalization time between the two groups (P>0.05); before operation, there was no significant difference in the serum E-selectin, sIL-2, HMGB1, TNF-α, IL-6 and MDA, SOD, MMP-2, MMP-9 levels between the two groups (P>0.05); 24 hours after operation, the observation group had no significant difference The levels of serum E-selectin, sIL-2, HMGB1, TNF-αand IL-6 in the observation group were lower than those in the control group (P<0.05); the levels of serum MMP-2, MMP-9 and MDA in the observation group were lower than those in the control group (P<0.05), and the level of SOD in the observation group was higher than that in the control group (P<0.05). 【Conclusion】 Sevoflurane anesthesia in radical operation of lung cancer can reduce the inflammatory reaction and oxidative stress reaction.
