Effect of Pelvic External Fixation through Iliac Screw Passage in the Treatment of Patients with Unstable Pelvic Fractures

【Objective】To investigate the clinical effect of pelvic external fixation through iliac screw passage in the treatment of patients with unstable pelvic fractures. 【Methods】Fifty-two patients with Tile C pelvic fractures who were treated in Tangdu Hospital (affiliated to Air Force Medical University) were included in this study. Patients treated with pelvic external fixator via iliac screw path were included in the observation group (n=32),while those treated with plate fixation in front of sacroiliac joint were included in the control group (n=20). Surgical conditions of the 2 groups were comparatively analyzed. The Matta score at 6 weeks after surgery,and Majeed functional scores at 6 weeks,6 months and 12 months after surgery were evaluated. Adverse events were recorded as well. 【Results】The surgical time of the two groups was similar (P>0.05). The intraoperative blood loss and hospital stay of the observation group were less/shorter than those of the control group (P<0.05). At 6 weeks after surgery,the excellent and good rate of Matta score in the observation group was significantly higher than that in the control group (P<0.05). At 6 weeks after surgery,the excellent and good rate of Majeed functional scores in the observation group was significantly higher than that in the control group (P<0.05),but there was no significant difference between the groups at 6 months and 12 months after surgery (P>0.05). Follow-up of 12 months found that there was no pelvic ring deformity,loose screw or nonunion in either group. There was no significant difference in the incidence of adverse events between the groups (P>0.05). 【Conclusion】The pelvic external fixator via iliac screw path can effectively improve fracture displacement in patients with unstable pelvic fractures,which is conductive to pelvic function recovery with few adverse events.

Relationship of Hemodynamic Parameters of Severe Stenosis or Occlusion in Different Segmental Vertebral Artery with Posterior Circulation Ischemia

【Objective】To explore the relationship of hemodynamic parameters of severe stenosis or occlusion of vertebral artery (VA) with posterior circulation ischemia (PCI). 【Methods】From September 2019 to March 2021,150 patients with severe stenosis or occlusion of V1~V4 segment of unilateral VA diagnosed by carotid ultrasound,angiography and angiography were retrospectively analyzed. According to the location of severe stenosis or occlusion VA,they were divided into the V1 segment (84 cases),V2 segment (9 cases) and V3 ~ V4 segment (57 cases). Carotid artery ultrasound was used to check and record the hemodynamic parameters of each part of VA. ROC was used to analyze the hemodynamic parameters of V1~V4 segments to predict the value of severe stenosis or occlusion of VA. Logistic stepwise regression analysis was used to test the correlation between hemodynamic parameters and PCI. 【Results】In severe stenosis or occlusion of V1 and V2 segments,peak systolic blood velocity (PSV),end diastolic blood velocity (EDV) and vascular resistance index (RI) of the affected side were significantly lower than those of the healthy side,and the diameter of VA intervertebral segment (VAD) was thinner than that of the healthy side. When V3~V4 segment was severely narrowed or occluded,the PSV and EDV of the affected side were significantly lower than those of the healthy side,and the VAD was thinner than that of the healthy side (P<0.05). When V3~V4 segment was severely narrowed or occluded,there was no difference in RI between the affected side and the healthy side (P>0.05). Through ROC analysis,it was found that the levels of PSV,EDV,RI,VAD in V1~V2 and PSV,EDV and VAD levels in V3~V4 can be used to predict the severe stenosis or occlusion of VA. Multivariate analysis showed that there was no significant correlation between PCI and severe stenosis or occlusion in V1 and V2 segments,while PSV,EDV and VAD in V3~V4 segments were significantly correlated with PCI,which were independent risk factors for PCI,while RI in V3~V4 segments had no significant correlation with PCI. 【Conclusion】PCI is related to V3-V4 segment stenosis or occlusion. The lower PSV,EDV and the thinner VAD can increase the risk of PCI.

Evaluation of the Consistency Between Contrast-Enhanced Ultrasound and Magnetic Resonance in The Diagnosis of Small Subcapsular Liver Cancer

【Objective】To contrast the value of contrast-enhanced ultrasound and magnetic resonance imaging (MRI) in the diagnosis of small hepatocellular carcinoma under the liver capsule. 【Methods】89 patients with suspected small subcapsular liver cancer in Baoji Hospital of Traditional Chinese Medicine were selected as the research subjects from July 2019 to September 2021. All patients underwent contrast-enhanced ultrasound and MRI examination, and pathological diagnosis was used as the "gold standard". The results of contrast-enhanced ultrasound and MRI, analysis of contrast-enhanced ultrasound and MRI diagnostic efficiency and consistency were recorded. 【Results】There were 95 subcapsular liver lesions in 89 patients with suspected small subcapsular liver cancer, of which 79 lesions (83.16%, 79/95) were confirmed to be small subcapsular liver cancer by surgical pathology and puncture pathological examination. The contrast-enhanced ultrasound examination showed 77 lesions (81.05%, 77/95) with "fast forward and fast exit" signs. The peak intensity (PI) of small liver cancer lesions was higher than that of benign lesions, and the rise time (RT) and peak time (PT) of small liver cancer lesions were lower than those of benign lesions (P<0.05). MRI diagnosis revealed that 75 lesions (78.95%, 75/95) showed significant signal enhancement in the arterial phase, decreased signal in the venous phase and equilibrium phase, and significantly low signal in the hepatobiliary phase. The peak value (PV), maximum ascending slope (MSI), and maximum descending slope (MSD) of small liver cancer lesions were all lower than those of benign lesions (P<0.05), and the Kep value, Ktrans value and Ve value of small liver cancer lesions were higher than those of benign lesions (P<0.05). The consistency kappa values of pathological diagnosis, contrast-enhanced ultrasound and MRI diagnosis were 0.801 and 0.789, respectively, and the consistency kappa values of contrast-enhanced ultrasound and MRI diagnosis were 0.806. 【Conclusion】Contrast-enhanced ultrasound and MRI have similar efficacy in diagnosing small subcapsular liver cancer, and the appropriate diagnostic method can be selected based on individual conditions.

