2289 Analysis of the Effect of Postoperative Functional Training with Local Drug Phonophoresis in Total Knee Arthroplasty

【Objective】 To investigate the effect of local drug phonophoresis on pain relief and functional exercise at the early stage after total knee arthroplasty (TKA). 【Methods】 Forty-one patients with unilateral TKA from June 2016 to May 2017 were randomly divided into observation group (n=21) and control group (n=20).Two groups of patients were given the same basic analgesia regimen. From 2 weeks after operation, the patients in the observation group received local ultrasound penetration (lidocaine + betamethasone) for 10 times in 4 weeks. The patients in the two groups were followed up to 3 months after operation. The preoperative activity and AKS scores were recorded respectively. The average activity,visual analogue score (VAS) and AKS score at 2 weeks, 6 weeks and 3 months after operation were recorded respectively. 【Results】 ① There was no significant difference between the two groups in preoperative activity, AKS score, activity at 2 weeks after exercise and VAS score after exercise (P>0.05);② At 6 weeks after operation, the activity of the observation group was (110.0±12.3)°, the VAS score was (2.0±1.0) points, the control group was (102.3±10.4)°,the VAS score was (2.9 ±1.3) points, the difference between the two groups was statistically significant (P<0.05).③ Three months after operation, the activity of the observation group was (116.4±10.7)°, the score of AKS function was (84.0±10.6), the activity of the control group was (111.0±10.9)°, the score of AKS function was (76.0±12.4). The observation group was superior to the control group(P<0.05).【Conclusion】 Ultrasound phonophoresis therapy with lidocaine and betamethasone can effectively relieve postoperative pain after TKA and is helpful to functional rehabilitation of patients with obvious early pain after TKA.

2293 Changes of EphA4,ephrinA3 Expression in Hippocampal CA1 Region of Epileptic Rats Induced by Pilocarpine and its Significance

【Objective】 To investigate the changes and significance of EphA4,ephrinA3 expression in hippocampal CA1 region of epileptic rats induced by pilocarpine.【Methods】 The SD rat model of temporal lobe epilepsy was established by pilocarpine. The changes of EphA4 protein and ephrinA3 gene in hippocampal CA1 at different time points were detected by immunohistochemistry and in situ hybridization.The morphological changes of dendritic spine in CA1 region were observed by rapid Golgi staining and the dynamic changes of ephrinA3 in astrocytes were observed by confocal microscopy.【Results】 The expression of EphA4 and ephrinA3 in hippocampal CA1 was high in temporal lobe epilepsy rats. Compared with the control group, the expression of EphA4 protein and ephrinA3 mRNA decreased to the lowest point at 7 days after epilepsy (P<0.01). After that, it increased gradually, but it was still lower than the control group at 15 days (P<0.05), and returned to that of the control group at 30 and 60 days (P>0.05). The dendritic spine of pyramidal cells in CA1 area became thinner, longer and denser at 7 days after epilepsy. EphrinA3 was expressed in the astrocytes of the hippocampus of the mature rats. The expression of ephrinA3 in the astrocytes of the hippocampus of the rats was down-regulated 7 days after epilepsy, and increased at 15 days after epileptic status, but it was still lower than that of the control group and returned to normal at 30 days.【Conclusion】 The down-regulation of the expression of EphA4 protein and ephrinA3 mRNA in CA1 participates in the regulation of postsynaptic dendritic spine morphology and is one of the important molecules regulating synaptic plasticity.

