miR-200b-5p、ATAD2在宫颈癌组织中的表达水平及其与患者预后的关系

摘要 【目的】探讨miR-200b-5p、三磷酸腺苷酶家族蛋白2(ATAD2)在宫颈癌组织中的表达水平及其与患者预后的关系。【方法】收集2019年1月至2020年3月在本院行手术治疗且经病理确诊的124例宫颈癌组织和癌旁组织(距癌组织≥5 cm)标本,检测并比较癌组织、癌旁组织中的miR-200b-5p mRNA、ATAD2 mRNA的表达水平;随访18个月,统计宫颈癌患者预后情况,并依据预后情况分为死亡组和生存组,比较两组患者临床资料;采用Logistic多因素回归分析影响宫颈癌患者预后的危险因素;制作受试者工作特征(ROC)曲线,以曲线下面积(AUC)分析宫颈癌组织miR-200b-5p、ATAD2表达对宫颈癌患者预后的预测效能。【结果】宫颈癌组织中miR-200b-5p mRNA水平低于癌旁组织(P<0.05),ATAD2 mRNA水平高于癌旁组织(P<0.05)。随访18个月,宫颈癌患者死亡发生率为17.74%。死亡组临床分期为Ⅲ/Ⅳ期占比、淋巴结转移、低分化程度占比、肌层浸润深度>1/2占比及宫颈癌组织ATAD2 mRNA水平高于生存组(P<0.05),宫颈癌组织miR-200b-5p水平低于生存组(P<0.05)。Logistic回归分析结果显示:临床分期为Ⅲ/Ⅳ期、低分化程度及宫颈癌组织中低miR-200b-5p mRNA水平、高ATAD2 mRNA水平均是影响宫颈癌患者预后的危险因素(OR=2.835、2.793、3.080、3.364,P<0.05)。ROC分析结果显示:宫颈癌组织中miR-200b-5p mRNA、ATAD2 mRNA水平预测宫颈癌患者死亡的AUC分别为0.793、0.860。【结论】miR-200b-5p mRNA在宫颈癌组织中呈低表达,ATAD2 mRNA则呈高表达;宫颈癌组织miR-200b-5p mRNA、ATAD2 mRNA对宫颈癌患者死亡的预测效能较高,可作为评估该类患者预后的重要参考指标。

Abstract:【Objective】To explore the expression of miR-200b-5p and Adenosine triphosphate family protein 2 (ATAD2) in cervical cancer tissues and their relationship with prognosis. 【Methods】A total of 124 patients with cervical cancer who underwent surgical treatment in our hospital from January 2019 to March 2020 were selected in the study. All cervical cancer tissue specimens and para-cancerous tissues (≥5 cm from the cancerous tissue) were collected. The expression levels of miR-200b-5p mRNA and ATAD2 mRNA in cancer tissues and adjacent tissues were compared. After 18 months follow-up, the prognosis of cervical cancer patients was counted and divided into the death group and the survival group according to the prognosis. Compare the clinical data of patients in the death group and survival group. Logistic multivariate regression was used to analyze the risk factors affecting the prognosis of cervical cancer patients. A receiver operating characteristic curve (ROC) was made, and the area under the curve (AUC) was used to analyze the predictive power of the expression of miR-200b-5p and ATAD2 in cervical cancer tissues on the prognosis of cervical cancer patients. 【Results】The miR-200b-5p mRNA in cervical cancer tissue was lower than that in adjacent tissues (P<0.05), while ATAD2 mRNA was higher than that in adjacent tissues (P<0.05). After 18 months of follow-up, the death rate of cervical cancer patients was 17.74%. In the death group, the proportion of stage Ⅲ/Ⅳ, the proportion of lymph node metastasis, the proportion of poorly differentiated degree, the proportion of muscle invasion depth>1/2, the proportion of ATAD2 mRNA in cervical cancer tissue was higher than those in the survival group (P<0.05), while cervical cancer tissue miR-200b-5p level was lower than in the survival group (P<0.05). Logistic regression analysis showed that the clinical stage Ⅲ/Ⅳ, poorly differentiated, low expression level of miR-200b-5p mRNA and high level ATAD2 mRNA in cervical cancer tissues were all risk factors affecting the prognosis of cervical cancer patients (OR=2.835, 2.793 , 3.080, 3.364, respectively, P<0.05). ROC analysis showed that the AUCs of miR-200b-5p mRNA and ATAD2 mRNA in cervical cancer tissues to predict the prognosis of cervical cancer death were 0.793 and 0.860, respectively. 【Conclusion】The level of miR-200b-5p mRNA is low in cervical cancer tissues, while ATAD2 mRNA level is high. Both miR-200b-5p mRNA and ATAD2 mRNA in cervical cancer tissue have a high predictive power for the prognosis of cervical cancer patients. Thus they can be used as important reference indicators for evaluating the cervical cancer prognosis.
