Effect of Icariin on Expression of miR-21-5p in Osteolysis Induced by Titanium Particles

【Objective】To investigate the effect of icariin(ICA) on the expression of miR-21-5p in osteolysis induced by titanium particles. 【Methods】Six-week-old male Balb/c mice were randomly divided into sham operation group(SOP group), the titanium particle group(TiP group) and the icariin group (ICA group).In the ICA Group, titanium particles were used to induce osteolysis in mice, and 300 mg/kg of icariin was given daily for 14 days by intragastric administration after the operation. Real-time PCR was used to detect the relative expression of miR-21-5p in mice serum before the operation and 3 days, 7 days, 14 days after administration of icariin. The relative expression of miR-21-5p in skull of mice was detected by Real-time PCR at 14 days after icariin administration. 【Results】There was no difference in the relative expression of serum miR-21-5p in each group before osteolysis-induced by titanium particles. Compared to the SOP Group, the relative expression of miR-21-5p in serum and skull from the TiP Group was significantly decreased after the operation (P<0.01). Compared to the TiP Group, the relative expression of miR-21-5p in serum and skull of the ICA group was significantly increased (P<0.01). 【Conclusion】Icariin can significantly promote the expression of miR-21-5p in serum and skull of mice after osteolysis induced by titanium particles. These results suggest that mir-21-5p can be used as a target for osteolysis therapy.

Clinical Effect of External-to-internal FasT-Fix Suture Combined with Fibro-hemagglutination Tamponade in the Repair of Refractory Meniscus Injury

【Objective】 To investigate the clinical effect of external-to-internal FasT-Fix suture combined with fibro-hemagglutination tamponade in the repair of refractory meniscus injury.【Methods】 Fifty-one cases of refractory meniscus injury in our hospital from March 2015 to March 2018 were retrospectively analyzed. Among them, 29 cases were treated by external-to-internal FasT-Fix suture combined with fibro-hemagglutination (observation group). 32 cases were sutured only by outward-to-inside FasT-Fix (control group). All patients were followed up for 11 months. IKDC score, Lysholm score and Tegner score were used to evaluate the clinical efficacy. 【Results】No complications such as joint interlock, knee joint movement disorder, vascular and nerve injury, wound infection and rejection occurred in all patients after operation. No obvious infection was found in the wounds of both groups, and no second operation was performed. The IKDC score, Lysholm score and Tegner score of the two groups were significantly higher than those of the pre-operation , and the IKDC score, Lysholm score and Tegner score of the observation group were significantly higher than those of the control group (P<0.05). 【Conclusion】The repair of refractory meniscus injury by external-to-internal FasT-Fix suture combined with fibro-hemagglutination technique is effective, and the functional recovery after operation is good. It is worth popularizing and applying in clinical practice.

Comparison of Effect, Recurrence, Metastasis and Adverse Reactions of TS and SOX in Postoperative Adjuvant Chemotherapy for Gastric Cancer

【Objective】To compare the effect of TS and SOX in postoperative adjuvant chemotherapy of gastric cancer. 【Methods】A total of 96 patients with postoperative adjuvant chemotherapy for gastric cancer were randomly divided into two groups. There were 49 patients in the observation group who were treated with SOX (Oxaliplatin + Teggio capsule), while 47 patients in the control group were treated with the TS regimen (paclitaxel liposome + tigeo capsule). The treatment course in both groups lasted for 5 cycles. The clinical efficacy, recurrence, and adverse reactions of the two groups were compared during the postoperative adjuvant chemotherapy with 3-year follow-up. 【Results】The total effective rate of the observation group was higher than that of the control group (59.2% vs 36.2%, P<0.05); The clinical benefit rate of the observation group was higher than that of the control group (83.7% vs 63.8%, P<0.05). The overall improvement rate of life quality in the observation group was higher than that of the control group (55.1% vs 31.9%, P<0.05). CEA and AFP of two groups showed no differences before and after treatment (P>0.05). The levels of CA199, CA125 and NSE in the observation group were significantly decreased after treatment ( P<0.05), while those tumor markers in the observation group were significantly lower than those in the control group ( P<0.05). The incidence rate of marrow suppression and liver function damage between the two groups did not show significant difference (P>0.05), while the incidence rate of nausea, vomiting, diarrhea and pain in the observation group were significantly lower than those in the control group (P<0.05). The RSF and OS in the observation group were significantly higher than those in the control group within 3 years (P<0.05); The peritoneal metastasis of the observation group was significantly lower than that of the control group (P<0.05). 【Conclusion】 Compared to TS chemotherapy, SOX chemotherapy has better clinical effects in adjuvant chemotherapy for gastric cancer with lower recurrence and metastasis rates and less incidence of adverse reactions.

