1873 Association between Retinol Binding Protein and Non-alcoholic Fatty Liver Disease: a Meta-analysis

【Objective】To evaluate the association between retinol binding protein (RBP) and non-alcoholic fatty liver disease (NAFLD).【Methods】A document retrieval was conducted by retrieval systems,including PubMed,Web of Science,Cochrane Library,Chinese National Knowledge Infrastructure(CNKI), CBM, VIP and Wan Fang Database, to collect the published papers of case-control studies on the theme of association between serum RBP and NAFLD up to Dec. 2017.Data collection and quality assessment were performed by two authors, respectively.All the meta-analysis were conducted with software RevMan 5.3.【Results】A total of 14 qualified studies were included in the meta-analysis.The analysis showed that serum RBP might be significantly higher in patients with NAFLD than non-NAFLD patients (SMD=0.96,95%CI:0.63~1.28,P<0.05)。Clinical subtype specific subgroup analysis showed that RBP might be higher in patients with type 2 diabetes mellitus and NAFLD than non-NAFLD patients (SMD=0.70,95%CI:0.42~0.97,P<0.001).Age specific subgroup analysis showed the RBP might be higher in patient aged ≥18y than the control (SMD=0.82,95%CI:0.55~1.09,P<0.001), while difference between patient and control aged <18y were insignificant (SMD=1.68,95%CI:-0.22~3.59,P=0.08). Sensitivity and publication bias analyses indicated that our results were stable and without obvious publication bias.【Conclusion】 The meta-analysis shows that serum RBP might be higher in patients aged ≥18y with NAFLD than non-NAFLD patients.

1877 Efficacy and Safety of Tigecycline in Treatment of Hospital Associated Pneumonia(HAP) Caused by Carbapenem-resistant Klebsiella(CRKP)

【Objective】To retrospectively analyze the efficacy and safety of Tigecycline in treatment of hospital associated pneumonia(HAP) caused by carbapenem-resistant Klebsiella(CRKP). 【Methods】 From September 2010 to March 2013, 22 patients with HAP infected by CRKP through sputum or alveolar lavage fluid culture were admitted to our hospital. Tegacycline was used for at least 5 days. Clinical efficacy, bacteriological efficacy and adverse reactions were retrospectively analyzed. 【Results】Among the 22 patients, 6 cases (27.27%) were cured, 4 cases were effective, the effective rate was 45.45% (10 / 22), and the clearance rate of bacteria was 54.55% (12 / 22). There was no significant difference in the clinical effective rate and bacterial clearance rate between the high dose group and the low dose group(χ2=0.028,P=0.867;χ2=0.566,P=0.452). There was no significant difference in the clinical effective rate and bacterial clearance rate between the carbapenem antibiotics group and the non-carbapenem antibiotic group(χ2=0.028,P=0.867;χ2=0.566,P=0.452). Of the 22 patients, 2 (9.1%) developed nausea and vomiting, and the symptoms disappeared after symptomatic treatment,the drug was not stopped. In the course of treatment, there were 2 cases of double infection, 1 case (9.1%) of Pseudomonas aeruginosa and 1 case (9.1%) of Pseudomonas maltophilia, and according to drug sensitivity, no serious adverse events occurred. 【Conclusion】Tegacycline can be used as a therapeutic option for HAP caused by CRKP infection. However, it is necessary to accumulate more clinical experience, especially for a large sample of prospective studies to explore the efficacy and safety of conventional and high dose therapy.

1880 Relationship Among Coagulation Function, D-dimer Level and Coronary Artery Lesions in Children with Kawasaki Disease