2297 Analysis of Lys847del Mutation Site of MYH7 Gene in Chinese Han Families with Familial Hypertrophic Cardiomyopathy

【Objective】 To investigate the mutation site of the main pathogenic gene MYH7(beta-myosin heavy chain gene) in Chinese Han family with familial hypertrophic cardiomyopathy (HCM), and to analyze the correlation between their genotype and clinical phenotype. 【Methods】 A total of 9 families suffering from HCM were chosen for the study. The exons in the functional regions of the beta myosin heavy chain gene were amplified with PCR and the products were sequenced, including TNNT2,MYBPC3,MYH7,TNNI3.The identified mutations were further detected through bidirectional Sanger sequencing in all family members and 60 healthy volunteers. Pedigree analysis included physical examination, clinical symptoms, electrocardiography (ECG), echocardiography(UCG) and 3.0T cardiac magnetic resonance (CMR), and regular outpatient visits or telephone follow-up. 【Results】 The Lys847del mutation in exon 22 of beta-myosin heavy chain gene was found in one family. However, no abnormality was observed in the same position in all healthy volunteers In this family, beside the proband showed Lys847del mutation, his mother and his daughter carried the same mutation of exon 22 in MYH7 gene. The mother of the proband was diagnosed as HCM following family screening and daughter did not meet the diagnostic criteria for HCM. 【Conclusion】 The Lys847del mutation in exon 22 of MYH7 may be the pathogenic mutation gene of familial HCM in China. Patients with hypertrophic cardiomyopathy with this mutant genotype may present a similar clinical phenotype.

2301 Effect of XIAP Gene Silencing Expression on Biological Characteristics of Ovarian Cancer SKOV3 Cells

【Objective】 To investigate the effect of XIAP gene silencing on the biological characteristics of ovarian cancer SKOV3 cells. 【Methods】 The human ovarian cancer cell line SKOV3 were transfected with XIAP/siRNA and XIAP/smRNA respectively, and the PBS were used as the control group. The cell transfection efficiency was detected. The apoptosis rate was detected by flow cytometry. The number of cell migration and invasion were detected by Transwell chamber, and the expression of related protein levels were detected by Western blot. 【Results】 Compared with the control group, the level of XIAP protein in XIAP/siRNA group decreased significantly, but no significant change was found in XIAP/smRNA group. The level of apoptosis in XIAP/siRNA group was significantly higher than that in the XIAP/smRNA group and the control group (P<0.05). The number of model penetrating cells and metastatic cells in the XIAP/siRNA group was significantly lower than that in the control group and the XIAP/smRNA group (P<0.05), but there was no significant difference between the control group and the XIAP/smRNA group (P>0.05). The relative expression level of E-cad protein in the XIAP/siRNA group was 0.312 ±0.08, which was significantly lower than that in the control group and the XIAP/smRNA group (1.065 ± 0.01, 1.026 ± 0.2), and the difference was statistically significant (P<0.05), but there was no significant difference between the XIAP/smRNA group and the control group (P>0.05).【Conclusion】 XIAP gene silencing can effectively inhibit apoptosis, metastasis and invasion of ovarian cancer SKOV3 cells, and its mechanism may be related to the regulation of E-cad expression.

2304 Effect of Introduction of Lappaconitone on Analgesia and Quality of Life in Patients with Cancer Pain by Ultrasonic Pulse Therapy Instrument

【Objective】 To investigate the effects of analgesia and quality of life in patients with cancer pain through acupoint introduction of lappaconitone by ultrasonic pulse therapy instrument . 【Methods】 A total of 98 patients with advanced stage tumors admitted to our hospital were divided into two groups. Patients (n=48) in the control group received standardized treatment for cancer pain , and patients (n=50) in the observation group were treated with ultrasound-guided analgesia on the basis of routine treatment. Both groups received continuous treatment for 7 days . Analgesic effect and quality of life between the two groups were compared. 【Results】 After treatment, the NRS scores (number rating scale) of the two groups were lower than those before treatment(P<0.05) , and the NRS scores in the observation group were significantly lower than in the control group (P<0.05) . The total effective rate of pain relief in the observation group was 90.00% (45/50), which was significantly higher than that in the control group 68.75%(33/48,P<0.05). Before treatment , there was no significant difference in life quality measurement scale (WHOQOL-BREF scores) between the two groups (P>0.05 ) . After treatment, both groups had higher scores in terms of subjective feeling of health condition, quality of life, physiological, psychological, environmental and social relations (P<0.05) . Compared to the control group, the scores of subjective feeling of health condition , physiological and psychological status in the observation group was better (P<0.05 ) . There was no significant difference in subjective feeling of life quality, environmental and social relations between the two groups (P>0.05 ) . 【Conclusion】 The introduction of lappaconitone by ultrasonic pulse therapy can significantly reduce the pain level of cancer patient and improve the pain relief rate and quality of life in patients with cancer pain

