Safety and Efficacy Analysis of Allogeneic Frozen Bone Graft in the Treatment of Large Bone Defects

【Objective】To compare and analyze the clinical effect of allogeneic frozen bone graft versus autogenous bone graft in the treatment of large bone defects, and to evaluate the safety and efficacy of allogeneic frozen bone graft. 【Methods】From July 2009 to July 2014, 60 cases of large bone defects treated in our hospital were selected as study objects. They were randomly divided into observation group and control group by random number table method. The observation group received allogeneic frozen bone graft treatment, the control group received autogenous bone graft treatment. The efficacy difference and complication between the two groups were compared and analyzed . 【Results】The success rates in the observation group were 93.3%, and 96.7% in the control group, but the difference was not statistically significant (P>0.05). The bone healing time in the observation group was(12.3±4.2)months, and(10.7±5.3)months in control group, but the difference was not statistically significant (P>0.05); The drainage 48h postoperation in the observation group were (262.6±38.9)mL, and in control group were(249.4±49.2)mL, the control group was lower than the observation group, but the difference was not statistically significant (P>0.05). There were no significant differences between the observation group and control group in the cases of rejection, infection, fever, wound exudate, delayed and healing (P>0.05). 【Conclusion】Allogeneic frozen bone has a similar effect and safety to autogenous bone graft, but will not cause secondary trauma, easily obtained, is worthy of learning to use.

Application of Ischemia Modified Albumin Inspection in Patients with Acute Chest Pain

【Objective】To investigate the application effect of Ischemia Modified Albumin inspection in patients with acute chest pain. 【Methods】 100 cases of patients with acute chest pain in 3 h admitted in our hospital from Feb 2014 to July 2015 were selected and divided into ischemic chest pain group (59 cases) and nonischemic chest pain group (41 cases). The Ischemic chest pain group was further divided into unstable angina group (39 cases) and acute myocardial infarction group (20 cases). The venous blood was collected at the instant, 3 h and 24 h after admission. Creatine kinase isoenzyme (CKMB), cardiac troponin I (cTnI) and IMA were determined. 【Results】At the instant and 3 h after admission, the IMA of the ischemic chest pain group was significantly higher than that of the nonischemic chest pain group (P<0.05), while there was no significant difference between cTnI and CKMB level (P>0.05). At 24 h after admission, there was no significant difference in IMA between the ischemic chest pain group and the nonischemic chest pain group (P>0.05); cTnI and CKMB of the ischemic chest pain group were significantly higher than those of the nonischemic chest pain group (P<0.05). At the instant, 3 h and 24 h after admission , IMA level between the unstable angina group and the acute myocardial infarction group was not statistically significant (P>0.05). 【Conclusion】 Ischemia Modified Albumin is a sensitive index for early diagnosing ischemic chest pain and nonischemic chest pain, but could not diagnose unstable angina and acute myocardial infarction.

Cerebral Protection Effect of Mild Hypothcrmia at Different Time for Rats after Cardiopulmonary Resuscitation in Anoxia

【Objective】To investigate the cerebral protection effect of mild hypothermia and timing choice for rats after cardiopulmonary resuscitation in anoxia. 【Methods】Thirtysix cases of SD mice were randomly divided into shamoperated group (A Group,n=6), normothermia group (B Group ,n=6) and mild hypothcrmia treatment group (C Group,n=24). C group was randomly divided into four subgroups of C1, C2, C3 and C4 groups, with 6 in each group. Rats in A Group just received anesthesia, tracheotomy, vascular access, without cardiopulmonary resuscitation in anoxia; rats in B Group were not treated with mild hypothcrmia; rats in C group were treated with cardiopulmonary resuscitation in anoxia at 2, 4, 6 and 8 min. immediately after heart arrest, respectively. The resuscitation successful rate of each group were recorded. Immunohistochemical method were used to determine the expression of Nrf2 and HO1 protein at cortex and hippocampus. Water content in brain tissue was calculated with Elliott method. 【Results】The expression of Nrf2 and HO1 protein at cortex and hippocampus of C Group was significantly higher than that of the other two groups (P<0.05). No significant difference was found among C1,C2,C3 and C4 Group (P>0.05). Water content in brain tissue of B Group and C Group was significantly higher than that of A Group (P<0.05), but there was no significant difference between B group and C group (P>0.05), and there was no significant difference in the four subgroups of C1, C2, C3 and C4 (P>0.05).【Conclusion】 The cerebral protection effect of mild hypothcrmia for rats after cardiopulmonary resuscitation in anoxia may be realized through Nrf2ARE signaling pathway, by upregulating the expression level of Nrf2 and HO1. Mild hypothermia treatment should be carried out in the early stage of cardiac arrest after cardiac resuscitation in order to relieve the brain injury and improve the success rate of cardiopulmonary resuscitation.