Analysis of Sleep Quality and its Influencing Factors in Elderly Patients with Type 2 Diabetes Mellitus and Osteoporosis

【Objective】To analyze the sleep disorder and its influencing factors in elderly patients with type 2 diabetes mellitus (T2DM) and osteoporosis (OP). 【Methods】A total of 136 elderly patients with T2DM and OP from January 2016 to February 2018 were enrolled for the study as OP group. 150 elderly T2DM patients without OP were randomly selected as non-OP group and 100 healthy elderly during the same period were randomly selected as control group. The score of Pittsburgh Sleep Inventory Index (PSQI) were compared among three groups, and the relationship between clinical data and sleep disorder were analyzed in the three groups. 【Results】The scores and total scores of PSQI in the OP group and the NOP group were significantly higher than those in the CN group. The scores and total scores of subjective sleep disorder, sleep quality and sleep efficiency in the OP group were significantly higher than those in the NOP group (P<0.05). The incidence of sleep disorder in the OP group was 74.3% (101/136), significantly higher than that in the NOP group and the CN group (60.7%,35.0%) (P<0.05). In the OP group, the incidence of sleep disorders in patients with age ≤70 years, divorce or widowhood, duration of T2DM > 15 years, HbA1c > 9.0%, diabetic neuropathy, insulin use, pain sites (≥3), moderate and severe pain, muscle spasm, spinal deformity, severe OP and other patients increased significantly, while the incidence of sleep disorders in patients treated with anti-OP decreased significantly (P<0.05). Multivariate logistic regression analysis showed that the duration of T2DM>15 years, HbA1c > 9.0%, moderate to severe pain and severe OP were independent risk factors for sleep disorders (P<0.05). 【Conclusion】The sleep status of elderly patients with T2DM and OP is poor, about 3/4 of them have sleep disorders. The duration of T2DM>15 years, HbA1c>9.0%, moderate and severe pain and severe OP are independent risk factors for sleep disorders in elderly patients with T2DM and OP.

Effect of Continuous Positive Airway Pressure on Serum Insulin-like Growth Factor-1 and Amylin in Patients with Severe Obstructive Sleep Apnea Hypopnea Syndrome

【Objective】To investigate the effects of continuous positive airway pressure (CPAP) on serum insulin-like growth factor-1 (IGF-1) and amylin in adult patients with obstructive sleep apnea hypopnea syndrome (OSAHS). 【Methods】A total of 118 patients with OSAHS diagnosed by polysomnography (PSG) were selected in the observation group, which were further divided into heavy, medium and light subgroups according to apnea hypopnea index (AHI). In the same time period, 55 healthy people without OSAHS were enrolled in the control group. Serum IGF-1 and amylin levels were measured by ELISA. Among OSAHS patients, 30 severe cases were followed up for 3 months of CPAP treatment. After half a year, IGF-1 and amylin were retested.【Results】With the severity of OSAHS, the patient's serum IGF-1 gradually decreased, and the level of amylin gradually increased (F=37.732,18.744;P<0.01). However, there was no difference between the mild group and the control group (P>0.05). Levels of serum IGF-1 and amylin levels were not correlated with BMI and age(P>0.05). There was a negative correlation between amylin and nighttime minimum oxygen saturation (LSaO
2
) and a positive correlation with AHI(P<0.01). In addition, there was a positive correlation between serum IGF-1 and LSaO
2
, and negatively correlated with AHI (P<0.01). After 3 months of CPAP treatment, serum IGF-1 level in 36 severe OSAHS patients was higher than before treatment, and amylin level was lower than before treatment. The difference was statistically significant (
P
<0.05). After half a year of treatment, serum IGF-1 was 56.61±5.46 ng/mL, which showed significant decrease, and amylin was 26.87±2.84 ng/mL, which showed significant increase (
P
<0.01).【Conclusion】 OSAHS patients have a decrease in serum IGF-1 level and an increase in amylin level, which may be associated with concurrent insulin resistance. CPAP treatment can significantly increase serum IGF-1 and decrease amylin levels in half a year. It is speculated that monitoring serum IGF-1 and amylin levels can evaluate the efficacy.

Clinical and UCG Features of a Family with Left Ventricular Noncompaction

【Objective】 To analyze the pedigree and ultrasonic cardiogram (UCG) of a family with left ventricular noncompaction (LVNC) to provide more evidence for the diagnosis and treatment of LVNC. 【Methods】A family of LVNC was investigated, clinical manifestations and UCG were analyzed, and the pedigree of the family was drawn. The clinical manifestations and UCG characteristics of the family were observed and recorded. 【Results】Of the 15 LVNC families of 3 generations, 6 had clinical manifestations or UCG changes of LVNC. In this family, the main manifestation of adult patients was left ventricular dysfunction, while the minor patients had no clinical symptoms. UCG showed typical LVNC changes: abnormal large trabeculae in left ventricular regional myocardium, deep recesses between trabeculae, non-dense layer/dense layer of left ventricular myocardium at the end of systole (N/C) >2 , ejection fraction < 50%. Through the analysis of the family pedigree, it was found that both men and women in the family were susceptible to the disease. One of the parents of the patient must be a patient. There were patients in every generation of the family, suggesting that LVNC was inherited by autosomal dominant inheritance in the family.【Conclusion】The inheritance mode of LVNC is autosomal dominant inheritance. The main clinical manifestations of LVNC in adults are left ventricular insufficiency, and there are no clinical symptoms in minors. UCG plays an important role in the diagnosis of LVNC and can detect typical changes of LVNC.