2307 Diagnostic Value of SWI Combined with DWI in Acute Ischemic Stroke

【Objective】 To analyze the imaging features of Magnetic sensitivity weighted imaging (SWI) combined with diffusion-weighted imaging (DWI) for acute ischemic stroke (AIS) and to evaluate its diagnostic value.【Methods】 From September 2016 to June 2018, 90 patients with AIS were selected and divided into cardiogenic AIS group (n=55) and non-cardiogenic AIS group (n=35). SWI,DWI and conventional MRI were given to the two groups, and the clinical data and imaging evaluation results were compared between the two groups.【Results】 The (NIHSS) score of stroke scale in the cardiogenic AIS group was significantly higher than that in the non-cardiogenic AIS group (P<0.05). There was no significant difference in the positive rate in DWI infarct volume and Intracerebral microhemorrhage focus (CMB) between the cardiogenic AIS group and the non-cardiogenic AIS group (P>0.05). The positive rate of protrusion cortical vein (PCV) and sensitive vascular sign (SVS) in the cardiac AIS group was significantly higher than that in the non-cardiogenic AIS group (P<0.05).【Conclusions】The positive rates of NIHSS, SVS and PCV in cardiogenic AIS patients are significantly higher than those in non-cardiogenic AIS patients. SWI combined with DWI can provide rich diagnostic information for AIS patients. It is of great significance to guide the clinical design of individualized treatment.

2311 Expression and Clinical Significance of PTEN and COX2 Protein in Patients with Recurrent Metastatic Colorectal Cancer

【Objective】 To study the expression and clinical significance of phosphatase and tensin homolog deleted on chromosome ten (PTEN) and Cyclic oxidase-2 (COX2) protein in patients with recurrent metastatic colorectal cancer. 【Methods】 From February 2016 to February 2017, 50 patients with recurrent and metastatic colorectal cancer (recurrence and metastasis group) and 50 patients with recurrent and unmetastatic colorectal cancer treated in our hospital from February 2016 to February 2017 were selected as simple recurrence group. Fifty patients with primary colorectal cancer who were treated in our hospital at the same time were selected as the primary colorectal cancer group, and 50 healthy persons who received enteroscopy examination (with intestinal discomfort and other related symptoms) as the control group. The expression of PTEN,COX2 protein was detected by immunohistochemical method in the four groups and the correlation between the expression of PTEN,COX2 protein and the prognosis of patients with recurrent and metastatic colorectal cancer was analyzed.【Results】 The positive rate of PTEN protein expression in patients with recurrence and metastasis was 20.00% (10/50), which was significantly lower than that in the other three groups, while that in the simple recurrence group was significantly lower than that in the primary colorectal cancer group and the control group. At the same time, the positive rate of COX2 protein expression in patients with recurrence and metastasis was 86.00% (43/50), which was significantly higher than that of the other three groups, and that of simple recurrence group was significantly higher than that of the primary colorectal cancer group and the control group. The difference was statistically significant (mean P<0.05). The positive rate of PTEN protein expression in the primary colorectal cancer group was lower than that in the control group, but the positive rate of COX2 protein expression was higher than that in the control group (mean P<0.05). The positive rate of PTEN protein expression in the survival patients was 32.14% (9/28), which was significantly higher than that in the dead patients (4.55%) (1/22). The positive rate of COX2 protein expression was 75.00% (21/28). It was significantly lower than that in the death patients (100.00%, 22/22), and the difference was statistically significant (P<0.05). The correlation analysis showed that the survival rate of patients with recurrent and metastatic colorectal cancer was positively correlated with the expression level of PTEN protein (r=0. 673, P<0.05). There was a significant negative correlation between the expression of COX2 protein and the expression of COX2 protein (r=-0.652,P=0.000).【Conclusion】 The expression of PTEN,COX2 protein can be used for early diagnosis of recurrent and metastatic colorectal cancer and as a prognostic indicator.

