神经内镜血肿清除术治疗成人慢性硬膜下血肿的临床疗效

摘要【目的】探讨神经内镜血肿清除术治疗成人慢性硬膜下血肿(CSDH)的临床疗效。【方法】回顾性分析2021年6月至2023年6月本院收治的80例成人CSDH患者的临床资料,根据不同的治疗方式将患者分为对照组(行经顶结节单孔血肿冲洗及引流术,n=38)和观察组(行神经内镜硬膜下血肿清除术,n=42)。分别采用改良Rankin量表(mRS)、Barthel指数(BI)评估患者术前及术后神经功能、日常生活能力,比较两组住院时间及复发率。【结果】术前,两组mRS分级情况比较,差异无统计学意义(P>0.05)。术后,观察组mRS分级情况显著优于对照组,差异有统计学意义(P<0.05)。术前,两组BI分级情况比较,差异无统计学意义(P>0.05)。术后,观察组BI分级情况显著优于对照组,差异有统计学意义(P<0.05)。观察组住院时间短于对照组,术后复发率低于对照组,差异有统计学意义(P<0.05)。【结论】神经内镜手术能够更好地改善患者的神经功能及日常生活能力,缩短患者住院时间,降低术后复发率,具有较高的临床应用价值。

Abstract：【Objective】To investigate the clinical effect of neuroendoscopic hematoma removal in the treatment of chronic subdural hematoma (CSDH) in adults. 【Methods】 The clinical data of 80 adult patients with CSDH admitted to our hospital from June 2021 to June 2023 were retrospectively analyzed. According to different treatment methods, the patients were divided into the control group (irrigation and drainage of single-pore hematoma via parietal tuberculum, n=38) and the observation group (removal of subdural hematoma by neuroendoscopy, n=42). The modified Rankin Scale (mRS) and Barthel index (BI) were used to evaluate the neurological function and daily living ability of the patients before and after surgery, and the length of hospital stay and recurrence rate of the two groups were compared. 【Results】Before operation, there was no statistical significance in the distribution of mRS Scores between the two groups (P>0.05). After operation, the improvement of mRS Score in the observation group was significantly better than that in the control group, and the difference was statistically significant (P<0.05). Before operation, there was no significant difference in BI index classification between the two groups (P>0.05). After operation, BI index improvement in the observation group was significantly better than that in the control group, and the difference was statistically significant (P<0.05). The duration of hospitalization in the observation group was shorter than that in the control group, and the postoperative recurrence rate was lower than that in the control group, the difference was statistically significant (P<0.05).【Conclusion】Neuroendoscopic surgery can better improve the neurological function and daily living ability of patients, shorten the length of hospital stay, and have a lower postoperative recurrence rate, which has high clinical application value.