Study on Effects of Methylphenidate Hydrochloride-controlled Release Tablets Combined with Electronic Biofeedback on Efficacy of Children with ADHD

【Objective】To explore effects of methylphenidate hydrochloride-controlled release tablets combined with electronic biofeedback on the clinical efficacy of children with attention deficit hyperactivity disorder (ADHD). 【Methods】A total of 80 children with ADHD were randomly divided into the observation group and the control group (with 40 cases in each group). The control group was treated with methylphenidate hydrochloride controlled-release tablets,while the observation group was treated with methylphenidate hydrochloride controlled-release tablets combined with electronic biofeedback. The course of treatment in both groups was 2 months. The therapeutic effect of the two groups before and after treatment,the adverse reactions and scores were observed and analyzed. 【Results】The total effective rate in the observation group was significantly higher than that in the control group,and the difference was statistically significant (P<0.05). After treatment and one month after the end of treatment,the scores of Conner factors in the two groups decreased if compared to the scores before treatment (P<0.05); And the scores in the observation group decreased even more significantly than those in the control group after treatment (P<0.05). The scores of various cognitive function items such as the learning factor,anxiety factor,behavior factor,hyperactivity factor and physical and mental factor in the two groups after treatment and 1 month after treatment were higher than those before treatment.The scores of vocal tics and motor tics in the two groups after treatment and 1 month after the end of treatment were all lower than those before treatment (P<0.05),and the scores in the observation group were significantly lower than those in the control group (P<0.05). The cognitive function scores in the observation group were significantly higher than those in the control group at each time points after treatment (P<0.05). 【Conclusion】The combination of methylphenidate hydrochloride and electronic biofeedback can effectively treat ADHD,improve children's cognitive function and tic disorder,and significantly improve the clinical effect.

Regulatory Effect of GADD45α of Liver Regeneration after Combined Hepatic Septation and Portal Vein Ligation in Cirrhotic Rats

【Objective】To discuss GADD45α regulatory effect of liver regeneration in cirrhotic rats after combined associating liver partition and portal vein ligation for staged hepatectomy(ALPPS). 【Methods】Ninety male SD cirrhotic rats fed with high-fat diet and induced by carbon tetrachloride were randomly divided into three groups:ALPPS group, surgical control group and normal control group, with 30 rats in each group. The ALPPS group was treated with ALPPS combined with hepatic septation and portal vein ligation. The operation control group only opened and closed the abdominal and perihepatic ligaments, and the normal control group was fed naturally. Ten rats in each group were killed at 1 day, 7 days and 14 days after operation. The expression of rat GADD45α gene was analyzed by Real time PCR, and the expression of rat GADD45α protein was detected by Western blotting. The liver function and liver regeneration rate of the three groups at different time points were analyzed and compared.【Results】On the 14th day after operation, the levels of serum ALT and AST in the ALPPS group were lower than those on the 1st and 7th day after operation, and also lower than those in the normal control group and the operation control group (P<0.05); The expression of GADD45α mRNA and GADD45α protein in the ALPPS group was significantly higher than that in the normal control group and the operation control group (P<0.05); The liver regeneration rate of rats in the ALPPS group was higher than that in 1 day and 7 days after operation, and higher than that in the normal control group and the operation control group (P<0.05). 【Conclusion】GADD45α has regulatory effect on liver regeneration after combined liver septation and portal vein ligation in cirrhotic rats, and can improve the rate of liver regeneration in rats.

Analysis of Risk Factors of Catheter Related Urinary Tract Infection in Children Undergoing Urological Surgery, Construction of Predictive Model and Nursing Countermeasures

【Objective】To investigate the risk factors of catheter associated urinary tract infection in children undergoing urological surgery. 【Methods】A total of 169 children with urinary tract infection who underwent surgical treatment in the urology department of our hospital from April 2020 to April 2021 and had urinary catheters retained after surgery were divided into infection group (n=31) and non infection group (n=138) according to whether the children had urinary tract infection after surgery. The clinical data of two groups of children were collected, and single factor and multi factor regression analysis was conducted on the factors that may affect the occurrence of urinary tract infection in children after surgery, and a prediction model was constructed. The ROC curve was used to analyze the predictive value of the prediction model on the occurrence of urinary tract infection related to urinary catheters in children after urological surgery. 【Results】Among 169 children with indwelling catheter after urological surgery, 31(18.34%) had catheter related urinary tract infection. 29 pathogens, including 18 gram-negative bacteria (62.07%), 10 gram-positive bacteria (34.48%) and 1 fungus (3.45%). Univariate analysis showed that there was a significant difference between the two groups in terms of catheter retention time, previous catheter retention history, hospital stay and catheter replacement times (P<0.05). Multivariate logistic regression analysis showed that the indwelling time of urinary catheter, previous indwelling history of urinary catheter, hospitalization time and the number of catheter replacement were independent risk factors for postoperative urinary tract infection in children (P<0.05). The ROC curve analysis shows that the area under the curve of the prediction model is 0.933 (95%CI:0.875~0.991), which has good prediction value (P=0.000). 【Conclusion】The indwelling time of urinary catheter, the previous indwelling history of urinary catheter, the length of hospital stay and the number of catheter replacement are independent risk factors affecting the occurrence of urinary catheter related urinary tract infection after urological surgery. The predictive model has good predictive value for the occurrence of urinary tract infection in children.

Preoperative Curative Effect of Acitinib Combined with Radical Surgery for Elderly Renal Cell Carcinoma

Clinical Analysis of High Intensity Focused Ultrasound or Uterine Artery Embolization Combined with Hysteroscope in the Treatment of Cesarean Scar Pregnancy

【Objective】To compare the effect of high intensity focused ultrasound(HIFU)or uterine artery embolization(UAE)combined with hysteroscope in the treatment of type Ⅱ cesarean scar pregnancy(CSP). 【Methods】A retrospective analysis was performed in 157 patients diagnosed with CSP ( type Ⅱ) and treated in our hospital from June 2018 to December 2019. Among them,82 patients were treated with HIFU combined with hysteroscope(the HIFU group)and 75 patients were treated with UAE combined with hysteroscope(the UAE group). Clinical efficacy was compared between the two groups. 【Results】The amount of bleeding during the operation in the UAE group(155.33±40.35)mL is much less than that in the HIFU group (189.84±50.64)mL(P<0.05);The operation time of the UAE group (26.12±10.02)min was shorter than that in the HIFU group (32.33±14.57)min (P<0.05);The decrease of hemoglobin (Hb) on the first day after surgery in UAE group (5.53±6.47) g/L was less than that in the HIFU group (7.93±7.85) g/L (P<0.05).Follow-up the menstrual recovery,patients with hypomenorrhea in the HIFU group (15.85%) were significantly less than that in the UAE group (26.66%) (P<0.05).Compared to the decrease of β-HCG on the third day after operation,the time of vaginal bleeding after operation,the number of cases with uterine diverticulum during the operation and the number of cases undergo second operation,there were no statistically significant difference between the two groups (P>0.05). 【Conclusion】Either HIFU or UAE combined with hysteroscope can effectively treat CSP. However,the UAE group is superior to HIFU group in reducing hemorrhage during operation and HIFU group is superior to UAE group in protecting reproductive function.

