摘要【目的】 探讨氨甲环酸(TXA)联合肾上腺素对全膝关节置换术后隐性失血量、输血率的影响。【方法】 将2015年10月至2017年10月在湘潭市中心医院进行全膝关节置换术的63例患者,随机分为观察组(n=38)和对照组(n=25)。观察组术前给予1 g TXA静脉注射,术中关节腔内注射2 g TXA +0.25 mg 肾上腺素(1∶200,000)混合液;对照组术前给予1 g TXA静脉注射,术中关节腔内注射2 g TXA。两组均术后夹闭引流管2 h,术后计算总失血量、隐性失血量、血红蛋白(Hb)下降值等指标,术后1周血管彩超检测下肢静脉血栓发生情况。【结果】 观察组总失血量、隐性失血量、输血率明显少于对照组,两组术后无下肢静脉血栓形成病例。【结论】 TXA联合稀释的肾上腺素可有效减少TKA术后隐性失血量和输血率,且无血栓形成并发症发生,值得临床推广应用。