2314 Clinical Observation of Different Doses of Intranasal Dexmedetomidine to Prevent Postoperative Shivering after Hepatectomy

【Objective】 To investigate the effects of different doses of dexmedetomidine(Dex) nasal drops in the prevention of postoperative chills after hepatectomy.【Methods】 A total of 120 patients with laparoscopic lobectomy were randomly divided into four groups with 30 patients in each group. Patients in the control group (group C) received normal saline before tracheal catheter extraction, while patients in group D1, D2 and D3 received Dex 0.5μg/kg、1μg/kg、2μg/kg, respectively.The incidence of nasopharyngeal temperature, grade of shivering, waking time, extubation time and other adverse reactions were recorded. 【Results】 There was no significant difference in the incidence and severity of postoperative shivering between group D1 and group C (P> 0.05). Compared to group C, the incidence and severity of postoperative shivering were significantly lower in group D2 and group D3 (P <0.05). However, there was no significant difference between group D2 and D3 (P>0.05). There was no significant difference in waking time, extubation time and sedation incidence among group C, D1 and D2 (P>0.05). The recovery time, extubation time and sedation rate of patients in group D3 were higher than those in group C (P<0.05). There was no significant difference in the incidence of nausea, vomiting and respiratory depression in all four groups (P>0.05). 【Conclusion】 Prophylactic intranasal administration of 1μg/kg of Dex in patients with hepatic lobectomy can effectively prevent postoperative shivering.

2317 Effect of General Health Education of Nursing Science of Liver Nourishing in Perioperative Period of Liver Lobectomy

【Objective】 To explore the effect of general health education of nursing science on promoting the rehabilitation of patients undergoing liver lobectomy.【Methods】 From February 2017 to May 2018, 69 patients with hepatic lobectomy were randomly divided into control group (n=33) and observation group (n=36). The control group adopted the traditional health education mode, the observation group was given the health education of raising liver and nursing science on the first day after operation. The first time of getting out of bed, the recovery time of bowel sound, the satisfaction degree and the ability of self-care were compared between the two groups.【Results】 The first time of getting out of bed, the recovery time of bowel sound, satisfaction and self-care ability in the observation group were significantly better than those in the control group (P<0.05), and the difference was statistically significant. 【Conclusion】 General health education of nursing science of liver nourishing liver can promote the rehabilitation of patients undergoing liver lobectomy.

2320 Investigation of Psychological Status and Related Factors in Elderly Patients with Habitual Abortion

【Objective】 To investigate the psychological status of elderly patients with habitual abortion, explore the related factors affecting their psychological status, and summarize the thoughts of formulating clinical psychological intervention strategies. 【Methods】 From August 2016 to August 2018, 52 elderly patients with habitual abortion (> 40 years old), 71 elderly patients with spontaneous abortion and 50 non-elderly patients with habitual abortion were enrolled in the observation group, the elderly control group and the non-elderly control group, respectively. The 90 items of symptom checklist 90 (SCL-90) and self-assessment of anxiety scale (SAS) and self-rating depression scale (SDS) were compared among the three groups. The correlations among SCL-90 score, SAS, SDS score, Eysenck personality questionnaire (EPQ) and social support rating scale (SSS) scores in the observation group were analyzed. 【Results】 The SAS score and SDS score of the observation group were higher than those of the elderly control group and the non-elderly control group, and the difference was statistically significant (P<0.05). The scores of SCL-90 in the observation group were higher than those in the elderly control group and the non-elderly control group (P<0.05). Pearson correlation analysis showed that SAS score and SDS score were positively correlated with EPQ P and N dimension score, and positively correlated with social support utilization and total social support score. Somatization and anxiety score were positively correlated with P dimension score of EPQ, and SCL-90 dimension score was positively correlated with N dimension of EPQ. The degree score was positively correlated with the utilization of social support and the total score of social support (P<0.05). 【Conclusion】 The mental health status of the elderly patients with habitual abortion is generally poor, their anxiety and depression are obvious. The personality characteristics and social support of the patients can affect their mental state, and should be paid attention to in psychological intervention.

