Abstract:【Objective】 To explore the correlation between thyroid hormone level and severity of type 2 diabetes mellitus(T2DM) complicated with acute cerebral infarction(ACI). 【Methods】 A total of 80 patients with T2DM complicated with ACI admitted to our hospital from January 2019 to January 2022 were selected as group A, and 80 patients with simple T2DM admitted to our hospital during the same period were selected as group B. After admission, blood glucose indexes [fasting blood glucose(FPG), mean daily blood glucose fluctuation(MAGE), standard deviation of blood glucose level(SDBG)] and thyroid hormone [triiodothyronine(T3), thyroxine(T4), thyroid stimulating hormone(TSH)] levels were detected and compared between the two groups. The National Institutes of Health Stroke Scale(NIHSS) was used to evaluate the degree of neurological deficit in group A patients. The patients were divided into mild group(n=27, NIHSS score ≤8), moderate group(n=32, NIHSS score 9-14), and severe group(n=21, NIHSS score ≥15). The differences of blood glucose and thyroid hormone in T2DM patients with ACI were compared, and the correlation between blood glucose, thyroid hormone and the severity of T2DM patients with ACI was analyzed. Three months after treatment, the improved Rankin scale was used to evaluate the neurological recovery of patients in group A, and they were divided into good prognosis group(n=50, grade 0-1 for good prognosis) and poor prognosis group(n=30, grade 2-6 for poor prognosis). The differences of blood glucose and thyroid stimulating hormone related indicators in patients with different prognosis in group A were compared, and the influence of the above indicators on the prognosis of T2DM patients with ACI was analyzed. 【Results】The levels of FPG, MAGE, SDBG and TSH in group A were significantly higher than those in group B(P<0.05), and the level of T3 in group A was significantly lower than that in group B(P<0.05). There was no significant difference in the level of T4 between the two groups(P>0.05). The levels of FPG, MAGE, SDBG and TSH in the mild group, the moderate group and the severe group increased in turn(P<0.05), and the level of T3 decreased in turn(P<0.05). There was no significant difference in T4 levels among the three groups(P>0.05). FPG, MAGE, SDBG, TSH were positively correlated with the severity of the disease(P<0.05), T3 was negatively correlated with the severity of the disease(P<0.05). The levels of FPG, MAGE, SDBG and TSH in the good prognosis group were significantly lower than those in the poor prognosis group, and the level of T3 in the good prognosis group was significantly higher than that in the poor prognosis group(P<0.05). There was no significant difference in the level of T4 between the two groups(P>0.05). 【Conclusion】The more severe the condition of T2DM patients with ACI, the higher the levels of FPG, MAGE, SDBG, TSH, and the lower the level of T3.