Effect of Nalbuphine on Intraoperative Analgesia and Hemodynamics in Children Undergoing Daytime Surgery

【Objective】To investigate the analgesic effect of nalbuphine during daytime operation in children and its influence on hemodynamics. 【Methods】The clinical data of 60 children who underwent unilateral hernia sac high ligation in our hospital from November 2020 to February 2021 were analyzed retrospectively. Diagnostic Value of CT Coronary Flow Reserve Fraction in Heart Disease

【Objective】To investigate the diagnostic value of CT coronary angiography blood flow reserve fraction (FFRCT) in heart disease. 【Methods】A total of 75 patients with heart disease in our Hospital were selected as the research objects. The corresponding images and data were obtained by coronary CT angiography (CCTA),coronary angiography (CAG) and invasive blood flow reserve fraction (FFR). The FFRCT model was established to analyze the sensitivity,specificity and accuracy of FFRCT in the detection of blood vessel as the unit and the number of vessels in coronary heart disease. The correlation between FFRCT and invasive FFR was also investigated. 【Results】A total of 100 lesions were found in 75 patients,including 58 left anterior descending lesions,16 left circumflex lesions,and 26 right coronary arteries. Based on the invasive FFR,the specificity,sensitivity,positive predictive value,negative predictive value,accuracy and κ of the FFRCT test results were 100.0%,87.6%,100.0%,50.0%,89.7% and 0.62,respectively. The specificity,sensitivity,positive predictive value,negative predictive value,accuracy and κ of the FFRCT test results in terms of vessel count were 100.0%,65.3%,100.0%,50.0%,76.4% and 0.51,respectively. In units of patients and blood vessels,FFR and FFRCT were positively correlated with good consistency(r=0.794、0.725,P<0.05). 【Conclusion】FFRCT technology has a high value in the detection of coronary artery disease in patients with coronary heart disease. The clinical application of FFRCT technology should be used to rationally select the diagnosis and treatment strategy for patients.

Risk Factors of Late-Onset Hemorrhagic Cystitis after Allogeneic Hematopoietic Stem Cell Transplantation in Children

【Objective】To retrospectively analyze the risk factors of late onset hemorrhagic cystitis (LOHC) in children with allogeneic hematopoietic stem cell transplantation (Allo-HSCT). 【Methods】The clinical data of 140 children treated with Allo-HSCT in our hospital from October 2018 to August 2022 were retrospectively analyzed, and their clinical characteristics were analyzed. Logistic regression analysis was used to analyze the related risk factors of Allo-HSCT with delayed hemorrhagic cystitis. 【Results】Among 140 patients receiving allogeneic hematopoietic stem cell transplantation, 36 patients developed hemorrhagic cystitis (25.71%). The occurrence time was 7-116 days after transplantation, and the median time was 30 days after transplantation. The duration varies from 4 to 60 days, with a median of 11 days. Two patients died due to other serious transplantation related complications at the same time, and 34 patients achieved complete remission. Allo-HSCT patients with treatment schemes including cetipar, aGVHD and CMV after transplantation had a higher probability of LOHC (P<0.05). Logistic regression analysis showed that pretreatment with Thiotepa and cytomegalovirus were independent risk factors for LOHC in Allo-HSCT patients. 【Conclusion】Pretreatment regimen containing Thiotepa and cytomegalovirus (CMV) after transplantation are independent risk factors for LOHC.

Analysis of Influencing Factors of the Efficacy of L-T4 in the Treatment of Subclinical Hypothyroidism during Pregnancy

【Objective】To analyze the influencing factors of the efficacy of levothyroxine (L-T4) in the treatment of subclinical hypothyroidism (SCH) during pregnancy and optimize the L-T4 treatment plan. 【Methods】A total of 365 patients with SCH during pregnancy from October 2019 to August 2021 were divided into the L-T4 treatment effective group (n=278) and the L-T4 treatment ineffective group (n=87) based on the clinical effects of L-T4 treatment. Data of medical history,the peripheral venous blood of the patients when fasting for index inspection,thyroid function-related indexes,lipid metabolism-related indexes,trace elements,thyroid ultrasound and spectral color Doppler blood flow-related indexes of the two groups were collected. Multivariate Logistic regression was used to analyze the influencing factors of the curative effect of L-T4 in the treatment of pregnant SCH patients.【Results】The effective rate of L-T4 in the treatment of SCH during pregnancy was 76.16%. The results of single factor analysis showed that the detection time of TSH,TSH level,diabetes complication,gastrointestinal diseases,iron (Fe),zinc (Zn),thyroid peroxidase antibody (TPO-Ab) (+),thyroid gland protein antibody (TG-Ab) (+),peak systolic flow velocity (PSV),end-diastolic blood flow velocity (EDV) were the influencing factors of the efficacy of L-T4 in the treatment of SCH during pregnancy(P<0.05). The results of multivariate analysis showed that the detection time of TSH,TSH level,TG-Ab(+),TPO-Ab(+),combined with intestinal disease,diabetes,and PSV were the influencing factors of the efficacy of L-T4 in the treatment of SCH in pregnancy (P<0.05). 【Conclusion】The effective rate of L-T4 in the treatment of SCH during pregnancy needs to be improved. There are many factors that affect the effect of L-T4 treatment. The maternal and child health departments of hospitals should pay more attention to the treatment of SCH during pregnancy to avoid unfavorable factors that affect the treatment and improve L-T4 treatment effect.