2323 Serum nesfatin-1 Level in patients with COPD and its Clinical Significance

【Objective】 To detect the serum level of nesfatin-1 in patients with stable chronic obstructive pulmonary disease (COPD) and explore its relationship with nutritional indicators and lung function. 【Methods】 A total of 100 patients with stable COPD and 100 healthy volunteers were selected as COPD group and healthy control group, respectively. Serum nesfatin-1 levels were detected by enzyme-linked immunosorbent assay (ELISA), and the nutritional indicators and lung function were detected and compared as well. The nutritional indicators included hemoglobin (Hb) and serum albumin (ALB). The lung function index included percentage of forced expiratory volume in the first second (FEV1),FEV1/FVC. Pearson correlation analysis was used to determine the correlation between serum nesfatin-1 level and nutritional status and lung function in patients with stable COPD. 【Results】 The serum nesfatin-1 level in the COPD group was higher than that in the healthy control group. The Hb, ALB nutritional, FEV1 and FEV1/FVC were significantly lower the healthy group. The differences were statistically significant (P<0.05). The serum nesfatin-1 levels were negatively correlated with Hb, ALB, FEV1, and FEV1/FVC in the COPD group (r=-0.608, -0.522, -0.529, -0.478,P<0.05). 【Conclusion】 Serum nesfatin-1 levels may be a potential biomarker for nutritional assessment and pulmonary function grading in patients with COPD.

2326 Effect of fQRS Wave Group on Survival Status of Patients with Coronary Heart Disease

【Objective】 To investigate the effect of fragmentation QRS (fQRS) wave group on survival of patients with coronary heart disease (CHD).【Methods】 The clinical data of 1043 patients with CHD admitted from June 2014 to June 2017 were analyzed retrospectively. According to the presence of fQRS in the 12-lead synchronous electrocardiogram, the patients were divided into two groups: non fQRS group (n=205) and fQRS group (n=838). The results of echocardiography, electrocardiogram and prognosis were compared between the two groups. 【Results】 The incidence of left ventricular ejection fraction (LVEF) in the fQRS group was significantly higher than that in the non-fQRS group (P<0.05). The incidence of left ventricular septal thickness, left ventricular end-diastolic diameter (LVDD) and the incidence of three-vessel disease was significantly higher in the fQRS group than in the non fQRS group (P<0.05). Left ventricular end-systolic diameter (LVSD), left ventricular end-diastolic volume (LVDV), left ventricular end-systolic volume (LVSV) were not significantly different between the two groups (P>0.05). The electric axis of the fQRS group was lower than that of the non-fQRS group. The QRS duration, P wave duration and ST segment change rate were significantly higher in the fQRS group than in the non-fQRS group (P<0.05). The incidence of angina pectoris, arrhythmia, cardiac death and death in the fQRS group was higher than that in the non-fQRS group, and the survival time was shorter than that in the fQRS group (P<0.05).【Conclusion】 The incidence of cardiac dysfunction, cardiovascular events, cardiogenic mortality and death are higher in patients with CHD complicated with abnormal Q wave. When fQRS wave appears in electrocardiogram, it has good prognostic value.

