Predictive Value of Seizure Duration, Cognitive Impairment and Electroencephalogram Grading for Prognosis in Patients with NCSE

【Objective】
To explore the predictive value of seizure duration, cognitive impairment and electroencephalogram (EGG) grading for the prognosis of patients with non-convulsive status epilepticus (NCSE).
【Methods】
Clinical data of 100 patients with NCSE were collected. A multidimensional prognostic evaluation system was established using the modified Rankin Scale (mRS) combined with the Mini-Mental State Examination (MMSE). Patients were divided into a poor prognosis group (n=37) and a good prognosis group (n=63). Clinical characteristics and EEG features of the two groups were compared. Univariate analysis was performed to screen factors related to prognosis, and multivariate Logistic regression analysis was used to identify independent risk factors affecting prognosis. The predictive performance of the regression model for patient prognosis was evaluated using the receiver operating characteristic (ROC) curve.
【Results】
The proportions of disturbance of consciousness, seizure duration ≥1 h, and EEG grades 3-6 in the poor prognosis group were significantly higher than those in the good prognosis group (P<0.05), while the proportion of generalized seizures was lower than that in the good prognosis group (P<0.05). Multivariate Logistic regression analysis showed that disturbance of consciousness, seizure duration, and EEG grading were independent risk factors for poor prognosis in NCSE patients (P<0.05), whereas generalized seizures served as a protective factor (P<0.05). The area under the curve (AUC) of the regression model was 0.929, with a sensitivity of 82.4% and a specificity of 90.4%.
【Conclusion】
Disturbance of consciousness, seizure duration, EEG grading, and generalized seizures are closely associated with the prognosis of NCSE patients. The regression model shows high predictive efficiency and is of significant value for guiding clinical treatment.

Clinical Efficacy of Sacubitril Valsartan Sodium Combined with Atorvastatin in the Treatment of Acute ST-Segment Elevation Myocardial Infarction Complicated with Heart Failure

【Objective】
To investigate the clinical efficacy of sacubitril valsartan sodium combined with atorvastatin in patients with acute ST-segment elevation myocardial infarction (STEMI) complicated with heart failure (HF).
【Methods】
A total of 80 patients with STEMI complicated with HF admitted to our hospital from June 2022 to October 2024 were enrolled and randomly divided into a control group and an observation group using a random number table, with 40 cases in each group. The control group received atorvastatin calcium tablets on the basis of conventional treatment, while the observation group was additionally given sacubitril valsartan sodium tablets on the basis of the control group. Clinical efficacy, cardiac function indexes [left ventricular ejection fraction (LVEF), left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic diameter (LVEDD)], oxidative stress indexes [superoxide dismutase (SOD), malondialdehyde (MDA)], myocardial injury markers [N-terminal pro-brain natriuretic peptide (NT-proBNP), creatine kinase isoenzyme (CK-MB), cardiac troponin I (cTnI)], vascular endothelial function indexes [vascular endothelial growth factor (VEGF), endothelin-1 (ET-1), nitric oxide (NO)], myocardial energy metabolism indexes [free fatty acid (FFA), circumferential end-systolic wall stress (cESS), myocardial energy expenditure (MEE)], and the incidence of adverse reactions were compared between the two groups.
【Results】
The total effective rate of the observation group was significantly higher than that of the control group (P<0.05). After treatment, LVEF in both groups was increased, while LVESD and LVEDD were decreased compared with baseline; the observation group had higher LVEF and lower LVESD and LVEDD than the control group (all P<0.05). MDA was decreased and SOD was increased in both groups, with lower MDA and higher SOD in the observation group (P<0.05). Levels of NT-proBNP, CK-MB and cTnI were all reduced in both groups, and were lower in the observation group (P<0.05). Levels of NO and VEGF were elevated while ET-1 was reduced in both groups, with higher NO and VEGF and lower ET-1 in the observation group (P<0.05). FFA, cESS and MEE were decreased in both groups, and were lower in the observation group (P<0.05). The incidence of adverse reactions in the observation group was lower than that in the control group (P<0.05).
【Conclusion】
Sacubitril valsartan sodium combined with atorvastatin shows favorable clinical efficacy in STEMI patients complicated with HF. It can regulate oxidative stress and myocardial energy metabolism, protect cardiomyocytes and vascular endothelial function, and presents good safety.

Application Effect of Social Support Combined with Psychological Intervention in Patients Undergoing Surgery for Cervical Insufficiency

【Objective】
To explore the application effect of social support combined with psychological intervention in patients undergoing surgery for cervical insufficiency.
【Methods】
A total of 100 patients with cervical insufficiency who underwent surgical treatment in our hospital from January 2022 to January 2025 were randomly divided into an observation group (received social support combined with psychological intervention) and a control group (received routine intervention) at a ratio of 1∶1, with 50 cases in each group. The hope level (Herth Hope Index, HHI score), health behavior (Health Promoting Lifestyle Profile Ⅱ, HPLP-Ⅱ score), family function (Family Adaptability and Cohesion Scale Ⅱ, FACES Ⅱ score) and pregnancy outcomes were compared between the two groups.
【Results】
After intervention, all dimension scores of HHI, HPLP-Ⅱ and FACES Ⅱ in the observation group were higher than those in the control group (P<0.05). The term delivery rate in the observation group was higher than that in the control group (P<0.05).
【Conclusion】
Social support combined with psychological intervention can improve the hope level, health behavior, family function and term delivery rate of patients with cervical insufficiency undergoing surgery, and optimize pregnancy outcomes.

