血清25羟基维生素D3联合骨代谢指标检测在老年骨质疏松症筛查中的应用价值

摘要 【目的】探讨血清25羟基维生素D3[25-(OH)D3]联合骨代谢指标检测在老年骨质疏松症筛查中的应用价值。【方法】选取2020年7月至2022年7月本院收治的96例老年骨质疏松症患者作为观察组,另外选取同期在本院体检的96例健康老年人作为对照组。比较两组腰椎骨密度、股骨颈骨密度及血清25-(OH)D3、骨代谢指标[骨钙素N段中分子片段(N-MID)、1型前胶原氨基端延长肽(P1NP)和Ⅰ型胶原羧基端肽β特殊系列(β-CTX)]、炎症因子[肿瘤坏死因子(TNF-α)和白介素6(IL-6)]水平。【结果】观察组的腰椎骨密度、股骨颈骨密度及血清25-(OH)D3水平均显著低于对照组,N-MID、P1NP、β-CTX、TNF-α、IL-6水平高于对照组,差异均有统计学意义(P<0.05)。【结论】血清25-(OH)D3联合骨代谢指标检测应用在老年骨质疏松症筛查中,可显著提高筛查效果,有助于评估患者的病情和生活质量,具有重要的临床应用价值。

Abstract:【Objective】To explore the application value of serum 25-hydroxyvitamin D3 combined with bone metabolism index in the screening of elderly osteoporosis. 【Methods】A total of 96 elderly osteoporosis patients admitted to our hospital from July 2020 to July 2022 were selected as the observation group,and 96 healthy elderly people who underwent physical examination during the same period were selected as the control group. Patients in both groups underwent lumbar spine bone density,femoral neck bone density measurements,as well as measurements of serum 25-hydroxyvitamin D3 [25-(OH)D3],bone metabolism indices (N-MID,P1NP,β-CTX),and inflammatory factors (TNF-α,IL-6). The test results of the two groups were compared. 【Results】The lumbar spine bone density and femoral neck bone density of the observation group were significantly lower than those of the control group,while levels of N-MID,P1NP and β-CTX in the observation group were higher than those in the control group. The serum 25-(OH)D3 level in the observation group was lower than that in the control group,and levels of inflammatory factors TNF-α and IL-6 in the observation group were significantly higher than those in the control group. The differences were statistically significant(P<0.05). 【Conclusion】The detection of serum 25-(OH)D3 combined with bone metabolism index in the screening of elderly osteoporosis can significantly improve the screening effect and early detection rate,help to assess the patient's condition and quality of life,and has important clinical application value.