Correlation between E2F2 Expression Level and Prognosis and Immune Infiltration in Patients with Prostate Cancer

【Objective】To investigate the relationship between the expression of E2F2 and the prognosis and immune invasion of prostate cancer. 【Methods】Based on the CancerGenome Atlas (TCGA) database, the expression of E2F2 in prostate cancer was analyzed. The correlation between the expression level of E2F2 and the prognosis of prostate cancer patients was analyzed by gene set enrichment analysis and immune infiltration analysis. The ROC curve was used to analyze the diagnostic value of E2F2 in prostate cancer. 【Results】The expression level of E2F2 in prostate cancer tissues was higher than that in normal prostate tissues (P<0.001); The expression level of E2F2 in prostate cancer tissues was higher than that in normal tissues adjacent to prostate cancer (P<0.001). Kaplan Meier survival analysis showed that the expression level of E2F2 was negatively correlated with the overall survival rate and progression free interval of prostate cancer patients (all P<0.05); There was no significant correlation between E2F2 expression level and disease specific survival rate of prostate cancer patients (P>0.05). ROC curve analysis showed that the AUC of E2F2 in diagnosing prostate cancer was 0.731 (95%CI:0.653~0.809). Spearman correlation analysis showed that the expression level of E2F2 was positively correlated with T-helper lymphocyte 2, T-helper lymphocyte, dendritic cells and T-cell enrichment group (all P<0.05); The expression level of E2F2 was negatively correlated with the concentration of natural killer cells and mast cells (all P<0.05). 【Conclusion】The expression level of E2F2 in prostate cancer patients is significantly increased, and the overexpression of E2F2 is closely related to poor prognosis and cellular immune infiltration.

Value of Th22 Cells and Serum IL-22 Level in Predicting the Severity of Lupus Nephritis

【Objective】To investigate the value of helper T cell 22 (Th22) and serum interleukin-22(IL-22) levels in predicting the severity of lupus nephritis (LN). 【Methods】Sixty two patients with systemic lupus erythematosus (SLE) were admitted to our hospital. The patients with more than 8 glomeruli in renal biopsy sections were included in the LN group (n=28),and the rest of patients were included in the SLE group (n=34). Meanwhile,30 healthy volunteers in the same period were selected as the control group. LN patients with SLE-DAI score ≥ 10 were set as active group (n=11),and those with SLE-DAI score<10 were set as stable group (n=17). The renal function indexes (BUN,Scr,GFR) and the levels of helper T cell 1 (Th1),Th22,IL-22 in each group were compared. The value of Th22 and IL-22 levels in predicting the severity of LN patients was analyzed. Spearman rank correlation analysis was used to reveal the correlation of levels of Th22 and IL-22 with SLE-DAI scores of LN patients. 【Results】The levels of Scr,BUN,Th22,and IL-22 in the control group were significantly lower than those in the LN group and the SLE group,while the levels of GFR were higher than those in the two groups (P<0.05). The levels of Scr and BUN in the SLE group were significantly lower than those in the LN group (P<0.05). There was no significant difference in the levels of Th22,IL-22 and GFR between the LN group and the SLE group (P>0.05). There were no significant differences in Th1 levels among the three groups (P>0.05). There were no significant differences in the levels of SCR,GFR,BUN and Th1 between the active group and the stable group (P>0.05). Th22 and IL-22 levels in the active group were significantly higher than those in the stable group (P<0.05). According to ROC analysis,the best cutoff values of Th22 and IL-22 levels for predicting the severity of LN patients were Th22≥7.180% and IL-22≥64.955 pg/mL,respectively (both P<0.05). Correlation analysis showed that Th22 and IL-22 levels were positively correlated with SLE-DAI scores of LN patients (P<0.05). 【Conclusion】The levels of Th22 and IL-22 in peripheral blood of SLE and LN patients were significantly higher than those of normal people,and there were significant differences among LN patients with different disease severity,which was significantly positively correlated with the disease activity of LN patients.

Efficacy of Ilizarov Transverse Tibial Bone Transport Combined with Platelet-rich Plasma for Refractory Diabetic Foot Wounds Treatment

【Objective】To investigate the efficacy and safety of platelet-rich plasma (PRP) combined with Ilizarov tibial transverse bone transport in the treatment of refractory diabetic foot wounds. 【Methods】A total of 80 patients with refractory diabetic foot wounds admitted to Hancheng People's Hospital from June 2018 to June 2021 were selected as the research objects. Patients were divided into the observation group (40 cases) and the control group (40 cases) by random number table method. The control group was treated with PRP external application,while the observation group was treated with Ilizarov transverse tibial bone transport on the basis of PRP in the control group. The clinical efficacy and limb salvage rate were compared between the two groups. Wound healing indicators (wound reduction rate,granulation tissue growth thickness,granulation tissue coverage rate,skin grafting time,and wound healing time) were compared between the two groups. The skin temperature of the affected foot,visual analogue scale (VAS) score and ankle brachial index were compared between the two groups before treatment and 6 months after treatment. Complications were observed and recorded in the two groups. 【Results】Compared to the control group,the total effective rate and limb salvage rate of the observation group were higher (P<0.05). The observation group had higher wound reduction rate,granulation tissue coverage rate,larger granulation tissue growth thickness,shorter wound skin grafting time and wound healing time,if compared with the control group (P<0.05). After treatment,the skin temperature and ankle brachial index of the affected foot increased,and the VAS score decreased in the two groups (P<0.05). Compared to the control group,after treatment,the skin temperature and ankle brachial index of the affected foot in the observation group were higher,and the VAS score was lower (P<0.05);There was no significant difference in the total incidence of complications between the two groups (P>0.05). 【Conclusion】PRP combined with Ilizarov tibial transverse bone transport in the treatment of refractory diabetic foot wounds is beneficial to improve the total effective rate and limb salvage rate and shorten the wound healing time. It has high safety,which has the value of promotion.