【Objective】To investigate the relationship among coagulation function, D-dimer level and coronary artery lesions in children with Kawasaki Disease.【Methods】A total of 194 cases with Kawasaki disease children who were diagnosed in the department of pediatrics in our hospital were collected in the study. According to the results of heart echocardiography for coronary artery lesions, patients were divided into the coronary artery lesions group (CAL, n=42) and the no-coronary artery lesions group (NCAL, n=152). At the same time 60 healthy volunteers were formed the control group. . Blood coagulation function was tested in all subjects, including thrombin time (TT), partial prothrombin activation time (APTT), fibrinogen (FIB) quantitative values, and D - dimer level. The laboratory results were statistically analyzed. 【Results】 APTT and TT in both CAL and NCAL groups of Kawasaki disease were shorter than in the control group, while FIB and D - dimer were higher than the control group. The difference was statistically significant (P<0.05). There was a significant difference in the TT and D-dimer levels between the CAL group and the NCAL group (P<0.05). Logistic regression analysis indicated that TT was an independent influencing factor of coronary artery lesions (P<0.05). For every one second increase in TT of Kawasaki disease, the risk of coronary artery lesions increased by 0.838 -fold . The ROC curve area (AUCROC) for TT to diagnose coronary artery lesions of was 0.608. 【Conclusion】 Children with Kawasaki disease are hypercoagulable in the acute stage. Coagulation function TT is an independent and sensitive factor to indicate CAL. At the same time, it can also be speculated that coronary artery lesions is associated with vascular inflammation activity.

1883 Effect and Novel Mechanism of IL-17A on Apoptosis of HepG2 Cell

【Objective】To explore the effects of interleukin-17A (IL-17A) on apoptosis of HepG2 cell and investigate the novel mechanism. 【Methods】Quantitative real-time PCR (qRT-PCR) and Western blot were used to detect IL-17A expression in normal hepatocytes (L-02) and hepatic cancer cell lines (Huh7, HepG2 and PLC/PRF/5). Subsequently, HepG2 cells were selected and treated with recombinant IL-17A (rIL-17A) and/or myeloid cell leukemia-1 (Mcl-1) siRNA. Apoptotic rate was analyzed by flow cytometry. The expression of Mcl-1 was measured by qRT-PCR and Western Blot. 【Results】 Compared to those in L-02 cells, the mRNA and protein levels of IL-17A were significantly increased in Huh7, HepG2 and PLC/PRF/5 cells, and HepG2 cells had the highest expression (P<0.01). Administration of rIL-17A reduced apoptotic rate and increased Mcl-1 expression in HepG2 cells (P<0.01). However, siRNA-mediated knockdown of Mcl-1 reversed the inhibitory effect of rIL-17A on HepG2 cell apoptosis (P<0.05).【Conclusion】IL-17A is highly expressed in hepatic cancer cells. IL-17A reduces HepG2 cell apoptosis by upregulating Mcl-1 expression.

1886 Construction and Identification of TNFAIP1-shRNA lentiviral Expression Vector

【Objective】To construct tumor necrosis factor alpha-induced protein 1 (TNFAIP1)-shRNA lentiviral vector.【Methods】Four shRNA sequences were designed and synthesized according to the sequence of human TNFAIP1 gene. These recombinant plasmids were transfected into 293T cells. RT-PCR assay was applied to detect the mRNA expression level of TNFAIP1 in each group and thus to choose the most effective TNFAIP1-shRNA. The HSH018143-LVRH1P-RNAi was constructed and confirmed by DNA sequencing. The selected TNFAIP1-shRNA-0S523409 was transfected into 293T cells. After 72 hours, the green fluorescence expression was observed and the lentiviral titer was tested.【Results】The TNFAIP1-shRNA lentiviral vector had been transfected into 293T cells efficiently and stably, the titer of lentivirus was 5×108 TU/mL.【Conclusion】The TNFAIP1-shRNA lentiviral expression vector has been constructed successfully, which lays a foundation for exploring for the role of TNFAIP1 in diabetic retinopathy.

1889 Diagnostic Value of Color Doppler Ultrasound in Children ' s Congenital Hepatic Portal Vein Shunt

【Objective】To investigate the diagnostic value of color Doppler ultrasound in pediatric patients with congenital extra-hepatic portosystemic shunt(CEPS). 【Methods】The ultrasonographic features of 7 children with CEPS diagnosed by ultrasound and confirmed by multi-helix CT or surgery were analyzed retrospectively.【Results】All the 7 cases were confirmed by CT or surgical operation were diagnosed as 6 cases with type Ⅱ, 1 case with type Ib and 4 cases with congenital heart disease. Color Doppler ultrasound can detect the abnormal blood flow sensitively and determine the direction and velocity of blood flow. The diagnostic accuracy is high and the results are consistent with the results of CT or surgery.【Conclusion】As a rapid and simple method, color Doppler ultrasound can clearly show the origin and route of extrahepatic portal-systemic venous shunt. It can be used as the first choice for CEPS.