Effect of Low Voltage and Low Contrast Agent Concentration on Imaging Quality of Head and Neck CTA

【Objective】To investigate the effect of low tube voltage combined with low concentration contrast agent on the imaging quality of head and neck CT angiography (CTA). 【Methods】The clinical data of 212 patients with head and neck CTA in our hospital were selected and divided into group A, group B, group C and group D according to random number table method, 53 cases in each group. Patients in the group A were given conventional tube voltage (120 kV), tube current (150 mAs) and contrast agent concentration (350 mg/ml of iodine). Patients in the group B were given tube voltage (120 kV), tube current (150 mAs) and contrast agent concentration (320 mg/ml of iodine). Patients in the group C were given tube voltage (100 kV), tube current (150 mAs) and contrast agent concentration (350 mg/ml of iodine). Patients in the group D were given tube voltage (100 kV), tube current (150 mAs) and contrast agent concentration (350 mg/ml of iodine). Tube current (150 mAs) and contrast medium concentration (320 mg/ml iodine). The image quality score, CT value of arterial segment, signal-to-noise ratio and radiation dose were compared among the four groups.【Results】There was no significant difference in image quality scores between group A, group B, group C and group D (Z=1.746,P=0.077>0.05). The CT values of aortic arch (AOAR), basilar artery (BA), left common carotid artery (CCA), right CCA, left internal carotid artery (ICA) and right ICA in the four groups were significantly different (P<0.05). The CT values of each artery segment in the group D were higher than those in the group A and the group B, lower than those the in group C (P<0.05). The signal-to-noise ratio of the group A and the group D was higher than that of the group B and the group C. The iodine intake of the group B and the group D was lower than that of the group A and the group C. The effective dose of the group C and the group D was lower than that of the group A and the group B. The difference was statistically significant (P<0.05).【Conclusion】Low tube voltage combined with low contrast agent concentration can effectively reduce radiation dose and iodine intake in patients undergoing head and neck CTA examination, and can ensure better image quality and meet the needs of clinical diagnosis.

Effect of Closed Negative Pressure Suction Combined with Microsurgery on Hand Function and TIMP-1 Expression in Patients with Scar Contracture after Hand Burn

【Objective】To investigate the effect of closed negative pressure suction (VSD) combined with microsurgery on hand function and TIMP-1 level of contracture tissue in patients with hand burn scar contracture. 【Methods】A total of 122 cases of hand burn scar contracture from November 2016 to November 2017 were randomly divided into two groups. The control group was treated with routine surgery, while the observation group was treated with VSD combined with microsurgery. The total activity of finger joint (TAM) value, ability of daily living (ADL) score and the in-situ expression of matrix metalloproteinase inhibitor-1 (TIMP-1) in the scar contracture tissue were compared between the two groups. 【Results】The TAM value and ADL score of the observation group were higher than those of the control group (P<0.05). The expression of TIMP-1 level in the observation group was lower than that of the control group. The difference was statistically significant (P<0.05). 【Conclusion】Closed negative pressure suction combined with microsurgery is effective in the treatment of hand burn scar contracture, which is worthy of clinical application.

Analysis of Clinicopathological Features of Different Subtypes of Papillary Thyroid Micro Carcinoma

【Objective】To explore the clinicopathological features of patients with different subtypes of papillary thyroid micro carcinoma (PTMC). 【Methods】A total of 367 patients with different subtypes of PTMC who were treated in our hospital from January 2010 to March 2018 were enrolled. The patients were observed for lesion size, calcification, extra-glandular microinfiltration, lymph node metastasis and Hashimoto's thyroiditis. Univariate and multivariate logistic regression analysis evaluated the influencing factors of lymph node metastasis in patients with PTMC. 【Results】Among all patients, there were 166 cases of columnar cell subtype PTMC, accounting for 45.2%; 152 cases of follicular subtype PTMC, accounting for 41.4%; 49 cases of other subtypes, containing 35 cases of intracapsular subtype, 8 cases of Warthin tumor-like subtype, 1 case of acidophilic subtype, 1 case of trabecular subtype and 4 cases of high cell subtype PTMC. Follicular subtype PTMC had the largest lesion diameter. Other subtypes of PTMC had the highest calcification rate and no extra-glandular infiltration. The lymph node metastasis rate of columnar cell subtype PTMC patients was significantly higher than that of follicular subtypes and other subtypes. Multivariate logistic regression analysis showed that males, younger than 50 years old, lesion calcification and extra-glandular infiltration were independent risk factors for PTMC lymph node metastasis. 【Conclusion】PTMC is mainly composed of columnar cell subtypes and follicular subtypes. The clinicopathological characteristics of PTMC in different subtypes are varied. Being male, age less than 50 years old, lesion calcification and extra-glandular infiltration are independent risk factors for PTMC lymph node metastasis.