Effect of Orthodontic Extraction on Thickness of Alveolar Bone in Angle Ⅱ Class 1 Malocclusion with Vertical Bone Surface

【Objective】To analyze the effect of orthodontic extraction on the thickness of alveolar bone in Angle Ⅱ class 1 malocclusion patients with vertical bone surface. 【Methods】From August 2016 to September 2020,a total of 80 patients with vertical bone surface type of Angle Ⅱ type 1 malocclusion,who were treated in Baoji Hospital of Traditional Chinese Medicine,Baoji Hospital of Stomatology and Weinan People's Hospital,were selected in the study. According to the Angle of mandibular plane Angle (MP-SN) and facial height index,patients were divided into the high Angle group,low Angle group and average Angle group. All patients were treated with orthodontic extraction and the changes of alveolar bone thickness were analyzed. 【Results】After treatment,the total thickness of alveolar bone at the maxillofacial distance of 6 mm and 9mm from the cementum enamel boundary(CEJ) in the high Angle group and the low Angle group decreased. The high Angle group was lower than the low Angle group; and the difference was statistically significant (P<0.05).In the high-angle group,the total alveolar bone thickness and labial alveolar bone thickness of the right mandibular occlusal surface from CEJ 3 mm,6 mm and 9 mm decreased the most,while the total apical point thickness and the total alveolar bone thickness from CEJ 9 mm and labial and palatal bone thickness from CEJ 6 mm decreased the most in the left occlusal surface (P<0.05).The thickness of palatal alveolar bone at 3 mm from CEJ in the high Angle group was significantly decreased,and the thickness of alveolar bone at 3 mm,6 mm and 9 mm from CEJ in the low Angle group was significantly decreased (P<0.05).After treatment,the buccal lingual thickness of the proximal and distal middle root in the low Angle group was significantly reduced,and the buccal alveolar bone thickness in the high Angle group was significantly reduced (P<0.05).The reduction in the labial side of the mesial root in the high Angle group was higher than that in the low Angle group (P<0.05). 【Conclusion】Orthodontic extraction can reduce the alveolar bone thickness in Angle Ⅱ type 1 malocclusion patients with high angle of vertical bone surface. The treatment should be carefully chosen.

Value of Nicorandil after PCI in Elderly Patients with Acute Myocardial Infarction and Diabetes

【Objective】To explore the application value of Nicorandil in elderly patients with acute myocardial infarction and diabetes after percutaneous coronary intervention (PCI). 【Methods】The clinical data of 98 elderly patients with acute myocardial infarction and diabetes treated by PCI were retrospectively analyzed. They were divided into the observation group (routine treatment+Nicorandil) and the control group (routine treatment) according to whether they took Nicorandil or not, with 49 cases in each group.The blood flow grades of myocardial infarction thrombolysis test (TIMI), creatine kinase isoenzyme (CK-MB) and troponin I (cTnI) before and 24 hours after operation were compared between the two groups;The left ventricular ejection fraction (LVEF), left ventricular end systolic diameter (LVESD), left ventricular end diastolic diameter (LVEDD), glycosylated hemoglobin (HbA1c), fasting blood glucose (FPG), and postprandial blood glucose (2hPG) of the two groups were also compared before and after treatment.The patients were followed up for 6 months to record the adverse cardiac events.【Results】The TIMI blood flow grading in the observation group was better than that in the control group, with a statistically significant difference (P<0.05). At 24 hours after operation, CK-MB and cTnI in the two groups were higher than those before operation, but those in the observation group were lower than those in the control group (P<0.05). LVEF after treatment was significantly higher than that before treatment, LVESD and LVEDD were significantly lower than that before treatment (P<0.05), and the difference between the observation group and the control group was statistically significant (P<0.05). After treatment, HbA1c, FPG and 2hPG in the two groups were lower than those before treatment (P<0.05), but there was no significant difference between the observation group and the control group (P>0.05). The total incidence of adverse cardiac events in the observation group was significantly lower than that in the control group (P<0.05). 【Conclusion】Nicorandil can improve microcirculation perfusion, alleviate myocardial injury, improve cardiac function and reduce adverse cardiac events in elderly patients with acute myocardial infarction and diabetes after PCI.

【Objective】To undertake three dimensional analysis of the effect of two different body surface labeling methods on cervical vertebra positioning error and organ endangering dose in patients with nasopharyngeal carcinoma undergoing intensity modulated radiotherapy.【Methods】From January 2019 to June 2020, 32 patients who were pathologically diagnosed as nasopharyngeal carcinoma in the Cancer Prevention and Treatment Center of Sun Yat-sen University were selected and divided into two groups according to the time of treatment, with 16 cases in each group. Group B patients fixed the facial mask with a conventional three-point marking method. The Group A added a positioning point between the left and right clavicular heads of patients on the basis of the Group B to guide the positioning of the neck of patients. Each patient underwent cone beam CT (CBCT) scanning once a week, and a total of 6 sets of CBCT images were obtained. Preventive Effect of Mecobalamin on Peripheral Neuropathy Induced by XELOX Regimen in Patients with Colorectal Cancer

【Objective】To explore the preventive effect of mecobalamin on peripheral neuropathy caused by XELOX regimen in patients with colorectal cancer. 【Methods】Ninety-two patients with colorectal cancer malignant tumors who were transferred to adjuvant chemotherapy after operation were randomly divided into the observation group( traditional XELOX+ Mecobalamin,n=46)and the control group ( traditional XELOX only,46 cases). Both groups took 21 days as a chemotherapy cycle and carried out at least 3 cycles. The nerve conduction velocity,incidence of acute and chronic toxicity, and the occurrence of other adverse reactions in the two groups were statistically compared between the two groups before chemotherapy and after 3 cycles of chemotherapy. 【Results】There was no significant difference in motor nerve conduction velocity between the two groups before and after chemotherapy (P>0.05). There was no significant difference in sensory nerve conduction velocity between the two groups before chemotherapy (P>0.05). After chemotherapy,the sensory nerve conduction velocity in the control group was lower than that before chemotherapy (P<0.05). There was no significant difference in the incidence of acute neurotoxicity between the two groups (P>0.05). After 1,2 and 3 cycles of chemotherapy,the incidence of chronic toxicity in the observation group was lower than that in the control group (P<0.05). There was no significant difference in the incidence of adverse reactions such as nausea and vomiting,bone marrow suppression and general fatigue between the two groups (P>0.05). 【Conclusion】Mecobalamin can effectively prevent the effect of chemotherapeutic drugs on the sensory nerves of patients with colorectal cancer. It can also effectively prevent the chronic neurotoxicity caused by chemotherapeutic drugs,but it has no obvious effect on the acute neurotoxicity caused by chemotherapeutic drugs. Mecobalamin is safe,too.