Analgesic Efficacy of Preoperative Flurbiprofen Axetil Combined with Transversus Abdominis Plane Block after Laparoscopic Total Hysterectomy

【Objective】
To investigate the analgesic efficacy of preoperative flurbiprofen axetil combined with transversus abdominis plane (TAP) block after laparoscopic total hysterectomy (LTH).
【Methods】
A total of 102 patients scheduled for LTH were randomly divided into a control group (receiving preoperative flurbiprofen axetil analgesia) and an observation group (receiving preoperative flurbiprofen axetil plus TAP block), with 51 cases in each group. 【Results】
The observation group required lower doses of remifentanil and propofol, had a lower rescue analgesia rate, and showed lower Ramsay sedation scores at 2 h postoperatively compared with the control group (P<0.05). Awakening time and extubation time in the observation group were shorter than those in the control group (P<0.05). At 48 h after surgery, levels of PGE

, TNF-α, and HMGB-1 in the observation group were lower than those in the control group (P<0.05). BCS and QoR-40 scores at 6 h and 1 day postoperatively in the observation were higher than those in the control group (P<0.05). There was no statistically significant difference in the overall incidence of adverse reactions between the two groups (P>0.05).
【Conclusion】
Preoperative flurbiprofen axetil combined with TAP block significantly improves postoperative analgesia, reduces the dosage of analgesic agents, accelerates postoperative recovery, and demonstrates a favorable safety profile in patients undergoing LTH.

Application of Serum Soluble MHC Class Ⅰ Chain-Related Molecule A in Clinical Diagnosis of Gastric Cancer

【Objective】
To explore the significance of serum soluble MHC class Ⅰ chain-related molecule A (sMICA) level in the diagnosis of gastric cancer.
【Methods】
The differences in serum levels of sMICA, carcinoembryonic antigen (CEA), carbohydrate antigens (CA19-9, CA72-4) were compared among 92 newly diagnosed gastric cancer patients (gastric cancer group), 89 gastritis patients (gastritis group) and 86 healthy people (healthy group). The relationship between serum sMICA level and disease course in gastric cancer group was analyzed. Receiver operating characteristic (ROC) curve was drawn to analyze the clinical application value of serum sMICA level in screening and diagnosis of gastric cancer.
【Results】
The serum levels of sMICA, CEA, CA19-9 and CA72-4 in the gastric cancer group were higher than those in the gastritis group and the healthy group, and the levels of all indicators in the gastritis group were higher than those in the healthy group, with statistically significant differences (P<0.05). The serum sMICA level in gastric cancer patients with different stages and different degrees of metastasis gradually increased with the progression of the disease (P<0.05), but there was no significant correlation with the degree of differentiation of gastric cancer patients (P>0.05). The combined detection of sMICA+CEA+CA19-9+CA72-4 had high diagnostic efficacy for gastric cancer patients (P<0.05).
【Conclusion】
The detection of serum sMICA level is helpful for the screening and diagnosis of gastric cancer, and can be used as a biomarker for the clinical diagnosis and treatment of gastric cancer.

Clinical Efficacy and Safety of Flexible Ureteroscopic Lithotripsy with Electronic Ureteroscope Versus Percutaneous Nephrolithotomy in the Treatment of Large Renal Pelvic Stones

【Objective】
To explore the clinical efficacy and safety of flexible ureteroscopic lithotripsy with electronic ureteroscope (f-URL) versus percutaneous nephrolithotomy (PCNL) in the treatment of large renal pelvic stones.
【Methods】
A total of 102 patients with large renal pelvic stones (long diameter > 3.0 cm) admitted to our hospital from March 2022 to March 2024 were selected. They were divided into a control group and an observation group using a random number table, with 51 cases in each group. The control group was treated with PCNL, while the observation group was treated with f-URL. Perioperative indicators (intraoperative blood loss, operation time, average length of hospital stay), incidence of adverse reactions, stone clearance rate, and proportion of conversion to open surgery during operation were compared between the two groups.
【Results】
The operation time of the observation group was longer than that of the control group, while the intraoperative blood loss was less and the average length of hospital stay was shorter than those of the control group, with statistically significant differences (P<0.05). There were no statistically significant differences in stone clearance rate or the proportion of conversion to open surgery between the two groups (P>0.05). The incidence of postoperative complications in the observation group was 5.88% (3/51), which was lower than 23.53% (12/51) in the control group, with a statistically significant difference (P<0.05).
【Conclusion】
Both f-URL and PCNL are effective surgical methods for treating large renal pelvic stones (long diameter > 3.0 cm). Compared with PCNL, f-URL has obvious advantages in terms of intraoperative blood loss, length of hospital stay, and incidence of adverse reactions.

Effects of Mind Map Combined with PBL Teaching Model on Theoretical Scores, Clinical Practical Skills and Teaching Satisfaction of Residents in Respiratory Medicine

【Objective】
To investigate the effects of mind map combined with problem-based learning (PBL) teaching model on theoretical scores, clinical practical skills and teaching satisfaction of residents in standardized residency training in respiratory medicine.
【Methods】
A total of 122 residents in respiratory medicine who received standardized training in our hospital from January 2022 to January 2025 were enrolled and randomly divided into a control group (received traditional teaching) and an observation group (received mind map combined with PBL teaching), with 61 cases in each group. The teaching effects were compared between the two groups.
【Results】
The scores of objective questions, subjective questions and total theoretical scores in the observation group were higher than those in the control group (P<0.05). The scores of medical inquiry, physical examination, basic skill operation, specialized skill operation in respiratory medicine, medical humanistic quality and total clinical skill scores in the observation group were higher than those in the control group (P<0.05). The teaching satisfaction rate in the observation group was 96.72%, which was higher than 83.61% in the control group, with a statistically significant difference (P<0.05).
【Conclusion】
Mind map combined with PBL teaching model can significantly improve the theoretical scores, clinical practical skills and teaching satisfaction of residents in respiratory medicine, and is an effective strategy to optimize the quality of standardized residency training.