2329 Risk Factors for Pregnancy with Subclinical Hypothyroidism and Its Effect on Maternal-infant outcomes

【Objective】 To explore the risk factors for pregnancy with subclinical hypothyroidism (SCH) and its influence on maternal-infant outcome.【Methods】 A total of 200 pregnant women with SCH receiving prenatal examination in the hospital from January 2017 to May 2018 were selected as the research group and 200 cases of non SCH were selected as the control group. The incidence rates of complications and pregnancy outcomes in these groups were observed. Univariate logistic regression analysis and multivariate logistic regression analysis were used to analyze age, pre-pregnancy body mass index(BMI) and educational level as well as the risk factors of pregnancy complicated with SCH. 【Results】 The incidence rates of gestational diabetes mellitus,anemia,and premature rupture of fetal membranes in the research group were significantly higher than those in the control group(P<0.05).The incidence rates of abortion, preterm birth, stillbirth in the research group were significantly higher than those in the control group(P<0.05).Univariate logistic regression analysis showed that age, pre-pregnancy BMI, education level, history of gestation, history of spontaneous abortion and smoking or exposure to smoking environment were correlated with pregnancy complicated with SCH (P<0.05).Multivariate logistic regression analysis showed that age-≥30 years, primiparity, pre-pregnancy BMI<18kg/m2 were risk factors of pregnancy complicated with SCH (P<0.05). 【Conclusion】 Pregnancy combined with subclinical hypothyroidism can lead to various complications, resulting in various adverse events, which seriously affects the safety of mother and child. In clinical, early detection and intervention should be made.

2332 Safety and Efficacy of Levosulpiride, Domperidone and Metoclopramide in the Treatment of Functional Dyspepsia

【Objective】 To evaluate and compare the safety and clinical efficacy of levosulpiride, domperidone and metoclopramide in the treatment of functional dyspepsia.【Methods】 One hundred and fifty patients with functional dyspepsia from June 2014 to March 2015 were randomly divided into three groups with 50 cases in each group. The three groups were treated with levosulpiride, domperidone and metoclopramide respectively. The degree of functional dyspepsia before and after treatment was evaluated by Leeds indigestion questionnaire and Likert five-point scale. 【Results】 143 of 150 patients (50 in the levosulpiride group, 45 in the domperidone group and 48 in the metoclopramide group) completed the test successfully. All three drugs can improve patients' symptoms of functional dyspepsia to varying degrees. However, the overall improvement of dyspepsia in the levosulpiride group was significantly higher than that in the other two groups (P<0.05), and the incidence of adverse reactions (including nausea, headache, diarrhea, etc.) was the lowest in the levosulpiride group.【Conclusion】 Levosulpiride has higher safety and efficacy than domperidone and metoclopramide in the treatment of functional dyspepsia.

2335 Effects of Hyperbaric Oxygen Combined with Lower Extremity Rehabilitation Robot Assisted Training on Lower Limb Muscle Tension and Walking Ability in Patients with Spinal Cord Injury

【Objective】 To investigate the effect of hyperbaric oxygen combined with lower extremity rehabilitation robot assisted training on the lower limb tension and walking ability of patients with spinal cord injury. 【Methods】 A total of 70 patients with spinal cord injury admitted and treated in our hospital from February 2015 to February 2017 were divided into the control group and the observation group according to the random number table method, with 35 cases in each group. All patients were given comprehensive rehabilitation therapy and lower limb rehabilitation robot assisted training. In addition to this, patients in the observation group were given hyperbaric oxygen therapy. The muscle strength and joint force of lower limbs were evaluated by the modified Ashworth scale (MAS) and lower limb rehabilitation robot feedback data, the walking ability, lower limb balance function and activity of daily living were evaluated by functional ambulation category scale (FAC), Berg balance scale (BBS) and modified Barthel index (MBI). 【Results】 The total effective rate of the patients in the observation group was significantly higher than that in the control group (P<0.05). The MAS scores of the adductor muscle, flexor knee, extensor and ankle flexion of the observation group were significantly lower than those of the control group after the treatment (P<0.05). The results of the robot feedback data showed that the force of the hip joint and the knee joint in the observation group was significantly higher than that in the control group (P<0.05), and the level of walking function and balance of lower limbs were significantly better than those of control group (P<0.05). However, there was no significant difference in MBI scores between the two groups after treatment (P>0.05). 【Conclusion】 The combination of hyperbaric oxygen and lower limb rehabilitation robot rehabilitation training can effectively improve the lower limb muscle tension and motor function in patients with spinal cord injury, and the clinical curative effect is remarkable.

