妊娠期高血压疾病对单双胎妊娠孕妇围产儿、分娩结局及妊娠期并发症的影响

摘要【目的】探讨妊娠期高血压疾病对单双胎妊娠孕妇围产儿、分娩结局及妊娠期并发症的影响。【方法】选取本院收治的50例合并妊娠期高血压疾病的双胎孕妇(双胎高血压组)、50例合并妊娠期高血压疾病的单胎孕妇(单胎高血压组),另选取同期在本院产检的50例健康孕妇为对照组,比较三组妊娠并发症(贫血、甲状腺功能减退、重度水肿、合并糖尿病、低蛋白血症)发生率、分娩结局(早产、足月产、经阴道分娩、剖宫产、严重产后出血)、围产儿结局(低出生体重儿、胎儿窘迫、新生儿窒息、围生儿死亡、低蛋白血症)。【结果】双胎高血压组贫血发生率、妊娠并发症总发生率、低出生体重儿、早产、剖宫产、严重产后出血发生率显著高于单胎高血压组和对照组,且单胎高血压组高于对照组,差异均有统计学意义(P<0.05);双胎高血压组足月产、经阴道分娩发生率显著低于单胎高血压组和对照组,且单胎高血压组低于对照组,差异有统计学意义(P<0.05)。【结论】双胎及单胎妊娠孕妇合并妊娠期高血压疾病对分娩结局、围产儿结局均有负面影响,增加妊娠并发症发生率,且双胎妊娠孕妇危险性更高。

Abstract：【Objective】 To explore the impact of gestational hypertension on the perinatal outcomes,delivery outcomes,and pregnancy complications of single and twin pregnancy women. 【Methods】 A total of 50 twin pregnant women with hypertensive disorder complicating pregnancy (twin hypertension group),50 singleton pregnant women with hypertensive disorder complicating pregnancy (singleton hypertension group),and 50 healthy pregnant women at the same time in our hospital were selected as the control group. The incidence of complications of pregnancy (anemia,hypothyroidism,severe edema,diabetes,Hypoproteinemia) in three groups were selected The delivery outcome (premature delivery,full term delivery,vaginal delivery,caesarean section,severe Postpartum bleeding) and perinatal outcome (Low birth weight,fetal distress,neonatal asphyxia,perinatal death,hypoproteinemia) were compared.【Results】 The incidence of anemia,the total incidence of complications of pregnancy,low birth weight,premature delivery,caesarean section,and severe postpartum bleeding in the twin hypertension group were significantly higher than those in the singleton hypertension group and the control group,and the singleton hypertension group was higher than the control group,with statistical significance (P<0.05); The incidence of full-term delivery and Vaginal delivery in the twin hypertension group was significantly lower than that in the singleton hypertension group and the control group,and the singleton hypertension group was lower than that in the control group,with a statistically significant difference (P<0.05). 【Conclusion】 Twin pregnancy and singleton pregnancy with hypertensive disorder complicating pregnancy have a negative impact on the delivery outcome and perinatal outcome,increase the incidence of Complications of pregnancy,and twin pregnancy has a higher risk.