Clinical Study of Lindholm Operation and Vulpius Operation on Chronic Achilles Tendon Rupture

【Objective】To investigate the clinical efficacy of Lindholm and Vulpius in the treatment of chronic Achilles tendon rupture.【Methods】The clinical data of 97 patients with chronic Achilles tendon rupture treated by surgery from February 2014 to December 2016 were retrospectively analyzed. According to the different surgical methods, they were divided into Lindholm group (n=50) and Vulpius group (n=47). The clinical efficacy and complications of the two groups were analyzed and summarized after operation. 【Results】There were 20 excellent cases, 26 good cases, 4 fair cases and 0 poor cases in the Lindholm group, the excellent and good rate was 92.0% (46/50); 20 excellent cases, 22 good cases, 2 fair cases, 1 poor case in the Vulpius group, the excellent and good rate was 89.4% (42/47); there was no significant difference between the two groups (χ
2
=0.2,P=0.646>0.05). There was no re-rupture of Achilles tendon and skin edge necrosis in both groups. In the Lindholm group, incision infection occurred in 3 cases and sural nerve injury occurred in 1 case. The incidence of complications was 8.0%. There were 4 cases of incision infection in the Vulpius group, and the incidence of complications was 8.5%. There was no significant difference in the incidence of complications between the two groups (χ

=0.008,P=0.929>0.05).【Conclusion】Lindholm operation or Vulpius operation is effective in treating chronic Achilles tendon rupture, and the incidence of complications is low. It can be selected according to the patient's own situation.

Clinical Observation of Combined Thoracoscopic and Laparoscopic Surgery for Esophageal Cancer

【Objective】To investigate the clinical effect of combined thoracoscopy and laparoscopy in the treatment of esophageal cancer.【Methods】135 cases of esophageal cancer treated in our hospital from July 2014 to July 2016 were divided into observation group and control group according to the different surgical methods. 68 patients in the observation group underwent thoracoscopic and laparoscopic resection of esophageal cancer, while 67 patients in the control group underwent traditional thoracotomy. The operation-related indexes, the incidence of complications 15 days after operation and the changes of immune function indexes before operation, 1 day after operation and 6 days after operation were compared between the two groups.【Results】There was no significant difference in operation time and tracheal extubation time between the two groups (P>0.05). The bleeding volume, hospital stay and fasting time in the observation group were significantly lower than those in the control group (P<0.05). The incidence of complications in the observation group was significantly lower than that in the control group 15 days after operation (P<0.05). Before operation, there was no significant difference in immune function between the two groups (P>0.05). On the 1st day after operation, the indexes of the two groups decreased, and on the 6th day after operation, the indexes of the observation group basically recovered to the pre-operation level, but the indexes of the control group were still lower than those of the pre-operation. At the same time, the indexes of the observation group were higher than those of the control group, and the difference was statistically significant (P<0.05).【Conclusion】The combined thoracoscopic and laparoscopic surgery for esophageal cancer has remarkable clinical effect, safety and reliability, effectively reduces the occurrence of complications, has little impact on the immune function of patients, and is conducive to the recovery after operation.

Analysis of Related Factors of Nosocomial Infection in Patients after Craniotomy at High Altitude

【Objective】To analyze the related factors of nosocomial infection after craniotomy in patients living at high altitude. 【Methods】The clinical data of 227 patients undergoing craniotomy due to trauma, tumors and other intracranial diseases from July 2010 to July 2018 were retrospectively analyzed. The incidence of nosocomial infection, types of infection and distribution of pathogenic bacteria were analyzed. The general clinical data of patients were compared. Logistic multivariate regression analysis was used to analyze the related factors affecting nosocomial infection of patients. The clinical preventive measures were summarized.【Results】Of the 227 patients, 59 had nosocomial infection after craniotomy, the incidence was 25.99%. The main site of infection was in the lungs (45.76%), followed by intracranial infection (30.51%). A total of 77 strains of pathogenic bacteria were detected in 59 patients. Acinetobacter baumannii was the main pathogen (44.16%), followed by Staphylococcus epidermidis (19.48%). Logistic multivariate regression analysis showed that age > 50 years old, operation time (≥4 hours), open injury and extubation time (≥3 hours) were independent risk factors for nosocomial infection after craniotomy in plateau patients (P<0.05).【Conclusion】The incidence of nosocomial infection after craniotomy in patients in plateau area is high and the influencing factors are complex. The key to prevent nosocomial infection and improve the safety of neurosurgery is to pay attention to the evaluation of high-risk patients, implement debridement, aseptic operation, perioperative management and targeted nursing.

Effect of Qing Re Xiao Du Soup Combined with Phentolamine on Immune Inflammation and Hemodynamics in Patients with Severe Sepsis

【Objective】To explore the effect of Qing Re Xiao Du Soup (dissolving decoction) combined with Phentolamine on immune inflammation and hemodynamics in patients with severe sepsis. 【Methods】A total of 96 patients with severe sepsis were random divided into two groups with 48 cases in each group. Patients in the control group were treated with Phentolamine injections were administered on the basis of routine treatment, while patients in the observation group were treated with Qing Re Xiao Du Soup on the basis of phentolamine injections. The clinical efficacy, immune inflammation index, hemodynamic index and adverse reactions of the two groups were recorded and compared. 【Results】The total effective rate was 97.9% (47/48) in the observation group, which was higher than 85.4% (41/48) in the control group (P<0.05). After treatment, levels of CD4

, IL- 6, CRP and PCT in the observation group were improved significantly better than those in the control group (P <0.01). After treatment, the cardiac index (CI), extravascular lung water index (EVLWI), systemic vascular resistance index (SVRI), and intrathoracic blood volume index (ITBVI) were significantly improved in both groups (P<0.05). The improvement of each index in the observation group was significantly better than that in the control group (P<0.05). There were no adverse reactions during the treatment period.【Conclusion】 Qing Re Xiao Du Soup combined with phentolamine is effective in the treatment of severe sepsis and can obviously improve the immune inflammatory reaction and hemodynamic index of the patients.