1892 Application Value of Simulated Defecography in Functional Constipation Detection

【Objective】 To explore the application value of emulation defecography in the detection of functional constipation.【Methods】The clinical data of 85 patients with constipation from May 2015 to May 2017 were selected. Among them, 43 cases were examined by traditional barium defecography and 42 cases by virtual defecography. The results of different detection methods and the changes of indicators in the detection process were compared.【Results】The time of excretion emptying in patients with virtual defecography was (9.2±2.1) s, which was significantly higher than that in barium defecography (3.1±0.4) s (P<0.05). The viscosity of contrast agent used in emulation defecation was (239±10.3) g, and that of barium defecation was (15.1±3.2) g. The difference between the two groups was statistically significant (P<0.05). The contrast agent of barium excrement was liquid, while the contrast agent of simulated defecation was soft and semi-solid, which was striped out. The detection rate of functional constipation by virtual defecography was 96.5% (82/85), which was significantly higher than that of barium defecography (80.0%) (68 / 85), and the difference was statistically significant (χ2=4.617,P<0.05).【Conclusion】Virtual defecography can achieve more accurate results in clinical treatment of functional constipation. At the same time, the operation is simple, the price is low, and it is worthy of clinical application.

1895 Meta-analysis of the Efficacy of Chemotherapy Alone vs. Chemotherapy Combined with Targeted Drugs for Advanced Biliary Tract Cancer

【Objective】To evaluate the role of chemotherapy and targeted drug treatment in advanced biliary tract cancer (BTC). 【Methods】By searching MEDLINE, EMBASE and other domestic and foreign databases, a randomized controlled trial (RCT) for advanced BTC was conducted. The patients in the observation group were treated with chemotherapy combined with targeted drugs, while the patients in the control group received chemotherapy alone. Meta analysis on the objectiv remission rates, median overall survival (OS), progression free survival (PFS), and adverse events was performed in both groups. 【Results】A total of 8 RCTs were enrolled, including 1052 patients in total. There were 549 patients in the observation group and 503 patients in the control group. The objective remission rate in the observation group was significantly higher than that in the control group (P<0.05). However, there were no significant differences in OS, the first half year of PFS and one year PFS between the two groups (P>0.05). The incidence of nausea / vomiting and bone marrow toxicity was not significantly different between the two groups (P>0.05). The incidence of skin toxicity and diarrhea in the observation group was higher than that in the control group (P<0.05). 【Conclusion】Based on the latest RCTs, meta-analysis shows that chemotherapy combined with targeted therapy can improve the objective response rate of late BTC. However, it does not have significant survival benefits.

1899 Clinical Effect of Drug-Eluting Balloon in the Treatment of ISR after PCI in Patients with Coronary Heart Disease

【Objective】To study the clinical effect of drug-eluting balloon (DEB) in the treatment of ISR after percutaneous coronary intervention (PCI) in patients with coronary heart disease.【Methods】The clinical data of 50 patients with coronary heart disease treated by ISR from January 2012 to May 2017 were analyzed retrospectively. According to the type of stent, it was divided into DES group and DEB group. The clinical features, coronary angiography features, and terminal events within 12 months after treatment were compared between the two groups.【Results】After 6 months of treatment, coronary angiography showed that the incidence of restenosis in the DEB group was significantly lower than that in the DES group (P<0.05).There was no significant difference in TIMI blood flow grade Ⅰ, Ⅱ, Ⅲ, stenosis degree, minimum lumen diameter and late lumen loss between the two groups (P>0.05). The incidence of secondary stent stenosis in the DES group was 24.00% (6 / 25), which was significantly higher than that in the DEB group (4.00%, 1 / 25). The difference was statistically significant(χ2=4.153,P=0.042<0.05). Kaplan-Meier survival analysis showed that there was no significant difference in MACE free survival rate between the two groups within 12 months after operation (χ2=0.467,P=0.494>0.05). 【Conclusion】The clinical efficacy of DEB in the treatment of ISR is comparable to that of DES with high safety, and no serious complications such as artery laceration dissection have been caused.

