YAO Nian, XU Ran, HU Jiaji, et al
DOI: 10.3969/j.issn.1671-7171.2024.01.002
【Objective】 To explore the efficacy of different traditional Chinese medicine external treatment methods in treating opioid related constipation through network meta-analysis. 【Methods】 Computer retrieval of literature on the treatment of opioid related constipation in cancer patients using traditional Chinese medicine external treatment methods in databases such as China National Knowledge Infrastructure, Wanfang, China Biomedical Literature Database, VIP, PubMed, Embase, Web of Science, and The Cochrane Library. The retrieval deadline is from the establishment of the database until March 20, 2023. At the same time, trace the relevant system evaluations and include references in the research. Two researchers independently screened the literature and extracted data to evaluate the quality of the literature. Statistical analysis was conducted using the Gemtc package from RevMan 5.3 software, Stata 17.0 software, and R 4.2.3 software. 【Results】 A total of 1570 patients were included in 17 studies, involving 7 intervention measures. The results of the network meta-analysis showed that the efficacy of acupoint application+conventional treatment (OR=7.00, 95%CI=3.90-13.00, P<0.05), moxibustion+conventional treatment (OR=6.20, 95%CI=2.20-21.00, P<0.05), and abdominal massage+conventional treatment (OR=8.80, 95%CI=2.40-46.00, P<0.05) was superior to conventional treatment. The SUCRA value showed that the top three traditional Chinese medicine external treatment methods in terms of efficacy were abdominal massage+conventional treatment (80.36%), acupoint application + conventional treatment (74.54%), and Moxibustion+conventional treatment (68.76%) respectively.【Conclusion】 It is recommended to use abdominal massage combined with conventional treatment to alleviate opioid related constipation. However, in practical clinical work, multiple factors such as patient willingness and physical condition should be considered for decision-making.

ZHANG Lixia, DONG Jinli
DOI: 10.3969/j.issn.1671-7171.2024.01.005
【Objective】 To explore the correlation between serum levels of Galactin-3 (Gal-3), Visfatin, and Chemerin and recurrence after first catheter radiofrequency ablation in patients with atrial fibrillation.【Methods】 A total of 184 patients with atrial fibrillation admitted to our hospital from August 2019 to August 2021 were selected. All patients underwent radiofrequency ablation treatment after admission. They were divided into a recurrence group (n=53) and a non recurrence group (n=131) based on whether they recurred within 6 months after surgery. We compared two clinical data sets and serum levels of Chemerin, Gal-3, and Visfatin before and 1 month after surgery, and analyzed the correlation and predictive value of these indicators with postoperative recurrence of atrial fibrillation at 1 month after surgery.【Results】 The proportion of recurrent group, concomitant hypertension, coronary heart disease, and left atrial volume index (LAVI) were higher than those of the non recurrent group, and the left ventricular ejection fraction (LVEF) was higher than those of the non recurrent group, with statistical significance (P<0.05). The serum levels of Gal-3, Visfatin, and Chemerin in the recurrent group were higher than those in the non recurrent group before and 1 month after surgery, with statistical significance (P<0.05). Logistic regression analysis showed that serum Gal-3, Visfatin, and Chemerin levels at one month post surgery were independent risk factors for postoperative atrial fibrillation recurrence (P<0.05). The risk of postoperative atrial fibrillation recurrence in patients with high levels of Gal-3, Visfatin, and Chemerin was 4.076 times, 2.847 times, and 6.272 times higher than that in patients with low levels (P<0.05).【Conclusion】 Serum Gal-3, Visfatin, and Chemerin are closely related to postoperative recurrence in patients with atrial fibrillation after the first radiofrequency ablation, which can predict the risk of postoperative recurrence in patients early and help clinical physicians formulate targeted treatment plans to reduce recurrence rates.

