体位调整联合抚触疗法对新生儿肺炎临床症状的影响

摘要【目的】探讨体位调整联合抚触疗法对新生儿肺炎临床症状的影响。【方法】选择2022年4月至2024年4月本院收治的100例新生儿肺炎患儿,根据随机数字表法将患儿分为对照组和观察组,每组50例。对照组接受常规护理,观察组在对照组的基础上给予转变体位护理和抚触疗法干预。比较两组症状改善情况(发热、咳嗽、咯痰等症状消失时间,住院时间)、呼吸功能[动脉血氧分压(PaO₂)、动脉二氧化碳分压(PaCO₂)、吸入氧浓度百分比(FiO₂)、血氧饱和度(SpO₂)、氧合指数(OI)]、发育情况[小儿智能发育量表(CDCC)评分]、情绪及神经行为情况[新生儿疼痛评分(NIPS)评分、新生儿精神行为评估量表(NBNA)评分]。【结果】观察组症状(发热、咳嗽、咯痰)消失时间和住院时间均短于对照组,差异有统计学意义(P<0.05)。干预后,两组患儿SpO₂、PaO₂及OI＞400占比高于干预前,且观察组SpO₂、PaO₂及OI＞400占比高于对照组;两组患儿PaCO₂低于干预前,且观察组PaCO₂低于对照组,差异有统计学意义(P<0.05)。干预后,两组智能发育指数(MDI)评分、心理运动发育指数(PDI)评分高于干预前,且观察组高于对照组,差异有统计学意义(P<0.05)。干预后,两组患儿NIPS评分低于干预前,且观察组NIPS评分低于对照组;两组患儿NBNA评分高于干预前,且NBNA评分高于对照组,差异有统计学意义(P<0.05)。【结论】转变体位护理联合抚触疗法能有效缩短新生儿肺炎患儿的症状消失时间和住院时间,增强呼吸功能,改善患儿情绪情况。

Abstract：【Objective】To investigate the effect of postural adjustment combined with touch therapy on clinical symptoms of neonatal pneumonia. 【Methods】A total of 100 children with neonatal pneumonia admitted to our hospital from April 2022 to April 2024 were selected and divided into the control group and the observation group according to random number table method, with 50 cases in each group. The control group received routine nursing, and the observation group received changing position nursing and touch therapy intervention on the basis of the control group. The improvement of symptoms of the two groups (fever, cough, phlegm and other symptoms disappeared time, hospital stay), respiratory function [Blood oxygen partial pressure (PaO₂), carbon dioxide partial pressure (PaCO₂), percentage of inhaled oxygen concentration (FiO₂), blood oxygen saturation (SpO₂), Oxygenation index (OI)], developmental status (CDCC score),emotional and neurobehavioral conditions [Neonatal Pain Score (NIPS) score, neonatal Psych behavioral Assessment Scale (NBNA) score] were compared between the two groups.【Results】 The duration of symptom disappearance (fever, cough, phlegm) and hospitalization in the observation group were shorter than those in the control group, the difference was statistically significant (P<0.05). After intervention, the proportion of SpO₂, PaO₂ and OI > 400 in the two groups was higher than before intervention; the proportion of SpO₂, PaO₂ and OI > 400 in the observation group was higher than that in the control group; the PaCO₂ in the two groups was lower than before intervention, and the PaCO₂ in the observation group was lower than that in the control group, with statistical significance (P<0.05). After intervention, MDI score and PDI score of the two groups were higher than before intervention, and the observation group was higher than the control group, the difference was statistically significant (P<0.05). After intervention, the NIPS score of the two groups was lower than before intervention, the NIPS score of the observation group was lower than that of the control group, and the NBNA score of the two groups was higher than that before intervention, and the NBNA score was higher than that of the control group, with statistical significance (P<0.05). 【Conclusion】Changing position nursing combined with touch therapy can effectively shorten the time of symptom disappearance and hospital stay of neonatal pneumonia children, enhance respiratory function and improve their emotional condition.