1902 Therapeutic effect of Tripterygium Glycosides Combined with Pancreatic Kininogenase in Treatment of Diabetic Nephropathy

【Objective】To investigate the therapeutic effect of tripterygium glycosides combined pancreatic kininogenase in the treatment of diabetic nephropathy. 【Methods】A total of 80 patients of diabetic nephropathy who received therapy from March 2016 to Nov 2017 in our hospital were selected as research subjects. According to the stochastic indicator method, patients were divided into the observation group and the control group. Patients in the control group received pancreatic kininogenase treatment, while patients in the observation group received tripterygium glycosides combined with pancreatic kininogenase treatment. Clinical efficacy, levels of homocysteine (Hcy),hypersensitive C-reactive protein (hs-CRP) before and after treatment, renal function indicators (blood urea nitrogen-BUN, urinary creatinine-UCr and 24h urine protein quantification-UP) and blood glucose index (fasting blood glucose-FPG, 2h post-prandial blood sugar-2hPG and glycosylated hemoglobin-HbAlc) were compared between the two groups. The adverse reactions were also observed during treatment.【Results】The total effective rate of the observation group was 87.50%, which was significantly higher than that of the control group (67.50%).The difference between the two groups was statistically significant (P<0.05). After treatment, the levels of Hcy, hs-CRP, BUN, UCr,UP,FPG,2hPG and HbAlc in the observation group were significantly lower than those of the control group (P<0.05). One patient in the observation group had a slight increase in alanine aminoase after 1 month of treatment, and returned to normal after symptomatic treatment. No adverse reactions occurred in the control group. There was no significant difference in the incidence of adverse reactions between the observed group and the control group (P>0.05). 【Conclusion】Tripterygium glycosides combined with pancreatic kininogenase in the treatment of DN can significantly reduce serum inflammatory factors and blood glucose levels, improve renal function. It is safe and worthy of clinical application.

1905 Application of Nursing Intervention of Information-motivation-behavior Model in Postoperative Rehabilitation of Knee Joint Replacement in Patients with Unilateral Posterior Chamber Degeneration

【Objective】To explore the application value of nursing intervention of information-motivation-behavior (IMB) model in postoperative rehabilitation of knee joint replacement in patients with unilateral posterior chamber degeneration of knee joint. 【Methods】A total of 90 cases of internal single compartment degeneration in the knee joint of our hospital were selected and divided into the observation and control groups according to the random number table method. Patients in the control group received routine nursing intervention, while patients in the observation group were treated with nursing intervention of IMB model on the basis of routine nursing. The self-efficacy rating (SER) score, the self-rating Anxiety Scale (SAS) score, the self-rating of Depression Scale (SDS) score, the change of the knee joint function (HSS) score and the compliance behavior before and after the intervention of two groups were observed and compared. The postoperative complications and nursing satisfaction were also compared between the two groups. 【Results】After intervention, the SER score of the observation group was higher than that of the control group, while the scores of SAS and SDS were lower than those of the control group(P<0.05). The total compliance rate of the observation group was 95.56% (43/45), which was higher than that of the control group 77.78% (35/45)(P<0.05). After intervention, the HSS scores of the two groups were higher than those before intervention, and the HSS score of the observation group was higher than that of the control group(P<0.05). The complication rate in the observation group was 2.22% (1/45), which was lower than that in the control group 17.78% (8/45)(P<0.05). The nursing satisfaction of the observation group was 97.78% (44/45), which was higher than that of the control group 80% (36/45)(P<0.05). 【Conclusion】The nursing intervention of IMB model after knee joint replacement surgery can improve the negative psychological emotion of the patients with unilateral compartment degeneration, improve their self-efficacy and compliance behavior, reduce the incidence of complications, and promote the recovery of the knee joint function. In addition, it has high nursing satisfaction and is worthy of clinical application.