Correlation Analysis between Waist-to-Hip Ratio and Blood Pressure, Blood Lipids, and Body Mass Index among 17,655 Elderly Physical Examinees in Qilian Community, Baoshan District, Shanghai

【Objective】
To explore the correlation between waist-to-hip ratio (WHR) and blood pressure, blood lipids, and body mass index (BMI) among 17,655 elderly physical examinees in Qilian Community, Baoshan District, Shanghai.
【Methods】
A total of 17,655 elderly individuals who underwent physical examinations in Qilian Community, Baoshan District from March 2018 to November 2024 were selected. Their height, weight, waist circumference(WC), hip circumference(HC), and blood pressure were recorded, and WHR and BMI were calculated. Based on WHR, the elderly were divided into Group W1 (WHR < 0.8, n=365), Group W2 (0.8 ≤ WHR < 0.9, n=6469), Group W3 (0.9 ≤ WHR < 1.0, n=9885), and Group W4 (WHR ≥ 1.0, n=936). The WHR levels of each group were compared. Pearson linear correlation was used to evaluate the correlation between WHR and blood pressure, blood lipids, and BMI.
【Results】
The levels of BMI, systolic blood pressure, diastolic blood pressure, WC, HC, and WHR in elderly male examinees were higher than those in elderly female examinees, while serum total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), and low-density lipoprotein cholesterol (LDL-C) were lower in elderly males than in elderly females, with statistically significant differences (all P<0.05). With the increase in WHR, blood pressure, TC, LDL-C, and BMI gradually increased, while HDL-C gradually decreased, and the differences were statistically significant (all P<0.05). Pearson correlation analysis showed that WHR had an extremely weak negative correlation with HDL-C (r=-0.105, 95%

:-0.120--0.090); and extremely weak positive correlations with systolic blood pressure, diastolic blood pressure, TC, LDL-C, and BMI (

Partial correlation analysis (after controlling for BMI) revealed that the negative correlation between WHR and HDL-C weakened (r=-0.082, P<0.001), while the positive correlations between WHR and blood pressure/blood lipids did not change in direction (|r|<0.05). The WHR of elderly males in Group W1, Group W2, Group W3, and Group W4 was significantly higher than that of elderly females, with statistically significant differences (all P<0.05).
【Conclusion】
WHR is associated with the health of the elderly, and maintaining a reasonable WHR is the foundation for the elderly to maintain good health.

Prognostic Impact of Stratified Intervention Strategies Based on Intoxication Severity in Patients with Acute Alcohol Poisoning under Observation in the Emergency Department

【Objective】
To investigate the prognostic impact of stratified intervention strategies based on intoxication severity in patients with acute alcohol poisoning under observation in the emergency department.
【Methods】
Clinical data of 240 patients with acute alcohol poisoning who received emergency observation in our hospital from January 2022 to August 2024 were retrospectively analyzed. The patients were divided into an observation group (n=122) and a control group (n=118) according to different intervention methods. The control group received routine emergency interventions, while the observation group received stratified interventions based on intoxication severity. The incidence of adverse events, emergency observation duration, and prognosis were compared between the two groups. Multivariate Logistic regression analysis was performed to identify factors associated with patient prognosis.
【Results】
There was no statistically significant difference in the distribution of intoxication severity between the observation group and the control group (P>0.05). The total incidence of adverse events in the observation group was 24.59% (30/122), which was significantly lower than 55.08% (65/118) in the control group, with a statistically significant difference (
χ
2
=8.364, P=0.004). The mean emergency observation duration in the observation group was significantly shorter than that in the control group (P<0.05). Statistically significant differences were found in the proportion of poor prognosis among patients with different intoxication severity, intervention methods, underlying diseases, alcohol consumption, and time to hospital presentation (P<0.05). Multivariate Logistic regression analysis showed that severe intoxication, absence of stratified intervention, and comorbid underlying diseases were independent risk factors for poor prognosis (P<0.05).
【Conclusion】
Conclusion Stratified intervention strategies based on intoxication severity can significantly reduce the incidence of adverse events and shorten the observation duration in patients with acute alcohol poisoning under emergency observation. Intoxication severity, intervention modality, and underlying diseases are key prognostic factors, and targeted emergency intervention protocols should be formulated clinically.

Application of Serum Tumor Markers Combined with Narrow-Band Imaging-Magnifying Endoscopy in the Differential Diagnosis of Benign and Malignant Early Gastrointestinal Tumors

【Objective】
To investigate the value of serum tumor markers combined with narrow-band imaging-magnifying endoscopy (NBI-ME) in differentiating benign and malignant early gastrointestinal tumors.
【Methods】
A total of 216 patients who underwent endoscopic examination and treatment were enrolled. According to postoperative pathological results, 104 patients with malignant gastrointestinal tumors were assigned to the observation group, and 112 patients with benign lesions were assigned to the control group. General clinical data, serum tumor markers [carcinoembryonic antigen (CEA), tumor-specific growth factor (TSGF), carbohydrate antigen 19-9 (CA19-9), and alpha-fetoprotein (AFP)], and NBI-ME features were compared between the two groups. The lesion characteristics and diagnostic accuracy obtained by conventional white-light endoscopy and NBI-ME were compared. Receiver operating characteristic (ROC) curves were used to evaluate the diagnostic performance of the relevant indicators.
【Results】
Serum levels of CEA, TSGF, CA19-9, and AFP in the observation group were significantly higher than those in the control group. The proportions of discontinuous lesion borders, irregular surface microstructures, and irregular microvascular patterns in tumor tissues were significantly higher in the observation group than in the control group (P<0.05). The NBI-ME image scores were significantly higher than those of conventional white-light endoscopy (P<0.05). ROC analysis showed that the combination of serum tumor markers and NBI-ME had the highest diagnostic efficacy, with the accuracy, sensitivity, and specificity of 94.44%, 95.19%, and 93.75%, respectively.
【Conclusion】
The combined use of serum tumor markers and NBI-ME demonstrates a high positive detection rate in patients with early gastrointestinal tumors and it is worthy of clinical application.

