吲哚菁绿荧光染色联合电生理监测在脑功能区胶质细胞瘤手术中的应用价值

摘要 【目的】探讨吲哚菁绿荧光染色联合电生理监测在脑功能区胶质细胞瘤手术中的应用价值。【方法】选取2012年4月至2018年3月本院收治的105例脑功能区胶质细胞瘤患者的临床资料,按照监测方法不同将患者分为观察组(n=51)和对照组(n=54),观察组患者采用吲哚菁绿荧光染色联合电生理监测引导脑胶质细胞瘤手术,对照组患者仅采用电生理监测下常规开颅手术治疗。观察两组患者神经电生理监测情况、手术切除肿瘤情况及患者术后恢复情况、神经功能改善情况。【结果】神经电生理监测发现患者肿瘤位主要于语言区、肢体运动区及感觉区。观察组肿瘤全切除程度高于对照组,差异有统计学意义(χ2=8.068,P=0.005<0.05)。观察组术后肌力恢复情况及KPS评分明显优于对照组(P<005)。观察组患者术后运动区及感觉区神经功能恢复情况明显优于对照组(P<0.05)。【结论】吲哚菁绿荧光染色联合电生理监测在脑功能区胶质细胞瘤手术中能够明确胶质细胞瘤内部的关系,有助于保护神经功能,提高肿瘤切除程度。

Abstract【Objective】To investigate the application of phthalocyanine green fluorescence staining combined with electrophysiological monitoring in the treatment of glioma in brain functional area. 【Methods】A total of 105 patients with brain glandular gliomas admitted to our hospital from April 2012 to March 2018 were enrolled. Patients were divided into observation group (n=51) and control group (n=54) according to different detection methods.The patients in the observation group were treated with phthalocyanine green fluorescence staining combined with electrophysiological monitoring to guide glioma surgery. The patients in the control group were only treated with conventional craniotomy under electrophysiological monitoring. Observed the neurophysiological monitoring of the two groups, the surgical removal of the tumor and the recovery of the patient, the improvement of neurological function. The accuracy of the phthalocyanoma boundary was evaluated by analyzing the fluorescence of the indocyanine green. 【Results】Neuroelectrophysiological examination revealed that the tumor location of the patients was mainly in the linguistic area, limb movement area and sensory area. The total tumor resection rate was significantly higher in the observation group than in the control group (P<0.05). The recovery of muscle strength and KPS score in the observation group were significantly better than those in the control group (P<005). The recovery of neurological function in the exercise and sensory areas of the observation group was significantly better than that of the control group (P<0.05). The sensitivity of the observation group using phthalocyanine green fluorescent staining to evaluate the glioma cell border was 92.15% and the specificity was 90.19%.【Conclusion】The use of indocyanine green fluorescence staining combined with electrophysiological monitoring technology in patients with glioma of brain function area can guide the surgical removal of tumors, which can accurately identify tumor lesions and maximize the protection of patients' nerve function.