1908 Analysis of Screening Results of Upper Digestive Tract Cancer in 6000 Cases of High Risk Population

【Objective】To investigate the incidence of upper gastrointestinal cancer in high-risk population in this area.【Methods】From January 2012 to December 2016, 6000 cases of high risk population aged 40 to 70 years old in our city were investigated. The cancer of upper digestive tract was surveyed and analyzed in order to analyze the carcinogenesis of upper digestive tract in high risk population in this city.【Results】Among the 6000 subjects, 4632 cases were normal esophageal mucosa, 1368 cases were abnormal, the abnormal rate was 22.80%. 391 cases (6.52%) of precancerous lesion, 112 cases (1.87%) of esophageal carcinoma, 94 cases (1.57%) of early carcinoma, 4689 cases of normal cardia and stomach, 1311 cases of abnormal gastric cardia and stomach, the abnormal rate was 21.85%. There were 176 cases (2.93%) of cardiac and precancerous lesions, and 25 cases (0.42%) of cardia and gastric cancer. Among them, 17 cases (0.28%) were early stage cancer. From 2012 to 2016, the detection rate of precancerous lesion and invasive carcinoma increased with the increase of year. The detection rate of invasive carcinoma was the highest in 2016, 10.73 % and 0.52%. The proportion of precancerous lesions, early stage cancer and invasive carcinoma in male was higher than that in female (P<0.05), and the detection rate of precancerous lesion of upper digestive tract, early cancer and invasive carcinoma of upper digestive tract gradually increased with age . 【Conclusion】The detection rate of precancerous lesions of upper digestive tract is higher in high risk population in this city, and the detection rate of upper digestive tract cancer is lower in men and elderly people, and the detection rate of upper digestive tract carcinogenesis is higher in male and elderly population.

1911 Effect of Oxycodone Hydrochloride Injection as Preemptive Analgesia on Analgesic Action and Pain-producing Factors in Segmental Mastectomy

【Objective】To explore the effect of oxycodone hydrochloride injection as preemptive analgesia on the analgesic effect and plasma pain-producing factors in segmental resection of breast cancer mastectomy. 【Methods】 A total of 60 patients who underwent segmental mastectomy in our hospital under local anesthesia were randomly divided into two groups: group A and group B. Patients in group A received intravenous oxycodone hydrochloride injection 10mg dissolved in 10ml of normal saline before local anesthesia, while patients in group B were injected with 10ml of normal saline. The vital signs of MAP, HR, SpO2 and VAS scores at the time of surgical incision (T1), 10 min after incision (T2), at the termination of operation (T3), and 3h after the operation (T4) were recorded. Peripheral venous blood(3ml) was drawn to detect 5-HT and substance P at T1~T4. The total dose of local anesthetic and adverse reactions during surgery were compared as well.【Results】 Compared to group B, MAP in group A was lower at T1 and T2, while HR was lower at T1, T2 and T3 (P<0.05). Patients in group A had significantly lower VAS scores at T1~ T4 than those in group B(P<0.05). Compared to T1, the plasma levels of substance P and 5-HT in group B were elevated to some degree at T2~ T4, which were also higher than those in group A (P<0.05). While there were no significant differences of substance P and 5-HT plasma levels in group A at T1~T4(P>0.05). Compared to group B, substance P and 5-HT levels in group A were lower than those in group B at T2~T4 (P<0.05). Meanwhile, the total dose of local anesthetic in patients of group A was significantly lower than that in group B( P<0.05). However, there was no statistically significant difference in the incidence of adverse reactions such as nausea and vomiting between the two groups (P>0.05). 【Conclusion】Application of preemptive analgesia with oxycodone hydrochloride injection in breast cancer patients having segmental mastectomy can significantly alleviate intraoperative and postoperative pain intensity, decrease levels of plasma 5-HT and substance P, and reduce the amount of local anesthetic used.

1914 Evaluation of the Effect of Quantitative CT on Bronchoalveolar Lavage in Patients with Pulmonary Alveolar Proteinosis

【Objective】To explore the result of quantitative computed tomography (CT) and its clinical significance on patients with pulmonary alveolar proteinosis (PAP) after bronohoalveolar lavage. 【Methods】The clinical data of 30 patients with PAP were selected. All patients were treated with BAL.The pulmonary function indexes of patients were measured by pulmonary function test before and after treatment [forced vital capacity (FVC), carbon monoxide diffusion volume (DLCO), forced expiratory volume (FEV1) in 1 second, maximum ventilation value (MVV) / percentage of predicted value. That is, FVC%, DLCO%, FEV1%,MVV%].Pulmonary ventilation function indexes [arterial oxygen / carbon dioxide partial pressure (PaO2 and PaCO2), blood oxygen saturation (SaO2)] were compared before and after treatment. The mean density of lung, the total volume of lung gas, the total mass of lung, the mean value of CT and so on were measured by quantitative CT. MMRC score was used to evaluate the degree of dyspnea. 6 min walking test was used to evaluate dyspnea symptoms and exercise endurance.【Results】The difference of FVC%, FEV1%, PaCO2 before and after BAL was statistically significant (P>0.05). The DLCO%, MVV%, PaO2, SaO2 value after BAL was significantly higher than that before BAL (P<0.05). The mean lung CT value, total lung weight and lung density were significantly lower than those before BAL (P<0.05). The total volume of lung gas was significantly higher than that before BAL (P<0.05). The MMRC score of patients after BAL was significantly lower than that before BAL, and the 6 min -walking distance was significantly higher than that of BAL (P<0.05). 【Conclusion】The clinical effect of BAL in the treatment of PAP is better. Quantitative CT can be used to evaluate the effect of bronohoalveolar lavage.

