Abstract:【Objective】To analyze the effect and safety of endoscopic retrograde appendicitis therapy (ERAT) on the levels of procalcitonin (PCT) and soluble intercellular adhesion molecule-1 (sICAM-1) in patients with appendicitis. 【Methods】The clinical data of 128 patients with appendicitis admitted to our hospital from October 2016 to March 2018 were analyzed retrospectively. According to the different operation methods, the patients were divided into control group (treated with laparoscopic appendectomy) and observation group (treated with ERAT operation), 64 cases in each group. The changes of PCT, sICAM-1, lipoxygenase A4 (LXA4) and soluble P-selectin (sP-s) in the two groups were observed, and the operation time, bed time, time of leukocyte recovery, hospitalization time and postoperative complications were compared.【Results】 The operation time, bed time, WBC recovery time and hospitalization time in the observation group were shorter than those in the control group, and the hospitalization cost was less than that in the control group, with a statistically significant difference (P<0.05). Before operation, there was no significant difference in PCT, sICAM-1, LXA4 and sP-s between the two groups (P>0.05). 72 hours after operation, the levels of serum PCT, sICAM-1, LXA4 and sP-s in the observation group were lower than those in the control group, the difference was statistically significant (P<0.05).There was 1 case of gastrointestinal hemorrhage in the observation group, 1 case of gastrointestinal hemorrhage, 1 case of abdominal abscess and 1 case of intestinal obstruction in the control group. The incidence of complications in the observation group was 1.56% (1/64) lower than that in the control group (4.69% (3/64). The difference was not statistically significant (χ2=1.032,P=0.310>0.05).【Conclusion】 ERAT treatment of appendicitis has the advantages of less trauma, less inflammatory reaction, quick postoperative recovery, less complications and high safety, which is worthy of clinical application.