Effects of Nutritional Support Intervention Combined with Staged Standardized Psychological Support on Patients Undergoing Radiotherapy with Head and Neck Tumors

【Objective】
To investigate the effects of nutritional support intervention combined with staged standardized psychological support on patients undergoing radiotherapy with head and neck tumors.
【Methods】
A total of 112 patients with head and neck tumors receiving radiotherapy were randomly divided into a control group and an observation group, with 56 patients in each group. The control group received routine interventions, while the observation group received nutritional support intervention combined with staged standardized psychological support. Nutritional indicators [prealbumin (PA), albumin (ALB), hemoglobin (Hb), and transferrin (TRF)], psychological status [Mini-Mental Adjustment to Cancer Scale (Mini-MAC-29) scores], quality of life [Quality of Life Instruments for Cancer Patients-Head and Neck Cancer (QLICP-HN) scores], and the incidence of oral mucositis were compared between the two groups before and after the intervention.
【Results】
After the intervention, levels of PA, ALB, Hb, and TRF in the observation group were significantly higher than those in the control group (P<0.05). The Mini-MAC-29 scores in the observation group were significantly lower than those in the control group (P<0.05), while the QLICP-HN scores were significantly higher (P<0.05). In addition, the overall incidence of oral mucositis in the observation group was significantly lower than that in the control group (P<0.05).
【Conclusion】
Nutritional support intervention combined with staged standardized psychological support can effectively improve nutritional status, psychological status, and quality of life in patients with head and neck tumors undergoing radiotherapy, and reduce the incidence of oral mucositis.

Clinical Effect of Combining Dapagliflozin and Qili Qiangxin Capsule on Conventional Therapy for Chronic Heart Failure with Heart-Kidney Yang Deficiency Syndrome

【Objective】
To investigate the clinical effect of combining dapagliflozin and Qili Qiangxin Capsule on conventional therapy for chronic heart failure (CHF) with heart-kidney yang deficiency syndrome.
【Methods】
Clinical data of 120 patients with CHF and heart-kidney yang deficiency syndrome admitted to Shibei Hospital of Jing'an District from January 2022 to December 2024 were retrospectively analyzed. Patients were divided into a control group (receiving oral dapagliflozin based on conventional therapy, n=60) and an observation group (receiving oral Qili Qiangxin Capsule in addition to the control group regimen, n=60) according to different treatment methods. Cardiac function indicators [left ventricular ejection fraction (LVEF), stroke volume (SV), cardiac index (CI)], traditional Chinese medicine (TCM) syndrome scores, and adverse reactions were compared between the two groups.
【Results】
After treatment, LVEF, SV, and CI in both groups were higher than before treatment, and these indicators in the observation group were higher than those in the control group, with statistically significant differences (P<0.05). After treatment, the primary syndrome scores, secondary syndrome scores, and total scores in both groups were lower than before treatment, and the scores in the observation group were lower than those in the control group, with statistically significant differences (P<0.05). The total incidence of adverse reactions was 8.33% (5/60) in the observation group and 6.67% (4/60) in the control group, with no statistically significant difference between the two groups (χ

【Conclusion】
Combining dapagliflozin with Qili Qiangxin Capsule in addition to conventional therapy can significantly improve cardiac function and alleviate clinical symptoms in patients with CHF and heart-kidney yang deficiency syndrome, with a favorable safety profile.

Clinicopathological Characteristics and Prognostic Analysis of BRAF V600E-Mutant Gastrointestinal Stromal Tumors

【Objective】
To investigate the clinicopathological characteristics of patients with BRAF V600E mutant gastrointestinal stromal tumors (GISTs) and their impact on prognosis.
【Methods】
A retrospective analysis was conducted on the clinical data of 58 patients with BRAF V600E mutant GISTs (mutation group) and 92 patients with BRAF wild-type GISTs (wild-type group). Clinicopathological features and survival outcomes were compared between the two groups. Univariate analysis and Cox proportional hazards regression models were used to identify independent prognostic factors for GIST.
【Results】
Compared with the wild-type group, the mutation group had a higher proportion of patients aged ≥55 years, tumors larger than 5 cm, and advanced TNM stage (stage Ⅲ+Ⅳ) (all P<0.05). Survival analysis showed that the mean overall survival (OS) and disease-free survival (DFS) in the mutation group were significantly shorter than those in the wild-type group (P<0.05). Univariate Cox regression analysis indicated that age, tumor size, TNM stage, and BRAF V600E mutation were adverse prognostic factors for GIST. Multivariate Cox regression analysis further demonstrated that age, tumor size, TNM stage, and BRAF V600E mutation were independent risk factors for poor prognosis in GIST.
【Conclusion】
Patients with BRAF V600E mutant GISTs exhibit poorer clinicopathological features and prognosis. Age, tumor size, TNM stage, and BRAF V600E mutation are independent prognostic risk factors for GIST.

Clinical Efficacy of Fangji Huangqi Decoction in Patients with Acute Ankle Sprain

【Objective】
To investigate the clinical efficacy of Fangji Huangqi Decoction in the treatment of patients with acute ankle sprain.
【Methods】
A total of 87 patients with acute ankle sprain treated in our hospital from January 2020 to June 2023 were enrolled and divided into an observation group (n=44) and a control group (n=43) according to the order of admission. The control group received conventional Western medicine treatment, while the observation group was additionally given Fangji Huangqi Decoction on the basis of conventional Western medicine treatment. Both groups were treated for 1 week. The swelling subsidence time, ecchymosis subsidence time, pain level [Visual Analogue Scale (VAS) score], adverse reactions, joint function recovery [American Orthopaedic Foot and Ankle Society ankle-hindfoot score (AOFAS)], and total effective rate were compared between the two groups.
【Results】
After treatment, the swelling subsidence time and ecchymosis subsidence time in the observation group were significantly shorter than those in the control group (P<0.05). The VAS scores in the observation group at 3 days and 1 week of treatment were lower than those in the control group (P<0.05). The AOFAS scores in the observation group at 3 days and 1 week of treatment were higher than those in the control group (P<0.05). The total effective rate of the observation group was 100.00%, which was higher than 88.37% of the control group, with a statistically significant difference (P<0.05). No obvious adverse reactions were observed in either group during treatment.
【Conclusion】
Fangji Huangqi Decoction can effectively relieve swelling and pain after acute ankle sprain, promote rapid recovery of joint function, and has high safety. It can be used as an adjuvant treatment for patients with acute ankle sprain.