Diagnostic Value of ThreeDimension Fast spoiled Gradient Sequence(3DFSPGR) at Evaluating Cartilage Injury of Knee

【Objective】 To investigate diagnostic value of threedimension Fast spoiled gradient sequence(3DFSPGR) at evaluating cartilage injury of knee by comparing with the conventional MRI. 【Methods】 A total of 45 patients with knee pain were treated in our hospital, who underwent 3DFSPGR and the conventional 2D sequences at 3.0 T. Through analyzing these images, the sensitivity, specificity and accuracy were evaluated by comparing 3DFSPGR with the conventional 2D sequence demonstrating degree of joint cartilage injury with the knee arthroscopy results as the gold standard. 【Results】 Fortyfive patients with 65 knees and 260 parts of the cartilage were totally analyzed. The arthroscopy results showed that 57 parts as Grade 0 and 203 parts as Grade 14. The sensitivity, specificity and accuracy of 3D FSPGR were higher than those of 2D sequences respectively, and the difference was statistically significant(P<0.05). 【Conclusion】3DFSPGR sequence is superior to the conventional 2D sequence in the diagnosis of knee cartilage injury, which has high clinical application value.

Evaluation of the Application of LigaSure in the Intestinal Closure in Dogs

【Objective】To explore the clinical value of LigaSure in the intestinal closer in dogs. 【Methods】Twenty dogs were randomly divided into the experimental and the control group. Dogs in the experimental group were performed on using LigaSure while dogs in the control group were performed on with traditional manual gut closure. According to intestinal different parts,these two groups were then further subdivided into two small subgroups each: intestinal and large bowel ( n=5). The gut closure time, intraoperative bleeding volume, postoperative complications, and intestinal histological effect were compared between two groups. 【Results】The gut closure times of the experimental and control groups were 3 s VS (8 ± 2.2) min in the small intestine and 7 s VS (12 ± 1.2) min in the large bowel, respectively. The amount of bleeding, 0 ml VS (27 ± 2.86) mL in the small intestine and 0 mL VS (29.65 ± 3.24) mL in the large bowel, of the experimental and control groups respectively, were statistically different (P<0.05). There was no significant difference in complication rate between the two groups (P>0.05). 【Conclusion】LigaSure on intestinal surgery closure is reliable; it can reduce the bleeding volume, operation time and intraoperative intestinal closure time and its postoperative complications were not found. It is worthy of further clinical research.

Correlation Analysis of Serum Inflammatory Factors Levels to Cognitive Dysfunction in Patients with Ischemic Stroke

【Objective】To analyze the correlation of serum levels of homocysteine (Hcy) and interleukin6 (IL6) to cognitive dysfunction in patients with ischemic stroke. 【Methods】84 inpatients with ischemic stroke were collected and divided into two groups according to occurrence of the cognitive functions: control group (38 cases, normal cognitive function), and research group (46 cases, cognitive dysfunction). Cognitive functions of all patients were assessed according to the Montreal Cognitive Assessment (MoCA). The indexes of Hcy, IL6, blood glucose, and lipid were detected and compared between the two groups. The risk factors to the occurrence of cognitive dysfunction were confirmed by the multivariate Logistic analysis. 【Results】Compared to control group, the values of Hcy, IL6, and oxLDL of the research group were higher (P<0.05), and the values of all indexes of blood glucose and other lipid indexes showed no difference between the two groups (P>0.05). Multivariant Logistic analysis showed that the Hcy and IL6 were independent factors to the occurrence of cognitive dysfunction. 【Conclusion】Serum levels of Hcy and IL6 were independent risk factors to cognitive dysfunction in patients with ischemic stroke.