Predictive Value of RDW Combined with hs-cTnT on Left Ventricular Diastolic Dysfunction in MHD Patients

【Objective】To analyze the predictive value of red blood cell distribution width (RDW) combined with high-sensitivity troponin T (hs-cTnT) in maintenance hemodialysis (MHD) patients with left ventricular diastolic dysfunction. 【Methods】A total of 80 MHD patients in our hospital from April 2019 to February 2021 were selected. The serum hs-cTnT level and RDW level of the patients before dialysis were collected. In addition,Doppler ultrasound was used to detect the early diastolic wave (E') and early diastolic peak flow rate (E) of the mitral valve. According to the test results,patients were divided into the diastolic dysfunction group and normal diastolic function group according to the test results. Logistic multivariate regression analysis was used to investigate the related factors affecting left ventricular diastolic dysfunction in patients; And receiver operating characteristic curve (ROC) was used to determine the effects of RDW and hs-cTnT levels on predictive value for patients to have complication with left ventricular diastolic disfunction. 【Results】Among the 80 MDH patients,49 cases (61.25%) had left ventricular diastolic dysfunction,and 31 patients (38.75%) had normal left ventricular diastolic function. The diastolic dysfunction group has a higher proportion of diabetes history,RDW,water load,and hs-cTnT level compared to the normal diastolic group. The ultrafiltration volume/BMI level of the diastolic dysfunction group was lower than that of the normal diastolic group (P<0.05). Logistic multivariate regression analysis showed that a history of diabetes,RDW,high water load,hs-cTnT expression level and low ultrafiltration volume/BMI expression level were the risk factors for left ventricular diastolic dysfunction in MDH patients (P<0.05). ROC analysis showed that the area under the curve (AUC) of RDW and hs-cTnT single and combined predictions for patients with left ventricular diastolic dysfunction were 0.804,0.807,and 0.888,respectively. The predictive power of RDW and hs-cTnT alone on patients with left ventricular diastolic dysfunction was lower than that of the combination of the two measurements (P<0.05). 【Conclusion】MHD patients with high expression levels of RDW and hs-cTnT are more likely to have left ventricular diastolic dysfunction. Thus,RDW and hs-cTnT have certain clinical predictive value for MHD patients with left ventricular diastolic dysfunction.

Correlation between Thromboelastography and Coagulation Indicators in Patients with AECOPD

【Objective】To investigate the correlation between thromboelastography (TEG) and coagulation indicators in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). 【Methods】All 106 patients with AECOPD were recruited from our hospital between January 2018 and January 2020. The coagulation indicators and TEG were detected. The correlation between TEG and coagulation indicators was analyzed by Pearson correlation. At the same time,patients were divided into the low-risk group (n=61) and the high-risk group (n=45),according to the number of AECOPD attacks and changes in lung function in the past year. Receiver operating characteristic (ROC) curve was used to explore the value of TEG in estimating risk of COPD. 【Results】Pearson correlation analysis showed that K (clot formation time) and R (coagulation factor activation time) had positively correlated with APTT (activated partial thrombin time),and the correlation coefficient correlation were 0.361 and 0.428,respectively. Meanwhile,MA (maximum clot strength)was positively correlated with FIB (fibrinogen),while MA was negatively correlated to D-D (D-dimer); and the correlation coefficient were 0.537 and -0.381,respectively. The K and R in the low-risk group were significantly shorter than high-risk group; the coagulation index (CI) was significantly lower than that of the high-risk group (P<0.05). The area under curve (AUC) of R time,K time and CI in the high-risk COPD were 0.87(0.81-0.94),0.80(0.71-0.89),and 0.66(0.56-0.77),respectively,and their optimal threshold was 3.62 min,4.75 min and 2.04,respectively. 【Conclusion】TEG was correlated with coagulation indicators in patients with AECOPD,and it can be used to identify high-risk COPD patients.

The baseline data of the two groups were compared,including initial labor,planned pregnancy,pregnancy habits (non-drinking,non-smoking and non-drinking tea),FA supplemental dose and the distribution of time to end supplementation. Relationship of TLR4/NF-κB Pathway with Related Cytokine Expression and Development of Acute Pancreatitis

【Objective】To explore the relationship of Toll-like receptor 4 (TLR4)/nuclear factor kappa B (NF-κB) pathway with related cytokine expression levels and the development of acute pancreatitis (AP). 【Methods】A total of 127 AP patients admitted to our hospital were divided into the AP group (75 cases) and the severe acute pancreatitis (SAP) group (52 cases) based on the severity of the illness. And 80 healthy volunteers who underwent physical examination in our hospital during the same time period were selected as the control group. The clinical data,TLR4,NF-κB expression levels and serum inflammatory factor levels were compared among the three groups. Pearson correlation was used to analyze the relationship of TLR4/NF-κB pathway with related cytokine expression levels and acute pancreatitis bedside severity index (BISAP). 【Results】The serum lipase and amylase of the AP group and SAP group were significantly higher than those of the control group (P<0.05). TLR4 mRNA and NF-κB mRNA in the AP group and the SAP group were higher than those in the control group (P<0.05). TLR4 mRNA and NF-κB mRNA in the SAP group were significantly higher than those in the AP group (P<0.05). The levels of interleukin-6 (IL-6),tumor necrosis factor-α (TNF-α) and interleukin-1β (IL-1β) in the AP group and SAP group were significantly higher than those in the control group (P<0.05). The levels of IL-6,TNF-α and IL-1β in the SAP group were significantly higher than those in the AP group (P<0.05). The levels of TLR4 mRNA,NF-κB mRNA,TNF-α and IL-1β were all positively correlated with BISAP (P<0.05),while there was no correlation between IL-6 levels and BISAP (P>0.05). 【Conclusion】The changes in TLR4/NF-κB pathway and related cytokine expression levels have a certain correlation with the development of AP disease,which can be a reference for clinical evaluation of AP disease progression.

