Abstract:【Objective】 To explore the effects of smoking cessation cognitive behavioral therapy (CBT) combined with repetitive transcranial magnetic stimulation (rTMS) on clinical symptoms and nicotine dependence in schizophrenia patients with tobacco dependence. 【Methods】 A total of 100 schizophrenia patients complicated with tobacco dependence admitted to the psychiatry department of our hospital from January 2023 to December 2024 were enrolled and divided into control group and observation group by random number table method, with 50 cases in each group. The control group received rTMS combined with original antipsychotic drugs; the observation group was additionally given smoking cessation CBT on the basis of the regimen for the control group. The clinical symptoms [Positive and Negative Syndrome Scale (PANSS) score], nicotine dependence level [Fagerstrom Test for Nicotine Dependence (FTND) score], smoking craving [Brief Questionnaire of Smoking Urges (QSU-Brief)], blood carboxyhemoglobin (COHb) and urinary cotinine levels before and after intervention were compared between the two groups, and adverse events were recorded. 【Results】 After intervention, the scores of positive symptoms, negative symptoms, and general psychopathology in both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). The FTND and QSU-Brief scores in both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). The levels of COHb and urinary cotinine in both groups were lower than those before intervention, and the observation group was lower than the control group, with statistically significant differences (P<0.05). There was no statistically significant difference in the incidence of adverse reactions between the two groups (χ2=0.045, P>0.05). 【Conclusion】 On the basis of conventional antipsychotic treatment, smoking cessation CBT combined with rTMS can remarkably alleviate tobacco dependence and smoking craving, improve psychiatric symptoms in schizophrenia patients with tobacco dependence with satisfactory safety, which possesses promising clinical application value.
