Abstract:【Objective】To explore the effect of cranioplasty timing on cerebral hemodynamics and cognitive dysfunction in patients with skull defects after decompressive craniectomy. 【Methods】The clinical data of 82 patients with skull defect after decompressive craniectomy in our hospital were prospectively selected, and they were divided into observation group (n=40) and control group (n=42) according to different skull repair timings. The two groups were further divided into mild group, moderate group and severe group according to the degree of cognitive impairment. The scores of the Mini-Mental State Scale (MMSE) before and after surgery were compared between the two groups of patients with different cognitive functions, and the cerebral blood flow before and after surgery in each group was compared as well.【Results】3 months after operation, the local blood flow of ACA, MCA and PCA in patients with mild, moderate and severe cognitive impairment in the two groups were all higher than those 2 days before operation, and the observation group was higher than the control group, and the difference was statistically significant (P<0.05); the MMSE scores of the patients with mild and moderate cognitive impairment in the two groups were higher than 2 days before surgery, and the difference was statistically significant (P<0.05); the MMSE scores of the patients with mild and moderate cognitive impairment in the observation group were higher than In the control group, the difference was statistically significant (P<0.05); the MMSE scores of the patients with severe cognitive impairment in the two groups were not significantly improved compared with those before operation, and the difference was not statistically significant (P>0.05).【Conclusion】Early cranioplasty can significantly improve local cerebral blood flow in the patients with skull defect, thus significantly promote the recovery of cognitive function in patients with mild to moderate cognitive impairment. However, for the patients with severe cognitive impairment, early cranioplasty and late cranioplasty have no obvious effect on the improvement of cognitive function.