1917 Predictive Value of Dynamic Enhanced Magnetic Resonance Imaging Combined with Laboratory Examination in Patients with Undifferentiated Arthritis

【Objective】To explore the effect of dynamic contrast-enhanced magnetic resonance combined with laboratory tests on the prognosis of patients with undifferentiated arthritis (UA). 【Methods】The clinical data of 180 patients with UA admitted in our hospital from April 2013 to April 2017 were selected. According to the follow-up results whether RA was developed, they were divided into RA group (n=78) and non-RA group (n=71). The data of the remaining 31 patients were not completely excluded. All patients underwent dynamic contrast enhanced magnetic resonance (MRI) and laboratory tests. The indexes of dynamic enhanced MRI and related laboratory indexes were compared between RA group and non-RA group. The diagnostic efficacy of dynamic enhanced MRI in predicting the outcome of UA to RA was analyzed.【Results】There was no significant difference in the proportion of (TTP), rheumatoid factor (RF) positive between the two groups at peak time (P>0.05). In the RA group, the relative RELENH, MAXENH, MAXRELENH, WASHIN, AREACURV, ESR, CRP, ACCP positive and MCV positive were significantly different (P<0.05). AREACURV had the highest sensitivity (96.0%), and AUC max. of WASHIN (0.955), CRP specificity max.(89.6%) and AUC of ACCP positive max.(0.972) had the highest diagnostic efficacy.【Conclusion】Dynamic contrast-enhanced MRI and laboratory related indexes can predict the outcome of UA to RA and can guide early diagnosis and treatment.

1920 Diagnostic Effect and Clinical Value of Urodynamic Examination in the Etiology of Urinary Frequency Patients

【Objective】To investigate the diagnostic value and clinical value of urodynamic examination in the etiology of urinary frequency patients. 【Methods】A total of 200 patients with urinary frequency were selected as subjects. All patients underwent urodynamic examination. According to the results of the examination, symptomatic treatment was given. The clinical efficacy of the patients was observed. The value of urodynamic examination in the diagnosis of urinary frequency etiology was evaluated. 【Results】Among the 200 patients, 80 cases (40.0%) were found to be bladder outlet obstruction by urodynamic examination, including 52 cases of urethral strictures and 28 cases of pre-existing prostate hyperplasia. The patients underwent urethral dilatation, transurethral resection of prostate and received medication. After treatment, the effective rate of treatment was 92.5% (74/80). There were 50 cases of unstable bladder (25.0%), of which 32 cases were chronic prostatitis and 18 cases were interstitial cystitis. The total effective rate after drug treatment was 92.0% (46/50). Thirty cases (15.0%) of bladder hypersensitivity were detrusor hyperreflexia. The total effective rate was 86.7% (26/30) after drug treatment, psychological intervention and detrusor reflex training. Low compliance bladder was found in 25 (12.5%) patients and the total effective rate was 88.0% (22/25) after drug treatment. Detrusor muscle without reflex in 15 cases (7.5%), and the effective rate after bladder spasm, intermittent catheterization, bladder training and intervention of symptomatic drugs, the effective rate was 80.0% (12/15).【Conclusion】Urodynamic test has beneficial application in determining the etiology of urinary frequency patients. It can provide a reference for selecting the correct treatment method and is worthy of promotion in clinical practice.