Effects of Online Cognitive Behavioral Therapy for Insomnia on Sleep Quality, Anxiety and Depressive Symptoms in Patients with Anxiety Disorders

【Objective】
To investigate the effects of online cognitive behavioral therapy for insomnia (CBTI) on sleep quality, anxiety and depressive symptoms in patients with anxiety disorders.
【Methods】
A total of 50 patients with anxiety disorders were recruited from Affiliated Encephalopathy Hospital of Zhengzhou University via outpatient service, posters and WeChat Moments from April 2025 to November 2025. They were randomly divided into an observation group and a control group using a random number table, with 25 cases in each group. The control group received sleep hygiene education and relaxation training, while the observation group received online CBTI intervention for 6 weeks, followed by a 3 months follow-up. Sleep quality (Pittsburgh Sleep Quality Index, PSQI), insomnia severity (Athens Insomnia Scale, AIS), anxiety symptoms (Hamilton Anxiety Scale, HAMA), depressive symptoms (Hamilton Depression Scale, HAMD) and self-efficacy (General Self-Efficacy Scale, GSES) were compared between the two groups before intervention, at 3 weeks, 6 weeks, 1 month and 3 months after intervention.
【Results】
The total scores of PSQI, AIS, HAMA and HAMD in both groups at 3 weeks, 6 weeks, 1 month and 3 months after intervention were lower than those before intervention, and the scores in the observation group were lower than those in the control group (P<0.05). The total GSES scores in both groups at the above time points were higher than those before intervention, and the score in the observation group was higher than that in the control group (P<0.05).
【Conclusion】
Online CBTI intervention can significantly improve sleep quality, relieve anxiety and depressive symptoms, and enhance self-efficacy in patients with anxiety disorders, which is worthy of clinical application.

Application of Prone Ventilation Positioning Intervention Based on the ABC-X Theory in Patients with Severe ARDS

Application of Different Cervical Pretreatment Methods in Preoperative Preparation of Postmenopausal Patients Undergoing Hysteroscopy Surgery

【Objective】
To explore the efficacy effects of different cervical pretreatment methods in the preoperative preparation of postmenopausal patients undergoing hysteroscopic surgery.
【Methods】
A total of 120 postmenopausal patients who underwent hysteroscopic surgery in our hospital from January 2023 to December 2024 were selected and randomly divided into group A, group B, and group C, with 40 cases in each group. Group A was pretreated with a disposable cervical dilator 60 minutes before the operation, group B was intravenously injected with 80 mg phloroglucinol 30 minutes before the operation, and group C was placed with 400 μg misoprostol in the posterior vaginal fornix 12 hours before the operation. The total effective rate of cervical softening, dilation time, and the incidence of adverse reactions were compared among the three groups.
【Results】
The total effective rate of cervical softening in group A and group B was significantly higher than that in group C (P<0.05), however, there was no significant difference between group A and group B (P>0.05). The cervical dilation time in group A and group B was significantly shorter than that in group C (P<0.05), but there was no significant difference between group A and group B (P>0.05). The total incidence of adverse reactions in group A and group B was significantly lower than that in group C (P<0.05), however, there was no significant difference between group A and group B (P>0.05).
【Conclusion】
In the preoperative preparation of postmenopausal patients undergoing hysteroscopic surgery, both the disposable cervical dilator and phloroglucinol can effectively soften the cervix, shorten the dilation time, and have a low incidence of adverse reactions, with better effects than misoprostol. They can be considered as preferred clinical options.

Analysis of the Prognostic Value of Serum TLR4, sIL-2R Combined with HLA-B27 Levels in Predicting Infection in Premature Infants

【Objective】
To analyze the predictive value of serum Toll-like receptor 4 (TLR4), soluble interleukin-2 receptor (sIL-2R) combined with human leukocyte antigen B27 (HLA-B27) levels for the prognosis of infection in preterm infants.
【Methods】
A retrospective analysis was conducted on the medical records of 197 premature infants with infections admitted to our hospital from December 2022 to April 2025. Before the treatment, pathogenic bacteria in the infected specimens from all children were identified. The infants were followed up until discharge and they were divided into the good prognosis group (166 cases) and the poor prognosis group (31 cases) according to the prognosis of infection in premature infants. The levels of serum HLA-B27, sIL-2R and TLR4 and the clinical data of the two groups were compared. The influencing factors for poor prognosis of infection in premature infants were assessed; and the predictive values of each detection index for poor prognosis of infection in premature infants were analyzed.
【Results】
A total of 157 pathogenic bacteria were detected in 197 cases of premature infants with infection comprising 93 Gram-negative bacteria and 64 Gram-positive bacteria . Serum TLR4, sIL-2R and HLA-B27 levels in the poor prognosis group were higher than those in the good prognosis group (P<0.05). The length of hospital stay in the poor prognosis group was longer than that in the good prognosis group (P<0.05), and the proportion of newborns with a 5-minute Apgar score of ≤7 in the poor prognosis group was higher than that in the good prognosis group (P<0.05). The ROC curve analysis revealed that the area under the curve (AUC) values of serum TLR4, sIL-2R, HLA-B27 level alone and the combination of the three for predicting poor prognosis of infection in preterm infants were 0.805, 0.846, 0.801, and 0.909 respectively (P<0.05). Moreover, the combined predictive value of these three markers was significantly higher (P<0.05). Logistic regression analysis revealed that serum levels of TLR4 > 404.65 mg/L, sIL-2R > 387.21 ng/L, and HLA-B27 > 99.23 U/mL, as well as a neonatal Apgar score ≤ 7, were independent risk factors for poor prognosis in preterm infants with infection (all P<0.05).
【Conclusion】
The levels of serum TLR4, sIL-2R and HLA-B27 have significant value in predicting the prognosis of infection in premature infants, and the combined predictive value of the three is even higher.