2338 Meta-Analysis of the Efficacy of Long-term Albumin Infusion for Patients with Cirrhosis and Ascites

【Objective】 The clinical efficacy and safety of human serum albumin for patients with cirrhosis and ascites were systematically assessed. 【Methods】 We searched Medline,Embase,Cochrane Library,Pubmed,China Biomedical Literature (CBM),WanFang Datebase,VIP and Chinese Journal Full-text database (CNKI). The search deadline from above databases was May 2018. A randomized controlled trial of long-term infusion of human albumin combined with diuretics (the experimental group) and separate diuretics (the control group) in patients with cirrhosis and ascites was performed. Two reviewers were independently involved in the studies using RevMan5.2 software, performed quality assessment, extracted the data, evaluated the risk of bias, and made the conclusions. 【Results】 A total of 803 patients with 5 articles were included. Meta-analysis showed that the overall survival rate was improved in the experimental group compared with the control group [HR=0.60,95%CI(0.44,0.83),P=0.002]. Data also showed the experimental group significantly reduced the incidence of complications [RR=0.35,95%CI(0.18,0.69),P=0.002], re-admission rate [RR=0.23,95%CI(0.13,0.43),P<0.001] and ascites recurrence rate [RR=0.17,95%CI(0.07,0.42),P<0.001]. There were no significant differences between the two groups in the incidence of serious adverse reactions and human serum albumin related adverse events. 【Conclusion】 Long-term infusion of human serum albumin for patients with cirrhosis and ascites can increase the overall survival and reduce the incidence of complications and re-admissions. It does not increase the incidence of adverse reactions.

2343 Study on the Expression of TC、ApoB and Hs-CRP in Patients with Subclinical Hypothyroidism of T2DM

【Objective】 To investigate the expression and clinical significance of total cholesterol (TC), high-sensitivity C-reactive protein (hs-CRP) and apolipoprotein B (ApoB) in the serum of type Ⅱ diabetes mellitus (T2DM) patients with subclinical hypothyroidism (SCH).【Methods】 From April 2015 to January 2018, 140 patients with T2DM combined with SCH were treated in our hospital (observation group) and 140 T2DM patients with normal thyroid stimulating hormone (TSH) were selected as control group. Age, sex ratio, blood lipid and serum hs-CRP levels were compared between the two groups, and the correlation between the above indexes and thyroid hormone level was analyzed.【Results】 The average age, the proportion of female and the level of serum TC,LDL-C,ApoB,hs-CRP of the patients in the observation group were significantly higher than those in the control group. Multivariate Logistic regression analysis showed that the increase of TC,LDL-C, ApoB, hs-CRP,TSH level was an independent risk factor for SCH in T2DM patients (mean P<0.05).【Conclusion】 Compared with the patients with T2DM alone, the level of lipid metabolism is abnormal in patients with T2DM combined with SCH, and the inflammatory reaction is involved in the formation of vascular lesions. Hs-CRP can be used as an effective index for monitoring the occurrence of SCH in patients with T2DM.

2346 Curative Effects of Rituximab Combined with Chemotherapy in the Treatment of Diffuse Large B Cell Lymphoma

【Objective】 To investigate the efficacy of rituximab combined with chemotherapy in patients with diffuse large B cell lymphoma (DLBCL). 【Methods】 According to different treatment regimens, 95 patients with DLBCL were divided into two groups. The observation group was treated with rituximab combined with chemotherapy (CHOP), while the control group was treated with chemotherapy alone. The short-term efficacy, adverse reactions and changes of lymphocyte subsets pre- and post- treatment were compared between the two groups. The 3-year survival rates of the two groups were also calculated. 【Results】 ①The short-term overall response rate of the observation group was 86% (43/50), which was significantly higher than that of the control group 62.22% (28/45) (P<0.05). ②There were no significant differences in levels of total T cells, CD4+, CD8+ or NK cells in the observation group before and after treatment (P>0.05), however, the level of B cells in the observation group after treatment was significantly lower than that in the control group (P<0.05). ③There was no significant difference in the incidence of adverse reactions between the two groups (P>0.05). ④During 1-year and 3-year follow-up, the 1-year progression-free survival rate (80%,40/50) and 1-year total survival rate (92%,46/50) of the observation group were significantly higher than those in the control group (57.78% and 75.56%, respectively) (P<0.05). The 3-year progression-free survival rate of the observation group was significantly higher than that of the control group (P<0.50), while there was no significant difference in 3-year total survival rate (P>0.05). 【Conclusion】 Compared to chemotherapy (CHOP) alone, rituximab combined with CHOP chemotherapy can significantly improve the short-term efficacy in patients with DLBCL, significantly improve the progression free survival rate, and, furthermore, does not increase the adverse reactions of chemotherapy. The regimen has little effect on T lymphocytes and NK cells though it significantly reduces B lymphocytes.