Effects of Nanostructure Coating Sealant Combined with Antibacterial Sealant on Periodontal Pathogens and Bone Mineral Density of Implants

【Objective】To investigate the effect of nanostructured coating combined with antiseptic sealant on periodontal pathogens and bone density of implants. 【Methods】A total of 190 patients with mandibular implants in our hospital from January 2018 to January 2020 were enrolled, and divided into five groups according to admission orders, each with 38 cases. Control group treated without sealant, routine group treated with implant sealant,nano group treated with nanostructured coating, antiseptic group treated with antiseptic sealant, and combined group received nanostructured coating combined with antiseptic sealant. Patients were followed up from treatment till post-treatment 12 months. Then the implant stability, implant success rate, periodontal pathogens and bone density were compared among groups. 【Results】At the moment of bonding and 3 months,6 months,and 12 months after loading,the implant stability quotient (ISQ) values from low to high were the control group,the conventional group,the antibacterial group,the nano group,and the combined group,with statistical differences(P<0.05),and the isq values at the moment of bonding in each group were higher than those after loading(P<0.05).With the extension of loading time,the number of Staphylococcus aureus,oral inherent Streptococcus sanguis and Porphyromonas gingivalis in the control group,routine group,nano group,antibacterial group and combined group increased.There was a statistical difference between the number of Staphylococcus aureus,oral inherent Streptococcus sanguis and Porphyromonas gingivalis 3 months,6 months and 12 months after loading and immediately after bonding(P<0.05).The increase of pathogenic bacteria in the combined group was the lowest and lower than that in the other four groups(P<0.05).At 6 and 12 months after loading,the bone mineral density of the combined group was higher than that of the nano group and the antibacterial group,and the bone mineral density of the nano group was higher than that of the antibacterial group,the difference was statistically significant(P<0.05). 【Conclusion】Application of nanostructured coating combined with antiseptic sealant can effectively ameliorate the periodontal pathogens and bone density, and improve the implant stability.

Study on the Relationship of Weight Gain and Serum Vitamin D Level with Glucose/ Lipid Metabolism during Pregnancy

【Objective】To explore the relationship of nutritional indexesand serum vitamin D[25-(OH) D] level with glucose/ lipid metabolism during pregnancy. 【Methods】A total of 120 pregnant women in the second trimester of pregnancy who were admitted to our hospital from March 2020 to July 2021 were selected as the research objects. The weight gain (GWG) of all pregnant women during pregnancy was measured. The levels of serum 25-(OH)D,fasting blood glucose (FPG),two-hour postprandial blood glucose (2hPG),glycosylated hemoglobin (HbAlc),triglycerides (TG),total cholesterol (TC),low-density lipoprotein (LDL-C) and high-density lipoprotein (HDL-C) were measured. The differences of glucose and lipid metabolism indexes among pregnant women with different levels of GWG and 25-(OH)D were analyzed. 【Results】According to the serum 25-(OH)D level during pregnancy,36 cases (30.00%) were in the normal group,49 cases (40.83%) in the deficiency group,24 cases (20%) in the deficiency group and 11 cases (9.17%) in the severe deficiency group. The levels of serum TG,TC,LDL-C,HbAlc,FPG and 2hPG in the normal,deficient,deficient,and severely deficient groups of serum 25-(OH)D levels increased sequentially; and the differences between groups were statistically significant (P< 0.05).The levels of serum HDL-C in four groups were normal group > insufficient group > deficient group > severe deficiency group (P<0.05). Pearson correlation analysis showed that 25-(OH)D level were negatively correlated with TG,TC,LDL-C,HbAlc,FPG and 2hPG, while HDL-C was positively correlated with 25-(OH)D levels (P<0.05). According to GWG,there were 31 cases (25.83%) in the excess group,62 cases (51.67%) in the appropriate group and 27 cases (22.50%) in the excess group. The levels of TG,TC,and LDL-C in the lack of group,the appropriate group,and the excessive group increased sequentially; and the difference between the groups was statistically significant (P<0.05). While the HDL-C level decreased sequentially,and the difference between the groups was statistically significant (P<0.05).The levels of HbAlc,FPG and 2hPG in the lack of group and the excessive group were higher than those in the appropriate group (P<0.05),but there were no significant differences in the levels of HbAlc,FPG and 2hPG between the two groups (P>0.05). Pearson correlation analysis showed that too little GWG was negatively correlated with TG,TC,LDL-C,HbAlc,FPG and 2hPG,and it was positively correlated with HDL-C (P<0.05). Also,the too much GWG was positively correlated with HDL-C,HbAlc,FPG and 2hPG,while it was negatively correlated with TG,TC and LDL-C (P<0.05). 【Conclusion】Lack of 25-(OH)D level and too much or too little GWG during pregnancy are closely related to glucose and lipid metabolism. Detecting 25-(OH)D and GWG in time during pregnancy and adjusting them within an appropriate range are the effective means to avoid the disorder of glucose and lipid metabolism.

Treatment of Congenital Talipes Equinovarus Deformity with Ponseti Method and Ilizarov Operation

【Objective】To investigate the clinical effect of Ponseti method combined with Ilizarov technique in the treatment of congenital talipes equinovarus deformity in children. 【Methods】A total of 100 children with congenital talipes equinovarus were randomly divided into two groups (50 cases in each group). The control group was treated with Ilizarov technique gradually by traction, and the observation group was treated with Ponseti method and Ilizarov technique. The follow-up period was 12 months. The clinical efficacy, ICFSG score and complications of the two groups were compared. 【Results】After treatment, the total effective rate in the observation group was 96.00%, significantly higher than that in the control group (78.00%, P<0.05); The ICFSG score in the observation group was lower than that in the control group after treatment (P<0.05); The total incidence of postoperative complications in the observation group was 6.00%, significantly lower than that in the control group (24.00%, P<0.05). 【Conclusion】The combination of Ponseti method and Ilizarov technique is effective in the treatment of congenital talipes equinovarus deformity, which can effectively improve the ICFSG score and reduce the incidence of postoperative complications, and has certain clinical.

Application of Naoxintong Capsules Combined with Rivaroxaban in the Interventional Therapy of Acute Pulmonary Embolism and Influence on Thromboelastography Parameters

【Objective】To investigate the application of Naoxintong capsule combined with rivaroxaban in interventional treatment of acute pulmonary embolism (APE) and its effect on thromboelastogram parameters. 【Methods】A total of 200 patients who underwent APE intervention in two hospitals were randomly divided into the observation group and the control group,with 100 cases in each group. The control group was treated with rivaroxaban,while the observation group was treated with Naoxintong Capsule in addition to the treatment in the control group. The recent total effective rate,arterial blood gas index,coagulation index,thromboelastogram parameters,inflammatory index and adverse events during treatment were compared between the two groups. 【Results】The total effective rate of the observation group was significantly higher than that of the control group (P<0.05). After treatment,the arterial oxygen partial pressure,arterial carbon dioxide partial pressure and thromboelastogram parameters (R value,K value and LY30) in the two groups increased,if compared with the baseline level,while the thromboelastogram parameters(α Angle,MA value),pulmonary artery pressure,D-dimer,tumor necrosis factor-α,high sensitivity C-reactive protein and N-terminal probrain natriuretic peptide decreased,and the activated partial thromboplastin time and prothrombin time got prolonged (P<0.05). The improvement of patients in the observation group was significantly better than those in the control group (P<0.05). There was no significant difference in the incidence of adverse events between two group (P>0.05). 【Conclusion】The anticoagulant effect of Naoxintong capsule combined with rivaroxaban in the interventional treatment of APE is better than that of rivaroxaban alone. It can significantly improve the parameters of thromboelastogram and reduce the inflammatory response. This method has high safety.