Comparison of Antihypertensive Effects and Impacts on Blood Pressure Variability between Nifedipine Controlled-Release Tablets and Amlodipine Besylate Tablets in Patients with Morning Hypertension

【Objective】
To compare the antihypertensive effects and impacts on blood pressure variability between nifedipine controlled-release tablets and amlodipine besylate tablets in patients with morning hypertension.
【Methods】
A total of 109 patients with morning hypertension admitted to our hospital from April 2021 to April 2024 were enrolled and randomly divided into group A (treated with nifedipine controlled-release tablets, n=54) and group B (treated with amlodipine besylate tablets, n=55). Both groups were treated for 12 weeks. The clinical efficacy, incidence of adverse reactions, morning systolic blood pressure (SBP), morning diastolic blood pressure (DBP), morning blood pressure target achievement rate, and blood pressure variability before and after treatment were compared between the two groups.
【Results】
The total effective rates of group A and group B were 96.30% (52/54) and 92.73% (51/55), respectively, with no statistically significant difference (χ
2
=0.667, P=0.414). At 4, 8, and 12 weeks of treatment, morning DBP and SBP in both groups were lower than those before treatment, and those in group B were lower than group A (P<0.05). There was no significant difference in morning blood pressure target achievement rate between the two groups at 4 and 8 weeks (P>0.05); at 12 weeks, the rate in group B was significantly higher than that in group A (P<0.05). After treatment, systolic blood pressure variability (SBPV) and diastolic blood pressure variability (DBPV) in both groups were significantly lower than those before treatment, and those in group B were significantly lower than group A (P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups (χ

【Conclusion】
Both nifedipine controlled-release tablets and amlodipine besylate tablets can effectively reduce morning blood pressure, but amlodipine besylate tablets perform better in stabilizing blood pressure, improving the morning blood pressure target achievement rate and reducing blood pressure variability.

Influence of Autologous PRP on Endometrial Thickness and Endometrial Receptivity in Patients with Thin Endometrium Evaluated by Three-Dimensional Ultrasound Volume Imaging

【Objective】
To investigate the effect of autologous platelet-rich plasma (PRP) on endometrial thickness (EMT) and endometrial receptivity in patients with thin endometrium by using three-dimensional ultrasound volumetric imaging.
【Methods】
From August 2023 to March 2025, 120 patients with thin endometrium (during the mid-luteal phase, EMT ≤ 7 mm) were selected. Autologous PRP was perfused into the uterine cavity at the late follicular phase, and three-dimensional ultrasound volumetric imaging was performed before 1 week and 48 hours after the treatment to evaluate the changes in EMT, endometrial volume (EV), vascularization index (VI), blood flow index (FI), and continuity of the endometrial-myometrium junction (JZ) before and after the treatment. The clinical pregnancy rate 4 weeks after the first embryo transfer was statistically analyzed, and the relationship between the improvement degree of three-dimensional ultrasound volumetric imaging parameters and the pregnancy outcome was analyzed.
【Results】
After autologous PRP treatment for patients with thin endometrium, EMT increased from (6.2 ± 0.5) mm to (7.8 ± 0.6) mm, EV increased from (2.1 ± 0.4) mL to (3.7 ± 0.5) mL, VI increased from (0.82 ± 0.21) to (1.53 ± 0.33), and the differences were statistically significant (P<0.05). The proportion of patients with JZ continuity interruption exceeding 50% of the circumference decreased from 56.7% to 24.2% (P<0.05). The clinical pregnancy rate after autologous PRP treatment for patients with thin endometrium was 42.5% (51/120). The clinical pregnancy rates of patients with a pre-to-post-treatment EMT difference ≥ 1.5 mm and those with a post-treatment VI ≥ 1.5 were higher than those of patients with an EMT difference < 1.5 mm and with a post-treatment VI < 1.5 (P<0.05). The difference in VI before and after treatment was positively correlated with the clinical pregnancy rate (r= 0.734, P<0.05).
【Conclusion】
Three-dimensional ultrasound volumetric imaging shows that autologous PRP treatment for patients with thin endometrium can effectively improve the EMT, endometrial blood flow status, and endometrial receptivity of patients, thereby increasing the clinical pregnancy rate. The improvement of VI may be an effective indicator for predicting the therapeutic effect of autologous PRP treatment.

