Abstract:【Objective】To explore the efficacy of Captopril with different doses in the treatment of adult viral myocarditis.【Methods】A total of 106 patients with viral myocarditis treated in our hospital were divided into the low-dose group (n= 50,25 mg) and the high-dose group (n= 56,50 mg) according to different doses of captopril on the basis of conventional treatment.The clinical efficacy and the levels of serum troponin T (cTnT),serum troponin I (cTnI),myocardial enzyme profile [α-hydroxybutyrate dehydrogenase (α-HBDH),lactate dehydrogenase (LDH),creatine kinase isoenzyme (CK-MB),aspartic aminotransferase (AST)] before and after treatment were compared between the two groups.The changes of cardiac function [left ventricular ejection fraction (LVEF),cardiac output per stroke (SV),ventricular short axis shortening rate (FS)] and the incidence of adverse reactions were compared between the two groups before and after treatment as well.【Results】The total effective rate in the high-dose captopril group was higher than that in the low-dose group (P<0.05).The levels of cTnT and cTnI in both groups were lower after treatment than before (P<0.05).The cTnT level in the high-dose group was lower than that in the low-dose group (P<0.05),while there was no significant difference in the cTnI level between the high-dose group and the low-dose group (P>0.05).After treatment,the levels of α-HBDH,LDH,CK-MB and AST in both groups were decreased compared to those before treatment (P<0.05).After treatment,the levels of α-HBDH,LDH,CK-MB and AST in the high-dose group were lower than those in the low-dose group (P<0.05).After treatment,the levels of LVEF,SV and FS in both groups were higher than those before treatment (P<0.05).The levels of LVEF,SV and FS in the high-dose group were higher than those in the low-dose group (P<0.05).The incidence of adverse reactions in the high-dose group was higher than that in the low-dose group (P<0.05).【Conclusion】Compared to captopril low-dose medication,high-dose captopril is more effective in the treatment of adult viral myocarditis,however,it increases the risk of adverse reactions.
