Abstract:【Objective】 To explore the effect of follow-up management based on Smart System of Disease Management (SSDM) on self-management ability and quality of life in patients with rheumatoid arthritis (RA). 【Methods】 A total of 112 RA patients discharged from hospital from July 2016 to June 2017 were selected, and divided into treatment group and control group with 56 cases by random number table. The control group was given routine follow-up management, and the treatment group was applied for follow-up management based on Smart System of Disease Management (SSDM) . Followed up for 6 months, the self-management ability, clinical efficacy indicators, and quality of life were compared between the two groups. 【Results】 After the intervention, for the treatment group, the scores of good habits in the treatment group were (8.12±1.02) points, the self-disposal drug behavior score was (8.25±1.14) points, the self-monitoring symptom behavior score was (6.35±0.82) points, the self-psychological adjustment behavior score was (7.68±1.21) points, the self-recovery training score was (7.52±1.21) points, and the ability to deal with new problems in a timely manner score was (7.26±1.06) points. The self-management ability scores of the treatment group were significantly higher than those before the intervention and the control groups (P<0.05). After intervention, the score of disease activity score in 28joints (DAS28) in the treatment group was (2.54±0.40) points, significantly lower than those before self intervention and the control group (P<0.05). After the intervention, the physical function score of the treatment group was (13.24±1.45) points, the psychological function score was (14.35±2.12) points, the social function score was (14.65±2.20) points, and the health self-recognition score was (13.45±1.52) points. The scores of quality of life in the treatment group were significantly lower than those before the intervention and the control group (P<0.05, P<0.01). 【Conclusion】 The follow-up management based on Smart System of Disease Management (SSDM) is helpful to promote the self-management ability of patients with rheumatoid arthritis, relieve their clinical symptoms and improve the quality of life.