【Objective】To investigate the efficacy and safety of amphotericin B inhalation in the treatment of HIV/AIDS patients with oral candidiasis. 【Methods】A total of 55 patients with HIV/AIDS and oral candidiasis admitted to Beijing You'an Hospital from June 2017 to June 2018 were enrolled. The amphotericin oxygen inhalation was used to observe the efficacy and adverse reactions. 【Results】The total effective rate of amphotericin B inhalation for treatment of HIV/AIDS with oral candidiasis was 96.4%. The incidence of adverse reaction like cough was 9.0%, and the incidence of nausea was 5.5%. However, both were tolerable. 【Conclusion】Amphotericin B inhalation is safe and effective in the treatment of oral candidiasis in patients with HIV/AIDS, which is worthy of clinical promotion.

Clinical Value of Controlling Delivery Speed of Fetal Head in Normal Labor to Reduce Perineal Laceration

【Objective】To explore the clinical value of fetal head delivery speed control during natural childbirth parturient in reducing maternal perineal laceration. 【Methods】A total of 400 pregnant women who underwent natural delivery in the obstetrics from February 2016 to February 2017 were enrolled in the observation group and the control group, with 200 patients in each group. Patients in the observation group received the control of fetal head delivery speed rate for the delivery assistance, while patients in the control group had the routine birth canal to assist the delivery. Indicators such as postpartum perineal laceration, the second stage of labor, postpartum hemorrhage, labor pain and childbirth control were compared between the two groups. 【Results】The perineal cut rate was 43.50% in the observation group, which was significantly lower than 63.00% in the control group (P<0.05). The perineal laceration rate in the observation group was 21.50%, which was significantly lower than that in the control group (34.50%) (P<0.05). The score of perineal laceration in the observation group was significantly lower than that in the control group, and the difference was statistically significant (P<0.05). The postpartum hemorrhage, labor pain and delivery control score in the observation group were significantly lower than those in the control group (P<0.05). The second stage of labor in the observation group was longer than the control group (P<0.05). There was no significant difference in Apgar scores between the two groups after 1 min and 5 min postpartum (P>0.05).【Conclusion】Natural childbirth parturient with control of fetal head delivery speed can reduce maternal perineal laceration and labor pain. It has positive effect on the sense of delivery control.

Effect of Intravesical Perfusion of Mitomycin after Transurethral Resection of Non-muscle Invasive Bladder Cancer

【Objective】To investigate the effect of intravesical perfusion of mitomycin after transurethral resection of non-muscle invasive bladder cancer (TURBt).【Methods】A total of 86 patients with non-muscle invasive bladder cancer were divided into the control group (43 cases, underwent TURBt ) and the observation group (43 cases, underwent TURBt and combined with mitomycin for bladder infusion). The differences in postoperative recurrence rate, serum tumor related factors, adverse reactions and quality of life were compared between the two groups.【Results】The recurrence rate of the observation group was significantly lower that of the control group (P<0.05). The average recurrence time was significantly longer than that of the control group (P<0.05). The serum levels of VEGF, FGF and MMP-9 in the observation group were significantly lower than those in the control group (P<0.05). The incidence of bladder irritation in the observation group was higher than that in the control group (P<0.05), while the incidence of adverse reactions such as nausea and vomiting, chest tightness and palpitations did not show statistically significant difference (P>0.05). The EORTC QLQ-C30 score and KPS score of the two groups were higher than those before operation (P<0.05), and the two scores in the observation group were significantly higher than those in the control group (P<0.05).【Conclusion】TURBt combined with early mitomycin-induced bladder perfusion therapy on non-muscle infiltration bladder cancer can reduce the recurrence rate, improve the serum levels of tumor related factors without obvious adverse reactions, and improve the quality of life of patients. It is worthy of clinical promotion.

Clinical Analysis of Ropivacaine and Levobupivacaine for Combined Spinal and Epidural Anesthesia in Caesarean Section

All patients received CSEA. The control group was anesthetized with levobupivacaine,while the observation group was anesthetized with ropivacaine. Clinical Efficacy and Prognostic Factors of Lens Implantation Combined with Anterior Vitrectomy for Malignant Glaucoma