【Objective】
To investigate the clinical efficacy of budesonide, glycopyrrolate, and formoterol fumarate aerosol combined with an internet-of-things (IoT) respiratory rehabilitation trainer in the treatment of patients with moderate-to-severe chronic obstructive pulmonary disease (COPD).
【Methods】
A total of 96 patients with stable moderate-to-severe COPD admitted to our hospital from July 2024 to June 2025 were selected and divided into a control group and an observation group using a random number table method, with 48 cases in each group. The control group was treated with budesonide, glycopyrrolate, and formoterol fumarate aerosol, while the observation group was additionally treated with an IoT respiratory rehabilitation trainer on the basis of the control group. The clinical efficacy, pulmonary function indicators [peak expiratory flow (PEF), forced expiratory volume in the first second/forced vital capacity (FEV
1
/FVC)], 6-minute walking distance (6MWD), and quality of life [St. George's Respiratory Questionnaire (SGRQ) score] were compared between the two groups.
【Results】
The total effective rate of the observation group was higher than that of the control group (P<0.05). After 8 weeks of treatment, the PEF and FEV

/FVC of both groups were higher than those before treatment, and the 6MWD was longer than that before treatment. Moreover, the PEF and FEV

/FVC of the observation group were higher than those of the control group, and the 6MWD was longer than that of the control group, with statistically significant differences (P<0.05). The symptom, activity, and impact scores of both groups were lower than those before treatment, and those of the observation group were lower than those of the control group, with statistically significant differences (P<0.05).
【Conclusion】
Budesonide, glycopyrrolate, and formoterol fumarate aerosol combined with an IoT respiratory rehabilitation trainer can further improve pulmonary function and exercise endurance in patients with moderate-to-severe COPD by strengthening respiratory muscle function and optimizing ventilation efficiency, reduce acute exacerbations, enhance quality of life, and demonstrate good safety, making it suitable for clinical promotion

Effects of Bilateral Quadratus Lumborum Block and Transversus Abdominis Plane Block on Postoperative Analgesia and Serum Inflammatory Factors in Women Undergoing Cesarean Section

【Objective】
To investigate the effects of bilateral quadratus lumborum block (B-QLB) and transversus abdominis plane block (TAPB) on postoperative analgesia and serum levels of monocyte chemoattractant protein-1 (MCP-1), substance P (SP), and 5-hydroxytryptamine (5-HT) in women undergoing cesarean section.
【Methods】
A total of 200 women scheduled for cesarean section were randomly assigned to the control group (n=100, TAPB) and the observation group (n=100, B-QLB). Analgesic efficacy, clinical recovery parameters, resting and movement visual analogue scale (VAS) scores at 4 h, 8 h, 12 h, 24 h, and 48 h postoperatively, serum biomarkers (MCP-1, SP, 5-HT), and adverse events were compared between the two groups.
【Results】
Resting VAS scores showed no significant differences between the two groups (P>0.05). The observation group had lower movement VAS scores at 8 h, 12 h, 24 h, and 48 h (P<0.05). The first pressing time of the analgesia pump in the observation group was longer than that in the control group. The number of effective pressings and the analgesia rescue rate 24 after surgery were lower than those in the control group, and the analgesia satisfaction score was higher than that in the control group (all P<0.05). The observation group also showed shorter times to first standing, first ambulation, and onset of lactation (all P<0.05). Postoperative serum MCP-1, SP, and 5-HT levels in the observation group were significantly lower than those in the control group (P<0.05). The incidence of adverse reactions did not differ significantly between groups (P>0.05).
【Conclusion】
Compared with TAPB, B-QLB provides superior postoperative analgesia effect for cesarean section, reduces analgesic consumption, significantly lowers serum MCP-1, SP, and 5-HT levels, and does not increase the risk of adverse events.

Application of Lamaze Breathing Pain Relief Combined with Birthing Ball in Vaginal Delivery Parturients

【Objective】
To explore the effects of Lamaze breathing pain relief combined with birthing ball in vaginal delivery parturients.
【Methods】
A total of 80 parturients who delivered in our hospital from January to December 2024 were selected and divided into the observation group (receiving Lamaze breathing pain relief combined with birthing ball intervention on the basis of routine midwifery care) and the control group (receiving routine midwifery care intervention) by random number table method, with 40 cases in each group. The pain degree, duration of each stage of labor and delivery outcomes of the two groups after intervention were compared.
【Results】
After intervention, the proportion of patients experiencing grade 0 and grade Ⅰ pain in the observation group was higher than that in the control group, while the proportion of patients experiencing grade Ⅱ and grade Ⅲ pain was lower than that in the control group, and the differences were statistically significant (P<0.05). The duration of the first, second and third stages of labor in the observation group was all shorter than those in the control group (P<0.05). The proportion of natural vaginal delivery in the observation group was higher than that in the control group, and the incidence of dystocia, postpartum hemorrhage and neonatal asphyxia was lower than that in the control group, and the differences were statistically significant (P<0.05).
【Conclusion】
The application of Lamaze breathing pain relief combined with birthing ball in vaginal delivery parturients can effectively relieve labor pain, shorten the duration of labor and optimize the delivery outcome, and is worthy of promotion.

Comparison of Efficacy of Hernia Sac Transection versus Complete Exfoliation in Laparoscopic Surgery for Oblique Inguinal Hernia

【Objective】
To compare the efficacy of hernia sac transection and complete hernia sac exfoliation for laparoscopic transabdominal preperitoneal (TAPP) hernia repair in patients with a maximum oblique inguinal hernia sac diameter of ≥ 5 cm.
【Methods】
A total of 90 male patients with oblique inguinal hernia admitted to our hospital from January 2020 to January 2023 were selected in this study. All patients received the TAPP laparoscopic hernia repair. According to different treatment methods of hernia sac, patients were divided into a transection group (in which the hernia sac was transected intraoperatively) and a complete exfoliation group (in which the hernia sac was completely dissected intraoperatively), with 45 cases in each group. The clinical indicators, pain levels [Visual Analogue Scale (VAS) scores], laboratory parameters [Total Antioxidant Capacity (TAC), Cortisol (Cor), Malondialdehyde (MDA), Tumor Necrosis Factor α(TNF-α), and Interleukin-6 (IL-6)], and the incidence of complications between the two groups were compared and analyzed.
【Results】
The operative time and hernia sac management time in the transection group were significantly shorter than those in the complete exfoliation group; and the intraoperative blood loss and incidence of inguinal seroma were significantly lower than those in the complete exfoliation group (all P<0.05). The VAS scores at postoperative day 1 and day 3 in both groups were significantly lower than those at 12 hours postoperatively, and the VAS score at postoperative day 3 was significantly lower than that at postoperative day 1. Moreover, the VAS scores in the transection group at 1 day and 3 days postoperatively were significantly lower than those in the complete exfoliation group (all P<0.05). At 3 days postoperatively, the serum levels of TAC, Cor, MDA, TNF-α, and IL-6 in the transection group were significantly lower than those in the complete exfoliation group (P<0.05). The total incidence of postoperative complications in the transection group was significantly lower than that in the complete exfoliation group (P<0.05).
【Conclusion】
For the treatment of patients with oblique inguinal hernia with the diameter of the hernia sac > 5cm, both laparoscopic transection and complete exfoliation of the hernia sac are effective approaches during TAPP surgery. Compared to the complete exfoliation of the hernia sac, the transection of the hernia sac can shorten the operation time and the hernia sac handling time, reduce the amount of intraoperative blood loss and alleviates postoperative pain; furthermore, it exerts a lesser impact on the levels of oxidative stress and inflammatory response, offering the advantages of fewer complications and higher safety.

