Abstract:【Objective】 To investigate the clinical efficacy of neoadjuvant chemotherapy combined with surgery in patients with resectable and borderline resectable pancreatic cancer. 【Methods】 A total of 70 patients with pancreatic cancer were enrolled in this study. Thirty-five patients who underwent surgery alone were assigned to the control group, while 35 patients who received neoadjuvant chemotherapy followed by surgery were assigned to the observation group. Perioperative outcomes and survival rates at 1 and 3 years postoperatively were compared between the two groups. 【Results】 There were no statistically significant differences between the two groups in terms of perioperative indicators including tumor diameter, surgical procedure, operative time, postoperative hospital stay, and postoperative complications (P>0.05). The 1-year and 3-year postoperative survival rates in the observation group were significantly higher than those in the control group, and the overall survival time in the observation group was significantly longer than that in the control group (P<0.05). In addition, the overall metastasis rate and local recurrence rate after surgery in the observation group were significantly lower than those in the control group (P<0.05). 【Conclusion】 Neoadjuvant chemotherapy combined with surgery is safe and feasible for patients with resectable and borderline resectable pancreatic cancer and does not increase perioperative risks. Moreover, it can significantly improve postoperative survival rates, prolong survival time, and effectively reduce local recurrence and distant metastasis rates, demonstrating substantial clinical value.