Urinary VEGF level and its Related Factors in Newly Diagnosed Type 2 Diabetes Meillitus

【Objective】To investigate urinary VEGF level and related factors in newly diagnosed type 2 diabetes mellitus (T2DM) patients. 【Methods】The urinary VEGF level was measured by enzymelinked immunosorbent assay (ELISA) in 71 patients with newly diagnosed T2DM. The urinary VEGF level and related biochemical indicators were compared between newly diagnosed T2DM patients with normal albuinuria (n=41) and microalbuminuria (n=32). The related factors for the urinary VEGF level in newly diagnosed T2DM patients with analyzed by Pearson and Liner linear regression analysis. 【Results】Compared with newly diagnosed T2DM patients with normal albuinuria, fasting blood glucose, glycated hemoglobin (HbA1c), total cholesterol, lowdensity lipoprotein cholesterol, serum uric acid and urinary VEGF level were significantly higher in patients with microalbuminuria. In newly diagnosed T2DM, the urinary VEGF level was positively related with fasting blood glucose, glycated hemoglobin(HbA1c), lowdensity lipoprotein cholesterol, serum uric acid and urine albuminuria. HbA1c and urine albuminuria were independent factors for urinary VEGF level in newly diagnosed T2DM patients (Beta=18.4,0.626;P=0.017,0.010 ). 【Conclusion】The urinary VEGF level is directly related with HbA1c and urine albuminuria in newly diagnosed T2DM patients. The urinary VEGF level could be a potential marker for renal damage in newly diagnosed T2DM.

Value of Susceptibility Weighted Imaging(SWI) in Diagnosis of Intracranial Cavernous Angioma(ICA) in Children

【Objective】To investigate the applicable value of susceptibility weighted imaging(SWI) in diagnosis of intracranial cavernous angioma(ICA) in children. 【Methods】 The clinical data of 23 children with ICA confirmed by operation were retrospectively analyzed, and the conventional MRI sequences and SWI sequences of the skull were scanned and examined. Among them, 14 cases were male and 9 were female. The size, shape and signal characteristics of the lesions were compared and analyzed. The advantages of SWI sequence in the diagnosis of ICA in children were comprehensively evaluated.【Results】All ICA lesions were displayed in SWI sequence, and had the typical characteristics of the "hoop sign, mulberry, popcorn syndrome". With round or about round shape, and irregular shape with very low signal and mixed signal, the boundary was clear, the focus diameter was about 0.20 ~ 5.71 cm. Conventional MRI sequence scan can not show the lesions of less than 3 mm, However, the SWI sequence is mainly based on the class of low signal and mixed signal, more sensitive to the detection of trace and still bleeding, and the display of small vein blood vessels is more clear.【Conclusion】 SWI sequence scanning in children with ICA has a high sensitivity to make up for the lesions that the routine MRI sequence scan fails to find, and has important value for the early diagnosis of ICA in children.

Clinical Performance of rhGMCSF Gargles combined with Ribavirin Spray in the Treatment of Children with Herpetic Angina

【Objective】To investigate the clinical performance of rhGMCSF Gargles combined with Ribavirin Spray in treatment of children with herpetic angina. 【Methods】150 children in our hospital from May 2013 to March 2014 with herpetic angina were chosen for this study in the First Clinical College affiliated to Harbin Medical University. The children were divided into two groupsthe control group and the observation group. Both groups were treated with Ribavirin Spray after each meal and before sleep, but the observation group was treated with rhGMCSF in addition to the Ribavirin spray. Success in this experiment was defined by three categories: clinical improvements, cure of herpes, and lack of ulceration. In addition, the two groups were compared with the efficiency, time to cure, and length of stay. 【Results】The total success rate in the experimental group was 96%, which was significantly higher than the control group's with 84% success rate, the difference was statistically significant (P<0.05). The time it took to cure herpes was 1.5 days shorter in experimental group than in control group, and the fever last time in experimental group was 8 hours less than in control group. The length of stay was 1 day shorter in the experimental group than the control group. The results are statistically significantly.【Conclusion】The clinical application of rhGMCSF Gargles combined with Ribavirin Spray in treating pediatric oral herpes leads to a quicker and superior improvement in children with herpetic angina.
