Relationship between the Expression of Serum Hep, TRF, SF, Fe and Left Ventricular Structure and Vascular Endothelial Function in Elderly Patients with Chronic Heart Failure Complicated with Iron Deficiency

Abstract:【Objective】 To investigate the correlation between serum ferritin (HEP), transferrin (TRF), ferritin (SF), iron (Fe) and left ventricular structure and vascular endothelial function in elderly patients with chronic heart failure (CHF) complicated with iron deficiency. 【Methods】 A total of 92 elderly patients with CHF complicated with iron deficiency treated in Xi'an International Medical Center Hospital from January 2019 to January 2021 were divided into MACE group (n=25) and no MACE group (n=67) according to whether MACE occurred after 3 months of treatment. The levels of serum Hep, TRF, SF and Fe, left ventricular structural indexes [left atrial diameter (LAD), left ventricular end diastolic diameter (LVEDd), left ventricular end systolic diameter (LVESD), ventricular septal thickness (IVST), left ventricular mass index (LVMI)] and vascular endothelial function indexes [nitric oxide (NO), endothelin-1 (ET-1)] were compared between the two groups.The correlation between serum Hep, TRF, SF and Fe levels and left ventricular structure and vascular endothelial function was analyzed, and the predictive value of serum Hep, TRF, SF and Fe levels to MACE was evaluated.【Results】The levels of serum Hep in the MACE group were higher than those in non MACE group, and the levels of serum TRF, SF and Fe were lower than those in the non MACE group (P<0.05); Serum Hep levels were positively correlated with LAD, LVEDD, LVESD,IVST , LVMI and ET-1 and negatively correlated with No. Serum TRF, SF and Fe levels were negatively correlated with LAD, LVEDd, LVESD, IVST, LVMI and ET-1 and positively correlated with NO (P<0.05).The area under the curve (AUC) of serum Hep, TRF, SF and Fe in predicting MACE in elderly patients with CHF complicated with iron deficiency was the largest, which was 0.957.【Conclusion】The levels of serum Hep, TRF, SF and Fe are significantly correlated with left ventricular structure and vascular endothelial function in elderly patients with CHF complicated with iron deficiency. The combined detection of the four is of great value in predicting the occurrence of MACE.
