Abstract:【Objective】 To analyze the relationship between the incidence of intradialytic hypotension (IDH) and cognitive function in hemodialysis patients. 【Methods】 A total of 182 patients with end-stage renal disease who underwent maintenance hemodialysis (MHD) in our hospital were selected for the study. The incidence of IDH within 3 months of MHD were recorded and the factors affecting IDH were analyzed. The cognitive function and quality of life in MHD patients with IDH and non-IDH were evaluated by assessment of MMSE and SF-36 for 6 months and 12 months. The survival rate of IDH and non-IDH patients were compared. 【Results】 (1) Of the 182 patients, IDH incidence was 33.0% (60/182). The age and ultrafiltration rate of patients in the IDH group were higher than those in the non-IDH group, while the plasma albumin and residual urine were lower than those in the non-IDH group (P<0.05). Logistic analysis found that age, ultrafiltration rate, and plasma albumin were independent influencing factors of IDH (P<0.05). (2)There were no significant differences in baseline cognitive function and quality of life score between the two groups before MHD (P>0.05). After 6 and 12 months of treatment, the MMSE and SF-36 scores in the IDH group were lower than those in the non-IDH group (P<0.05). The incidence of cognitive dysfunction in the IDH group at 6 and 12 months were 13.3% and 23.3%, respectively, which were higher than those in the non-IDH group 3.3% and 8.2%, respectively, (P<0.05). (3) Kaplan-Meier method and Log-rank test analysis showed that the survival time of IDH group was shorter than that of non-IDH group (P<0.05). 【Conclusion】 IDH has a high incidence in MHD patients. Age, low protein and ultra-ultrafiltration are risk factors of IDH. The occurrence of IDH is closely related to cognitive impairment and poor prognosis.
