Abstract:【Objective】To explore the influencing factors of coronary microcirculation resistance index (IMR) in acute myocardial infarction (AMI). 【Methods】A total of 67 AMI patients admitted to our hospital from March 2019 to June 2021 were selected in the study and they were divided into a normal IMR group (the control group,n=29, IMR≤25) and an observation group (n=38, IMR>25) using a binary classification method with a normal IMR value of 25 as the cutoff. Clinical data of the two groups of patients were compared, using Pearson correlation analysis to analyze the relationship between IMR and various clinical factors and multivariate logistic regression curve to analyze the correlation between various clinical indicators and high-level IMR. 【Results】The results showed that the age, LDL-C (low-density lipoprotein cholesterol), cTnI (cardiac troponin I), CK-MB (creatine kinase isoenzyme), and lesion vessel length of the observation group were higher than those of the control group, while the postoperative TIMI blood flow grade and LVEF (left ventricular ejection fraction) were lower than those of the control group, with statistically significant differences (P<0.05). Pearson correlation analysis showed that IMR was positively correlated with cTnI and age (P<0.05), but negatively correlated with postoperative TIMI blood flow grading and LVEF (P<0.05). There was no significant correlation between IMR and lesion vessel length, and no correlation was seen between IMR and CK-MB (P>0.05). Multivariate logistic regression analysis showed that cTnI, postoperative TIMI blood flow grade, and LVEF were the influencing factors of IMR, with odds ratios(OR)(95%CI) of 2.01 (1.02-3.88), 1.03 (0.87-1.95), and 0.36 (0.07-0.93), respectively, with statistical significance (P<0.05). 【Conclusion】The cTnI postoperative TIMI blood flow grading and LVEF are the influencing factor.