DOI: 10.3969/j.issn.1671-7171.2024.01.022
【Objective】 To explore the expression and clinical significance of SOX2 and CARM1 in stage Ⅰ endometrial adenocarcinoma tissue.【Methods】 Pathological paraffin specimens of cancer tissue from patients with total hysterectomy for high and medium differentiated endometrioid adenocarcinoma, who admitted to the Department of Gynecology at the Affiliated Hospital of Inner Mongolia Medical University from January 2013 to December 2015, were enrolled in the observation group. While proliferative endometrial pathological paraffin specimens from patients who underwent total hysterectomy due to benign diseases such as uterine fibroids and adenomyosis were recruited in the control group. Immunohistochemical SP method was used to detect the expression levels of SOX2 protein and CARM1 protein in two groups and the results were compared between the two groups. 【Results】 The positive expression rates of both SOX2 protein and CARM1 protein in the observation group were significantly higher than those in the control group, with a statistically significant difference (P<0.05). The positive expression of SOX2 and CARM1 protein was related to histological differentiation in endometrioid adenocarcinoma (P<0.05). However, their expression was not related to patients' age, International Federation of Gynecology and Obstetrics (FIGO) and CA125 value (P> 0.05). There was a significant positive correlation between SOX2 and CARM1 protein expression in in endometrioid adenocarcinoma (P=0.010). 【Conclusion】 Highly expressed SOX2 protein and CARM1 protein may be involved in the pathogenesis of endometrioid adenocarcinoma. The two synergistically promote the occurrence and development of endometrioid adenocarcinoma. The higher expression level of SOX2 and CARM1 was found in moderately differentiated EAC compared to well differentiated ones, which may be novel factors that cause malignant behavior.

The control group received a small amount of blood transfusion intervention, while the observation group received a large amount of blood transfusion intervention, closely monitoring the changes in vital signs of patients after blood transfusion intervention. We compared the incidence and mortality rates of inflammation, coagulation function, blood gas, and multiple organ dysfunction syndrome (MODS) in two groups of patients before and three days after intervention. 【Results】 Three days after intervention, the levels of C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-α(TNF-α) in the two groups of patients were significantly higher than before intervention (P<0.05), and the levels of various indicators in the observation group were higher than those in the control group (P<0.05); Three days after intervention, the levels of prothrombin time (PT), activated partial thromboplastin time (APTT), and thrombin time (TT) in two groups of patients were significantly higher than before intervention, while the levels of fibrinogen (FIB) and platelet count (PLT) were significantly lower than before intervention (P<0.05). In addition, the levels of PT, APTT, and TT in the observation group were higher than those in the control group, while the levels of FIB and PLT were lower than those in the control group (P<0.05); Three days after intervention, the pH and arterial oxygen pressure (PaO2) levels of the two groups of patients were significantly higher than before intervention, and the arterial carbon dioxide pressure (PaCO2) levels were significantly lower than before intervention (P<0.05). In addition, the pH and PaO2 levels of the observation group were higher than those of the control group, and the PaCO2 levels were lower than those of the control group (P<0.05); The incidence and mortality rate of MODS in the observation group were significantly lower than those in the control group (P<0.05). 【Conclusion】 The application of extensive blood transfusion intervention in elderly patients with severe trauma can effectively supplement their blood volume, improve blood gas indicators, and reduce mortality, but it is prone to trigger inflammatory reactions and lead to coagulation dysfunction. Therefore, when applying blood transfusion intervention to elderly patients with severe trauma, it is necessary to reasonably control the transfusion dose and closely monitor relevant indicators such as coagulation function.

【Objective】 To explore the clinical efficacy of Wushe Rongpi decoction in maintenance hemodialysis (MHD) patients with skin itching and its impact on their immune function. 【Methods】 A total of 100 patients with MHD accompanied by skin itching admitted to our hospital were randomly divided into an observation group and a control group, with 50 cases in each group. The control group received routine clinical treatment, while the observation group received Wushe Rongpi decoction. The clinical efficacy and incidence of adverse reactions were compared between the two groups after treatment. The TCM symptom score, immune function indicators, and serum levels of interleukin-2 (IL-2), interleukin-6 (IL-6), and tumor necrosis factor-α(TNF-α) were compared before and after treatment. 【Results】 The total effective rate of patients in the observation group was 92.00%, which was higher than 78.00% in the control group (P<0.05); The scores of skin itching, dark red rash, and increased erythema in the observation group after treatment were lower than those before treatment and in the control group after treatment (P<0.05); After treatment, the levels of CD3+ and CD4+ in the observation group were significantly higher than those before treatment and the control group, while the levels of CD8+ were significantly lower than those before treatment and the control group (P<0.05); The levels of IL-2, IL-6, and TFN-α in the observation group after treatment were lower than those before treatment and in the control group (P<0.05); There was no statistically significant difference in the incidence of adverse reactions between the two groups (P>0.05).【Conclusion】 The use of Wushe Rongpi decoction in the treatment of MHD patients with skin itching has a significant therapeutic effect, can improve the patient's immune function, reduce serum IL-2, IL-6, TFN-α levels, and traditional Chinese medicine symptom scores, and has minimal adverse reactions.