Effect of Personalized Exercise Guidance on Early Recovery after Lower Extremity Varicose Veins Surgery

【Objective】
To explore effects of personalized exercise guidance on early functional recovery after lower extremity varicose veins surgery.
【Methods】
A total of 128 patients who underwent surgery for lower extremity varicose veins in Zhumadian Traditional Chinese Medicine (TCM) Hospital from Jan 2023 to Jan 2025 were randomized into two groups, with 64 cases in each group. Patients in the control group received routine systematic nursing, while patients in the observation group received personalized exercise guidance in addition to routine nursing. Pain (Pain Intensity Scale, PPI), functional recovery and nursing satisfaction were compared between the two groups before and after treatment.
【Results】
After the intervention, the PPI scores of both groups were lower than before the intervention, and the observation group's scores were lower than the control group's scores, with statistically significant differences (P<0.05). The time for hematoma disappearance, ulcer healing, and lower limb numbness were significantly shorter in the observation group than in the control group, with statistically significant differences (P<0.05). The nursing satisfaction rate in the observation group was 92.19% (59/64), significantly higher than the 79.69% (51/64) in the control group, with a statistically significant difference (χ

【Conclusion】
Personalized exercise guidance reduces postoperative pain, accelerates recovery of hematoma, ulcer and numbness, and improves satisfaction.

Application Value of Pneumatic Therapy Combined with Enoxaparin Sodium in Preventing Lower Extremity Deep Vein Thrombosis after Cesarean Section

【Objective】
To explore the application value of pneumatic therapy combined with enoxaparin sodium in preventing lower extremity deep vein thrombosis (DVT) after cesarean section.
【Methods】
A total of 130 puerperae who underwent cesarean section in our hospital from May 2023 to May 2024 were enrolled and randomly divided into an observation group and a control group using a random number table, with 65 cases in each group. The control group received pneumatic therapy after cesarean section, while the observation group received pneumatic therapy combined with enoxaparin sodium after cesarean section. The incidence of lower extremity DVT, adverse reactions, hemorheological indexes, coagulation function indexes, inflammatory factor levels, and vascular endothelial function indexes of the two groups were compared after cesarean section.
【Results】
The incidence of lower extremity DVT in the observation group was 1.54% (1/65), which was significantly lower than 10.77% (7/65) in the control group, with a statistically significant difference (χ
2
=4.795, P=0.028). After treatment, the plasma viscosity, whole blood low-shear viscosity, and whole blood high-shear viscosity in the observation group were lower than those in the control group; the fibrinogen level was lower than that in the control group; the activated partial thromboplastin time, thrombin time, and prothrombin time were shorter than those in the control group, and all differences were statistically significant (P<0.05). After treatment, the levels of intercellular adhesion molecule 1, platelet-activating factor, matrix metalloproteinase 9, endothelin-1, and vascular endothelial growth factor in the observation group were lower than those in the control group (P<0.05).
【Conclusion】
Pneumatic therapy combined with enoxaparin sodium can prevent the occurrence of postoperative lower extremity DVT by improving the postoperative hypercoagulable state of puerperae after cesarean section, reducing the levels of inflammatory factors, and reducing vascular endothelial damage.

Construction and Validation of a Risk Model for Anterior Lateral Ligament Injury in Individuals with High BMI

【Objective】
To construct a risk model for anterior lateral ligament (ALL) injury in individuals with high body mass index (BMI) and to validate it.
【Methods】
The clinical data of 110 patients with non-contact anterior cruciate ligament (ACL) rupture in the Department of Orthopedics ward Ⅲ of our hospital from January 2022 to December 2024 were retrospectively analyzed. Among them, patients with simple ACL rupture were set as the control group (n=60), and those with concomitant ALL injury in addition to ACL were set as the observation group (n=50). Multivariate Logistic regression analysis was used to determine the independent risk factors for ALL injury in high BMI individuals, and a nomogram model was established based on this. The calibration curve and receiver operating characteristic (ROC) curve were used to validate the model.
【Results】
The results of multivariate Logistic regression analysis showed that meniscal injury, multiple sprains, history of re-injury, large posterior tibial plateau inclination angle on the lateral side, and large posterior meniscal inclination angle on the lateral side were all risk factors for ALL injury in high BMI individuals (P<0.05); The calibration curve of the model showed good consistency between predicted probability and actual observation results. The ROC curve analysis results showed that the model had high diagnostic accuracy, with an area under the curve (AUC) value of 0.983, a sensitivity of 92.0%, and a specificity of 100%.
【Conclusion】
The risk model for ALL injury in high BMI individuals has a high predictive value for ALL injury in high BMI individuals, which can help clinicians identify high-risk patients early and make appropriate treatment decisions.