Changes of Inflammatory Factors and Coagulation Function in Patients with Pulmonary Infection after Cerebral Aneurysm Surgery and their Correlation with Prognosis

【Objective】To investigate the changes of serum interleukin-6 (IL-6), matrix metalloproteinase-9 (MMP-9) and coagulation function in patients with pulmonary infection after cerebral aneurysm surgery and their correlation with prognosis. 【Methods】A total of 157 patients with cerebral aneurysms admitted to our hospital from January 2017 to May 2020 were divided into pulmonary infection group (n=56) and non pulmonary infection group (n=101) according to whether the patients were complicated with pulmonary infection after surgery. The coagulation function indexes and serum IL-6 and MMP-9 levels were compared between the two groups and patients with different prognosis. The predictive value of FIB, D-D, IL-6 and MMP-9 levels on the prognosis of pulmonary infection after cerebral aneurysm surgery was evaluated by using the receiver operating characteristic (ROC) curve. 【Results】 The levels of FIB, D-D, IL-6, MMP-9 in patients with pulmonary infection were higher than those in patients without pulmonary infection (P<0.05). The levels of IL-6, MMP-9, FIB and D-D in severe patients were significantly higher than those in mild and moderate patients (P<0.05). The levels of IL-6, MMP-9, FIB and D-D in patients with good prognosis were lower than those in patients with poor prognosis (P<0.05). ROC curve results showed that the AUC of FIB, D-D, IL-6, MMP-9 predicting the prognosis of pulmonary infection after cerebral aneurysm surgery were 0.763, 0.835, 0.755, 0.837, respectively. The AUC predicted by the above indicators was greater than that by a single indicator (P<0.05). 【Conclusion】The high expression of FIB, D-D, IL-6, MMP-9 in patients with pulmonary infection after cerebral aneurysm surgery is closely related to the severity and prognosis of pulmonary infection, which is of great significance for guiding clinical treatment.

Effect of Probiotics on Intestinal Flora in Elderly Patients with Abdominal Trauma and Liver Injury

【Objective】To observe the effect of probiotics on intestinal flora in elderly patients with abdominal trauma combined with liver injury. 【Methods】A total of 116 elderly patients with abdominal trauma combined with liver injury were randomly divided into the observation group and the control group, with 58 patients in each group. The patients in both groups received surgical treatment such as drainage, ligation or partial resection according to the situation of abdominal trauma and routine perioperative therapy. The patients in the control group were treated with artificial liver support system on this basis, while the patients in the observation group were treated with probiotics on this basis, To compare the clinical efficacy of the two groups of patients after treatment, to compare the levels of various indicators of liver function [alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), albumin (ALB)], and the number of intestinal flora (Escherichia coli, bifidobacteria, lactobacillus, staphylococcus) of the two groups of patients before and after treatment, and to count and record the occurrence of postoperative complications of the two groups of patients during treatment. 【Results】The total effective rate of the observation group after treatment was 84.48%, which was significantly higher than that of the control group (65.52%), with a statistically significant difference (P<0.05). After treatment, ALT, AST and TBIL of the two groups were significantly lower than that before treatment, and ALB was significantly higher than that before treatment, with a statistically significant difference (P<0.05); After treatment, ALT, AST and TBIL in the observation group were significantly lower than those in the control group, and ALB was significantly higher than those in the control group, with a statistically significant difference (P<0.05). After treatment, the number of Escherichia coli and Staphylococcus in the two groups was significantly lower than that before treatment, and the number of Bifidobacterium and Lactobacillus was significantly higher than that before treatment (P<0.05); After treatment, the number of Escherichia coli and Staphylococcus in the observation group was significantly lower than that in the control group, and the number of Bifidobacterium and Lactobacillus was significantly higher than that in the control group (P<0.05). There was no significant difference in the total incidence of postoperative complications between the two groups after treatment (P>0.05). 【Conclusion】Probiotic adjuvant therapy can significantly improve the efficacy of elderly patients with abdominal trauma combined with liver injury, can significantly improve the liver function of patients, regulate intestinal flora, and is safe to use, which is worthy of clinical promotion.

Value of DSA and Gadolinium Bemeglumine Enhanced MRI in the Diagnosis of Small Recurrent Lesions of Hepatocellular Carcinoma after TACE

【Objective】To investigate the diagnostic efficacy of digital subtraction angiography (DSA) and gadolinium enhanced magnetic resonance imaging (MRI) in the recurrence of small lesions after transcatheter arterial chemoembolization (TACE) of liver cancer. 【Methods】A total of 85 HCC patients treated with TACE in our hospital from January 2017 to January 2021 were selected. All patients were examined with DSA and gadolinium enhanced MRI, and the pathological results were taken as the recurrence results of small liver cancer. The results of DSA and gadolinium enhanced MRI in the diagnosis of recurrence of small liver cancer were recorded, and the efficacy of the two diagnostic methods was analyzed. 【Results】After TACE, 41 out of 85 HCC patients (48.2%) were confirmed to have relapsed small lesions by biopsy. DSA and gadolinium bemeglumine enhanced MRI showed that 38 cases and 39 cases were small recurrent lesions, respectively. The sensitivity and specificity of DSA were 92.7% and 100.0% respectively for the diagnosis of recurrent small lesions, and the sensitivity and specificity of gadolinium bemeglumine enhanced MRI for the diagnosis of recurrent small lesions were 95.1% and 100.0% respectively, with no significant difference (P>0.05). The consistency between DSA and gadolinium bemeglumine enhanced MRI diagnosis and pathological diagnosis was 0.791 and 0.778 respectively, and the consistency between DSA and gadolinium bemeglumine enhanced MRI diagnosis was 0.802. 【Conclusion】DSA and gadolinium bemeglumine enhanced MRI have similar efficacy in the diagnosis of small recurrent lesions of hepatocellular carcinoma after TACE, and suitable diagnostic methods can be selected reasonably according to individual conditions.