2349 Expression of MMP and VEGF in Primary Renal Cell Carcinoma and its Relationship with Clinical Features and Prognosis of Renal Cell Carcinoma

【Objective】 To investigate the expression of MMP and VEGF in primary renal cell carcinoma and its relationship with clinical features and prognosis of renal cell carcinoma. 【Methods】 A total of 72 cases of renal cell carcinoma confirmed by pathology in our hospital from January 2009 to June 2014 were selected, immunohistochemical staining was used to observe the expression of MMP-2, MMP-9 and VEGF in renal cell carcinoma, and the relationship between MMP-2, MMP-9 and VEGF expression and renal cell carcinoma was studied by unconditional Logistic regression. 【Results】 The expression of MMP-2,MMP-9,VEGF in renal cell carcinoma (RCC) increased gradually with the increase of TNM stage, and the difference was statistically significant among the different stages (P<0.05). In this group, 50 cases survived (the survival group), 22 cases died (the death group), the proportion of TNM stage Ⅰ-Ⅱ patients in the survival group was higher than that in death group (P<0.05). The expression of MMP2, MMP-9,VEGF in the dead group was significantly higher than that in the survival group (P<0.05). In TNM stage, MMP-2, MMP-9,VEGF was the main risk factor for the prognosis of renal cell carcinoma (OR=1.326, 1.559, 1.734, 1.846,P<0.05 ).【Conclusion】 The expression of MMP-2, MMP-9, VEGF in renal cell carcinoma is correlated with TNM stage and increases with the increase of TNM stage. MMP-2,MMP-9,VEGF are risk factors for the prognosis of renal cell carcinoma.

2352 Clinical Characteristics of and Countermeasures for Infantile Hand Injury in An Industrialized Township in the Past Ten Years

【Objective】 To analyze the clinical characteristics of infantile hand injury in industrialized towns in China, and to propose targeted prevention measures. 【Methods】 A total of 326 infants and young children with hand injuries were admitted to our hospital from January 2007 to January 2017. Of these children, 300 were followed up with intact data. Retrospective analysis was performed for clinical characteristics to provide a scientific basis for the development of preventive measures. 【Results】 The study showed that in the past 10 years the number of children with hand injuries increased yearly. Most injuries occurred within the 8:00 am to 16:00 pm timeframe of a day. Mild (34.00%) and moderate (40.67%) injuries accounted for the majority of injuries, with relatively few serious injuries. More than half of kids' parents were workers (51.67%), the others were educators and clerks. 【Conclusion】 The results of this study suggest that the lack of parental supervision of children is a risk factor for hand injury in infants and young children in this area. It is suggested that community health education can prevent and reduce the occurrence of hand injuries.

2355 Clinical Analysis of 22 Cases of Humeral Fracture Caused by Throwing Exercise

【Objective】 To investigate the etiology, treatment and curative effect of humeral fracture caused by throwing exercise. 【Methods】 The clinical data of 22 patients with humeral fracture caused by throwing exercise were analyzed retrospectively from April 2013 to April 2017. The etiology and treatment were analyzed, followed up and the curative effect were observed.【Results】 Twenty-two cases were followed up for 10~22 months, and the fracture line disappeared 4~6 months after operation. According to the Neer score, 18 cases were excellent (81.82%), 4 cases were good (18.18%), the excellent and good rate was 100%.【Conclusion】 Rotational violence is the main cause of humeral fracture in throwing exercise, and satisfactory results can be obtained after early diagnosis and timely treatment.