【Objective】To analyze the influencing factors of clinical efficacy of phacoemulsification and intraocular lens implantation combined with anterior vitrectomy in patients with malignant glaucoma. 【Methods】From April 2013 to April 2017, 88 patients (96 eyes) with malignant glaucoma underwent anterior vitrectomy combined with phacoemulsification and intraocular lens implantation. The patients with normal intraocular pressure and anterior chamber formation were regarded as good group, while those with poor anterior chamber formation were regarded as ineffective group. The changes of visual acuity and intraocular pressure before and after operation were compared, and the risk factors affecting the therapeutic effect were analyzed by Logistic.【Results】After operation, 61 of 88 patients (96 eyes) had visual acuity improvement of two lines or more; 20 patients (22 eyes) had visual acuity higher than 0.3; 41 patients (44 eyes) had visual acuity of 0.1~0.3; 27 patients (30 eyes) had no significant change; and no visual acuity decreased. After operation, 42 patients (46 eyes) had intraocular pressure control, 34 patients (37 eyes) had partial control, and 12 patients (13 eyes) had no effect. The effective rate of intraocular pressure control was 86.46%. There was significant difference between the good group and the ineffective group in the length of eye axis and the distance between scleral processes before operation (P<0.05). Axis length and age before surgery were independent risk factors affecting the outcome of surgery (P<0.05).【Conclusion】The age of the patients and the length of the ocular axis affect the surgical treatment of malignant glaucoma, which should be paid attention to clinically.

Study on the Correlation between the Expression of Vascular Endothelial Growth Factor, Hsp70 and IL-1β and the Prognosis of Patients with Spontaneous Cerebral Hemorrhage

【Objective】To explore the correlation between the levels of vascular endothelial growth factor (VEGF), heat shock protein 70 (Hsp 70), interleukin-1β(IL-1β) and prognosis in patients with spontaneous intracerebral hemorrhage (SICH).【Methods】From August 2016 to August 2018, 96 patients with SICH diagnosed and treated by surgery were divided into poor prognosis group (n=32) and good prognosis group (n=64) according to their prognosis. Serum levels of vascular endothelial growth factor, Hsp 70 and IL-1β were detected by ELISA. GCS scores and hematoma volume were measured. ROC curve was used to analyze the predictive value of each index for SICH prognosis.【Results】The levels of vascular endothelial growth factor, Hsp70 and IL-1beta in SICH patients with poor prognosis were significantly higher than those in patients with good prognosis (P<0.05). Pearson correlation analysis showed that the amount of hematoma at admission was positively correlated with the expression levels of VEGF, Hsp70 and IL-1β(P<0.05), while the GCS score at admission was negatively correlated with the expression levels of VEGF, Hsp70 and IL-1beta (P<0.05). The levels of VEGF, Hsp70 and IL-1β had predictive value for the poor prognosis of SICH, with statistical significance (P<0.05); the GCS score at admission had the highest diagnostic value for the poor prognosis of SICH, with AUC (0.768 ± 0.047); there was significant difference between the levels of VEGF and the AUC score of GCS (P<0.05); there was no significant difference between the levels of plasma Hsp70 and IL-1β and the AUC score of GCS (P>0.05). )【Conclusion】Serum levels of vascular endothelial growth factor (VEGF), Hsp70 and interleukin-1β are helpful to reflect the severity of SICH patients and have high clinical value in predicting poor prognosis.

【Objective】To explore the effect of feedback health education on postoperative rehabilitation training in aspects of knowledge, belief and practice (KBP) in elderly patients with femoral neck fracture.【Methods】Nightly-six elderly patients with femoral neck fracture treated in our hospital from January 2014 to March 2017 were randomly divided into observation group and control group, 48 cases in each group. The control group was given routine health education for post-operative rehabilitation, while the observation group was given feedback health education on the basis of the control group. The scores of knowledge, belief and behavior, rehabilitation effect and satisfaction of post-operative rehabilitation were compared between the two groups.【Results】At discharge, the knowledge, attitude and behavior scores of rehabilitation training in the observation group were significantly higher than those in the control group (P<0.05); the Harris hip function score and improved Barthel index (MBI) in the observation group were significantly higher than those in the control group (P < 0.05) after 6 months of follow-up (P<0.05); and the satisfaction was significantly higher than that in the control group (P<0.05).【Conclusion】Feedback-based health education can help to improve the level of postoperative rehabilitation training and KBP level of patients with femoral neck fracture in the elderly, promote joint function recovery, and maintain a good relationship between nurses and patients.

Effect of Valsartan Combined with Calcium Dobesilate on Renal Function and Renal Artery Hemodynamics in Patients with Diabetic Nephropathy

【Objective】To investigate the effects of valsartan combined with calcium dobesilate on renal function and renal artery hemodynamics in patients with diabetic nephropathy (DN). 【Methods】A total of 126 diabetic nephropathy patients were randomly divided into two groups with 62 cases each. All patients received routine treatment for diabetes mellitus to control blood glucose after admission. On this basis, the control group (Group B) was treated with valsartan, while the observation group (Group A) was treated with valsartan combined with calcium dobesilate. The course of treatment was 3 months.The clinical efficacy, renal function and renal artery hemodynamics were compared between the two groups. 【Results】After treatment, the total effective rate in Group A was 93.6%, which was significantly higher than 80.9% in Group B (P<0.05). There was no significant difference in the indexes between the two groups before treatment (P>0.05). After treatment, UAER,SCr,BUN and Cys-C of the two groups were significantly lower than those before treatment, and Group A was significantly lower than that of Group B (P<0.05). The systolic maximum flow velocity (Vs max) and the diastolic minimum flow velocity (Vd min) of the renal artery hemodynamic parameters in both groups were higher than those before treatment, and the pulsatility index (PI) and resistance index (RI) were lower than before treatment (P<0.05). Those observations of Group A were better than those of Group B (P<0.05).The incidence of adverse reactions in Group A was 15.8% (10/63), which was significantly lower than that in Group B (30.1%, 19/63) (P<0.05). 【Conclusion】Valsartan combined with calcium dobesilate has better clinical effects on renal function and renal artery hemodynamics in patients with DN, which is worthy of clinical application.