1922 Therapeutic Effect of Hyaluronic Acid Gel Combined with Momethasone Furoate Ointment on 45 Children with Eczema

【Objective】To investigate the clinical effect of hyaluronic acid gel combined with amethasone furoate ointment in treating children with eczema. 【Methods】 A total of 90 cases of eczema children admitted to our hospital from February 2017 to October 2017 were randomly divided into the experimental group and the control group,45 patients in each group. The experimental group was treated with hyaluronic acid gel combined with mometasone furoate ointment. The skin lesion score, pruritus score, serum interleukin-4 (IL-4), interleukin-2 (IL-2), interferon γ (IFN- γ), clinical efficacy and adverse reactions were compared between the two groups before and after treatment.【Results】Before treatment, there was no significant difference in (EASI), pruritus score, IL-4, IL-2 and IFN- γ levels between the two groups (P>0.05). After treatment, the EASI score, pruritus score and IL-2,IFN- γ levels in the two groups were significantly lower than those before treatment (P<0.05). The level of IL-4 was significantly higher in the experimental group than in the control group (P<0.05). The total effective rate of the two groups was 100%. In the experimental group, the cure rate was 64.44%, the markedly effective rate was 28.89% and the effective rate was 6.67%, which were significantly higher than those of the control group, the cure rate 44.44%, the markedly effective rate 40.00% and the effective rate 15.56%, the difference was statistically significant (P<0.05). The adverse reaction rate of the experimental group was 13.33%, which was not significantly different from that of the control group (6.67%) (P>0.05).【Conclusion】Hyaluronic acid gel combined with momethasone furoate ointment can effectively treat children with eczema, improve local skin damage score, relieve pruritus symptoms, and improve the clinical therapeutic effect of eczema, at the same time, no obvious adverse reactions occurred, which is safe and reliable and worthy of clinical application.

1926 Value of MRI in the Early Diagnosis of Postoperative Intervertebral Space Infection in Patients with Lumbar Disc Herniation

【Objective】To investigate the clinical value of MRI technique in early diagnosis of postoperative intervertebral space infection in patients with lumbar disc herniation.【Methods】The clinical data of 86 patients with intervertebral space infection after lumbar disc herniation in our hospital from January 2014 to January 2017 were analyzed retrospectively. All the patients were examined by MRI and given active treatment after diagnosis. The results of MRI diagnosis were analyzed. The changes of WBC, erythrocyte sedimentation rate and the level of CRP, interleukin-6 (IL-6) were observed and compared before and after treatment.【Results】Staphylococcus aureus and Escherichia coli were the main pathogens in 86 patients with intervertebral space infection. Most of the infection sites of the patients were in L4/5 or L5/S1 intervertebral space. The lesions showed different degree of disc fragmentation, becoming smaller, disappeared and irregular margin. The results of MRI showed that the intervertebral disc space lost its normal shape and signal intensity, including long T1 low signal in 77 cases, T1 signal in 9 cases, long T2 high signal in 82 cases, short T2 low signal in 4 cases. The vertebral cartilage endplate and adjacent cancellous bone in the intervertebral disc space showed varying degrees of damage.There were no obvious changes or slight wedge-shaped changes in the vertebrae and no kyphosis in the spine. Six months after operation, the clinical symptoms of 63 patients with intervertebral space infection were completely disappeared (73.26%), and the remaining 23 patients still had mild intermittent low back pain. The levels of WBC, ESR and CRP, IL-6 in all patients were significantly lower than those before treatment (P<0.05). 【Conclusion】MRI can clearly display the site of infection in patients with intervertebral space infection after lumbar disc herniation. It is of great significance for the early diagnosis and treatment of intervertebral space infection.

1929 Application of Sevoflurane Inhalation in Fast-track Infant Operation

【Objective】To evaluate the clinical application value of sevoflurane inhalation anesthesia in fast-track infant operation. 【Methods】A total of 68 children undergoing fast-track infant operation were divided into two groups by odd and even numbers, with 34 cases in each group. The observation group was given mask inhalation of sevoflurane for induction while the control group was given ketamine anesthesia induction. The anesthesia induction time, postoperative recovery time, mean arterial pressure (MAP), heart rate (HR), SpO2 and the incidence of adverse reactions were recorded and compared. 【Results】The anesthesia induction time and postoperative recovery time of the observation group were significantly shorter than those of the control group (P<0.05). After the induction, MAP and HR of the observation group were significantly lower than those of the control group (P<0.05). The incidence rates of nausea and vomiting, increased secretions, laryngeal spasm in the observation group were significantly lower than those in the control group (P<0.05). 【Conclusion】Sevoflurane inhalation anesthesia has the advantages of short induction time, quick postoperative recovery and little effect on hemodynamics in fast-track infant operation. It is safe and worthy of clinical use.