【Objective】To investigate the effects of edaravone on the expression of inflammatory factors TNF-alpha, IL-6p, p38MAPK, p42MAPK and p44MAPK in cerebral ischemia of ischemic stroke (CIS) rat model.【Methods】 Eighty rats were randomly divided into saline group (40 rats) and edaravone group (40 rats). The edaravone group was intraperitoneally injected with 5 mg / (kg d) edaravone for one week. The saline group was intraperitoneally injected with the same amount of saline. One week after administration, the edaravone group and the saline group were randomly divided into edaravone sham operation group (20 rats), edaravone model group (20 rats), saline sham operation group (20 rats) and saline model group (20 rats). CIS rat model was established by thread embolization. Inflammatory factors TNF-alpha and IL-6 were detected by ELISA. Western blot was used to detect p38MAPK, p42MAPK and p44MAPK proteins in cerebral ischemic area of rats.【Results】The expression levels of TNF-alpha, IL-6 and p38MAPK protein in cerebral ischemic tissue of rats in the saline model group and the edaravone model group were significantly higher than those in the saline sham operation group and the edaravone sham operation group, and the expression levels of TNF-alpha, IL-6 and p38MAPK protein in cerebral ischemic tissue of rats in the saline model group were significantly higher than those in the edaravone model group (P<0.05). However, there was no significant difference in the protein expression levels of p42MAPK and p44MAPK between the groups (P>0.05).【Conclusion】 Edaravone can inhibit the inflammatory response in cerebral ischemic area of CIS rats, which may inhibit the inflammatory response in ischemic area through p38MAPK pathway.

Effects of Three-dimensional Laparoscopy on Cholecystolithiasis with Common Bile Duct Stones and its Influence on PCT and CRP

【Objective】 To investigate the effect of three-dimensional laparoscopy in the treatment of cholecystolithiasis with common bile duct stones and its influence on serum procalcitonin (PCT) and C-reactive protein (CRP).【Methods】 The clinical data of 72 patients with cholecystolithiasis complicated by common bile duct stones admitted to our hospital from March 2015 to March 2018 were retrospectively analyzed. According to the different surgical methods, the patients were divided into three-dimensional operation group (n=32) and two-dimensional operation group (n=40). The patients were treated with three-dimensional laparoscopy and traditional two-dimensional laparoscopy, respectively. The related indexes of operation, changes of serum PCT, PCR before and after operation, and the occurrence of complications and the recurrence of stones were compared between the two groups.【Results】The operation time and intraoperative bleeding volume in the three-dimensional group were significantly lower than those in the two-dimensional group (P<0.05). There was no significant difference in abdominal drainage volume, drainage time and hospital stay between the two groups (P<0.05). The levels of serum PCT and CRP in the two groups were significantly higher than those before operation, while the levels of serum PCT and CRP in the two-dimensional group were higher than those in the three-dimensional group (P<0.05). Bile leakage occurred in 1 case in the 2D operation group and the incidence of complications was 2.5%. No complications were found in the 3D operation group, but there was no significant difference between the two groups (χ

【Objective】To investigate the correlation between ambulatory pulse pressure index (APPI) and flow-mediated endothelium-dependent vasodilation(FMD)in patients with type 2 diabetes mellitus (T2DM) complicated with hypertension. 【Methods】Thirty patients with primary hypertension and 30 patients with primary hypertension and type 2 diabetes from January 2018 to October 2018 were enrolled for the study as hypertension group and hypertension combined with diabetes mellitus group, respectively. Further, 30 healthy adult volunteer who received physical examination in our hospital were selected as the control group. The results of blood pressure and FMD were compared among three groups, and Pearson correlation coefficient was performed to analyze the correlation of APPI and FMD. 【Results】 There were significant differences in SBP, DBP, 24h SBP, 24h DBP and APPI among three groups (P<0.05). The SBP and DBP in the hypertension combined with diabetes mellitus group and the hypertension group were significantly higher than those in the control group. The 24h SBP, 24h DBP and APPI in the hypertension combined with diabetes mellitus group were significantly higher than those in the hypertension group and the control group. The 24h DBP: and APPI in the hypertension group were significantly higher than those in the control group. There were significant differences in basic vascular diameter, maximum diameter after reactive congestion or nitroglycerin administration, and FMD among the three groups (P<0.05). The diameter of the vessel under the basal state and the maximum diameter of the blood vessel after reactive hyperemia or nitroglycerin in the hypertension combined with diabetes mellitus group were significantly higher than those in the hypertension group and the control group. The FMD in the hypertension combined with diabetes mellitus group was significantly lower than that of the hypertension group and the control group, and FMD in the hypertension group was significantly lower than that in the control group. FMD was negatively correlated with 24h SBP and APPI in all subjects (P<0.05). 【Conclusion】The APPI of T2DM patients with hypertension was significantly increased and FMD significantly decreased. There was a significant negative correlation between APPI and FMD.

Changes of PCT and CRP Levels in Elderly Patients with Urinary Tract Infection and their Clinical Significance

【Objective】To analyze the changes of procalcitonin (PCT) and C-reactive protein (CRP) levels in urinary tract infection of elder patients and its clinical significance. 【Methods】The clinical data of 68 elderly patients with urinary tract infections admitted to our hospital from January 2017 to October 2018 were selected. They were divided into upper urinary tract infections (group A,n=32) and lower urinary tract infections (group B,n=36) according to the location of urinary tract infections. Thirty healthy volunteers with normal physical examination in our hospital during the same period were selected as control group (group C). Blood and urine white blood cell count (WBC) and serum PCT and CRP levels were measured, and the correlation between PCT and CRP levels was analyzed.【Results】The levels of WBC in blood, WBC in urine, PCT and CRP in serum of the group A and the group B were significantly higher than those of the group C, and the levels of PCT and CRP in serum of the group A were significantly higher than those of the group B, with statistical significance (P<0.05). The levels of serum PCT and CRP in the group A and B after treatment were significantly lower than those before treatment, and the levels of serum PCT and CRP in the group A were higher than those in the group B, with statistical significance (P<0.05). Pearson correlation analysis showed that serum PCT level was positively correlated with CRP (r=0.73,P=0.009<0.05). 【Conclusion】The combined detection of serum PCT and CRP levels can help to diagnose the occurrence of urinary tract infections in the elderly, and is helpful to clinical medication. It is worth popularizing and applying in clinical practice.

Predictive significance of Blood Pressure Variability on White Matter Lesions in Patients with Ischemic Cerebrovascular Disease

【Objective】To study predictive significance of blood pressure variability on white matter lesions in patients with ischemic cerebrovascular disease. 【Methods】Ninety-five patients with ischemic cerebrovascular disease admitted to our hospital from June 2015 to May 2017 were selected as subjects. According to the results of cranial MRI, they were divided into WML group (n=55) and control group (n=40). The severity of WML was assessed by Fazekas scale. WML patients were divided into mild-moderate group (n=25) and severe group (n=30). 24-hour ambulatory blood pressure [24-hour mean systolic blood pressure (24-hour SBP), day mean systolic blood pressure (dmSBP), night mean systolic blood pressure (nmSBP), 24-hour mean diastolic blood pressure (24-hour DBP), day mean diastolic blood pressure (dmDBP), night mean diastolic blood pressure (nmDBP)], coefficient of variation [diurnal systolic blood pressure variability (dSBPV), diurnal diastolic blood pressure variability (dDBPV), nighttime closure ,compression coefficient of variation (nSBPV), nocturnal diastolic pressure coefficient of variation (nDBPV)]were observed. Multivariate logistic regression analysis was used to analyze the risk factors of WML lesions.【Results】The 24h SBP, dmSBP, nmSBP, 24h DBP, dmDBP, nmDBP, dSBPV, dDBPV, nSBPV and nDBPV in the WML group were higher than those in the control group (P<0.05), and the 24h SBP, 24h DBP, dSBPV, dDBPV, nSBPV and nDBPV in the WML severe group were higher than those in the mild-moderate group (P<0.05). Multivariate logistic regression analysis showed that 24h SBP, dSBPV and nSBPV were independent risk factors for WML lesions (P<0.05).【Conclusion】BPV is closely related to WML lesions in patients with ischemic cerebrovascular disease. 24-hour SBP, dSBPV and nSBPV can predict the degree of WML lesions, and should be monitored clinically.

Changes of Urinary NGAL Levels in Patients with Diabetic Nephropathy and its Clinical Significance

【Objective】To investigate the changes and significance of serum neutrophil gelatinase-associated lipid carrier protein (NGAL) in patients with diabetic nephropathy (DN). 【Methods】 Fourty healthy volunteers were selected as group A, 40 patients with type 2 diabetes mellitus (group B), 40 patients with diabetes mellitus and microalbuminuria (group C, urinary albumin/creatinine 30-300 mg/g), 40 patients with diabetic albuminuria (group D, urinary albumin/creatinine greater than 300 mg/g). NGAL, serum creatinine (SCr), urea nitrogen (BUN) and urinary microalbumin (UmALB) levels were measured and compared among the four groups.【Results】The levels of NGAL and UmALB in the group C and D were significantly higher than those in the group A and the group B (P<0.05), and the levels of NGAL, SCr, BUN and UmALB in the group D were significantly higher than those in the group A, group B and group C (P<0.05). NGAL was positively correlated with SCr, BUN and UmALB in DN patients (P<0.05). The critical value of NGAL in diagnosing DN was 80.04 ng/mL, the sensitivity was 85.29%, the specificity was 76.04%, the missed diagnosis rate was 14.71%, the misdiagnosis rate was 23.96%, and the AUC value under ROC curve was 0.807.【Conclusion】The level of NGAL increased in the early stage of DN patients, and it has a certain correlation with the degree of renal function damage. Early detection of NGAL has a certain value in the differential diagnosis of DN.
